How to fill the D4 Medical Form - HGV Medical form - Bus Medical form-www.vitalmedicals.co.uk

Today I will be going through how to fill out the D4 form. A lot of our customers are finding it difficult to fill it out.
This medical form can be used to apply for the renewal or provisional licence for horse boxes 🐎, ambulances 🚑, Lorries 🚛, busses 🚎 and motor homes. Was after this purely for the the 'date first licensed' section. The form says to complete all sections yet the dvla information document stupidly refuses to added a note for those applying for the first time =_=
