    Very interesting video. I'm 55j old and training from my 15j 5 days a week. I never used anything.     Age 49, been on TRT for over 10 years. One thing not talked about is other meds you may be taking and how they may affect Test. If you are on any kind of anti anxiety or anti depressant you may not get the full affect of test as those meds can affect libido, sexual performance and energy levels. Also, I've noticed TRT being way more beneficial for those that lift weights or exercise as that is when you really notice the difference. Not sure if it is worth taking if you just want for libido and/or other health benefits as it is hard to maintain a steady balance of hormone levels without the ups and downs. You also have to keep an eye on RBC's and Hematocrit levels.

      I stopped getting erections. I think it's bc I weightlift too much. My testosterone levels dropped significantly. I have heard from guys on TRT, that it does not help libido. Is that true?
      Gaining libido is more important to me than gaining muscle.

      Man im 31. My sex drive is so high now. I never masturbated before. I feel the need to now. My erections are harder than ever and i feel bettter all around. My test was at 320 in the evening and 440 in the morning befoee test. So i didnt think it was gonna help. But man what a differebce in 3 weeks i was like wow. Im using the lowest dose gel too. @MilkyWayGalaxyy

      @@darrenbeeken6783 You are using the gel?? Not TRT? Everyone I spoke to says the gel is messy and not effective.
      But it's working for you?
      I am 530 total testosterone.
      But I had varicose vein surgery and am having complications post surgery. Blood isn't going to my penis so I stopped getting erections. I am trying to see the surgeon again to fix it. But my libido was fine before the surgery.

      ​@@darrenbeeken6783I am 36 and my test levels came back at 620,my libido is not the best,but my doctor says its because of anxiety,which she was right I guess,she prescribed cialis and it works great I believe its 10mg of cialis so in the day that I am going to perform I take one in the morning and the other one 30 minutes before my duty,but what you need is your partner to help you stimulate you and that got me hard and ready for 2+ hours,but I also drank each time although its not recommended at all,I did it to bring my anxiety down,and it's worked every time.

    Thank you so much for putting this piece of content out. I just turned 40 and have been feeling a drop in energy and libido over the last 2 years. Got tested two weeks ago, and although my total test was 617, my SHBG was was 86 and my free was 6.7! I start 200mg of cyp/week next week and I am beyond stoked. The funny thing is that I still compete in BJJ, surf regularly (2 hour sessions), and stay ripped. People don’t realize that you may look great on the outside but feel like total crap under the hood.

      I have similar ranges with lower SHBG and slightly higher free test. Also very active and turning 40 soon.I'm in great shape but exhausted all the time past few years. Looking at options too

      Does standard blood work show the shbg or does the doctor have to request that I have the testosterone level, etc. but I don’t see that

      At 40 I was in shape and energized. I'm 46 now and had to jump on the TRT, I gained weight and just felt tired all the time within just a few short yrs. It will get worse so take care of your self

      Be careful man - I got snowballed into thinking the same thing and much like you (lifelong wrestler, soldier, BJJ brown belt) I got with Defy medical who convinced me I had something wrong and put me on 200mg / weekly HCG. Felt great for the first month, then I felt like fucking trash after and started really hitting the redline. High BP, extreme aggression, couldn't sleep. I got freaked out that I was killing myself and did a hard stop, continuing with HCG and Tamoxifen for 3 weeks. I backed off from training so hard and put some weight back on, and my SHBG went down, freeing up more T. I started at 879 TT at 37 years old, and recovered to 796 in 1 month after quitting. I was on T for 9 months. It's just not for me (right now). I, like you, walked around shredded, but I feel better off it. Just my anecdotal experience, although that first 3 weeks was fkn awesome.

      ​@@AN-jz3px Hey man 200mg is high for starter dose! What levels were out of whack that caused them to give you it? They never tell people this, 100mg is a wayyyy better start. I've done enough reading and seeing anecdotal evidence from people such as yourself stating 100mg was a way better option. Then again, they could of just wanted your money and you didn't need it, thats why you felt like that! Very broad subject lol.

    Great video. Just wondering if Doc has ever dealt with an patients that have a mutation in AR gene, leading to androgen insensitivity syndrome? There would definitely be some sort of genetic component to your question about receptors…

    This was super insightful. Some of it still went over my head, but the key takeaway for me is that I need to have a deeper talk with my doctor at my next wellness check about my bloodwork. It's something we usually gloss over but I'll be 43 soon and feel like I've been experiencing changes that I've not been able to put my finger on.

      My 2c on your extremely common question. It's best to learn how to manage yourself. You can learn a lot from various doctors but people do have individual response. Some guys have problems elevating hemoglobin, some blood pressure. Some with the dose response. Some with other medications and so on and so forth. Doctors have roughly 15 minutes to see you. They aren't going to figure all that out in your brief visits but they can point you in the right direction.

      Be prepared to fight a bit with your doctor. I had to force mine to even offer to order bloodwork. I paid big bucks out of pocket for a solution.

      @@jacobdrollinger4802 I was able to get a referral. No appointments until July. I started TRT 3 weeks ago. Chronic pain gone, energy is up and sense of well being is back. Do your homework but don't be afraid to start living again.

      Sam, I'm in Airdrie Alberta. I just saw your comment. Did you use Regenesis for TRT? My doctor just shut me down because my bloodwork showed right on the edge 12nmol/L even though my first bloodwork a few weeks earlier was 9.2. Very frustrated my doctor didn't care and/or know about this. We are short family doctors taking new patients in Calgary area.@@sammasic5849

    Great video. I'm 46yo, and just started my hormone replacement therapy under my doctor's guidance. For reference, my total T was 320, free T was only 4.6ng/dl, SHBG was good at 30, and my available t was only 100 ng/dl. My doctor started me on gel, but I plan to do test cyp at 200mg/week starting next month.

    Thank you for a super informative Q&A with Dr Brett. I have just got my blood test results and the exact scenario highlighted in this video is me. I'm 53, healthy, in good physical shape, I have Total T = 713 (great) but Free T = 7.55 due to SHBG at 90. So Nettle Root Extract and Milk Thistle are my supplements to see if I can lower this and increase my T levels. TRT is off the table until my SHBG is lower and I get some free T pumping, hopefully this will help with my clarity.

    Great vid James thanks again. I got a T test from my physician and the test only shows total T and Free T. If somebody has a low total T number and "normal" free T. Do you think its possible for the doc to make a sound medical decision without SHBG and Albumin #'s?

    I’ll be 55 in June and I’ve been increasing my lifting everyday.I haven’t waked up with erecting for a long time.I don’t smoke or drink.I’m not a diabetic. I’m skinny fat. I eat 4 organic eggs everyday with avocado. With Greek yogurt and organic blueberries. I’ve been trying to get my V shaped look back.

    James you are my fitness idol. I'm in my 30s so not quite at the level of needing to worry about TRT but such great information in this video and very candid. Gonna give stinging nettle a try for a few months and see if I notice a difference. Thanks for the years of great content.

      Bro my test levels were shot at 32yrs old and NI no PEDs. On TRT.1000mg every 10wks. This is what's most commonly given here in New Zealand.
      Saved my marriage if you know what I mean.Im 56 and just started training 6 mths ago.

    Hi do I see an endocrinologist for my trt dialing in. Or is it just a primary care physician. I. Considering strongly on starting this year. Hard to stay in shape with metabolic syndrome.

    Im 45 and my Testosterone has dropped by 360 pts in the last 7 yrs. I was at 890 now at 530 and they have started me on T cream twice a day. I have also started Sermorline once a day at bed. My liver function was very good but my main concern is my estrodial number is at 13.9 and my DHEA was the lowest she said she’d ever seen at 75.9. I will also be supplementing DHEA. Should I be taking something to keep my estrodial numbers down or do they need to increase? I have been taking finasteride for 20 yrs do to hair loss in the family. I am a little concerned that my hair loss will increased by the Test therapy.. I should note my Free T is 93.2. Any recommendations are greatly appreciated

    Hi. I am trying to find info about my hot and cold flashes that i got after stopping testosterone. But i cant find anything on this..
    So i am hoping for som insight on this from you guys.
    I am 50ys old and i already passed menopause with normal hot flashes some years ago. Every 5th day I took 0,1ml / 25mgs of testo E. For half a year. And then i stopped suddenly at christmas when i discovered hair loss and some more.
    But i have had increasingly more frequent problems with these hot cold sweaty flashes for a month or more now. Its impossible to stay dry. Goosebumps at least 2times an hour.
    I also have a male 40yr old friend experiencing about the same but he hasnt stopped exept from taking more in periods.
    Pls have you some info on how long this can go on. And can i stop it somehow?

    I have used Testosterone over the past 10 years. In the last 2 years I have not used any. Feeling flat and low did bloods and was diagnosed with secondary hypogonadism. The problem is when I started with test enanthate 250mg once a month I experienced severe bloating and a very uncomfortable gut. As soon as I'm done eating I would get very uncomfortable and need ed to use the loo. Moved over to test cypionate, 50mg once a week. Same result. It's almost as though I can't tolerate it internally. I have chatted to my endocrinologist and few other well versed people however nobody seems to understand why or how this happens. Any feedback would be highly appreciated.....🙏

    My doctor put me on bioidentical testosterone troches. 50 mg twice a day and 50 mg Dhea. What do you think about this? Been on this for 6 weeks now. The only thing that l have noticed is that my libido is some what better. I am 60

    I was so worried about going on TRT, I'm 50, had blood work done in november 2022, test level came back low 301, forced myself back in the gym since i had balloned up to 420lbs from my inshape weight of 245lbs, was still tired abnormally weak, like I struggled with 175lbs on the bench, but before I gained all this weight, i could still bench over 300lbs, so now I just had my bloodwork done again and now my test is 199! wtf. So my dr has me seeing a urologist next month, I pretty much know I need test. My previous test levels prior to the weight gain was 989, that was when I was 44. So I'm hoping TRT works for me.

      You need to loose weight. Its actually converting to estrogen

      @@TiberiusWorld I’m 30 and was told I’m very anemic for a while so tried b 12 injections but didn’t help. Finally tested testosterone and I’m very low. I abused heroin and meth for many years and am on methadone now so that’s probably why. I work out 5 days a week and work in construction weight is about 250 pounds . Got too much fat around belly but it’s been hard to get the same results the last few years. I haven’t woke up with an erection in a long time and it doesn’t get hard enough for sex.
      I’m starting it on Wednesday my doctor who gives me methadone is prescribing it . We’re gonna figure out a dose

      @Richard Bell i do get that Rich...I'm 45 myself go to the gym 4/5 days a week,and I've been doing it for years,but I've reached my natural limit man...but that's not why I'm considering trt..just want that 20 yo feelling,you know...that strength...the feeling you can do anything and you can back it up as well...i will get tested to see where i am,although trt here in UK,done properly in a clinic, it's pricey...

      @@TiberiusWorld thanks I know the feeling I used to be in great shape and very muscular even when shooting up drugs. Then I ended up on the streets and lost all control and then got on methadone after getting some charges. Met a woman and had to use viagra sometimes or a tiny ring thing that goes down there to keep blood flow there. Then she realapsed on drugs and died in a pretty traumatic way so I never tried to find anyone else to be with since . It’s been 3 years though since she died and want to be able to feel like a man again.
      The only thing is my bottle of test says 100 mg once monthly. I’m gonna talk to doctor and see if that’s right . I also got some testosterone coming from another source to have on hand but that’s not legal so not covered by my work insurance.

    Great vid, I'm turning 50 this yr an have been on TRT for about 2 years. I strength train with kettle bells an resistance bands. I was a teen body builder an have been in fitness for about 70 percent of life. No problems with sex drive but I have been experiencing PE, it sucks. I'm on lexapro. Any advice.
