  • Mike Stella demonstrates soft tissue techniques for treating and managing shoulder pain arising from scalene and upper trap referral. These techniques can be very effective for arm care in baseball pitchers, especially for conditions like Thoracic Outlet Syndrome (TOS), Biceps tendon issues, forward head posture, shoulder & cervical pain syndromes, and general neuropathies.     Great vid. I think one stealth problem in the mix with all this can be sleeping position. If your nick sits a certain way on the pillow or you sleep on your side, stomach, etc., you can just redo all the problems every night after stretching your ass off all day! It's really hard to change how you sleep, too. And your body basically wants to find the position that's most "comfortable", meaning not stretching anything, which is probably reinforcing the bad patterns. I think all this needs to get talked about more.

    This is amazing. I am a professional violinist and florist and am experiencing this with the tightening of my muscles in the shoulder/upper back. Do you know of anyone in Minneapolis who does treatment like this? Not extremely effective working with PT. Thank you so so much!

    Super great information, though for anyone else watching, might need to hold those trigger points longer, especially the anterior and lateral scalenes. Those hold a lot of tension for anyone with forward head translation especially those who sit at a computer for long periods of time. 30sec to 2min holds to release trig points can give enormous relief.

      That is the traditional approach, yes, but there is no magic window of time that "must elapse" in order to get down-regulation of tissue. I go mostly by feel and patient comfort. Trigger points alone are controversial, as much isn't really known of this phenomena in terms of repeatable, measurable outcomes, let alone how much time is needed to treat. appreciate the comment.

    I can't tell you how much I wish I could find someone like you here in the UK. The problems you describe in this vid are pretty much me. I need relief from it. My neck feels permanently jacked up and I'm currently suffering from a right shoulder condition.

    I do these similar techniques in my services as an L.M.T.
    Its not comfortable but these scalenes can be the toughest to work on.

    I had most of my right pec minor removed years ago due to symptoms in my shoulder/chest and down my arm. If the web had been around back in the early 1990s, I may have had more options.

    Hey man, I was wondering (after reading anatomy trains) if you were thinking about setting up a training course. I think what you do is badass and you explain the work logically in a way that everyone from athletic trainers to massage therapists would understand and excel at. I’m a massage therapist and I’ve brought a lot of the information and techniques into my practice and they have taken what I do to a different level. Schools don’t go nearly this in-depth into sports massage/muscle work.

    I appreciate watching your videos for the many modalities in treatment you use, plus the good analytics in kinesiology and anatomy. Im considering complementing my massage therapy training with techniques such as Graston/IASTM, k-taping, etc.
    One bit of constructive criticism for the videos themselves: for us visual learners, maybe try to get better camera angles and shots of the body parts youre working on. For instance, you'll be working on this clients neck, and I cant see whats going on in the shot.
    Keep up the great work! Looking forward to watching a whole lot more!

    When I mentioned TOS to my pcp he looked at me like I had two heads. I had to beg this guy to get a referral to see a specialist. It shouldn’t have to be that complicated. I mean being that it’s my body what would I know about it? That’s how they make you feel. Then frustration sets in. They are just not right most of the time. That’s why they call it PRACTICING MEDICINE!

      commentary of what they teach in medical school. most so called "doctors" don't practice medicine. they just recite it. then they criticize us when we go to dr google. i have had much more success with TOS thru a chriopractor. at least they think outside the box. good luck.

    You're a genius, this is exactly the same symptoms I have, I've seen a sports masseuse and she touched up on the same area and I felt good afterwards, however no form of stretching the neck helps, what can I do to prevent tightness coming back?!

      Joe, You are too kind!!! I am not a genius, but I consider myself a good evaluator, and I assume nothing, using my treatments as a diagnostic tool to help find the most effective techniques! Stretching the scalenes are tough, as the attachment at the rib is not 100% stable...Meaning if you try stretching the scalene, you may just cause more 1st rib elevation....try pinning down the scalenes with your fingers...just push straight down behing your collarbone, then turn your head away and extend back...Pair this, the tissue work, and do some chin tuck exercisesto strengthen your deep neck flexors, and presto! Also, watch your posture!!! if you sit at a desk, or are on your phone a lot, poor cervical posture can exacerbate symptoms. Thanks for the kind words and thanks for the support!!! Good luck!

    Hello Sir i have a question i have read many articles about tos. On the side the say that we have to release the muscles that compress tos on the other side they say that the muscles compress tos because they are to weak and they dont need myofascial release but strenght Training and when we start strenghtening for example the scalenes that the symptoms for the first gets worser but then better im very confused whats your opinion about that

    My upper traps has been a bother for me for years. They're real jumpy and spasmy. I also have somewhat tight pec minor muscles and rather weak low traps. Gotta reverse it all!

    I'm currently going to school for Massage therapy and have background in the OT/PT field. In the program we briefly discussed TOS. How would you approach a patient/client that is hypersensitive? Very curious to learn and know, and I'm also very interested in ART. Thank you for the info!

      Cherrie Laxamana key is consistency. Start as light as you have to. Reduce the time of sessions, or work on less sensitive areas first. Over time, the client will acclimatize to the pressure and you'll be able to access deeper layers. Build trust. Be patient. Be empathetic. Remember the mind is a powerful thing and some of the sensitivity could be anxiety related, so reassure them that your intentions are not to hurt them and you are willing to work at their pace. Give them the control but preach consistency.
      You could even try having them put their hand on top of yours and that they can control the depth. Control definitely helps people feel more at ease. The more they can relax, the less force you'll need and more effective your treatment will be. Thanks for the comment!!!!! Hope his helps!

    Just a question / observation, but being an orthopedic therapist myself for 20+ years, doesn't it take away from the myofascial work to use so much of the cream? Because we were trained to use a teeny tiny bit, or none at all, in order to be able to really grab and pull the fascia, to hook it for the movements and NMT part, and to soften the muscle tissue. And also, is it necessarily sanitary to dip your hands in the same container as that of the one that is being used for other clients? I would be careful with that contamination.

    Do you also help with elevated ribs? My first two are popped out along with my scalenes & scms being tight ropes
    You guys are 2 hours from me but it may be worth the drive if you think you can help. My TOS got so bad and is bilateral affecting almost all fingers but I don’t want surgery 😭

      Hey Kallie, sorry to hear. We could certainly take a look. Rib flare is a common finding in TOS patients. Can be a result of breathing habits, diapahragmatic control, etc. When I have clients from out of town, I generally do a 2 hour eval, treat, and program design session, and then we can collab remotely.     I am a professional dancer and struggling with the same issues, tightness in the neck and in mid-back between the scapulas. Did some PT, exercises, acupuncture, cupping massage but the effect is not very noticeable. Do you know someone who is good enough in Seattle who could perform similar techniques? Would love to schedule with you next time i am in NY!

    Do you use PNF or PIR techniques for lengthening the scalenes and traps?

      Sure you could definitely do some PNF stretching, I like to really work isometrics on the neck, find them easier to do at home for my clients. Just some way to have a person be active in the new ROM we gain after treatment.

      I use Nitrile gloves for a few reasons. They are more sturdy, meaning they dont rip as opten when doing manual work, and I don't have to worry about people with Latex sensitivity. The brand I use is called SkinTX, which I have found to be the best combo of price and quality. Hope this helps and thanks for the comment!

      Yes! It is a process to make the necessary changes to posture, movement, and whatever dysfunction is causing your pain. Consistent, proactive manual treatment by a good therapist, paired with a corrective exercise program, good nutrition and lifestyle and workplace ergonomics can make a huge difference!
