Thanks for this video. How should a diabetic manage their insulin for the glucose test? I need to do the test and no one in my care team is able to give me an answer on how to dose insulin for such a high amount of sugar.
What probiotic strains do you recommend for testing sibo? Do they differ for sifo?
What are your thoughts on the Trio Smart at-home breath test, which claims to be more accurate and tests hydrogen, methane, and hydrogen sulfide?
From the lactulose test, for hydrogen I saw just 1ppm at 80 mins, 13ppm at 100 mins, and 42ppm at 120 mins. Yet they said positive for SIBO and that Rifaximin is needed. I'm afraid of taking it because of side effects. I can't seem to find a place in Connecticut that does the glucose breath test. Any help would be appreciated 🙏

Hello doctor i am naman from india it would be of much help to me i had done my hydrogen breath test and my doctor said it is negative but when i see the videos on youtube it seems positive i had already gone to 9 doctors and my stomach goes like i am pregnant at the night time plz help i am sharing my h2 concentration values if you allow me later i will send you my h2 breath chart here are the values

it seems positive because after 60 min there is rise of 20 ppm and it is usually positive before 90 min so help me also here in my city only hydrogen is available no methane and glucose test and i am constipated like hell and in constipation methane is more accurate so plz tell the above values is sibo positive or not

i may have developed my sibo by consuming a drink that had lots of fungus/mold in it which was stupid of me for not checking...the mold probbly carried the bacteria to places it shouldnt have been.. i have diarreah everytime i eat carbs/ fruictose.... hopefully the carnivore diet will slow my digestion down combined with many anti fungal /bacterial/ and viral herbs untill those bacteria are killed....as a vegan im now temporairly on omad Carnivore diet😒.. hopefully it doesnt take me longer than 2 weeks.

It seems to me that you lack knowledge about this subject.
You claim that a glucose test is more reliable than a lactulose test, but you omit to mention that glucose, unlike lactulose, is digestible. So if the microbiomes are further down in the small intestine, a glucose test might miss the SIBO. Therefore a glucose test is not considered reliable for SIBO in the ileum (near the large intestine).
You also overlook the fact that a SIBO test needs to be longer for people with a slow transit time (typical when you have chronic constipation). Otherwise the SIBO might be missed, since the sugar solution won´t reach the part of the small intestine where those microbiomes exist. That is one of the reasons that many prefer a 3 hour test.
While much of the mechanistic theory is true, it does not hold up against further scrutiny when we look at actual OUTCOME data. For example, a few studies examining the accuracy of glucose vs lactulose, found glucose to be superior. Furthermore, lactulose may lead to more false positive findings (~30%) which may lead to unnecessary treatment and retesting. Remember, to treat the symptoms, not the labs. @@DrRuscioDC Show me a study that proves that a glucose test is better at discovering SIBO in the ileum (near the large intestine) for people with a slow transit time (typical for chronic constipation), and you have a point. (Lactulose increases the transit time while glucose does not. So with a glucose test the risk is greater that the sugar solution don´t reach the area of the small intestine where the microbiomes are.)
To me the glucose test seems to increase the risk of a false negative result in those patients. You are right about that healthcare should treat symptoms and not test results, but conventional medicine in my country doesn´t work like that. A false positive test for a patient with typical symptoms is much less of a problem. A false negative test means no treatment.
