Are you suppose to see good through the one eye you had cataract surgery on or do you need both eyes done. What if you need reading glasses after but have to hold what your reading real close to your nose with over the counter reading glasses. Maybe I'm still healing not sure. I do have some astigmatism. Unhappy right now though. I chose standard lens can't afford others due to bills and needing better income and car is old. I guess you would have to know my particular case to know for sure. Adjustable Lenses (LAL) - My Personal Experience - No prior issues and or surgery’s.. As someone in their 50s who still snowboards in the BC mountains (big falls, rapid impacts), weight trains (heavy) four days a week, and spends significant time working on my laptop and dual screens, it’s important for me to understand the nuances between the different IOLs. Also worth noting, that I have been deemed a candidate for the Light Adjustable Lens (LAL). Since then, I have been desperately seeking an alternative point of you regarding this technology. While it sounds groundbreaking at first, I’ve come across some significant concerns that I think more people need to hear about. Let’s uncover what I call “the dragon in the details” when it comes to this lens. The Adjustment Process: The Hidden Catch The adjustment process isn’t as simple as it sounds. Most surgeons start by setting both eyes to Plano, meaning they initially prioritize distance vision. Then comes the tweaking stage, which involves multiple UV light treatments to refine your vision. In my case, what wasn’t fully explained, or what I may have misunderstood, was how significant these tweaks can be. More specifically, some patients end up with mini-monovision, blended monovision, or even full monovision during these adjustments. Why? Because the adjustments can range anywhere from 0.25 diopters to 1.50 (or more) per eye. For anyone unfamiliar, monovision (not mentioned in your video) means one eye is optimized for distance while the other is set for near or intermediate vision. And here’s the kicker: Approximately 20-30 percent of patients struggle to adapt to monovision. Some can’t tolerate it at all, experiencing issues with depth perception, eye strain, and overall discomfort. This is definitely me, so no monovision! The Dragon in the Details: My Expectations vs. Reality For me, the biggest issue was not fully grasping how much the adjustments could impact the final outcome. Even with the adjustability that makes the LAL unique, achieving balanced vision across all distances, near, intermediate, and far, without glasses often means embracing some level of monovision if I understand correctly?? (Hopefully I’m wrong). Additionally, while the promise of adjustability sounds fantastic, the process can be tedious and involve multiple visits. You might find yourself chasing a vision (my concern) goal that feels elusive, all while unknowingly being nudged toward monovision, even if that wasn’t your original goal. The Alternatives: Are There Better Options? This brings up an important question: with so many advanced lenses and technologies available today, is the LAL really the best choice for most people? I’m currently also considering the Johnson & Johnson TECNIS Odyssey lens that you mentioned in this video. Interestingly, there are other concerns with multifocal that I am worried about besides aberration’s halos, glare, and contrast issues. Most importantly, there are some ophthalmologists, who have indicated that the majority of their colleagues requested monofocal lenses, ( very interesting ) rather than more advanced options like multifocals or the LAL. I’d love to hear your thoughts on why that might be and how this fits into the broader conversation about these newer lens technologies. The only people that I have talked with tells me far vision is the only way to go. This way readers for up close and perfect vision normally.
Great video. I have RLE surgery in a week and now I have serious concerns. I’m mildly myopic (-1.75) and have decent near vision. Distance vision (8ft to infinity) is my priority. So I’m not sure trifocals will give me this.

Informative video. Can you say a little more about what IOLs have the best light transmission? Are Toric OK? Are LAL good for high myopic cases? I have myopia approx -9 and (per your video) have moderately bad floaters since age 50ish that I tolerate. Not sure if it might make sense to treat the floaters first (replace vitreous or laser). Does vitreous replacement make sense overall to improve the fluid clarity for the bigger myopic eyes?
What about flickering light around the lens edge every time I move my eye. I see that along with an arcing shadow off to the side constantly unless I’m in a very dark environment. I’m 12 weeks out from IOL surgery and it isn’t getting any better.

Well i am 75 years old, and i am not expecting miracles and getting back to my younger years vision. As long as getting the Mono Focal lenses in Nov 2024 make me see a little better in the distances and wear glasses for seeing things close I will be happy. Not expecting 2020 vision or how they work in different light conditions. All i can do expect a little better vision than what I see now.
Can you explain Posterior capsular opacification (PCO)aka secondary cataract after IOL replacement. I’ve read that type of material the IOL is made from may cause incidence of PCO. Hydrophobic vs hydrophilic lenses. Also risks of using Toric lenses. My astigmatism got worse after using a toric IOL.

Lal went wrong!! The lens was perfect ( no adjustment needed) but one day after lok it ,distance gone!! No real answers . Told might be dry eye, mild film but not in 24 hours. Also, I can no longer drive at night. Help!!😢

Well this video sounds all well and good, but most of us can't afford the high "out of pocket" costs for lenses since insurance only pays for the basic mono focal lenses and also does not pay for advanced laser surgery procedures, etc. here in the US. Kind of a bummer, but i guess we have to live with it and hope for the best.
That's the problem of healthcare worldwide.
Cant see how anybody would choose anything but the monofocul plus like the eyenhance jnj. The others above those seem to have prominent sideaffects to make people comment often on the net about halos, not crisp etc.
it depends on personal preferences. I have talked to thousands of people and, believe me, personal preferences differs so much..

EDOF is a technology which as a result gives Increased (Extended) depth of field or range of vision, other words focal point is extended (EDOF) which gives a patient insted of 50cm of sharp vision let's say 100 cm of sharp vision - as an example

depth of focus is physical measure of let's say sharp projection on retina. depth or field is a physical measure of area in front of your eyes, where you can see subjectively sharp

Thanks for the info on Mono Focal lenses - That is what I am intending to get for both my eyes in November surgeries. I probably have some kind of astigmatism in my eyes, but not getting Toric lenses - insurance does not cover these Toric or enhanced lenses - which is kind of a bummer. I hope glasses can correct my astigmatism like it did before I had cataracts. I was also near sighted. Time shall Tell how it goes for me. Watching allot of You Tube videos.
thank you for your comments,. yes, glasses will correct any remaining optical issues. the current trend is to reduce dependence of glasses with enhanced, toric and premium presbyopia corretcting IOLs
Thank you for your videos. I have my cataract surgery consultation next week. I’ve been trying to educate myself as much as possible, so that I can have an intelligent discussion with my doctor. So many choices. I’m a professional truck driver and also drive at night. I have a mild astigmatism. It’s difficult to know which lens to pick. I’m not trying to be free of glasses , but I do want to minimize adverse side effects. At the same time, I don’t have a premium budget. But whatever is the best choice, is probably worth the costs.
For your case I believe that bilateral Tecnis Eyhance will be the best choice.

Just wondering what the perceived difference would be going with the toric version as opposed to the non-toric version. I know that’s probably not a question that you can answer without specific examination numbers on the degree of astigmatism. But I’m sure it would cost more with the toric lens. Something I will have to discuss with my doctor.
Toric vs non-toric answer depends on astigmatism type, it's power and your individual perception of astigmatism. If it is a clear lens exchange, or your cataract is not too much affecting vision, you may try to simulate is by trial glasses to see if you (and how) react to residual astigmatism. Honestly, if it's low astigmatism like about 1D..1.5D it might be neglectible, but always better to try and to discuss with your doctor. Higher amount more probablt need a toric IOL, and of course, astigmatism shall be regular (refer to my recent astigmatism video)

It is now Oct 2024. Well, there maybe several choices of IOL Lenses, but for the majority of us that are retired and on Medicare and a Supplemental in the US, Lens and Surgery is only covered for the Mono-Focal type lenses. All the other Lenses/procedures are out of pocket costs and can be very expensive which I cannot afford. I am 75 years old and in the process of getting both eyes taken care of because of having Cataracts - right eye is worse than the left. I will probably go for the "Distance" Mono-Focal lenses and then wear glasses for seeing things closer to me. I have always been near sighted and wore glasses most my life. Right now I am not sure what the "Distance" Mono Focal lenses will give me. I go in for measurements on the 18th of Oct and will ask the Doctor what to expect with these lenses for both eyes.
as a nearisghted person you may discuss either mono-vision or both eyes calculated for near focus and distance glasses use.

THank you for being honest about IOLs etc. Its refreshing. Similar advise is much needed for Laser eye surgery too. Not just a big sell from vendors.

If you had cataract surgery today, which lens or combination of lenses would you put on your eyes?
Every eye and every case is different. Choice for one patient may not suite for another.
today I'd put PureSee bilaterally or PureSee + as an idea (my personal situation and my risks - PureSee for dominant + Odissey for non-dominant)

@@atalayonline I can not answer about "cm to be clear" as it is very subjective. with that type of IOLs (increased range of vision IOLs, EDOF technology) you shall expect fucntional reading wich means able to read smartphone but will have to wear glasses for prolonged reading or small tasks as a basic expectations. Realistic near vision acuity will depend on many factors like individual eye optical specific, residual refraction, light conditions, neuro procesing and personal visual experience before the surgery
