All the guests you see that show up were people that answered an ad on Craigslist or similar publications. They all get a free room & meal as provided by the production company and usually something like $50. It's the same for Kitchen Nightmares except there it's just a meal and a drink which usually means a bottle of something. Most of the people wind up giving that money to the servers.
