With SmoughTown and Tarnished Archeologist now covering Bloodborne lore theres alot of content out there that goes much deeper. If you want to go as deep as it gets you should check them out.
The Paleblood Hunt narrated by Jay Briton. It’s got some girth (5 and a half hours of content) but it breaks down and adds in much more information (DLC, Chalice Dungeons, and the game proper, how they all link and the significance of Kos) and gives some interesting insight in to what Pale Blood truly is. If you still have questions that you want answers too The Paleblood Hunt is the definitive collection of Bloodborne Lore.
And there's still more that isn't covered in these videos. So much lore in these games. I would HIGHLY recommend Elden Ring. It is absolutely glorious with similarly awesome lore. And yes, Sluglife!

I believe pale blood is the blood of the moon presence as vati calls it the pale moon presence here too 1:07:30 and when you defeat it and spill it’s blood you finally transcend fulfilling the beginning notes request of seeking out pale blood. Also have to keep in mind the Japanese to English translations with all the lore.
I'd say the doll is both a doll and a person. It's complicated, but TV Tropes implies that the Doll is only able to move if it "feels loved" by someone with the insight to look at the doll and see something more. I'd say the site's article on Bloodborne is worth a look on your own time. My favorite is the "alternative character interpretation" entry, which asks: did Gascoigne kill his wife, or did he just get done butchering the S.O.B. that did? Either which way, his wife's death seems to ultimately be what broke him for good.

I thought a Paleblood was any cosmic entity in the game. With their blood literally being silvery-pale. The only npc's to bleed that color.

The game says what Paleblood is. It's not that much of a mystery. It's a the Moon and Sky after the Moon descends.
