Patients can complain of neck pain from a variety of reasons and in this video I am going to try to outline the most common causes of neck pain and how to
treat it.
The first thing you want to identify is if the patient has neck pain or shoulder pain. Shoulder pain and neck pain problems overlap. You need to know if the origin
of the pain is the cervical spine or if the shoulder itself is the origin of pain.
Does the patient symptoms improved by shoulder abduction and placing the hand over the head, then the pain has the cervical spine origin.
Shoulder abduction test
Lifting the arm above the head usually relieves the symptoms because it decreases tension on the affected nerve.
Is the pain more when you move the shoulder?
Then the shoulder itself is the source of the pain.
An important cause of neck pain is disc herniation. The patient will complain of numbness, paresthesia and radiculopathy, which is shooting pain that radiates
from the neck to the arm. The patient will have unilateral arm pain, numbness and tingling in a specific dermatome in the hand. As you see here in this diagram.
There will be weakness in a specific muscle group. And cervical radiculopathy, the pain is sudden and goes to the arm. Radiculopathy occurs due to compression
of the nerve root either by a disc herniation or by arthritis that narrows the foreman. Cervical disc herniation occurs most frequently at the level of C6-C7 and it will affect the C7 nerve root. It can also occur at C5-C6 and in this case, it will affect
the C6 nerve root.
How to you test for cervical spine disc herniation?
Neurological exam of the patient will include:
Motor testing
Sensory testing
Reflex testing
This is how you test the motor strength of the nerve root from C5-T1. This is how you test the sensory as you see her in this diagram. This is how you test the
reflex as you can see in this diagram.
Spurling test for cervical spine radiculopathy
What is the Spurling test?
The Spurling test is considered to be positive with neck extension and rotation towards the painful side reproduces symptoms in the ipsilateral arm. The
natural history of cervical radiculopathy is favorable with resolution of the symptoms in most cases. And cervical radiculopathy, the patient will have 70 to 80% successful outcome after 2 to 3 months with conservative treatment.
How do you really treat cervical disc herniation?
We are going to start nonsurgical treatment first. Use anti-inflammatory medication, isometric exercises, physical therapy, and muscle relaxers. Get an MRI if
symptoms do not improve after 6 weeks of conservative treatment.
The MRI result should be correlated with the clinical symptoms. False positive rate of the MRI is high.
. In asymptomatic patients older than 40 years old, you will find that 57% of these patients will have at least 1 degenerative disc on the MRI. The MRI may show you a problem with the disc in patients
you are not complaining of any neck pain. Therefore, use a result of the MRI wisely and do not scare the patient, because what you find on the MRI could be a normal process.
Persistent disabling pain for 6 to 12 weeks despite nonoperative treatment can be an indication for surgery. The surgery is usually anterior and it involves decompression
and fusion of the involved disc space. Asked the patient if the patient has gait disturbance (unstable gait when walking, wide-based gait) or hand clumsiness, may indicate cervical myelopathy due to compression of the spinal cord (serious condition). The
patient may have decreased manual dexterity and difficulty manipulating fine objects such as buttoning and on buttoning shirts. The patient may have occipital headache and a sense of discomfort in the neck. The patient may not have severe neck pain. In
general, and cervical myelopathy the onset of pain is insidious, gradual, poorly characterized, and localized. Cervical myelopathy is a slow progressive process with stepwise progression and deterioration over time. In cervical myelopathy, the MRI will show
compression of the spinal cord. Cervical myelopathy you will find upper motor neuron signs including a positive Hoffmann sign. This is how you will see a positive Hoffmann sign.
What are other upper motor neuron signs?
You will find the patient will have hyperreflexia, positive Babinski test and clonus test. Early recognition and early surgery is important for a good outcome.
The severity of the symptoms and early treatment is most important to the outcome. Surgery is done for any functional impairment of the gait or the hands. Usually the treatment in this situation is surgery back decompression and fusion.

I’m scared as hell. I got into a car accident over ten years and I recently got into an accident and slipped n fell and ended up clipping the top of my head, which cause my head n neck to snap back and now I feel fucked up my right hand is numb and tingling and I sweat my head feels like it’s gonna fall off my shoulders and I’m now gonna go to the hospital tomorrow thanks to ur videos. Thank you, I've had problems with my neck and shoulder since Jan 2020 that keeps having problems daily, even with stretching the pains come back. I've been told it's cervical spondylosis and radiculopathy but I think maybe there is a secondary issue of a lateral winged scapula. Watching your videos helped me to understand how to explain my symptoms better to the Dr and I think I'm finally going to find out if there is anything secondary to look into!

@@Soundkings429 not yet I'm afraid, still ongoing, now with muscle knots which I keep releasing but they keep reforming even with continuous stretches and trying swimming - on the whole I have more movement and can do more things and muscles strengthen, but the pain is still ongoing and constant and fascia/muscles bulk together into huge clumps that are painful to release - I'm not sure what this is but thinking it could be myofacial pain syndrome

i am facing exactly same thing....i got the microcurrent massager so i wil try to do that frequently plus i think i figured out one exercise that really works- sort of leaning your head backwards, adjusting your neck to /match/ the line of the head and move the head back upwards....hold for about 10secs and repeat throughout the day; i have daily pains since 2019-2020 as well, damn!
they also say to trat with anti-inflammatory medication so instead i will incorporate turmeric and ginger interchangeably in form of a tea.....good luck!

When I was a kid I would wake up looking straight up at the ceiling. My head wouldn't move. My parents would work it around until I could move. It went away for a while but now I get swelling between virtabrae in my neck. I get headaches, numb finger tips, balance loss, vision problems. Drs haven't been able to figure anything out. They put me on celebrex and an snri but it doesn't really help. They won't send me for an MRI. Can anyone tell me what might be causing this?

Wearing a neck brace now as I'm watching this cuz they done everything and nothing has worked including a spine stimulator i was tested for damage on my hands but they only found a lil bit of carpal tunnel they told me pain in hands and fingers comes from the neck but it had gotten worse Drs have no idea wat else to do or check cuz neck pain is not going away any suggestions you recommend for them to check can't get an MRI cuz i have 2 spine stimulators their MRI comparable but wen i went to have one done they dint want to do it meanwhile i haven't had a good night sleep for months I'm very frustr6 at this point and feel I'm going insane from lack of sleep

I could understand ur pain... Im 28 yrs old..fr past 3 yrs im suffering from cervical and lumba pain. C2 to c6 and L4 to S1 i have radiating pain in both legs and hands... I tried all the treatments ( that are non-surgical) nothing brought me back to my normal life. At last I found some solutions.. Yoga and Satvic Deit ( Healthy diet) I almost recovered 80% .. Walking is must for at least 30mins... If u get pain stop walking and try some stretching yoga... If u feel better from yoga go ahead for walking.

I've been diagnosed of endometrial cancer in 2016 and of left breast cancer in 2020. Had submitted for chemotherapy twice. Doc, is there a treatment for a neck pain caused by cancer? Physical therapy worsens the pain and anti-inflammatory medication only works for a few hours, and the pain comes back. Even after you take it for a week, next time you do some physical activity and your neck pain is back. I don’t understand why you have to have radiculopathy to get surgery, whereas neck surgery also relieves neck pain in over 70% of cases. Why doctors sentence us (neck pain only patients) to suffering, and people with radiculopathy get a lucky ticket and can enjoy full life after surgery???
If your pain comes back after a few days of physiotherapy, you might be focusing on the wrong areas. For example, if you have a pain in your neck and you did exercises for your neck, but the symptoms came back again. Now, your focus should be on physiotherapy of your shoulder area. Even it is not helpful, you have to focus on the biceps and triceps muscles and the nerves there. There is a still chance that it may not be the cause. Finally, you may have to focus on your elbows.
If you are clever enough, it's better to correct from elbow to neck as most of the neck problems are not directly related to neck issues.
I studied and experimented for my neck and shoulder pain myself without any medical help. However, during a couple of years, I got rid of most of the pain issues. Now I am strengthening my muscles to make sure I won't go back to getting the pain again.
