Actually the controller was partly responsible. Had to read up on it. Main issue of the crash was the ground indicator did not work properly.
@@tonymerritt7141 you are right, the ground proximity warning system in the plane didn't work, pilots had no way of knowing they were flying into a terrain in that poor visibility. Also the airport used a super outdated radar system that refreshes every 12 seconds, the ATC had no idea the pilots were correcting their initial left turn and was turning right. The airport also didn't have adequate vectoring charts. Go check out The Flight Channel's recreation of this accident, the pilot who created the channel does a much much much better job than this crappy video that misled people into thinking the pilots cannot distinguish right from left. Honestly the people who are managing this channel, if you are reading my comments, you seriously need to do better research, and please present more accurate information.

On Friday evening, three planes were trying to land at the same time due to delays caused by the forest fire haze: Bouraq 683, Garuda 152 and Merpati 152.
About four minutes before the crash, there was some confusion because two planes had the same flight number. Then, after Garuda 152 was in radar range, air traffic control pulled it off what the pilots say is the normal landing approach, and told it to turn left at 2,000 feet, about 14 miles away.
The directions brought the plane to a mountainous area requiring an altitude of at least 7,500 feet, the pilots say. Normally, the plane would descend to 2,000 feet at 6.6 miles.
According to the transcript, the pilot asked for confirmation of the instructions and was told to go on.
The last miscommunication happened one minute before the crash, when pilots say air traffic control told Garuda 152 to turn right when he should have turned left. One minute after the plane began turning, the pilot's screams were heard.

Avoid tiny airlines with cowboy pilots, this is an area with a lot of water and I assume night landings that are probably as blind a landing as there is. He is obviously instrument rated and has made night landings, he didn't follow the procedures he should have and landed in a forest instead of a runway.

Findings by the NTSB :
"There was confusion regarding turning direction of left turn instead of right turn at critical position during radar vectoring that reduced the flight crew’s vertical awareness while they were concentrating on the aircraft’s lateral changes. These caused the aircraft to continue descending below the assigned altitude of 2000 ft and hit treetops at 1550 ft above mean sea level."
The GPWS (Ground proximity warning system) also malfunctioned on the plane which also played a role in the disaster.

Just watched the whole documentary. In shorts, there are several errors led to this incident.
1. Miscommunication by ATC leave out the critical information of expected runway approach than usual.
l
2. Captain approach habit + distractions on the temperature in the cockpit led to the wrong turn by captain and failed to be noticed by FO in reasonable time.
3. Outdated ATC radar that updated in 12 seconds interval.
4. GPWS failure / unable to warn the pilots which by that time should have been triggered about four times.
5. Unable to hold altitude given by ATC, the plane descended below 2000ft which still unclear was due to instrument failure since the AP instrument recorder has been completely destroyed in the incident. (my opinion was that due to high level of familiarity and experience of the captain, the planes was flown manually by the captain all the confusions and distractions led to him unrealizing that he's descending below what was given by the ATC)

Till this day, the remains of the planes have not really been cleared because of the mountain terrains. I remembered my dad said a person gathered the local people to go to the crash site, and what they found was chilling. Most of the planes part are still there like the seats, bags and they even found some skulls there
For everyone that asked in the comments "where is the GPWS alarm": the GPWS did not sound at all. The investigators determined that, sometimes, the GPWS system couldn't provide a correct alarm in time, probably due to steep/irregular terrain. So the GPWS system had some kind of error.
The GPWS was malfunctioning on that flight.
Did They not have GPWS and Alarms On? Where is “Terrain, Terrain” and “Terrain, Ahead.”
The GPWS wasn’t working at the time of the accident (Garuda had pretty shoddy maintenance policy) and TCAS only detects other aircraft, not terrain.

The plane that crashed was an Airbus A300. It uses a yoke instead of a modern Airbus plane like the A320 or A330, which uses the side stick. The only Airbus commercial jets using a yoke are the A300, A310 and Belugas.

It was the worst open fire that year. I remember I went back a week before this crash to my hometown in Sarawak, Borneo. The airport was closed for 10 days because it was blanked by thick haze. Many camped out at the Subang airport desperately wanted to go home. I am lucky to managed to board a plane last minute on the 7th day when there weather cleared suddenly. There were only 2 flights that managed to land on Kuching international airport that day before it was closed for another 3 days.
The condition was so dangerous that when the plane landed, we all cheered and clapped. What an ordeal. So I understand the difficulties faced by the pilots as the haze was so thick and they cannot see the nearby mountain. It just that why they flew in the first place? It was 1000 API.
For your information, the open burning this magnitude never happened before. Indonesia wanted to open up thousand acre of land for the palm trees plantation. The thick haze lasted for a month but the 10 days where the planes cannot land was the most intense condition. I never forget that day and may the victims rest in peace.

great videos by the Smithsonian. I have learned a lot about the mechanics of the airplane. Glad to see these videos investigate pilot error and the shortcuts the maintenance crew take in order to get a plane off the ground. The fact is the Airlines could care less if people are killed on a flight. It is just one more unfortunate situation that the public risks
I lived in Indonesia when those fires were burning. We often flew from home (Irian Jaya) to Jakarta and during that period there was some haze the whole way.

Earlier in the day, another Flight 152, Merpati Nusantara Airlines Flight 152, was handled by the same air traffic controller. This led to the controller mistakenly saying "Merpati one five two turn left heading 240 to intercept runway zero five from the right side"; as the wrong call sign was used, the Garuda pilots disregarded these instructions. The controller, on not receiving a response, queried the pilots on if they heard the instructions; this time the correct call sign "Indonesia 152" was used. The controller then repeated his instructions, though he did not say that the flight would be making its approach on the south side of the runway, or right side. The pilots believed they were flying the approach on the north side of the airport, as was the approach on the approach chart the pilots were using. Thus, when the pilots were instructed to turn right to a heading of 046 maintaining 2,000 ft to capture the localizer for the ILS to Runway 05, out of habit - or possibly due to the approach chart - the captain initiated a left turn to a heading of 046.

On the other, there are 6 million car accidents every year. Of those accidents, 32,000 dies, 3 million injured... of those 3 million injured, 2 million is permanently injured. And that statistics is in the US alone.

The ATC accidentally used the wrong call sign, "Merpati 152" when instructing the aircraft on approach, as a result Indonesia 152 pilots ignored the instructions. The controller received no response from the pilots and repeated his instructions using the correct call sign "Indonesia 152". The controller repeated most of the commands but crucially failed to relay "turn left heading 240". This lead the pilots to believe they were landing from the north side of the runway, when the controller intended an approach from the south. This lead to the aircraft crashing in high terrain after confused pilots attempted a fatal approach path.

If you search Garuda Flight 152 one of the articles that comes up is from Wikipedia. The explanation is that several factors combined to cause a lot of confusion in both the ATC and the aircraft. There were two separate flights with #152 coming in, the ATC radar was slow, the aircraft warning system wasn't functioning, the pilots were confused. It was basically a perfect storm of what could go wrong.

It's very much probable that one of the captain must have tried to use accidentally the yoke to turn, thus, it made the HDG select to become inoperable, which I believe, none of the pilots noticed!
