It's not that scary. Not too many jumpscares and not that much gore. More of a slow burn with suspense/thriller/sci-fi elements

the evil one switched with the innocent one when she was a child , so the evil one is the one in the real world !
The thing is, it isn't black and white. Red may have been the original and is trying to get revenge for what the fake Addy did to her, leading the revolution of the tethered people. At the same time, Red turned into animals like the other tethered people, just much more intelligent (i.e. being able to speak) and with a strong motive because she originally came from the surface.
On the other hand, there's the fake Addy (I'll just call her Addy). You could say that Addy was a victim of her environment and just did whatever she could to escape and have a normal life, as well as she was a child who didn't understand the repercussions of her actions on the original Addy. You could also argue that Addy premeditated the kidnapping of Red and knew fully well what she was doing. But she went on to live what could have been Red's life anyway, even making a family of her own. She became humane, while her counterpart in the underground became more animalistic. Throughout the movie, Addy demonstrates that she'd keep her family safe at any cost. As the viewer progresses through the movie, with every kill Addy makes she appears to become more animalistic, just like one of the tethered.
With this information, you can see the situation from both sides: a woman who's lost her humanity is taking her life back from the one who made her this way, or a woman whose family is being attacked by their inhuman doppelgängers. There is truth in both of these viewpoints, and the movie wants us as viewers to form our own opinions about it. So, it really depends on your perspective as to which one is evil. My standpoint is there isn't an evil one, but instead two characters who are in the gray area.
