Can't see the detail of how the actual flow adaptor gets removed using those long grippers!

Why do these need replacing would you be getting poor flow ?

Because they leak mate and drip onto the diverter valve motor. Sometimes the plastic goes brittle and cracks and goes into the plate heat exchanger causing poor hot water
