Can I fill in the grey parts of the form prior to going to the medical to save time or do they have to be filled in,in the presence of the medical examiner at the medical?

Thank you for such a useful video.
My HGV license is going to expire on my 55th birthday (24th Nov 2024).
I will be abroad for a month before that and won’t be back until the 2nd week of November 24.
My queries are:
1. Is form D4 the ONLY form that I and the doctor need to fill out, or are there any additional forms required?
2. How long in advance can I fill D4, do the medical and send the signed (form D4) before my license expires (24/11/24)?

I made a couple of single-letter typo errors as I have a long full name. Should I get a new form, or can I use a correction marker like Tipex?

Hi Dan, if the form hasn't already been signed by a doctor then please get a new form. DVLA scan all forms t read them, so this will avoid them not being able to read it properly.
