  • For many individuals with Acne, the fight doesn't end once their lesions resolve... it leaves battle scars in the form of flat, pigmented and red lesions and depressed or elevated ones. And so another journey begins, that of achieving a smoother, more even skin.⁠

    Flat scars are what majority of my patients experience, and they can be distressing. Pigmented ones are more easily address than red ones, the so called Post-Inflammatory Erythema (PIE) or Post-Acne Erythema.⁠

    In this post, I talk about what PIE/PAE is, why it happens and what can be done about them.⁠ This video serves to educate and give information about anything related to skin and skincare and shall in no way take the place of a proper consultation with an expert, such as a board-certified dermatologist.

    Hello doctor. This might be a little long but please bare with me. I've been struggling with PIE for 3 years now and have tried almost everything. I'm feeling so hopeless. I have brown skin and I am 22 years old now (male). 95% of my acne is gone now on it's own even though I sometimes get acne here and there. But the redness still persists.
    I've tried the Candela V beam laser. I've done 4 treatments with this setting:
    7mm 10 ms,8.5 j/cm2 30/20
    I did not get any results. I feel really hopeless doctor. I even used the Niacinamide 10% by The Ordinary and it did not suit my skin. Can you help me? Can you tell me if the settings for the PIE is right?

      Hello! PIE is truly very challenging and lasers are for me the best treatment modality yet. But I do not give laser settings or any form of treatments this way. Please come to the clinic for a formal consultation :)

    Hello Doctor. I had a mole removed about 8 months ago with a CO2 laser. The mole is gone, but it left a pinkish/purple mark on my face. I noticed it's a bit less noticeable in the morning and tends to even border the "red" spectrum during a heat flush. Will this ever go away? Also, I can in lighting that the scar it left is a bit shiny. However, the spot is not raised, nor indented. It's flat and you wouldn't be able to tell if you ran your finger across it. However, I'm just wondering when the pinkish/purple mark will go away, since it's about 8 months and it's still not gone?

      I personally do not remove moles using CO2 lasers so I have no experience with how they would look afterwards. It would be best for you to go back to the doctor who did the procedure to you and discuss your options :)

    Hello doc. Hope you could give me an advice. I don't have acne or big pimples, I had facial then the aesthetician extracted "blackheads" after 2 days, I still had this red marks, like post hyperpigmentation 😭 I still have 7 sessions for Facial, DP and Rejuv laser 😢 How many days would it go away?

      PIH resolves, on average, in 12 weeks :) PIE longer if it's due to inflammation. But iba iba ang tao e meron iba mabilis magfade ing flat scars, for others mas matagal.

      You’re welcome and thank you for the support. Many have asked me to talk about those but I believe prescription and systemic medications are best explained by your attending physician during a proper consultation :)

      @@willmae2519 i honestly don’t give spironolactone. I give other anti-acne meds :) but ginagamit talaga cya for acne and in general, we usually modify the dose of a medication when we feel na kulang or sobra. But with spironolactone, best to ask your derm, since mas familiar cya with it :)

      in my opinion laser is best in improving PIE. BPO and tret can help but not so much. Anyway PIE resolves on its own dn naman even without treatment :)

    Doc, can you layer cerave's resurfacing retinol serum and the good molecules discoloration correcting serum? Or cerave's resurfacing retinol serum and jumiso all day brightening serum? Thank you~

      Anong ingredients nung 2 other serums?
      If u ask me ok naman to layer wag lng 2 ingredients that go against each other or can cause too much irritation etc.

      @@irenegaile Doc, the GM discoloration correcting serum po has 2% cetyl tranexamate mesylate and 4% niacinamide while the jumiso one po has vitamin tree extract and 2% niacinamide.

    Doc, is picosure laser good for treating PIE?

      Picosure is a pigment laser used in removing tattoos and other pigmented lesions :) Vascular lasers like the Pulsed dye laser or yellow laser are those used in improving PIE.

    Hi doctor I’m on acne treatment, BP and Adapaline it seem to be they flare up my PIE scars and also newly forming acne will also leave a PIE scar.

      You will benefit from prescriptions medications. I recommend that you have a proper consultation to get assessed and be given the right meds :)

      Retinoids in a way because they are keratolytics, licorice is more of a lightening ingredient.

    What are your vitamin c recommendations doc??

      I like to combine my vitamin c (concentration of 5-20%) with vitamin e and ferulic acid -- these make a powerful and stable combination. Although there are so many vitamin c derivatives that are more stable than vitamin c and can be used alone.

    Doc, what kind of ingredients to use that is best with Epiduo Anti-Acne gel kasi po mostly red marks nalang meron po ako pero nagkakaacne parin that's y im using epiduo po. Epiduo lang po gamit ko for night and suncreen lng po sa umaga. Thankyou po.

      Oh pinanood mo na pala hehe if you're asking about examples of brands, hmmm for niacinamide there's cetaphil bright healthy radiance, peter thomas roth pro strength niacinamide discoloration treatment yan palang nasubukan ko but actually maraming brands ang may niacinamide ngayon.
      as for azelaic acid i only know of skinoren na prescription-strength brand :)

    hi Dra, can a 17year old female with acne and acne scars start a skin care routine including a low concentration of Retinol?

      Retinol is an anti-aging ingredient. At 17, you don’t need it yet. You can however benefit from a basic skincare routine that consists of a gentle cleanser, a sunscreen, anti acne medications and moisturizer (if necessary)

    Hlw doc.i m from bd.And i m also a doc.i have post inflammatory pigmentation. Is laser only one way to treat it???
    I used hydroquinone containing cream,solution.but no result. I dont want to do laser.is there any way to cure it???

      Sun protection is very important. But it is still best to have a proper consultation so that your skin can be assessed. Sometimes, lasers may not be the best solution especially if you have dark skin type.

    Hello po, Doc. Cetaphil Cleanser, Serum, at Sunscreen lang po gamit ko, are these enough po? Do I need to use products contains salicylic acid, or aha-bha po? Ano pong kulang? Salamat po nang marami.

      Best to consult so that your skin can be assessed and proper recommendations be given. If you have no skin issues and are 25 or younger, a gentle cleanser, moisturizer (if needed) and sunscreen would suffice :)

    I'm using sugar dolls sunscreen 2 months doc, but I still have pimples and small bumps, I have also marks they are all visible, should I stop using these sunscreen? And what sunscreen would you recommend for sensitive and acne prone skin doc? THANK YOU in advance doc Happy new year.

      Try lightweight, gel- or water-based sunscreens. If not, spray sunscreens may be a good alternative. for sensitive skin, choose those that do not contain other ingredients other than UV filters.

      Please consult a dermatologist to get properly assessed so that you will be given recommendations that will suit your condition.

      Come to my clinic! We have the yellow laser and soon we will have a new vascular laser for red marks, broken capillaries etc
      Price kasi depends on the lesions.

      Lasers still the best treatment option for PIE
      Azelaic is ok to use but i recommend 15% and it might take at least 6 months to see significant results

    Gumagamit po ako ng glycolic with kojic cream every morning at epiduo forte sa gabi.. almost 7 months na po na routine medyo matagal mawala ang red marks ng acne😔

      Red marks are really stubborn and in my experience the best way to improve them is using lasers :)

      For me minimum of 4 weeks up to 12 weeks to see improvement. Once youve achieved the desired result, pwede mo na stop.

    Good day po Doc! Sorry to ask as im not familiar with the first 3 topical creams you mentioned, mga over-the-counter lang po ba to? And if im already using cream with niacinamide, should i still use any of the first 3 topical creams po ba? Is niacinamide enough na po ba? Thank you in advance, po!

      The first 2 I mentioned -- brimonidine and oxymetazoline -- are prescription medications na hindi na ata available dito satin. All the rest can be bought over-the-counter (niacinamide, vit c, tranexamic acid and azeleic acid) -- all 4 of them are good for PIE and PIH. I recommend trying one first, and then if hindi enough u can try using 2 kinds. For PIE pinaka effective parin ang procedures but I have patients who have responded well naman to topical creams

    Doc, meron din po akong PIE or the redmarks then my derma binigyan po ako ng clindamycin phosphate topical solution sa morning po then acne astrigent and then she also gave me a whitening saop po. Makakatulong puba yung mga gamot na binigay po ng derma ko sa pagtanggal po ng mga PIE kopo doc?

      Clindamycin is an antibacterial so hindi cya for PIE. As for the other meds, ask your derma kung anong ingredients, I cannot answer for her because I dont know the components of the astringent and whitening soap.

      @@irenegaile Almost 9 months napo yung treatment kupo sa kanya doc and i see na nawala po talaga mga acne and comedones kopo may naiwan ngalang pong mga bakas witch is yung mga redmarks po yung pung binigay na whitening saop po doc is from bioderm po yung thioderm po ata yon and then yung pung astringent doc yung pung brand nya is acne cure.

      Hello! based sa studies, 5% tranexamic ung ginamit nila, but it should still have an effect especially combined with 5% niacinamide. I will look into it too and let you know if i find anything on 2% tranexamic acid.

      For PIE a combination of topical creams and lasers give the best results. but if i were to choose between the 2, i would do lasers. :)

      I give that to my patients with acne but not for long periods of time. Its a prescription medication that u can buy from drugstores :)

      12-24 months on average.
      In my experience lasers pinaka effective. Sa topicals naman, azelaic acid and exfoliants. Sometimes niacinamide and tranexamic acid.

    Hello po, Doc Gaile😊 My face is about to be covered with these red marks caused by acne. Is it okay for my age (15 years old) to use a Vit C serum? The Garnier Vit C serum, to be exact.
    Here's the products I'm currently using:

    doc maganda rin po ba ang mandelic and malic acid for acne and sensitive skin?

      They are alpha hydroxy acids that exfoliate the skin.
      I wouldn't recommend to those with sensitive skin until after their skin barrier has been restored. They may help improve acne but are not considered first line of treatment.

    Omg doc super thank you! Now I know kung ano yung case ko. I thought pigmentation lang siya but its PIE! Buong right na pisngi ko pa naman meron.       You can go ahead with your current skincare routine naman if it works for you and add products containing ingredients I mentioned in the video.
      If you want to really make sure of what you have or get specific product recommendations, I suggest a consultation para ma-assess not just your skin issues but your lifestyle, habits and preferences as well :)

      Yes minsan more than 1 year talaga especially if new pimples keep coming out. I recommend a proper consultation, para ma asses ka ng mabuti at mabigyan ng tamang gamot.

    Hi doc i am using BPO (day) and akneclic (adapalene and clindamycin) at night as prescribed by my derma. If im going to use niacinamide saan ko po sya pwede iinsert sa routine ko? I have lots of PIE and everytime my acne ako red marks tlaga sya 😔

      You can use it in the morning after your bpo and/or evening after your adapalene+clinda :) best way to prevent pie is to control ur acne and by minimizing inflammation. Thats good youre treating your acne :)

Or do you have any product recommendation po? Can I use also Belo Kojic + tranexamic toner doc? Dont know lang the exact % of tranexamic first time ko lng kasi itry.

      Nope its a pigment laser, the target chromophore is melanin, not hemoglobin :)

      Alam ko pwede nmn yan pagsamahin. Kasi binabawasan ni b3 ung irritation from tret. Ang tanong lang kasi, kaya mo ba ma handle ung 5%? May iba kasi naiirritate padin sila sa 5%

      Purging is often due to keratolytics (retinoids, benzoyl peroxide) or exfoliants (AHAs, BHAs). Niacinamide is not one of them. But you can definitely use it with tretinoin. It is not directly beneficial in treating comedones but may have sebum regulatory effects.

      Hello! yes ok naman, exfoliants sila so they can speed up PIE resolution in a way :) best if combined with topical or procedural treatments. Dont forget lng to use sunscreen!

    May tanong po ako. Currently red spots tlga ung problem ko. Di ko masabi na PIE sya. Kasi di namn ako tinigyawat kung saan sya nag manifest. Natulog lng ako tapos pag gising ko, meron na. Bandang left temporal and left cheek ko. Considered as PIE din po ba yun?
    may Niaciamide 10%, Vitamin C, Arbutin 2%, Tranexamic Acid and Azelaic acid 20% akong mga skin care. Ok lang bang pagsabayin sabayin ko po yun?

      PIE can result from any form of inflammation and as a consequence of healing of inflammatory lesions. Kung ung red spots mo are temporary then it is not PIE.
      For me overkill ung sobrang daming products with similar mechanism of action which might lead to irritation rather than benefiting the skin.

      For acne yes you should use that. Dont be afraid of purging. Its all part of the process :) plus it doesnt happen to everyone.

    Confused tuloy ako now if bakit cream na may hydroquinone yung prinescribe sa akin. Almost 3 weeks ko na din po ginagamit, walang effect. Online lang din ako naassess.

      Hydroquinone is a skin lightening cream which we prescribe for post inflammatory hyperpigmentation. Baka mix ka ng PIH and PIE :)

    Hello, Iv had this red blemish on my forehead now for probably over 3 months now, i remeneber having 2 pimples close to the area and im not one who deals with ance, other then a few pimples here and there, im not sure why i have this blemish..it doesnt seem to be going away, im thinking it might be PIE but i really dont know and trying to see a doctor is almost impossible where i live.

      Yes ok lng to use BHA. I suggest you try laser treatment :)
