Aged 73 started intermittent fasting 1 meal a day with low carbs 3 1/2 years ago, after being diagnosed with diabetes 2 and A1c 11.5. Within 4 months it fell to 5.9. Reduced A1c still further within 1 year to 5.3, which is normal. This has been maintained even though I only fast now 20 hours per day and eat 60 gms carbs per day. No medications at all. Many other benefits such as normal weight, more energy, sleeping better, lower blood pressure, and rarely sick. Sailed through COVID without contracting it, unless I was asymptomatic and didn’t know ( also supplemented with vitamin d3).

I've been doing 18/6 2MAD and unprocessed food for almost 5 years. The biggest surprise has been that it gets easier and better over time. When I do eat processed foods or an extra meal it is so I can accommodate someone else(for example, my mom wants to make me a birthday cake) and the natural thing to do is get back to normal, no willpower or effort needed. Actually, the most shocking thing is that my asthma went away, and stayed away. That is life changing, and I'm never going back.

I gained 60 pounds in 6 years, became obese, constantly stressed and found out I was diabetic last March. I have a doctor who encouraged me to reverse my diabetes, started IT, changed my diet with lowering daily calories and no processed food and exercise. To reverse type 2 and prediabetes, there are 3 important things we should do, every single day, exercise, eat healthy with proper nutrition and intermittent fasting. It does not happen overnight. It requires patience and persistence. I reversed my stubborn 20 years prediabetes (did not know I had it. Doctors never mention on my yearly blood work). I started IF and exercise daily, vigorously religiously and eat low carb foods over a year. Blood glucose down from 117 to 75 - 85 fasted test. A1C from 5.9 down to 4. - 5.2. There are still the days up and days down depending the food I eat and the quantity. Overall not bad like a year ago. I noticed a huge improvement in Health and also even the days I break rules during vacation and occasionally, blood glucose spike and go down quickly at the base after 3 to 4 hours eating. Am 70, athletic and fit. I believe also the age make a difference too. If you're younger and take good care of yourself and following strictly the rules then you can reverse your health issues faster.
Thank you for a such informative video.

I've consistently done one protocol or another for intermittent fasting for 7 years now. Before that I suffered with acute pancreatitus. The diet was terrible and barely kept it at bay. The medication made me dizzy and I would still have flare-up, sometimes requiring hospital stays. All of this was caused by hypetrigiserdes. Finally I told my Dr I was going to start fasting.
At the start my triglycerides were 780 and my hdl was 29. 3 months later all of my numbers were in normal ranges. Plus I was finally able to look at a glass of wine without having an attack. Intermittent fasting not only works for me, it gave me my life back.

I was addicted to food and always hungry. 2 months....I repeat 2 months and my levels went from ridiculously high to never higher then 6.4 in about 60 days of intermittent fasting mostly 20-4 but started with 16-8. I ate healthy, stopped from 8pm onwards to about noon or later and I'm feeling better then I ever have. Have blood work tmr. Going to compare.
My mood is better, I stopped napping and feeling miserable and edgy. I sleep better.
I also exercise moderately and do strength training but it's never more then 1.5 hours a day and usually around 30 mins or 1 hour. Nothing crazy.I sometimes have a day where I end up skipping but try not to. I do weight lifting 3 times a week sometimes 2 for a hour and usually bike several times a week. Pretty big change for me but nothing to difficult. I had diabetes for 8 years at age 27. I'm now 35.
I think intermittent fasting is the biggest reason for my turn around.
Thanks for info it keeps my head on right.
Also to note: I no longer feel extreme hunger believe it or not it's actually taught me self control and I had absolutely no self control ever in my life with anything including drugs. I stopped those also. This was by chance and I can't explain it but it happened and it work.

Very interesting video. Thanks for sharing.
Diagnosed almost six years ago as T2d, I was fortunate enough to be given the opportunity to try lifestyle and diet interventions before resorting to more conventional poly-pharmacy . With an A12 of 12.5 the diabetic nurse initially recommended insulin, along with statins and the usual candidates. Luckily, I walked out with only test strips and a glucose meter. Six weeks later, my A12 was 8.8 . Quite a few heads appeared in the door to see who this individual was.
The nurse was interested to see how I’d achieved this miraculous drop in A1c in such short space of time. When I explained that I’d cut out bread, rice, potatoes and pasta - not to mention sugar and junk food - the reaction was disbelief. Clearly, a first time in her experience.
Six weeks later, with an A1c of 6.1 the diabetic nurse was delighted. When I explained that I’d also started intermittent fasting, she admitted to following a similar pattern of eating. In her case, the primary goal was weight loss. Of course she would never be able to suggest this potential intervention to a patient.
Today, I am still in remission, with the most recent results of an A1c test coming in at 5.8.
I still get the post test call from a GP suggesting that I take statins to lower my risk of premature death, but I always decline the generous offer.

Thanks for putting this one together. I have lived the scenario you outlined... and to honest I retain some disappointment with the system that channeled me down that path. I am in my 67th year, I have always been active, I have been managing Type 2 Diabetes, one way or another, since diagnosis at age 34 - oral medication included over the last 1.5 decades. Additionally, I have always been active, and still am relatively. I have had Cancer and a heart attack and had surgery for both. Recovery was successful and I have enjoyed a continuation of the pass times I enjoy so much - trekking and exercise.
I began to investigate Intermittent Fasting about 4 years ago while still taking oral medication for Diabetes (I have hyperglycemia). Eventually I found my way to a 24 hour fasting cycle ... and experienced occasional Hypoglycemia. I was struggling with knowing what was ACTUALLY happening with my body. This confusion and dependency on the medical system was getting me down, I wanted to be done with medication all together. So thought long and hard and ... I tossed the medication. My thinking was that I needed to know what my baseline is. In other words 'how long do I have to fast to achieve normal blood sugar levels'?
I learned that for me that duration was 48 hours. I researched food options and fasting strategies to try to get a routine that was optimal. That process is still ongoing. Until recently I had achieved a resting level of 8.0, which is not in the range I would like, but headed in the right direction. A couple of days ago after a difficult trek I achieved 4.8, which is very good.
There are issues, ongoing. I have a calorie deficit, and am losing weight (the Cancer had already plundered my muscle mass). I am on the look out for a high calorie zero carb component to add to my diet. I am already eating salmon, meats, fats. I had tried avocado but found that the carb content was affecting overall levels. Given the recent low reading I added a small avocado to that evenings meal, WIN!
If you can provide some input I would very much appreciate it.

I eat healthy lower carb diet now. Intermittent fasting gives me a lot of energy, especially mental energy. If I want, I can go 36-48 hours without food, but I get cold which is unusual for me. But I feel mentally very sharp when I do not eat! Like having lots of coffee without the palpitations.

Intermittent Fasting = Do IT! lol. The easiest for me has been 16/8, simply cutting out breakfast most days. I've seen a tremendous improvement to quality of life over the past year or so I've been doing it. Incorporating whole foods has been a challenge but I'm getting better and better at it as the days go by...plus it can be costly...in the end it's worth the improved health. Thank you Doctor Mike for making this easy to share with my loved ones!

Great talk Sir. Really appreciate it. Surprisingly, if you do some reasearch on JAIN food patterns, you will find that they have been observing intermittent fasting for last 2600 years. In Jainism, it is referred to as Ekasan (eating once a day), Biyasan (two meals a day) or Chouvihar ( no food or water from sunset to sunrise). Most Jains do not eat after sunset, as presence of Sun is considered an important aid in digestion. Good luck in your future posts. Thanks again! Keep up the good work. God bless. ❤

I am a poster child of insulin resistant weight gain. My doctor r did not help me at all even when he told me I was IR. At the time exercise alone did not help. And I did plenty of it while I went from 170 to 192 pounds.
Enter keto. Over a matter of 6 months, my weight dropped 12 pounds and after about a year, I was below my goal of 170 where I am today 6 years later. I had been playing around with IF and was on and off it quite a bit but last year I began to do it daily with either 16 or 18 hours of fasting and 2 meals. (Sometimes I go longer than 18 if I'm not hungry.)
Bottom line is I'm doing well and my blood work remains great even though I am now eating more of a low-carb and balanced diet regimen. What that means is I do eat some healthy carbs almost daily. I do not deprive myself on holidays and special occasions so dessert is on the menu then.
I do eat my sourdough bread; I do eat high quality pasta; I do eat my homemade yogurt and other cheeses and I do eat sweet potatoes and an occasional Idaho. This all seems to be OK now.
My basic tenant is to let no corporation prepare anything I eat. I live by that and at 76, I plan to continue for quite a while longer.

It doesn't seem to have any relationship to my weight gain or loss. For example, my last checkup showed I'd gained 5 pounds but my A1c level had dropped to 6.2. That was after 3 months of intermittent fasting which helped me drop 2 inches off my waist. I recently added a rebounder and near-infrared light and between fasting, exercise, and light therapy I hope to get my A1c level down to less than 6.0 despite my love of grains and dairy.

Thank you so much. I became very ill when taking that new "wonder drug". I was confused as hearing you, I was "You are right, it seems to be a sign, not THE disease." One thing is so many of the doctors I have seen, with "Hey I'm putting on weight, why?", was met by "You have to lose weight." I was "So how? What do I change?" and it was all "Lose weight". I changed my diet drastically but also found the weight stayed the same. I went to a special nutritionist who had me doing special recipes, even though I told her because of my disability I can't stand for long and my husband does the cooking. Well, he could NOT go as the insurance would not cover him attending her cooking classes. It was, and still is, lose weight, only it appeared to me no one was losing weight. Until the new drugs that have come out. But, I just wanted to know WHAT caused the weight gain, I didn't wake up one day and start eating more and if anything move MORE because of my disability and attending physical therapy several times a week (plus working out at home). The belief, still, that if I lose that 20 pounds, I'll be fine, and this is TOTALLY ON ME, and some woman that wants me to cook three meals a day, is insane. This is at least something to discuss with my primary physician. One thing, the focus, non stop, on weight, with little support or help, is very depressing. I like doing my PT, I like to manage my disability, but there is no help at all with weight management which is simply a sign of what's going on.

So now it's 15/9 or 14/10 depending. It's so easy. But you have to be somewhat aware of your intake during the eating phase. Best wishes.

Oh thanks doctor for this info and Inter. Fasting does work for me. But in this age of COVID and the season of influenza, how important is it to get a balance of nutrients, that is, load up on nutrients, when you are sick rather than do a fast. I know that there is research done that shows fasting does boost the immune system to some extent. Now that I have some respiratory symptoms, whether it's a cold or something else, I would rather eat and get high density nutrients in food. Thanks!

You don't have to double down on fasting. You can ease it from time to time as long as it emcompasses the meta of your eating habits. Though it is worth noting that it is probably best to eat even more healthy when you are not doing IF.

Without telling my doctor, I took myself off of prescribed statins (cholesterol), high BP meds, and metformin (diabetes) and changed my diet to keto with intermittent fasting. 9 months later, I dropped from 192 to 150 pounds, ~20% body fat, fasting glucose 101 mg/dl, A1C 5.5%. BP is lower than when I started this journey and doc did not put me back on BP meds. However, I was living high-on-the-hog cooking way too much delicious saturated fat and my cholesterol/apoB levels went thru the roof. The food was expensive, delicious, and very sustainable during those 9 months, but now I'm back on statins and saddened that I need to significantly cut back on saturated fat. But that's an easy change compared to weening myself off the addiction of sugar and junk food.
Also, exercise, alone, does not help at all. In fact, even with a poor diet, I gained more weight with exercise. It wasn't until I changed what and when I was feeding myself did things turn around for me... and quickly, too. For me, I had to do all 3: exercise, keto, and IF.

Thank you thank you thank you! I have type 2 and am in the beginning stages of reversing, working with doctor residency changes, so moving doctors doesn't always help. I am being prescribed the new injectable to lose weight, so that should be interesting. I had to cut sugars completely, increase fiber, and learn to use net zero carbs, and consume high amounts of proteins, some healthy fats so I like to eat avocado right out of the skin, not sure if I am stating all that correctly... :) I hope to reverse this disease for good. Also trying to figure out how to regenerate healthy beta cells. One thing I learned recently this week is that caffeine inhibits proper insulin release, it blocks the insulin. So, I am going to probably cut coffee. I try to drink no more than 12oz per day, but I think it is an important step to stop all caffeine. I use Erythritol in place of sugar and is supposed to be a healthy substitute and tastes just great in my coffee. I appreciate the advice and just trying to work out a fasting schedule and balance work and life and other activities. My friend is diabetic and reversed by fasting, eating once every 24 hours with net zero carbs, no sugars, good fats and proteins, etc. So, I am confident I can reverse mine. Plus, I joined Planet Fitness and working out helps. At my age, my legs get soar unless I work out on the elliptical, walk, or use a cycler. Sometimes I just lift my legs in the air and cross over them for a few minutes at a time. Thank Dr Mike, I appreciate you and I am happy to hear that you have been able to reverse diabetes. 😊

I too am trying to keep my eating window to 8 hrs or less. Eating mostly plant based, some meat. Staying away from processed food. My blood work showed a pattern of rising cholesterol and my doctor said I could take a statin to lower it. One doctor said it would take a year to lower, with drugs and the same for my lifestyle changes. I am always shying away from drugs.

By default, I live the a lifestyle very close to what you suggest with a daily 40 minute brisk walk, light weight training three times a week and a (mostly) healthy vegetarian diet. My weight and resulting BMI, muscle mass and general health are better than they were 30-40 years ago when I had no anatomical knowledge and used to over-indulge in food and booze like many young men of my generation.
However, it is far easier to live such a lifestyle now without the pressures of a demanding job that I had at that time, and I'd guess this applies to other people.

I had a 12.7 A1c when diagnosed with TII. Within 6 months I had a A1C of 5.6 with changing diet alone. I added fasting to be able to increase my carb intake without high blood sugar spikes. I am still experimenting and learning. Some of what is recommended isn't always the best. I never understood why the nurse told me to check my blood sugar 3 times. Once in the morning, once 1 hour after a meal and once two hours after eating. I think the only time to measure it that makes much sense is 1 hour after eating. A high blood sugar is damaging to the cells and that is caused by eating too much carbs or also not expending enough energy to use up your glucose that's in the blood. Bottom line is anything over 125-150 or so is an indication that you're damaging your cells with causes further health complications.

    • @rprastein

Morning glucose measurement is basically a fasting glucose level. If your blood glucose never gets down into a normal range, even when you haven't eaten anything in the last 8 hours, then it's not well controlled. Fasting blood sugar will show this more quickly than HbA1c will, but it is also more likely to show variation. Conversely, if your blood sugar drops too low when you've fasted overnight, then you are at risk for hypoglycemic coma (or lesser degrees of signs and symptoms of inadequate brain fuel), especially if you are taking diabetic medications, and most especially if you are taking insulin.
Measuring blood sugar 2 hours after eating and comparing it to the level 1 hour after eating shows how rapidly you are clearing that glucose load from its peak. Sure, a high peak value is bad for your health, but so is a less high but still elevated value over an extended period of time. Again, if you are taking insulin (in particular, but also oral hypoglycemic agents), monitoring for low blood sugar is actually just as important, if not more so, than monitoring for elevated blood sugar.
With all that said, if your HbA1c is down to 5.7 and you're not on any diabetic medications, I don't know why you would need to be taking your blood sugars 3x daily; I would think a fasting level would be enough. But if you're actively making changes your diet, and/or are being monitored for whether or not you need to be on medication, perhaps that's the explanation.

@@rprastein yeah, no reason to measure blood sugars 3 times a day. A type 2 diabetic usually doesn't have too low blood sugars with diet controlled diabetes. The ones that have to worry about hypoglycemia are the T 1 diabetics.

It is not just processed food. I have a lot of fruit trees and bushes and my problem started at the end of summer eating a lot of fruit, especially grapes, apples, buying bananas and some occational too much alcohol (fermented fructose). My processed food eating was very low and eating overall NORMALLY with eating much more fruit rather healthy. Spontaneous gout attacks stopping me to work/walk for a day, myopia , started all suddenly and schocked me. Intermittend fasting and cutting down on ALL FRUCTOSE so including (high sugar) fruits was what reversed everything.

Yes. No diabetes or anything but (while on WFPB diet) fructose - even in whole fruits - is my kryptonite, too. As well as fluid calories, that is to say juice and plant milks (no fibers)

What do you recommend as a minimum fasting time, Doctor?
I brought my A1c down from 6.8 to 6.1 by cutting my carbs and general food intake. I lost over 35 pounds and now am almost the same weight and waist size as I was when I was 19. My VA pharmacist cut my metformin from 2000 MG to 1000 MG.
My a1c went back up to 6.3 but has stayed there.
Most days, I eat only one meal and minimal carbs at that.
The problem with the VA is that they seem to have a hobby horse about cholesterol and taking statins. They must get a commission off that. They give no practical advice about lowering a1c.

84 year old male. Diagnosed with pre-diabetes, high blood pressure, and using 10-15 mg lisinopril a day, overweight (100 kg), although I was relatively active with daily walks. Eight months ago joined the keto community. Lost 25 lbs, after 6 months my A1c went down to 5.2, and I have stabilized my B-p at about 115/66 with no drugs.
Daily using 5000 i.u. of vitamin D3 plus K2, CQ-10 along with Mg and K. I feel that my blood vessels opened up. Btw, since being on the keto, I‘ve been visiting a gym every second day doing exercises with 80-90% of max weights. Feeling great... and groovy!
