FYI: The accident was caused by an unmanned deployment of reverse thrust during the takeoff, due to mechanical failures.

* The uncommanded deployment of the thrust reverser _combined_ with the co-pilot holding both throttles full forward because he mistook a mechanical safety feature (which was responding to the malfunctioning engine stuck in reverse by physically pulling the right throttle to idle) for a problem with the out-of-service autothrottle system (which the pilots had been told about before the flight.) With the left engine at full-forward thrust and the right engine at full reverse, it bled away their airspeed and stalled the right wing which resulted in a complete loss of control.
also both pilots were not trained to deal with this kind of situation. The co pilot noticed that plane's right throttle is in idle and he tried to override it but he accidentally cut the cord with so much force and allowing the right engine to speed up and cause plane to crash

I'm from São Paulo and I was 10 at the time, but I remember this vividly on the news. There were almost 100 casualties and the Jabaquara district looked like a war zone.

This was caused by a short circuit in the reserve thruster that caused one turbine to spin the opposite. They crew would have had no warning or indication as to what was wrong. Had they know right away there might have been another option, but that is the case with a lot of these crashes. Simply not knowing the problem is often the real danger. This was in no way the fault of the pilots.

TAM Transportes Aéreos Regionais Flight 402 was a scheduled domestic flight from Congonhas-São Paulo International Airport in São Paulo, Brazil to Recife International Airport in Recife via Santos Dumont Airport in Rio de Janeiro. On 31 October 1996, at 08:27 (UTC-3), the starboard engine of the Fokker 100 operating the route reversed thrust while the aircraft was climbing away from the runway at Congonhas. The aircraft stalled and rolled beyond control to the right, then struck two buildings and crashed into several houses in a heavily populated area only 25 seconds after takeoff. All 95 people on board were killed, as well as another 4 on the ground. It is the fourth deadliest accident in Brazilian aviation history, the second at the time.
This reenactment is so well done, one can feel the sheer terror the passengers and crew must have gone through. It always makes the tragedy worse somehow when its maintenance, computer or parts failures, and the pilots don't know what they are fighting against and are helpless😥

Oh... I notice now! The right engines reverse thrust was on, causing it to loose airspeed and bank to the side!
It's worse than you think. The F100 is designed to bring an engine to idle if its reverser deploys when there's no weight on the landing gear. There's no indicator in the cockpit (or wasn't, anyway; they may have added one) to tell the pilots the reverser is out, and Fokker told airlines "the reversers will never deploy in flight, don't worry about training your pilots what to do if it happens." The correct procedure, since the plane can take off on one engine, is to increase the thrust on the still-working engine, declare an emergency and land as soon as you possibly can. Since the first officer didn't know what was happening, he tried increasing thrust on the reversed engine. It went back to idle, so he strong-armed the throttle lever. Eventually the cable pulling the lever back broke. He had full forward thrust on one engine and full reverse on the other...and the plane just spiraled in.

It's been 25 years. 25 years have gone since this terrible disaster. I was 20 years old and i was ready to take over my afternoon shift at multinational company when i was astounded by some live breaking news saying about this crash.
After years of studying and researching, it's claimed that the causes of the crash could have been determined and the bodies of the victims could have been identified much earlier if there wouldn't have been a bunch of poor quality during the investigation and if the procedures to keep the site of the crash out wouldn't have been so amateur.
Many lessons from this tragedy were learned in order to avoid mistakes, poor quality, lack of training and neglect.

My mother saw this accident with her own eyes, when she went to the balcony to see the morning light, and noticed this beautiful blue plane, but realized that it was going very low, in a matter of seconds a fireball went up.. it was so nearby, my mom noticed that the plane had crashed in the back street, near the school where my older brother was studying, so classes were suspended that day! the street where the plane crashed was covered by fire and the firefighters collected at least 100 dead people, it was very sad.

O TAM 402 teve uma abertura de reversor de empuxo durante a decolagem ocasionada por manutenção precária.
A falha ocorreu no circuito/chip eletrônico da trava hidráulica do reversor de empuxo devido a má manutenção e ao elevado número de horas de voo e de ciclos de voo (decolagens e pousos) dada a elevada carga de operação que os jatos tinham na TAM.
Combinado com isso, o primeiro-oficial, que tinha entrado na linha aérea a poucas semanas, não teve o treinamento da abertura do reversor de empuxo durante a decolagem.
A TAM não queria gastar dinheiro além do necessário em termos legais com o simulador de voo, naquela época só existiam os simuladores de voo full motion, do tipo usado para certificar pilotos no curso de ground School de determinado avião tipo. Hoje existem simulador do tipo Homecockpit, flight training device, etc.
Como a TAM não queria gastar dinheiro, ela fez uma "consulta informal" a fabricante do Fokker 100, a Fokker, perguntando sobre a necessidade do treinamento para a abertura do reversor de empuxo durante a decolagem.
A Fokker disse que não seria necessário, pois a probabilidade de um dos reversores abrir durante a decolagem era de 1 em 1 bilhão, pois a velocidade necessária para a abertura do reversor de empuxo seria de pelo menos 200 nós, e dificilmente isso ocorreria no primeiro, ou segundo SEGMENTOS DE DECOLAGEM.
Isso ocorreu pela filosofia gananciosa da TAM sob o comando de Rolim Amaro, que tinha dentre os seus mandamentos o seguinte: "Nada substitui o lucro" e "a maneira mais fácil de ganhar dinheiro é parar de gastar."
Durante a corrida para a decolagem do voo TAM 402, os dois canais automáticos do autothrotlle (Controle de aceleração automática) saíram e seria necessário a operação manual, como o controle de aceleração automática do tipo autothrotlle move as thrust levers (manetes de potência) diferente do autothrust, quando o avião levantou voo e o reversor do motor 2 abriu e fechou por duas vezes e depois abriu, o avião se inclinou para a direita, o primeiro-oficial com desespero achou que seria uma falha do autothrotlle, pois a manete de potência do motor 2 estava em idle (marcha lenta), porém era o flex cabel (cabo de segurança) que estava puxando a manete de potência para idle para minimizar os efeitos da abertura do reversor de empuxo.
Por não ter tido o treinamento da abertura do reversor de empuxo o primeiro-oficial não sabia desse sistema e ficou a todo o custo puxando a manete de potência do motor 2 para potência TO/GA (takeoff: decolagem Go around: arremetida)
O cabo de segurança suportava por volta de 500 a 600lbs de força, porém a força combinada do primeiro-oficial junto do sistema fez com que o cabo se desconectadas e um ponto de conexão.
Depois disso o avião em o motor 2 em potência de decolagem com o reversor aberto. Isso causou um estol (perda de sustenção aerodinâmica) do tipo estol lateral ou estou de asa, quando uma aeronave se inclina tanto que o fluxo de ar sobre as asas se solta e a aeronave cai. Aviões turboélice, por motor turboélice trabalhar com sustenção e não ação e reação como o motor a jato, podem fazer curvas fechada com mais de 60°, jatos comerciais não podem se aproximar ou ultrapassar 60° de bank angle (ângulo lateral) se não entram em estol.
A TAM foi a maior culpada, pois fez uma manutenção precária e não deu o treinamento de abertura do reversor de empuxo para o primeiro-oficial, por ganância do presidente Rolim Amaro.
O relatório do CENIPA INOCENTOU a TAM de forma descarada, isso ocorreu porque a maior parte dos investigadores era tinha relação com a TAM.
E 11 anos depois houve o acidente do voo TAM JJ3054, que teve por principal fator o reversor de empuxo.
"Nenhuma vida perdida em um acidente aéreo é em vão"

Se a aviação tivesse aprendido com o acidente do Voo South African 250 o Voo UPS 6 não teria caído.
Assim como se a aviação tivesse aprendido com o incidente do Voo Airfrance 296 o voos LionAir 610 e Ethiopian 302 não teriam caído.
Se a aviação tivesse aprendido com o incidente do voo Airfrance 354 o Voo TAM JJ3054 não teria caído.
Assim como se a aviação tivesse aprendido com o incidente do voo American Airlines 96 o voo Turksh Airways 981 não teria caído.
Então a aviação aprende com seus erros, mas dizer que ela aprende com todos os erros é uma mentira e uma falta de respeito com todas essas famílias.

RBLXanime it wasn’t his fault. It was Fokker’s fault for never taking into account a situation involving reverse thrust on the engines.
when you hear this, you just know everyone is going to die.
It was the copilot not the pilot
As the pilot in command it’s really hard to watch the thrust reverser as you normally shouldn’t have too and you have to keep track of so many other parameters. The copilot saw it but with only a weeks experience on this particular type of aircraft he didn’t know the cause of it nor how to respond to it. He should have told the captain in the first place but instead just pushed it again. The airplane now automatically pushed it all the way back once again to make sure the thrust reversers weren’t accidentally being turned on - which it shouldn’t have but did due to possibly faulty circuit breaker - and all the stress caused something to break (can’t remember what) and the plane started banking right.
Plus, they weren’t trained for this situation. They should’ve shut down the right engine, declared an emergency and landed. But since they didn’t know what was happening, the copilot pushed the right throttle back to full power. The safety system pulled it to idle again, so he held it at full, with the reverser deployed, the plane spiraled in. They thought it was the autothrottle, so yeah...

As the aircraft lifted off the runway on the accident flight with both engines at full power, a faulty switch in conjunction with a possible short circuit caused the right engine's thrust reverser to deploy. The imbalance of power resulted in the aircraft rolling and veering to the right as it was climbing away from the runway. The safety system automatically cut the power to the engine suffering the thrust reverser malfunction. There was no alarm in the cockpit to indicate that a thrust reverser had been accidentally deployed. The crew had no way of knowing what the true problem was. The copilot, seeing the right engine power lever automatically move to the closed position, thought that the lever had slipped back and pushed it back to the full power position alongside the left engine throttle lever. Once again, the automatic safety system closed the right engine throttle and the captain, who was conducting the takeoff, called for the autothrottle system to be switch auto throttle system to be switched off. After switching off the system, the copilot again pushed the right engine power lever fully forward and forcefully held it there.
The safety system cable, responsible for pulling the power lever to the idle position, was no longer able to withstand the physical strain of being pulled one way by the actuator, while the copilot forced it the other way by pushing the throttle lever to the fully open position - the cable soon parted at a maintenance connection. With the lever no longer restrained by the safety system, the copilot continued to hold the right throttle fully open. The combination of the right engine at full thrust in reverse and the left engine still at normal forward take-off thrust caused the aircraft to roll violently to the right and descend into the ground.
