Why Does My Shoulder Hurt?

  • It is incredibly common that people who have gone through Breast Cancer will experience shoulder pain after their primary treatment has finished. I'm not talking about the post surgical pain and stiffness that happens for days or a few weeks after a lumpectomy or mastectomy....
    I'm talking about a very common condition that can seemingly come out of nowhere. That condition is Shoulder Bursitis. And with the amount of women I see every single week that have been through Breast Cancer and were presenting with Shoulder Bursitis, I HAD to make a video on it.
    In this video I discuss what is bursa, what is bursitis, why does bursitis occur, what symptoms you get with bursitis, and how we treat it. The topic of cortisone injections for bursitis is covered in detail in this video, because I believe from my clinical experience that there is still good clinical reasons to perform a cortisone injection, even if the patient is at risk of Lymphoedema. I explain why in this video.

    Thanks for this information been suffering severe pain in my left shoulder over recent months had bilateral surgery 10 months ago find it so hard to sleep on my right side but pain down my left arm after sleeping left side is terrible also get sharp dart if i jerk it back quickly folding clothes etc have painted my living room recently with no problems just the backward motions seem to affect hopefully with time i will see an improvement not doing too bad otherwise for 64 thankfully

    I was going through the same pain on my left shoulder as i also had lumpectomy on left side. Out of no where this video came in feed. Now i am at peace to know what i have

    Hello there- thank you so much for making such in depth videos. They have given me so much calm and comfort as I go through physical therapy. I was diagnosed with mild rotator cuff tendonitis a year post lumpectomy and radiation but nothing has really helped me. Could this be bursitis instead ? Curious to hear what you think.

      @@TheBreastCancerPhysio it is tight but not too terribly restricted. It feels fragile at the tippy top of the shoulder and I hear lots of cracklings sounds when I turn around in bed. I also very very recently developed cording ( also one year after surgery and radiation) and it has baffled my PT. And I am sorry to go on and on about this but my neck and shoulders are so so incredibly tight and low grade painful all the time.

    This is a useful video! I had an axillary lymph node dissection (and lymphovenous bypass) in early April. No major pain issues until just a week or two ago--now getting shoulder pain which seems to happen in a lot of different planes of motion...but particularly when I reach out to the side, or stretch my (right) arm to the left across my body. And sometimes, strangely, I get a sudden sharp pain when I am withdrawing my arm from a stretch--rather than going up or out into the stretch. Does that match up with bursitis? Besides my post-surgery prescribed arm exercises, I have now been given the o.k. to start back with my gym workouts, where I have been returning to all my previous arm exercises (which I could always do with no pain) but now using very much lighter weights for now. What is upsetting me most is that I don't feel I can return to playing tennis at this point! I will be starting radiation later this week (15 sessions planned). What should I discuss with my radiologist or radiology technician? What can be done to get me back to playing tennis as soon as possible?

      Hi Heather! Have you been able to see a Physiotherapist or Physical Therapist to assess your shoulder? Hard to tell if it is bursitis or scar tissue.     Thanks Jen great video. Very odd I have developed what now appears to be bursitis after watching your video - have all the symptoms as you said out of the blue. Odd thing its the opposite arm to the surgery arm. Very odd

      Hi Pantano, this is actually relatively common. The unaffected side can become over used because the affected side is recovering / sore. So I definitely see people get bursitis in the opposite shoulder. Jen x

      @@TheBreastCancerPhysio Thank you, I just asked this question too, and now see this comment. Mine has started on the opposite side to the surgery side - I was very protective of my surgery side, so I most likely over-compensated on the other side.

    Hello Jen, I have just watched your You Tube Vlog on ‘Why does my shoulder hurt’? . I had Right side Breast Cancer at the start of March 2018 and finished treatment in 6th Aug 2019
    I had Chemo, a Lumpectomy operation on my right hand side and then Radiation. It was all ok at the end and my Cancer Surgeon signed me off.
    So from the middle of Aug 2019 I have been in Remission.
    All has been fine no probs until now.
    I was wondering if I could have Bursitis in my ‘ opposite side’ ? . When I lift my left arm it hurts when I lift it half way and higher. It feels heavy and is sore on the outside area of my shoulder. I have had it for about a month now . Do you think I should go and see my doctor? Thanks Jennie. X

      Hi Jennie, thanks for watching and commenting. It is certainly possible to get bursitis in the opposite side to your treatment. I suspect it may occur because of overload on your good side, if that makes sense? Definitely discuss it with your GP and hopefully they will refer you for an ultrasound scan to clarify. Jen

    Hi there, I have had arm pain on and off since treatment 3 years ago. Exercising aggravates it and I've been living weights for about 6 weeks which seems to have brought it on again. Today my arm and shoulder feel achey. Is this normal and not necessarily a sign of lymphydema? I don't have swelling that I can see.

      Hi Miriam, I am so sorry for my delayed reply. I would definitely try to see a qualified Lymphoedema Therapist to assess your arm. It may be lymphoedema but if the only symptom you have is sore it may not necessarily be lymphoedema as other issues after Breast Cancer Treatment cause pain.     Hello have been feeling some chest ,bellow the back and shoulder pain to the left that extend to the centre of my shoulder, I've got also a swollen lump on my breast collarbone which is painful with a stomach ache that feels like daeriea, what could that possibly mean

      I'm also having the same problem.. i went for consulting a doctor and took ultra sound scan also.. ap per USS have some lumps which is normal.. but I'm really worrying about this pain.. can u pls advise what it can be?

    Can it be on the opposite shoulder the breast cancer side? Due to over-compensating and using the other arm?

      Yes it sure can, I see it happen on the opposite side when someone has over compensated for the pain / weakness / tension in the treated arm - hope that makes sense.     Is it possible to have bursitis in my wrist also? My entire right arm is painful andthe pain has moved into my wrist. Feels like carpol tunnel or arthritis. Wearing a wrist splint to help with pain

      Hi Patti! The description you have provided doesn't sound like bursitis. You are possibly correct- it does sound like carpal tunnel. Speak with your GP and they should organize the appropriate investigations to clarify a diagnosis or refer you to a specialist that can diagnose and treat. I hope this helps. Jen

    I was diagnosed with DCIS breast cancer in April. I am due to have surgery in two weeks. I have developed shoulder pain. I thought that it was due to the cancer spreading, but I'm skeptical of it. I thought that I slept wrong, so I have been putting heat on it. Is this bursitis?

      Hi Teresa, I would suggest seeing your Doctor to get a formal diagnosis as to why you have shoulder pain. Usually you will require some scans such as an x-ray and / or ultrasound to confirm a diagnosis. Best to get this done prior to starting any treatment.     Is it normal to have upper back or shoulder pain while receiving chemo?

      Hi Bridget, any upper back or shoulder pain that is not explained should be investigated to obtain a diagnosis and then hopefully it can be treated. I would discuss this with your Medical Team and they may refer you for scans. I hope this helps. Jen :)

      I went through chemo 1st radiation then mastectomy 7 months later I started have my shoulder pain. I got cortisone shot at a bone clinic which made my shoulder worse. I go to a pain clinic and my doctor is going to inject my shoulder tomorrow. After watching this video it shows my just how inadequate my doctor are. I wished I lived near you so I could come to your clinic but Tennessee is to far away. Thank you for the knowledge. By the way it was triple negative invasive ductal carcinoma stage 3 b I did not finish in 1st post

    I never had any issues with shoulder pain until after my BMX. It feels like the pain I’m having is at the biceps tendon and bicipital groove. Have you seen this in your practice?

      Hi Alynda, it is highly possible the shoulder issues you're experiencing could be linked with your bilateral mastectomy. Do you have access to a Physiotherapist to be assessed? Jen x
