    Therapist have trouble understanding what BPD is and its manifestation.
    Most are not trained in working with PD, which is not the same as treating those without PD. There is often confusion over comorbidity (Complex BPD), therapists are worried or concerned due to increased rates of self-harm and suicide, and the challenge to manage resistance and willingness/willfulness.
    To overcome this, you must get into alignment with your therapist. Many individuals with BPD feel their therapist just doesn't care, but I haven’t met one therapist who asked me about BPD that didn’t genuinely care about the welfare and success of their client with BPD.
    We're going to talk about strategies -core/surface content, self-talk, adaptive strategies in this video as well. Regretfully, there is no magic wand. Therapists can only do so much, and it’s going to be up to you to take these strategies and incorporate them. Don’t fight yourself, or engage in self-attack. Resisting that old urge is tough, and it doesn’t go easy. It will go away by staying the course. You need encouragement, understanding and patience from your therapist but you must also have that with yourself as well. It's treatable and you can manage your issues with BPD.

    I think the point he said it the end is so important. We need to treat ourselves the way we would want to be treated by a therapist. Listen to yourself, be curious about why you do things, be compassionate to yourself, give yourself encouragement. I think there's a feeling like, once someone gives me those things it will feel like I deserve them and I can finally give them to myself. But I think the opposite is true, you deserve them now and you need to give them to yourself first before you will allow them to be taken in by others. We don't trust our therapists and are willful because we imagine they think about us the same way we think about ourselves.

    The first time I heard the term bpd was in my behavioral health clinical in medic school. Back then, students didn't really interact one-on-one with pts and were just "flies on the wall" during group therapy. I really related to one particular girl who was completely withdrawn and lashed out in record time when it was pointed out that she wasn't participating in the therapy. Afterwards, I started asking my preceptor about her dx/behavior and she cut me off and said "she's just an angry drama queen with bpd". I still think about that almost every single day (13 years later). I just graduated with my BSN and that stigma was still there during my behavioral clinicals for it (from the RNs and techs, not from psychiatrists/psychologists).
    Its truly disheartening to know there's people working in the field with that level of ignorance.

      I agree , mostly because, I guess, the first attempts to get help come from really bad situations , in my case it was met with a very unprofessional therapist. Fortunately We live in the internet era, and also my second attempt was met with a great mental health provider. It makes me happy to read your thoughts on this, wish you the best, and congrats.

    I wish you could do a video on how family members and friends who try to deal with a person with BPD. It’s overwhelming and exhausting when you care about them and their attacking you.

    Dr. Fox is so adorable in his mannerisms. While I highly appreciate the research and work that he puts into his productions, he also just has a very sweet and likeable personality. Very genuine. What a precious soul 🌸🦊💜

    I haven’t been able to find a mental health provider who can treat my daughter. It may be true that they want to help but money is definitely a factor in the equation. The therapists that accept insurance don’t know how to treat BPD and the therapists who know how to treat BPD are cost prohibitive.

      I have cried before too when I finally spoke with a therapist who understood what was going on! I spent so much time in therapy struggling to convey what happens in my heart and mind. But when somebody gets it and you feel seen, it's easier to discuss solutions and a process that will have results

      He’s awesome. At my new job I can watch videos and what not through out the night and my bpd has gotten sooooooo much better by practicing what he explains. I’m very analytical and he helps break things down so well and it’s easy to understand. He’s the BEST!

    After going through some folks, I think I've finally found the therapist that is right for me. She actually listens and doesn't try to be too by the book. It's really helped me to go to therapy more.

    I will watch this video after it premieres. I have had the reaction from a few therapists that because I say I have bpd that it means I like having it or use it as a label..or I want to have it all my life. I mention diagnosis because I think it is clinically relevant..eg at recent eating disorder treatment..and also because the diagnosis gives a quick explanation of some of the difficulties I face. For me bpd has been enduring (decades)... all my adult life. And not voicing bpd diagnosis doesn't make bpd go away. However the reaction that I 'shouldn't' mention diagnosis feels like it nullifies my whole lived experience and the significant impact of living with bpd. On the positive side..I have had much beneficial treatment.

      Whenever I tell a mental health professional I have it they act like I don’t and I had a therapist say I wasn’t “crazy enough” to. Which is obviously just messed up. Just because I can act pretty rationally and calm doesn’t mean I don’t flare up and explode when I’m angry or rejected. That’s what they don’t get I guess.

      You just expressed my feelings exactly. I started therapy again after not having a therapist since 2016 and I'm hesitant to mention my previous BPD diagnosis from ten years ago exactly because I don't want that same reaction.

    I have never been able to have the right insurance or enough money for therapy. I was diagnosed in 1987 or so and have searched for years for help and never got it. I am now 77. I am reading the book right now and realize I have come a long way by my own fighting to get well. I still cannot find a therapist that does this kind of work because I am on SSI and medicare/Medicaid insurance and there is no one who will take my insurance that can help BPD. I have called over 25 therapists just in my local area and there is no help for the less insured. I have cried my eyes out begging and getting nowhere.
    I am so grateful for the book you wrote, Dr. Fox because it gives me some support and ideas on how to help myself. I have accepted I will never get any real professional help but I am willing to do what I can on my own!! I never wanted to be this way and I am crushed that I have lost so much in my life because of the BPD, so many members of my family will no longer have anything to do with me, and friends, well, I keep my distance so I don't ruin them.

    So my therapist and I have been working hard against my BPD for 2 years now and we have made amazing progress, to the point where I completely agree that it is treatable and where I can see myself living a symptom free life. I've always known that I was lucky finding my therapist, she's never for one second not been 110% convinced that we can kick BPD butt and I've always appreciated her but after watching this video it's become very clear just how lucky I am with her and it's an amazing feeling. Thank you so much for your video 😊

    I hope you read this, Mr. Fox. I've yet to find any therapist that will even take me on. The last one I saw threatened to call the cops on me if I showed back up at her office because I was really uncomfortable with schedule changes. (I am 90% positive I have BPD, I have Complex PTSD, and I might be on the autistic spectrum too). I really want to see a therapist and s tart treatment and get better but nobody ever gives me the time of day and sometimes I just give up and never try. Every time i reach out, I never find anyone. I wish I had you as my therapist. You'd probably help me change my life a lot - instead I'm stuck trying to fix myself on my own.

    Thank you dr for another informative video. I have BPD and I worry about my therapist lol when I see her. She’s stuck by me for over 20 yrs! I honestly don’t know how she didn’t give up on me. She’s amazing . BPD is forever present in my personality..sometimes intense and sometimes not but always present…. I admire you that you have committed yourself to helping people with BPD.     I wish I could find a therapist who saw things the way you do. Years and tens of thousands of dollars for therapy and I don’t feel equipped to deal with internal battle I struggle with. I feel more vulnerable internally now than I ever have.

      Hi Kendi, I can relate. I had a therapist for over 17 years who I believe with the knowledge I have now, only encouraged and strengthened some of my maladaptive patterns. It is really hard work, and I am lucky now to have found an incredible therapist for my BPD experience, but you can do this. Do you have the ability to access Dr. Fox's workbook? I have found it to be super helpful (the Co-morbid one), and it helps to bring to sessions and use it to facilitate my sessions and to focus my thoughts. I know how difficult the internal struggle is, if only people could see me in my dark moments, I would scare everyone, but just baby steps, you got this. Message me if you need to.

    Dr. Fox, one of your best videos! I think so many of us who struggle with BPD wonder if we matter and how we impact our therapist's world. I still wonder every single day if I am thought of negatively, or if I am worthy of their time and energy. It's tough with such a poor self-image. I do want to point out that from my experience, the willfulness component I have seen in both myself (obviously), but quite intensely from the other side of the couch. I am grateful to get to work with the greatest therapist I have ever found currently however, I previously worked with a therapist for 17 years that almost encouraged some maladaptive patterns for me often with his willfulness. I believe pretty strongly that the willfulness can be pretty strong from the other side of the couch if they are not prepared or trained in understanding PD's. So, with that, thank you so much for putting this content out there, it is so incredibly valuable.

    Thanks for all your work Dr. Fox! You deserve so many more views. I had a CBT therapist and a DBT therapist... and they helped a little. But only with surface issues. I'm glad to have found a Jungian therapist who is more focused on core content.

    Wow, when you talked about downward spirals, that was my experience dealing with a homeless shelter GP that refused to give me a referral for a mental health assessment. He clearly had no idea what he was dealing with. Every time I tried to get help for 4 years, I was getting second guessed and would spiral out. I guess since I am not getting any help from the system here, your book is my only option.

    I have BPD and have put my therapists through some shameful behavior from my part. I thank all of them for not giving up on me.

    This video is gold doctor. Loved the concept of the Wise mind...funny what clarity lil diagrans can bring. ...and finally a medical professional acknowledging that meds do not help you achieve that state, it only brings surface lvl relief, ...NOT undermining the importance of it.
    I wish n hope all ppl who are in the spectrum learn to balance medication with medtitation.
    Pls do a vid on how to deal, if at all, with ppl with BPD....the other side of the table perspective.

    I feel like Doctors think I am coping because I work and I am articulate. Even appear self aware. I am in my thirties and still self harm during intense moments. I also have downward spirals, dissociation and act out e.t.c On Sertraline highest dose and Mirtazapine. Also, Zopiclone as needed. Diagnosed with Mixed Anixiety and Depressive Disorder. They label social factors as the issue, yes I've a lot going on but I know it isn't just this. Thanks to Dr Fox's content and workbook, I've started to make progress. I've told two close friends and they are being supportive. I feel much better now I have more awareness and I am consciously trying to be a bit kinder to myself. Finally, someone who nails it to a tee. Thank you.

    I recently fired a therapist who would not look at her counter transference. She only checked things off (in my mind) that whenever I did or said confirmed to her that I had bpd. She heard me say things that fit her narrative and failed to see that she was hearing what she thought I said, not what I did say. I felt like I wasn’t seen or heard and that mirrored my upbringing so much. She chose to use the model by John Gunderson where severe boundaries were set with ‘no more then 50 minutes /wk were needed.” She pounded bpd education into me without me understanding her words to the point where I felt like a complete failure and suicidal and she just told me that I should call my pcp and she’d talk to me in a week. So frustrating- I added another therapist to help me figure out what was going on as I suspected the countertransference and I found someone who was able to be an ally with me. Things are much better since I changed therapists -however my new one is leaving practice in April. I’m working very hard to make as much progress as possible until that time comes. Thanks for providing some solid info for balance.

    I was told recently it's like I disburse a lot of energy on the couch and my therapist has shared he needs weeks off to make sure he can still help himself and other clients after seeing me.

    I had a great therapist who really did not judge and was into dbt but I had to move to a different county so I had to get another from a different agency. I refused to go to a last appointment and “terminate”. She was a favorite person and I hold on to hope that I can get back to her someday.

    I understand Dr. Fox to a degree, he often speaks very fast and I often get lost in the flow. I think his new book is valuable because many like myself deal with the complex BPD. So many other things are mixed in with the main dx. Hey Dr. Fox can you slow down a bit? Lol maybe not seems like you really enjoy talking about this. Oh and by the way can you do a talk on Validation? I seriously struggle with receiving validation and I’m not sure why that is; I know the concept and purpose of it but I struggle accepting it for myself and sometimes receiving from others.

      Growing up and until just recently (and im 50 now) i've always found it really hard to make decisions. Or i make decisions at the moment that usually aren't the best ones.
      I think for me ive always wanted to be validated, and to know that i made a good life choice..

    After watching your videos and many others...I believe my husband has BPD. It has been a very difficult 25 years. He is emotionally and mentally aggressive. We have twin boys that are 18. My husband was told in childhood he has ADD. I can only take so much of the abuse. I am no longer resilient and struggle with leaving. However, I need to leave so he can get better or at least not have me on a daily basis to regulate his emotions and the expense of my health. If I stay he will drown both of us and the children will suffer as they begin their adult lives. Thank you so much for putting your knowledge out to the general public. I am so torn that I have to leave him especially knowing that he is not well. Life is a struggle and will always be a struggle. That's how we learn. I always thought it was me he was very good at keeping me guessing and thinking it was all me. I was too anxious to see what was really going on. The more I tried to fix the things he said I was doing the worse it got. The push and pull, manipulation, twisting words, and lack of insight caused so much anxiety in me that I didn't know what was up or down. I have been in therapy for 6 months now and my therapist has helped so much.

      ADD can manifest in a similar way to BPD. It's not "he's abusive so he must have BPD". He can have ADD and be abusive because of that.

    Dr. Fox, I again thank you for your time and your videos. This one has helped so much with perspective. But, how do I go about finding a good therapist here in Phoenix Arizona? I'm going to share some of the staff at my mental health clinic. Particularly my clinical director. He is the only person at my clinic who has been there since me. Well not since me but he's the only one out of for example case managers, doctor, benefit specialist, him and I have been at odds I have felt not hurt, lied to by him and last year I was hospitalized and went back to the clinic I told him how I felt and he really listened. He has been very attentive, he has had so much empathy for me. Very willing to help me. Even a willingness to watch some of your videos with me. I am struggling with getting a therapist that knows BPD and I know now how desperately I need that. Do you have any suggestions?? Please thank you again

    Would you be able to do a video on BPD and how it relates to trauma associated psychotic features? I know people with BPD can experience features much like such when under a great deal of stress but for the longest time I’ve been told by professionals that they’re not real/true psychotic traits and that people with BPD cannot experience such a thing. Hence why I’ve now been told the psychotic symptoms I experience are trauma related. But BPD is trauma related. I’m not sure exactly what I’m asking. Merely if you could shed some light on the relationship between the three presentations I suppose. Thank you! I love your videos!! I always send them to people when they ask about my BPD or trauma 💚

    My new therapist said about my BPD diagnosis that she is against labeling but she would like to hear more about my childhood that I have alluded to as being traumatic. I do like her and think she is smart but I really want to get to my Core issues and not just all the surface stuff. It is hard to find BPD specialist.

    The more I learn about my BPD, the more I feel like a robot running a program. I feel like free will doesn't exist more and more every day.
    Don't let BPD become you're identity. Don't manifest your future based on what you learn about yourself. We are pushed in these directions of thought and reaction, but we are not unable to overcome our programming and do better.

    Lose the gate on your audio. There is a reason why sound techs use room tone. The problem is the condenser mic in a ringy room, so try a dynamic lavalier or a dynamic handheld mic.

    This sounds almost like determine a cancer in a xray photo: there are now computer programs that are able to determine this cancer patterns. May be this can also be done in this field to determine the core problem(s)

    Hey doc.. can you make a video on BPD and the perspective of time. I feel that I have a distorted perception of time where I sometimes believe that there is plenty of time so that I can procrastinate now. Also i have this problem of forgetting people too soon if I don't meet them often. It's like i couldn't remember that i love them. It's literally out of sight out of mind for me. Can you cover this topic too? Also on perfectionism please. I am a chronic perfectionist. And it's killing me.

    Say Doc. What would be a fairly simple test for a person to suspect they have BPD? I know only a trained professional can diagnose. But why would a person be able to think that they might have it? or at least symptoms. And like everything is this also measured on a spectrum?

    I have seen behavioral therapists, took a year of Dialectic Behavioral Therapy… the DBT was rough… Sold as a miracle cure and way out. Then covid hit. Went all on screen and the doctors lost complete touch. Even lost a fellow patient.
    Still healing. But my trust for the ‘system’ is very damaged.

      I can certainly understand that your trust would be questionable after an experience like that. I think that you may want to try again in regards to DVT or other treatment methods. Learning strategies is critical and it is very important. This is the way!

    What do you think ab them changing the name of bpd? The comorbid stuff is super confusing for me too. Is it CPTSD, or pure BPD just presenting itself thru comorbid things like anxiety, ocd, adhd, etc or do I have BPD and some adhd, depression anxiety, etc? Trying to figure out w my therapist and dr if maybe I’m bipolar also. Borderpolar. I guess I wasn’t aware bipolar is complex as well. So is it BPD and or just how BPD presents itself?

    Appreciate the videos and the obvious passion for the approaches, however I have a few questions on the alignment bit and the “therapist doesn’t care” thing. I think they are both related as you say, when “aligning with the BPD” maybe you can’t see the therapist as caring. However in my experience with a few different therapists now and different modalities, they all demonstrate a very distant style. I can be having a melt down or getting very emotional and they will just sit there watching. It seems verboten for a therapist to actually demonstrate care or even suggest they do, so when wise mind pops his head up, there is some space to breathe from the earlier alignment, but even then it doesn’t get any encouragement or reward. How are people supposed to align with a therapist who they may only know a short time (due to financial constraints of people with this diagnosis), and who cannot or are not “ethically” able to demonstrate caring? Therapists are paid so for me it is hard to believe that brief visits once a week lead to anything other than challenges.

    Therapists, by definition, ARE (should be) trained to deal with emotionally disregulated individuals. If your therapist doesn't understand you, RUN FAST OUT OF THERE before they damage you, which they can and will do.

    I’m from New York I’m 30 with BPD who just lost my father to Covid suddenly who was a larger than life person to me and then my fiancé left me because I got “ too depressed to be with” ad now I’m here and alone and never been into drugs and now I’m dabbling with it. My psych has no idea what she’s doing . And my psychiatrist put me on Pristiq and it does NOTHING. - where can I see you in a seminar ? I’ll buy plane tickets I don’t care I need help HELP ME please 😔🙏🏼

    At 6 I was diagnosed ADD ADHD put on Ridellan and a sleeping pill. At age 10 or 11 I attempted subside the first time I had go through abuse and trauma . At age 12 I suffered an open TBI and my left frontal lobe is dead, things got worse in and out of psychiatric ward multiple suicide attempts, very hard relationships and in my late 20’s I was diagnosed BPD, depression, and I suffered drug addiction and in my 30’ arrested the first time and then 3 more times and sexually was very very bad I even escorted a year ago. It seems to be getting worse and I wondered if my brain damage makes it worse?

      Interesting you ask that . . . I was just re-reading The Willpower Instinct by Kelly McGonigal, and she talks about how the left, right and central part of the frontal lobe are where the "I will", "I won't" and "I want" willpower parts of the brain are located. That said, I have also seen some amazing recovery in a stroke survivor friend who had to learn to walk again. Our ability to heal and the brain's neuroplasticity is an amazing thing.

    I literally can't find any help my last psychtrist never just listened to me and wanted to treat me like a lab rat and give me medication that I didn't agree with when told him Aderall is ONLY thing that has helped me with my ADD and depression but they will not give it to me. Ugh...Ithey wrote some bullshit about me In my chart and now I can't get Xanax or Adderall. Literally the only 2 medication I've ever found help me.

    Finding a good therapist is hard. I don't care if you are dealing with PTSD, alcoholism, personality disorders, or any specific disorder. There are a small percentage who are actually good at what they do. Many therapists and people in the psych field have narcissistic traits and take advantage of people who are dealing with mental illness. If you aren't careful, you will land in an office of someone who is abusing their position.

    How does my therapist know when I’m ready to lessen the frequency of our sessions? He recently told me (about 2 years ago, after I got out of a very abusive relationship), that I was ready to choose when to have a session, that it wasn’t mandatory anymore after 17 years of appointments once a week, sometimes twice. I’m just wondering how a therapist knows when a patient is ready for that step.

    This may be out of bounds, but do you know any male mental health professionals in the sf area, preferably not psychiatrists, who are accepting new clients that you recommend for bpd diagnosis and treatment? Between the stigma and the ignorance in the field I just don’t trust a rando off the street, but I trust your judgment.

    I'm in one on one therapy and DBT..... So far ..... I'm not getting too far.... I don't see the point in 50% being mindfulness and repeating all of it.....
    Seems to me if you don't sort the original trauma...you'll always have the issues. DBT just teaches you how to keep calm and react calmly...
    I don't see how DBT helps other than temporarily...trauma is still there......Alot of DBT is common sense. I find unless your therapists are trained in BPD..they are just patronising. I seem to get good feedback from the group because my points make sense.
    I find DBT very invalidating...seems to be a feeling shared...the classes are just Marsha workbook... All the paperwork is from her book.. They read through the guidelines..tell u to do it and that's it....
    It's not what I expected.....
    Anyone feel the same way? Or advice..
    Just seems pointless.
