Testosterone Therapy (TRT) - The Good, The Bad & The Ugly | With Dr. Osborn

  • What is legitimate Testosterone Replacement Therapy (TRT) and when is just a guise for steroid use? There can be a very fine line between testosterone replacement (TRT) and medically supervised steroid use for the purpose of enhancement. I sat down with my good friend Dr. Brett Osborn in West Palm, FL, who is a neurosurgeon and an expert in endocrinology, to get his opinion.

    Good video. I wouldn't agree about a certain age to begin TRT. I started at 25 and I was below 300ng/dL and plenty of low T symptoms. Im 40 now. TRT isn't about treating numbers, including age. Lots of info available about modern men, even in their 20s having low T issues. All the side effects he mentioned are manageable by donating blood, clean diet, and taking an ARB. One issue that really is a bunch of BS, is how a young female can get test to transgender, but if you want some to better your life as a man, it's looked down upon due to toxic masculinity. Never forget who signed into law the Steroid Control Act of 1990. FJB.

      I agree, for the most part. I do, however, have an issue when any healthcare practitioner start to use superlatives in their speech since we don't have nearly enough scientific data to form any absolute conclusion when it come to maximum safe test levels or doses. Even if we began tracking it intensely today, it would still take decades before we got to that point where we could form those conclusions.

    I am just commenting for the algorithm as more people would benefit from watching this. Thank you for that. I have a few people I will share this with. I am a personal trainer and primarily train seniors, but have some younger ones. One of my clients (76 y/o) has a cousin that is on TRT (do not know if it is prescribed), and now he wants to look like his cousin who is 2 years younger than him. I told him to talk with his doctor as that is outside of my scope. Only a doctor would be able to determine if it is the right thing for him.
    I suspect in the field I am in, far too many people are slanging PED’s left and right. I have never seen so many 16 and 17 y/o’s looking like freaks of nature. This influence really does a number on 60+ y/o when they expect to look like a 20 y/o.

Most aptly - more productive, safer, and conservative AAS use than most back in the early 2000s. Got a degree in exercise science and nutrition with sports endo hybrid. Got a MS in human performance+ injury prevention. Kept studying endo. Kept studying both synthetics (AAS)and bioequivalents through my 20s and 30s. Plenty of A4M seminars and worked with plenty of MDs. 20 yrs of consulting later - I have never stopped studying. Also have never stopped taking test because crashing T after 5 years on gear is just silly when thinking a few shots of hCG and some clomid tabs will fix the HPTA (aka T-production) - even when I was 25. For the past two decades I've used approximately 250-300mg every 10-12 days. Almost exclusively Sustanon but cypionate and enanthate can work well also. Recently I've compounded isocaproate because it has the highest of test weight to ester ratio of any T ester short of propionate or suspension while still being a viable alternative with a 9 day half life (I can take less test). I feel doctors in the states will eventually look into this method.
    Lastly:
    I've had very elite clientele that previously worked with doctors virtually absent of any real knowldge in this space.

I like to target the mid-700 to the very low 900 range on my end of week test. I have a metric ton of autoimmune issues including thoracic outlet syndrome, reactive arthritis, and closing spondylitis to a degree and hashimoto's thyroiditis all triggered by an immune system scrambling during a meniscus repair back when I was 34 years old. I've been taking Enbrel immunosuppressant for the last 18 years to control the random inflammation and damage but have only stopped working out for one year way back then. I've also recently introduced Deca Just to see what that does for my physiologically induced joint pain in my lower vertebraes and knees to see if I can get some relief there to allow me to continue working out. All of this I religiously use your very structured workout routines to protect those joints ligaments and tendons. My shoulder workout never exceeds 20 lb on any movement isolating small parts of each muscle group instead of overburdening the joints and ligaments. I have enjoyed sizable games from your workout techniques in concert with reasonable TRT supplementation. Best of health to you sir and to the doctor in this interview... For those of you considering trt, just KNOW, it causes your natural T to not produce, unless you have HCG. That's a serious commitment, so think wisely. If you're over 50, OK, which has less implications. Although, HCG is now illegal. Be wise it's a lifetime decision, yes tons of benefits, but also count the cost, my guy sold me into it. Started trt a month ago and decided on ageless men's health. I didn't know what to expect before but got my first labs back with a mid 200's total test. Bioavailable and free were beneath even the acceptable range. Went to the men's clinic and got more labs to find that the numbers were low 200's.
    All that explained my sleep being strained, mood swings, disinterest in sex, gym gains slow, motivation gone, etc. The doc was real certain that we could reverse all that.
    On April 3rd they started my treatment and adjusted it once, I'm doing 170mg test cyp, split dose on Mon+Thur. I'm glad I started it because I've been going slowly uphill and I've got more energy for my family and I'm finding my motivation again. My doc said we're just scratching the bubble and there's more room to grow. I'm 32, 5'8 197lbs 30% bf.
    It was a hard decision to hop on this treatment but videos like this give me more fuel and remind me that I'm doing this for more than me but my family too. But I can tell you that the workload I can handle at the gym is more towards what I expect for myself and I love that. I would encourage more men to make sure you know what you're doing this for and stick to it if you pull the trigger because if I can expect a lot more benefits including becoming a better version of myself then I'd do it again for myself and my family. I'm seeing some benefits to start so I'm stoked to see more. Take care of your health guys ✊

After about 3 months (maybe less) the volume started coming back. I'm working my way up to a year here in April and I've got no complaints now. Everything either is to the level it was before or is satisfactory to my standards now, since before TRT it wasn't an issue.

      @raidenjames8075 Месяц назад

      @@gsomethingsomething2658 I've noticed a lot less than I would have produced before, but drinking a ton of water fixes some of that. It's less than 50% what I used to produce (if I remember correctly, it's been over a year now). Initially there was a drastic drawback, but now I've increased from that spot. I've learned that water is incredibly important to load size and consistency. I use some ZMA and that helps too, honestly this is a big topic and there's a subreddit for this talk too lol.
      Volume was honestly probably the thing I missed the most since I didn't recognize how manly I felt with it until I didn't have the same amount. The pros are that I'm feeling and looking bigger, my energy and stamina are way up and my workload at the gym is intense. I'm down to 184, 16% bf, I can keep up with my kids and my mood is more consistent. Total upside and small sacrifice so for me it was a win.
      Now I coach other guys on nutrition to improve their levels naturally and how to eat when they're on TRT. It's similar, but most times that structure is comforting when the labs come back. Being on trt means watching total health too and there's never a shortness of benefits to that.

      @@IgorTravelsTheWorld for me man, no hair loss at all. In fact I might even grow a beard finally lol. I'm noticing more body hair but mostly on my stomach, some hairs along the facial path that could make a beard eventually, and I shave my head weekly.
      Some acne yes, but only on my back. A good exfoliating brush on a stick takes care of it. And as far as side effects, it's usually only if I have been dehydrated. Blood thickens when you're not getting enough water so I got myself scared one day when I cut myself shaving and I didn't bleed. Got labs done and everything was alright, turns out I just didn't get enough water for a few days.
      Eat a good balanced diet, get enough sleep and water, exercise (things we should do anyway) and there are no major side effects (other than decrease in seminal volume). It's actually been a crazy boost in performance at work, gym, family, and for myself in general.

    That doctor seems good and intelligent. I get a "no BS" impression from him.
    I think I'd really benefit from TRT. I fit just about all symptoms of low T and certainly do have low T when I got tested via LetsGetChecked in 2020 (think sub-400, and I was 37 then). But I dunno, I'm extremely hesitant because I don't like the idea of being on a drug for the rest of my life like that. And if I didn't have it, I'd feel even worse. That just seems like a prison to me, almost. I'm trying to unfvck my sleep (when I tested myself in 2020 I had been on 4 hours of sleep a weeknight for a year or so) which is still horrible, 4 hours of sleep a weeknight roughly, and the crap water where I live I'm sure does not help. I think a lot of environmental things can impact your hormones, even what you use to wash your clothes for example. So I'm trying to clean up that stuff and gonna test myself again, maybe this coming summer.

    I recently watched a video(Thomas DeLauer) that recommended DHEA, which is supposed to work as a hormonal precursor as a more natural way to increase testosterone, but it's also something not evaluated by the FDA. Any thoughts?

    • @JamesGrage
      @JamesGrage  Год назад +6

      Hey my friend. Great question. Definitely one of my biggest issues with social media is quasi-experts making recommendations when they are not qualified to do so. DHEA is a pro hormone that can act on more than just testosterone. There can be less desirable responses such as changes in estrone and estradiol and reductions in HDL (your good cholesterol). To call it a natural alternative is a misnomer. It’s a hormone produced in our body, just like testosterone, so to try and manipulate that without the supervision of a physician is inadvisable in my opinion.

      Its the most abundant hormone in your body . Each person is different when taking . Most men do good at 100mg but you can get headaches at first usage and your stool will lack water making it dryer very much harder to shit . Maybe try 50mg at the beginning of the day EOD and adjust as you go. One thing is it will help with body fat and chaff hunger. If you want a better way to increase testosterone . Take a spoon full of Tongkat Ali in liquid form every morning . This will increase your free testosterone.

    I've held off getting on TRT for the last 3-4 years in spite of a significant 5 year decline in some of the physical and most of the mental aspects associated with low T. I've strength trained, eaten healthy, kept muscular and in good shape since I was a teenager. The last thing I wanted or ever thought I'd have to do was get on T therapy. However, in spite of my best efforts, my numbers kept declining. Currently 350 total and 6.8 free T at 49 years old. Willpower cannot consistently overcome this lack of drive, confidence, and energy (which I was overflowing with until my early 40's.) Everything, even the things I used to love to do, seem....all vanilla. I still hit the gym, outlift, and have a better physique than 80+% of the (natural) 20-40 year olds there. On some level I thought if I still had good muscle I couldn't be low T but the mental effects have just gotten to be too much. After a final extended effort to turn it around I've concluded it's, unfortunately, not going to happen. I want to enjoy life and feel unstoppable again like I did before. This morning I was prescribed an injectable T therapy program from a clinic who's philosophy is in line with the good Doctor from this video. Don't like needles, but I'll get over it. Will be starting next week. I suspect I will wish I'd started 3 years earlier.

      I feel sorry for you. You feel the need for a steroid to complete your life, its sad really. I mean no disrespect here but sad. My wife's call name here and I'm a 63 year old male hitting the weights 4 days a week. There may be multiple reasons for your declines not just test; too much of the stress hormone cortisol, chronic stress, hyperthyroidism and hypothyroidism, diet, etc. Sounds like you are looking for the easy way out, drugs. Now I know why the FDA is looking closer at this. Remember pain killers started as a trouble free way.

      @@dawnmarie5872 Thank you for your sincere concern. I'll take the time to give you more information so you may avoid jumping to conclusions and irritating people in the future.
      I've had broad hormone/nutrient profile testing done over the last 20 years because health is my utmost concern No, there is not a chronic stress or other underlying thyroid hormonal issue. I've spent the last 4 years working on ways to avoid T therapy. It has always been my last option. I've always been vehemently anti anabolic steroid use and never needed them to build muscle even though my T levels have never tested above 550 (late 20's). I have a varicocele which could explain my historically lower levels. Last year I suffered a concussion injury which my doctor feels may have contributed to this recent decline. Please direct your "pity" towards the 20 somethings looking for any way to get on drugs to get buff fast and easy. I'm ok with you feeling somehow superior to me. Thank you for taking the time to let me know about it on a public forum. Consider this, you too depend on a steroid, Testosterone, to complete your life. It's just that your body is still making levels above the threshold where you would start feeling how I do. If you get a TBI tomorrow and your T levels tank in spite of your best efforts, please, by all means don't resort to a drug. Endeavor to persevere as you rapidly decline mentally and physically and make your family suffer right along with you. Sound fun?
      Maybe consider walking a mile in other peoples shoes? If not, perhaps next you can visit a type 1 diabetes video and try to shame someone off of another anabolic drug, insulin. Let them know you don't need it and that you pity them. Maybe they will find your positive input valuable. No offence intended. Thanks again.
