How to fill the D2 Form- HGV application form - Bus licence application form-www.Vitalmedicals.co.uk

Today I will be going through how to fill out the D2 form.
A lot of our customers are finding it difficult to either get the form in the first place or fill it out.
This form can be used to apply for the renewal or provisional licence for horse boxes 🐎, ambulances 🚑, Lorries 🚛, busses 🚎 and motor homes.
We normally provide you with the D2 form when you come for your medical, and are more than happy to help you fill the form too.

Good video. Wasn’t aware of the ticking all boxes for getting a provisional with all entitlements at once.
One thing you never revisited was when your driving licence is due to expire within the period of 1-3months (don’t remember exact duration) of the application date. I’ve had mine returned and I need to submit a new photo and pay the fee. Do I need this new photo signing at all?
I don’t have the information portion of the D2 any more so can’t check and I can’t find it on the DVLA website to read it.

Hello Andy, if you are adding an entitlement to your already existing license you do not have to pay anything with this application.

I did tried to contact them directly or email but unfortunately they never get back to me. My question is do I need to go for medical again or just send them new D2? I had bus license and it was revoked due to my heart attack as I failed a test after the stent. They revoked only bus entitlement, and issued me another licence with car only.
Now after another test, it came positive. And dvla said to re-apply I need to fill D2 form.
Can you please make a short video explaining how to fill the form for a revoked entitlement.

I want my HGV provisional. In the "Your conduct section" would i have to put an offence from when i were younger although they now have come off my licence? It was "IN10" No insurance. It would be a great help as i already made a mistake and have had to wait another week for another D2 to be sent to me. Problem being i cannot remember dates and didnt go to court. It were a long time ago

So both my main licence and my CPC have expired. It feels like quite a faff to sort this out, especially as I no longer need my CPC. I am going to assume I need to complete the D2 and D1 forms and send them off, whilst selecting surrender C1 and D1.

I am sorry if i misunderstood you, but, if you need glasses for a lorry then you will need them for a car and vice versa, as seeing whilst driving to the standard required is a legal requirement no matter what you are driving
