    I have this issue . Been doing physical therapy for almost a year ..no help . No one knows how to solve this problem . Its insane how the a small patella mal tracking can destroy a life. Can you explain how to test a medial torsion of femur ? Can you try to explain the bio mechanism of lateral patella tracking ?

      Did you figure anything out?? I'm in p.t.. AGAIN! They mentioned arch support and it makes sense when I think about it. Also they can cinch up ligaments or add lateral ones to keep it stable. I had a doctor mention grooving out the kneecap socket.

    Wow. The description of the mechanics involved in patella maltracking at 3.00 has finally brought understanding to an issue ive watched 100s of hours of youtube vids trying to fix. You are awesome, im impressed with the depth and clarity of the tutorials, thanks

      @@violetgodwin677 It Did! Understanding the why's of the problem, led me to knowing which muscle groups I needed to work on to stabilise the joint. Seen great improvements, especially in conjuction with soft tissue work as he demonstrates here and elsewhere. Oh my surname is Goodwin BTW, which is the newer version of Godwin!

    does having patella tracking issues increase my chances of patella dislocation?

      The medial patellofemoral ligament is the primary stabiliser against lateral displacement/dislocation of the patella. Imbalance between stronger lateral tissues like the vastus lateralis, overcome weaker medial structures, especially the medial patellofemoral ligament .
      Often the mechanism for injury is associated with twisting of the leg, with internal rotation of the femur on a fixed foot. Valgus stress (internal movement of the femur) is also associated with dislocation.

    This is the best descriptive video I have seen regarding this issue. I had a right knee replacement in august. A few weeks later good left knee started with issues due to overcompensation. Mri confirmed patellar alta which I didn’t have previous to this surgery. I also have stage 4 chondromalacia.

    Thanks for the video. My right knee has shifted to the inside and so I have problems running with my right leg dragging, I will try these exercises. Are there other exercises? Will orthotics help?

    I’ve been having tracking issues on my right knee since last week and I sprained my left ankle a couple days before the tracking issue. I’m just guessing but maybe my sprained ankle caused the tracking issue

    great videos! I just watched both - as a personal trainer, this information is super helpful! I have a client that has been told she has patella tracking issues - but her pain is below the kneecap, not above, like you mentioned. Would this potentially change what muscles are involved?

      Hi Cora-Ann.. thanks for the watch. Without an assessment it would hard to say. Pain below the knee could be a number of things, including but not limited to patella tendon strain, meniscus, or cartilage damage. Generally position of the hip, hemisphere dominance, and center of mass has a large part to play in whether the knee tracks functionally. Knowing these things would help to tease out which force couples are out of balance.

    Have you seen anyone’s knee start tracking normally after maltracking?

      Absolutely. Often folks focus on the tracking of the cap in the groove, rather than adjusting the femurs relative position (there by making the groove more congruent), or improve the global distribution of force throughout the gait cycle, which can lead ro less than desirable outcomes. Applying local and global tactics usual can resolve the issue.

    Do i have to sleep a certain way, I feel like my spine alignment affects my pelvis.

      How you sleep can position your back. Side sleeping can bow your back to the opposite side, essentially hiking the hip on the opposite side of that on the ground. The top leg can also drag the spine forward into the opposite side, twisting the spine clockwise if left is up, or counter clockwise if right is up.

    At 18:30 when you’re showing the exercise do you want to resist the pull to the outside or do you want to gently allow it?

      The band tension is engaging the quadriceps (ideally vastus medialis) at terminal knee extension to assist with tibial and patella stability. Focus on keeping the knee at no deeper than 13 degrees of internal rotation.
