Signs Your Thyroid Medication is Too Low

  • Do you feel like your thyroid medication just isn't working as it should? Do you still have symptoms of low thyroid even though you are taking your thyroid medication faithfully? It could be that your dose is just not high enough and that you need to increase it.
This problem is actually more common than you might think.
It's been estimated that as many as 25-30% of thyroid patients may be taking too much thyroid medication at any given time. We don't know the numbers for how many people are not taking enough but it's probably safe to assume that it's at least equal to the number of people taking too much.
This promotes a big problem for thyroid patients because you simply won't feel better if your dose is not sufficient.
The good news? There are ways to determine just based on your symptoms if your dose is too low.
Let's talk about those right now.
Signs that your thyroid medication dose is too low include:
#1. Gaining weight despite taking your thyroid medication every day
#2. Fatigue that is getting worse or doesn’t get better
#3. Hair loss that either gets worse or persists
#4. Continued constipation (may get slightly better or worse)
    #5. Cold intolerance that persists
#6. Continued menstrual problems (may also get worse with some thyroid medication)
#7. Depression and/or anxiety that persists or get worse
    #8. Pain in the joints/muscles
    #9. Dry skin
Do you feel that your dose of thyroid medication is optimized? Or do you think it needs to be adjusted?

I wish GPs would take under active thyroid seriously. They just aren’t interested and are quick to dish out inadequate doses of cheap, ineffective bog standard levothyroxine. No sympathy at all for patients whose symptoms can make them feel as though life is not worth living. GPs need much more training on such a common complaint. Doses solely based on TSH is just not good enough. We’re talking about quality of life here! Change is needed!

Absolutely agree 100%. It took me 8 years of hair loss, weight gain, intolerance to cold, hot sweats at night, pain in joints etc fighting with doctors and them refusing point blank to test me. Eventually after changing docs a number of times and getting an at home test done, I was put on eltroxin (levothyroxine) by my new gp. However, she's saying that they are only allowed to prescribe eltroxin for underactive thyroid and the bloody thing is making my hair fall out quicker. Sick and tired of being sick and tired.

In case you haven't noticed, the medical profession has been proven corrupt over the past 2 years; they're beholden only to Big Pharma, not the patients they're supposed to help. Expecting them to take patient complaints seriously is like expecting the gov't to care about high taxes.

Be sure to ask your dr to run a full thyroid panel and not the typical labs they run! I really am glad you are shining a light on this subject. The issue is when you feel these symptoms the most of the drs don't listen.

Why do Drs. Not take full panels, I don't understand, you tell the Dr. what you went through ,from beginning, I was graves, overactive thyroid, had to drink the nuclear radioactive ,had a nodule on right side, of my thyroid, this is what I tell Drs. now I've went to so many Drs. just to find a Dr.that will take a full panel of freet3, free t4 ,T3, t2,t1,t5 . I've fallen into depression, cognitive, my muscles ache, 1Dr. said he wants to see if he lowers my mcgs if it will slow down bone lose, I was overactive 20 yrs. Was diagnosed in 1994.
😢 Is it I'm not important, is it I can't remember what and they want to get me as low as they can go,
I'm just the mouse to experiment.
I don't like living shut out of the world, afraid I'll say something wrong, what will people think of me now the Dr. has me on a cholesterol medicine, antidepressant, wouldn't it be better to make PT. fill better?
      Confused , unworthy mouse.🥹🥺🤯😡

I have benefited from your videos in last two years, but I noticed weight gained with nothing change, Does it means that we have to continue adding dosage over the time? I had very good results on levothyroxine & Liothyronine , but seems they don’t do the same after 2 years now, should I change to other medicine or should I just adding more dosage?

The other way to adjust your thyroid med is the TSH level that should be between .5-1.5 at least 4-6 weeks after any change in the dosage. Besides the requirement of thyroxine changes with age pregnancy and weather conditions.

@Dr. Westin Childs How does it come that my body needs more and more thyroid medication overtime? I have Hashimoto and I started with 100mcg L-Thyroxin which seemed to be fine due to TSH being optimal 4 years ago. Then I had to adjust to 125mcg and now I am up to 150mcg because my TSH went to 5 which is not optimal. I still have to check if 150mcg is enough to get into that 1-2 range. But there is a clear porgressive increase of dosages and I wonder why, what does that mean?
I supplement with Zinc, Selenium, Iodine, Ashwaghanda, Vitamin D3.
And I still have high antibodies and my body seems to ask for higher doses.
Does that mean nothing is working? Is my thyroid being destroyed despite all the supplements and adjustments of dosages? Is 150mcg very high?
I hope you can help me!

Thanks for sharing this information Dr Child,I’m 65 , I have been taking levothyroxine 25 mg for 6 + yrs. Went for a physical labs showed test wasn’t normal. My doctor went up on my medication to 37mg. I feel worse my hair is thinning more . I have more heat intolerance and I’m very frustrated. Also I’m wondering if I should be taking iron to see if some of my hair might regrow and what type and the dosage . Which other thyroid medications would you recommend few or less side affects with my hair I have to pretty much starve myself to lose weight the things you mention, in the video are the things that I have issue with while taking this medication. I actually stopped taking the meds for a whole year and my test showed normal go figure and I felt fine or the same. My apologies for the long comment. This has been on my mind for years. Why am I taking this medication if it makes things worse.

Hello and thank you for all your very informative youtube videos.I am watching them all .
I take Thyroid-S, which is an NDT medication as I'm sure you know. My question is that surely NDT doesn't have T3 in it because that is made by converting T4 to T3 in the liver. I read that the thyroid gland makes T1 T2 T3 T4 and calcitonin.Which is correct, does the thyroid gland make T3, or is T3 made from T4 by the liver? Also, could you please tell me how dangerous overdosing on thyroid medication is? Recently my GP's surgery informed me that I was taking too much medication, yet they only ever do a TSH and a peroxidase test. They never do a free T3 and T4 test, let alone reverse T3 and T4.I did not feel any symptoms of taking too much and now I have decreased my dose, I have symptoms of hypothyroidism again.I think that you must be aware that in the UK there is no choice of thyroid medication, only Levothyroxine is ever prescribed. There are a few docto
rs who will prescribe it, but I believe they must be private ones.I buy mine fromThailand and have done for fifteen years.

Hi Dr. Childs thanks for making this video. I just checked my lab work. My previous PA who worked for a endo put me on a child's dosage only 5 mcg of cytomel. I am with a new Endo doctor and my T3 levels are still low. I only found out this by reading the patient leaflet online at the manufacturers website that 5 mcg is a child's dosage. I also started zoloft and I found out from the patient pamplet that my dosage also needs to be increased while on cytomel because zoloft increases the need for more cytomel. I am going to ask him to put me on a adult dosage of cytomel so I can get my T3 levels up.

Hi Dr Childs, several years ago I was completely over weight. I’m 4’10 and was weighing at about 140 lbs. I tried losing the weight my exercise and eating healthier meals but it didn’t work. My daughter got sick from gluten allergy/intolerance of some sort and It was hard on her so I decided to go gluten free with her. My heart broke for her since she couldn’t have all the same meals as a family I guess I could say. So I noticed the weight started coming off just by my diet alone. I then, started to excersice because of it. I got a mommy makeover surgery and then afterward I went and got a physical my lab work and my doc said my TSH levels were elevated. So she put me on 25 mcg Levothyroxine. I’ve had the symptoms of hypothyroidism I just didn’t know that’s what it was.. I’ve been on it for about 4-5 weeks now and have seen a huge difference but I still wake up tired some days, I get sometimes get extremely dizzy sometimes and I slowly gaining weight despite eating healthy meals at home low in sodium and going on my 60 minute 3-4 mile walk/trots. Should I ask my doc to up my dose or should I just slowly get off of it ?
My labs showed my tsh was elevated to 6.5….

I’ve been thyroid free since radioactive iodide treatment in 1988 - it just kept dissolving away and I kept gaining weight even after diagnosis of hypo in 1989, less than a year after treatment weighing in at 120 and a year later up to 140. Gaining weight every year and going through menopause at 40 years old. I ended up weighing over two hundred pounds even though my gp was giving me Levothyroxine-increasing dose every six months. Finally went to an endocrinologist when my gp suggested bariatric surgery. She wasn’t listening to me at all for all those years, thinking I couldn’t control appetite or exercise enough. Still battling to get to my weight of 135. 2023 and there was only one time I could lose and maintain with help of my mental health practitioner. She gave me Evekeo in 2020,2021 and in 2022 my insurance and drug store said it wasn’t available and gave me amphetamine salts. So for two and a half years I could fit into my size 8 clothes again! So careful with eating, skipping 2 meals a day, walking, yoga, and hard labor in the yard and house. But right back to 170 again, unable to lose. The Evekeo was key,but my endo just keeps lowering my meds. I was on 250 mcg in the 90’s all the way up to 2016. Then endo tried to help, but he’s hooked on fact my TSH is symptomatic of hyper, even though my t-3 and t-4 are in “normal” range. 5’9” tall and killing myself to get to under 170. So unhappy to be in this state for so long. Up until I was 37, I kept weight at 135. Then wham… now 67 and still not right.

My normal weight until I was 60 years old was 122lbs. In one year I went up to 170 and then 215 even on Levothyroxine. I realized my body was not making T3 from the T4. In desperation I went online and found natural, dessicated thyroid hormone. It worked! I am no longer so depressed that I cry myself to sleep every night, I have more energy and am down to 170lbs and eat whatever I want. I no longer can exercise as I am now 80 with COPD and diabetes but I feel good, look okay and am happy. By the way, the doctor still thinks I am taking Levothyroxine. What he doesn't know won't hurt either one of us.

I was taking synthyroid 50 and T3 Lio 35 mcg my TSH went down to negative 0-01 my T3 went to 8.8 It took me 3 years of slowly adding 5 mcg. I turned hyperthyroid and it happened very fast some of the symptoms are the same as hypo. I was irritable, anxious, cold, my hair wasn’t falling off it was actually breaking off, and thinned out my nails would not grow and started lifting from the nail bed. I had to start again with the initial dose I got of Levo 75 mcg in 2018 just so that I could get my TSH up, I will see her in four weeks to see it goes. Symptoms are very tricky you can get cold feet with hyper too. My TSH came up from the -01 but my T3 was 2.2 and my total T3 was 133 it’s really crazy. It’s important to journal your symptoms and when you change your doses so you know when you did what or felt what it’s easy to get overwhelmed in this process it took me 3 years to tolerate Liothyronine in increments of 5 mcgs we went slow too but if you don’t track it, a lot of things can go wrong. It’s a long journey to find your right dose even when you have a Dr that is working with you.

I'm lucky to have a GP who looks @ T4,T3,+ TSH & listens to how I feel, what I think. We do my labs every 3-6 months. I get labs drawn the week before my appt. so we can go over the results together.
We have lowered my dose steadily over the last 2-3 years. I was @ 130 mg NP Thyroid twice a day, now @ 90 twice a day. My heart rate kept being high (100) + lab results agreed. I put myself on many supplements over the years. Zhou Thyroid Support has helped.
1 frustration is that I abruptly became very Anemic (9/29)with Very low iron levels (Can cause higher heart rate).
No wonder I was so tired + very pale
Last Fall, My Opthamologist had me reduce red meat for the beginnings of Macular Degeneration. I'm Keto + didn't eat many vegetables + can only eat berries, no other fruits as I am a Stable Pre-diabetic.
Also, I've been on Prilosec for GERD for years.
I have Gastritis + Diverticulosis (upper + lower GI Scopes looking for bleeding/ causes anemia.), My H+H was 10/32 Had been taking Hemaplex iron TID.. (Now I know why I don't tolerate black pepper + spices)
I removed one of my digestive enzymes pills that had black pepper in it to one containing BetainHCL (Low Thyroid peeps have low stomach acid/ upper opening of stomach doesn't close well with low acid)
BetainHCL is supposed to increase stomach acid.
2 days later I skipped the prilosec, no Reflux. I took 1 more Prilosec + stopped. I've only had 1 episode of Reflux in 2 weeks! Yay!! no more Prilosec.
I am still taking Hema Plex iron + Added B12 sublingual, eating more lean beef, increased veggies.
My color is better + I'm not so tired. Will repeat labs next week.
Hopefully, my anemia + low iron will be improved.

So I was put on just the 75mcg. That's when the shaking started. Got TSH checked again (maybe too soon) and it was 1. So good. Evaluate what tests are in normal levels. If only testing for TSH, T4 and T3 they almost never will never diagnose a thyroid issue. I even had an endocrinologist who couldn't diagnose thyroid issues.

i am so confused, started this levoxyrhine 25mg in late july . i have gained 10 or 12 lbs in 4mths,keeps going up despite uping dose to 50mg twice a week, hunger increased, fat increase. i am also on HRt for osteporoiss at 45. so i dont think this drug is working, i am depressed and anxious , i also have POTS and usually very fit person but the dizziness has curtailed activty. my appetite increased since october , i am thinking this dose too much or maybe i dont need this. my tsh was 2.68 now its .9. my t4 just normal at .9, same with t3 it is 2.1 just normal. this is only sign meidcine is working , my t4 was .7 in june. but i dont feel better and weight is piling on quikcly. was always 106lbs for 2 years now 118 in 4mths
