    Excellent points regarding people who spend 8 hours a day staring at a computer screen. The other 16 hours are spent sleeping and doing other things like you mentioned so trifocal lenses are not always the right choice.

    Hello Oleksii, my left eye sees 100% and my right eye sees 60%. I have cataracts in both eyes. It is a slowly progressing type of cataract, developed due to allergy eye drops. I think my right eye will have surgery before my left eye. What would you recommend to someone like me who is healthy? I'm 38 years old. Your general opinion is incredibly important to me.

    My surgeon has recommended the Oculentis lens. I read there were problems with opacification with these lenses a few years ago.Has this problem been fixed now with the latest Oculentis lenses?

    *Quality* of vision: this is so so so important. I have struggled with this just over the course of worsening presbyopia - I literally have not been able to see anything clearly at any distance for years now. The fact that our vision is "tested" using only black letters on a white background, as if this has much to do with how let alone what we *really* see on a daily basis is ridiculous imo. Yes it measures something which a I understand it, is labeled "acuity", but come on - how often do we encounter this level if visual contrast in daily life? Just because I can manage to make out what a letter is, doesn't mean I see it *well*. Yes I can read that last line, but it looks horrible! Yes with progressive lenses I can kinda see at all distances, but none of it actually looks good. It's functional, at best, but not "good" at all, which is hard to explain but if you've experienced it you know exactly what I mean. Yet the optomotrist is happy to usher me out the door, claiming great success with my new prescription :/ I now have a cataract and I'm very concerned that I won't be able to find a doctor who understands that I care about being able to see really well again, at any distance! ETA I don't mean all distances, I mean any of them :D I'm hoping it's possible to stretch that a little bit so I can get top notch distance vision but also comfortably cook or shave my legs with just a bit of blur, similar to how it was when I first started developing presbyopia (with micro mono vision? and/or an enhanced mono lens?) but I'm not cool with gaining a wide range of mediocre visual quality. It's what I've got now and I genuinely hate it.

    i know someone who got monofocal recently. Sees great in day time but disappointingly she sees halos at night such that she doesn't want to risk driving at night. But then again she had nighttime halos from cataracts before the surgery and didn't drive at night. Overall she's glad she got the surgery.

    Can computer guided laser "enhancement" (limbal incisions?) at the time of a standard Eyhance IOL placement for cataracts neutralize 1.5 D corneal astigmatism or is it better treated with a toric lens or a combination of both? What is considered most predictable in neutralizing this amount of astigmatism?

    Which IOL will you suggest for my 60Y old Mother, who is high myopic (Even after Cataract Surgery, she will need glasses for myopia, that's what Doctors said) and hyperopia with weak retina? standard monofocal or Enhanced Monofocal? these are my options: Hoya Vivinex / Johnson & Johnson Technis / Eyhance or Zeiss ?     Well, the first surgeon I saw said he doesn't use or feel the need for laser. Turns out, he also failed to detect that I had something called an epiretinal membrane in one eye. According to the literature, even if minimal, it is a definite no no for multifocal lenses which his office had dates scheduled me for placement until I chickened out for other reasons. My reason for changing course to enhanced monofocals was that getting multifocals with greater likelihood of night time glare, halos and decreased contrast would fail to address my chief complaint of poor vision while driving at night. The undiagnosed epiretinal membrane issue raised doubt as to proper initial treatment planning. The second surgeon is suggesting J&J Eyhance enhanced monofocal IOL implants in both eyes and laser corneal reshaping of the minimal (-0.50 or -0.75) regular astigmatism in each eye. I have an appointment for a laser scan tomorrow and will report back. Sites like this one hosted by Oleksii are of great benefit in discussing the latest updates and many options in cataract surgery in ways that a layperson can understand.

    I have +6.25 hyperopia with mild astigmatism in both eyes. -.75 and -.50. The astigmatism is described as "regular." If cost is no object, can laser cataract surgery also be used to recontour the cornea (through a releasing incision?) and would it be preferable over a toric lens implant to correct the mild astigmatism? I am considering Eyhance in both eyes. Your videos are clear and concise. Thank you for your guidance in this complicated field.

      What does your surgeon say? The first ophthalmologist that I consulted wanted to do the relaxing incision on one eye due to mild astigmatism (currently my prescription has -1 cyl in both eyes). He never even mentioned the possibility of a toric lens. I don't know if one solution is preferable over another. I have read that it's very important to get the toric lens placed in the right position. I'm hoping any astigmatism I have can be corrected post-surgery with glasses, as I see myself wearing glasses most of the time anyway.

    Thanks for this video and others. There are so many things that go into good vision that the lay person has not considered until cataract surgery. There is no one roadmap for achieving good vision, as lifestyle, age, tradeoffs in the choices, should all be considered. An example of monofocal toric lens not extending depth of focus was my neighbor who had cataract surgery recently. He chose distance as his target. He uses two pairs of readers, +3 (for close work) and +1.5 (for computer) for closer distances. The toric lens in one astigmatic eye enables image clarity.

      @@CC-uh8gl He never could get used to wearing progressives so instead had two pairs of bifocals (before cataracts). One was computer/reading and the other distance/reading. Now he doesn’t need any distance correction, so just uses readers.

    alcon IQ monofocal was my choice after much discussion with my doctor. Was -16 left eye, -12 right eye. wore contacts 30 years. Excellent vision. After surgery, could not be happier. 20/15 left eye,20/20 right eye. just use -2 readers. Am 68, had surgery 11 years ago. Had one Yag procedure. Amazing

    Your insight is always appreciated. Is there any difficulty getting best visual acuity when using advanced monofocal lenses when combined with progressive eyeglasses? I read the following: "The other issue to consider is that if you use the Eyhance lens it will compromise your ultimate vision with eyeglasses. Eyeglasses can make a correction when the diopter amount is off. However, the Eyhance lens has a progressive power change with the radius of the lens. That is how it gets the EDOF effect. It smears the focus point over a range. A good aspheric monofocal like the Tecnis 1 will bring all the light to a single focus point, and will be easier to correct with an eyeglass lens. The contrast sensitivity or MTF will also be higher with the true monofocal." per Dr. Ben LaHood

      Interesting, I was considering the Eyhance since the first ophthalmologist I saw was touting it, in that with distance selected as the target, I'd get some intermediate without glasses. The second ophthalmologist suggested I pick intermediate as the target distance, and her office uses Alcon Clareon for standard monofocal. I'm going with her. It took me awhile to come to the conclusion I'm simply going to replicate my current situation, nearsighted needing progressives, only without the cataracts. Progressives are expensive but I figure I won't be changing my prescription often, if at all, with the cataract afflicted lenses gone.

    Dear Oleksii, you wrote JnJ Symphony ( -> PureSee). Does it mean that PureSee is a new version of Symphony? Still don’t know if PureSee Toric is any better than RayOne EMV Toric. There is no review from people regarding PureSee. I found just one thread online that’s it.

      correct, PureSee is a new replacemend for Symfony. Not awailable in all the countries yet, but the initial resutls I have from the markets where it has been released are very promising

    Post op two weeks on my second eye. I'm 20/20 in my left eye and 20/25 in my right eye so far. Considering my extreme near-sightedness, this is more than amazing. I opted for the Panoptix trifocal lenses after much research. So far, wow! It's night and day. Literally! Thanks for the videos. I've learned a ton.

    As always, thank you for sharing your knowledge and experience. These decisions are daunting for many patients and as you say, knowledge is power. You didn't mention Clearview 3 (SBL 3). Where does this lens fit into the scheme and could you share your experience?

      Good question. It is bifocal with high near add, so basically it is a full range of vision lens with lack of good intermediate by design.

    I have been wearing mono vision contacts for 40 years with astigmatism in the left (far vision) eye. Is it possible to continue with mono vision after cataract surgery and avoid the very expensive premium lenses?

      You are not obligated to get premium lenses. It's a personal choice and you can continue to use monovision if you wish. You should discuss it with a eye doctor but, you shouldn't be pressured or obligated to get a "premium lens". Take care and good luck.

    Thanks Oleksi. Sorry I got your name wrong. I got a monofocal iol set for distance in the left eye. The right eye is legally blind because of epiretinal membrane growing back after vitrectomy. I am happy I went for monofocal iol set for distance. My left eye has diabetic retnopathy. Both eyes have glaucoma. I am using over the counter reading glasses to see close. They are not the best so I might make typing/spelling mistakes ( lol ). I am waiting for my vision to stabilise so I can get prescription reading glasses. Your videos are very informative because doctors do not tell you everything or anything sometimes. Keep up the good work.

    Thank you Alexee ( hope I got your name correct ) . You are correct about compromise for visual outcome. I think safety is a factor especially when you are old so seeing far and clearly may override seeing near and clearly. Of coursw we live in the computer age and one wrong click of the mouse can get you in trouble. Or signing a legal document you should read fullly.

There is no generalization possible in terms of age or occupation, it is what I’ve discovered with years of my work and direct to patients discussions. All the best for you!

      @@iol-adviser I choose a toric. With my astigmatism there's a fair chance I won't need glasses for driving and distance activities. The adjustable would make that more likely but the cost difference is just not worth it. I've been wearing glasses since 2'nd grade so I'm used to it. Also for driving I almost always wear sunglasses. Uncorrected distance would be a plus, but not a great advantage. A cost benefit analysis. Thanks for your videos.

    contact wearer 30+ years. very miopic. -16 left eye, -11 right eye. started reading glasses late 30s. multi-focal contacts did not work for me. Had slight cataracts late 50s. Had rle at 58. wanted best distance vision possible. Had monovision Alcon IQ both eyes. Now 10 years later still have 20/15 left eye,20/20 right eye. no glare no halos. wear reading glasses.+2. no big deal. Best decision I ever made. Shoul have done it years earlier. just amazing to wake up and see clearly

      Do you need glasses for medium distance when using a PC? How does your reading in dim light compare to your old healthy eye?

      @@atalayonline computer.......just my readers , at home screen is my 50 inch tv on wall, no glasses. reading in dim light no problem with readers. i fix watches for a hobby, sometimes use 2 pairs, need to buy a jewellers magnifier. Actually vision is better than before surgery when using contacts and readers

    As an Ophthalmologist I have used most of the lenses available.In my own eyes I would want a Rayone EMV or a Hoya vivinex in a mini monovision configuration. Good far ,intermediate and fair close vision with very little disphotopsia symptoms . Some EDOFS lenses work well but are exspensive

      Thank you for your feedback. My personal choice for now is PureSee, which is similar to Eyhance and delivers way more near vision. Or, if not available mini-mono vision with Eyhance or EMV/ Vivinex as a reasonable option.

When I woke up from the procedure, my left eye experienced a sharp stabbing pain coming from the inside to the outside of the eye, and fluid squirted out. Do you have any idea what could have caused this reaction?

      @@iol-adviser I wanted to thank you for your information. There is no infection or dry eye. However, after the procedure, I did develop a small apparent inclusion cyst on the inside of my eye whites. I'm not sure if that could be causing the blurriness and discomfort I'm experiencing. I'm thinking, when I had the sharp pain and fluid squirt from the eye, could that have moved the lens?

    I have high myopia, -9, -9.5 . Light astigmatism. Doc recommended me Panoptic tonic lens. Which is a multi focal lens.Do you think that will fit ? Thank you for your reply
