I am a nephrologist and kidney doctor who believes that the best tool to reduce the burden of kidney disease globally is not medication, lab tests, or even genetics research. Instead, knowledge of kidney disease can best protect our kidneys.
This channel is part of the kidney disease awareness project I have worked on for years. It started with long conversations with kidney patients in the office and hospital. During these conversations, I was perplexed to see patients’ lack of knowledge about their kidney disease.
To fill this gap, I developed a blog for kidney disease patients, “Kidneyblogs.com,” where patients could find the correct information-and access a nephrologist for kidney-related questions. But then, I realized that not all Urdu- and Hindi-speaking patients could read English, and many preferred listening. This revelation led to the formation of the RUclips channel you will explore.

Thanks for your kind words. No, sir I am not aware of Vit A toxicity causing proteinuria but vit a toxicity can casue kidney injury just like it can cause fibrosis in other organs like liver.

Sir I am ckd patient using Aavsar 5/80 but my bp is in control without taking drug so my dr suggested to take a half dose and I feel beter alternate day
I am not in favor of taking these medicines on and off. Please take the medicine daily and try to use the full dose if your blood pressure can tolerate. Most medicines work best when they are taken in full dose.

I am a long-term Nephrotic Syndrome patient (for the last 20 years). I have taken homeo herbal treatment for the last 15 years as steroids were not effective for me. I have not taken a biopsy. I am currently not on immunosuppression or steroids, but I keep getting 400-600 mg protein in 24 hours. I have not taken herbal treatment either for the last 9 months. Nephrologist says biopsy can not be done unless I am spilling 2 grams in 24 hours. How do you reduce this protein to under 300 mg? Please reply.

My father is age 71. His diagnoses are DM (1999) CVA (2006) PCI in 2009 and again in 2014, First time he was diagnosed with Kidney disease in 2014. Since then we are visiting a nephrologist every month. His last labs are EGFr 15, Complete blood counts are normal. Potassium is 5.4. Last month his UCAR was 58mg/day. Can he take Flozins? Currently, his vitals are stable with a control BSR under 200.
Thanks for sharing your father's experience. I don't see any problem using Flozins in a 71 year old person with Stage 5 CKD. But your doctor may not go for it because research on this topic has not published yet.

Hi I am a proteinurea patient since 10 years I had kidney biopsy two times and got diagnosed as minimal change disease both times. I on Tab prednisone since than when I take prednisone my proteinurea gets normal but when I try to get off from prednisone my proteinurea comes back. My doctor said I am prednisone dependent. I have to take prednisone rest of my life. Could you please give me piece of advice thanks
Ji your doctor is saying right. But you can discuss with him and try other stornger immunosuprressive medicines like cyclosporin or tacrolimus. There are no specific guidelines for them, but between your doctor and you, it can be tried.

If you are talking about Filspari, this drug has not been studied in CKD other than IgA nephropathy. So, we cannot use this drug in non-igA patients.

Assalamu Alyekum. My Albumin/creatinin ratio was 50. I was put on Invacano and my ratio was dropped to 15.5. My question to you is, is it possible I can use the 2nd group of medicine, or is it OK to be on Invacano? Also, I would like to know that Albumin/creatinine ratio can be lower than 15.5, if I continue Invacano?
Moreover, how do you know that protinuria is because of glomerules or tubules?

First of all you need to know why do you have proteinuria. Second, if your albuminuria has resolved with Invacaon that is enough. Third there is no easy way to differentiate between glomerular and tubular proteinuria.

There is no such case. Doctor should always give medicine for proteinuria.
Sir plz help me.. I had kidney stone and it was removed by urethroscopy and then they put a stent.. Then after 2 weeks they removed it.. But I started feeling pain on my right side again it became worse..I went to hospital then did a urine test.. They told me there is protein in my urine it was not normal.. It was 50 to 60% out of 100..but they did a blood test and ctr scan.. Then they told me that my kidney is working properly n there is no infection in ctr scan and in blood test.. Now I am home and my pain is getting worse day by day but doctors are unable to find the problem.. They say I have no infection.. Plz doctor help me.. I really need your help

I am extremely sorry for your father, extremely extremely sorry. If I ever converted into someone who hurls insults on others, my first target would be a doctor who doesn't prescribe ACEIs or ARBs to a CKD, but I have promised everybody that I will just convey the positive message as long as I can. I am extremely sorry on behalf of doctor community that they don't prescribe ACEIs or ARBs to a CKD or DM with CKD patients in Pakistan. Would you please ask him next time why not. You father should be on one of them unless he has high potassium level or very high Cr.

I'm 35 male, diagnosed in 2021, at stage 3b with egfr 33. I have been taking valsartan 80 and farxiga 10. Alhumdulillah stable currently. Doing exercise and trying to eat healthy, with low protein. Please keep making such videos. I'm still confused about low protein diet
Thanks for sharing your experience. First of all, there shouldn't be any confusion about your protein intake. CKD doesn't need any protein restriction. You can eat upto 60gm protein per day. Plus, you should be taking maximum dose of valsartan and Farxgia if your blood pressure tolerates, for maximum benefits. Discuss with your nephrologist
@kidneyblogs thanks. Which protein is better? Animal or plant based? Lately my bicarbonate is fluctuating between 16 to 25 in the last 6 tests with 2 months gap each. My nephrologist says no need to worry as it is not consistent yet and uric acid has remained below 7

First of all you, let me clarify that you need to see at least three doctors all the time-an endocrinologist, a cardiologist, and a nephrologist. And you are not in position to use any medicine just from internet or from any over the counter source. You should strictly stay with your doctors and consult all three of them even before changing the dose of any of your medicine.

Sir there are no meds to reverse the fibrosis of kidney disease so far. If we will make them in the future is unknown. Probiotics and Nefrosave are not medicines to reverse the kidney disease.

My uacr .30mg/L...Normal range .24 hour urine 164(normal 150)....But still frothy urine ...for 1 year...what more test can be done to detect proteinuria?
If your creatinine is normal and 24 hour urine is just 160, I would suggest healthy life style and check these two things every 6 months. But make sure you dont have diabetes or high blood pressure.
My mother is kidney transplant patient and is having proteinuria for 3-4 years now after an episode of acute kidney rejection. Usually one-two + in urine. She has renal parenchymal disease grade 3. Taking calcium channel blocker nifidipine twice/thrice a day for bp and aimpra plus glucophage for sugar. Can medicine be given to her for proteinuria?
Ji definitely she can be given the medicines from the first group ACEIs or ARBs, but there is no research for SGLT2 Inhibitors and the third group in transplant patients.
Stop tab glucophage. Start short acting insulin Humalin R in low dose.
And please don’t try any medication without consulting your transplant team. I appreciate.

I would advise low potassium diet. If it doesnt work, ask your physician for some medicine.

Yes, you will continue these medicines lifelong. And ACR 300 is not bad. Most likely, it will get even better.

Hi doc , My Doctor had given me ARB ( Telmisartan 40mg ) , i still have Trace Protein but my BP has reached 105/65 . I do exercise daily and don't take more 1/10th tbsp salt . Should i stop taking ARB as my BP is under control
No you shouldn't stop Telmisartan. 105/65 is a very beautiful blood pressure
@@kidneyblogs but my blood pressure keeps lowering down . Wouldn't my diastolic blood pressure get even lower than 60 with telmisartan which I have read isn't safe . What do you think ? I also take 10 mg dapagloflozin daily

Ji Sir. thanks for watching from India. Yes, your ACR of 62 is nothing to worry about. It should completely go away with RAAS and SGLT2 inhibitors. And if you dont do exercise yet, do it regularly.

Aoa. Sir very informative episodes. My wife is kidney patient having problems of SLE and glumarlnephrtics 1gm. In her recent test urine protein spot is 816.1 mg/dl, urine creatinine spot is 213.6 mg/dl and protein/creatinine ratio is 3.82 mg/mg. Sir yeh test kasy hn
sir 3.82 mg/mg is still nephrotic syndrome range. If she improved from 10mg/mg to 3.8 then it is significant improvement but is she hasn't shown any progress then I am not sure what is wrong with the treatment.
