    I met a highly paid cardiologist surgeon in Cabo San Lucas, and I asked him if high cholesterol can cause a heart attack and he said to me that if high cholesterol caused heart attacks then he would have had one years ago since, as he said, his has been hovering around 300 for years and that statins haven’t moved the market on the rate of heart attacks since their inception.

    When it comes to a Coronary Artery Calcium (CAC) Test, anything greater then zero is undesirable. It is a good test, but most people would also recommend a Carotid Intima-Media Thickness Test (CIMT), since it appears to be the gold standard when testing for soft plaque. Also, keep an eye on your A1C & triglycerides. As you may know, prolonged high blood glucose & pressure can lead to Endothelial Dysfunction.

The categories are not labelled exactly as you are describing here: LPL particle num: 1962, LDL particle Size: 22, Total Cholesterol: 317, Triglycerides: 90, HDL Cholesterol: 80, LDL Cholesterol: 219. I just went off my Statin for three months and made some dietary changes and lost 15 pounds. I am a 6'0" male at 172 pounds that exercises 6 days a week and competes in National Tennis Tournaments. I am offering myself up to a clinical/medical/scientific study as it relates to blood work (cholesterol, glucose,A1C, etc.) and a low carb/keto/carnivore diet.
    I have tracked my labs going back to 2012. Went low carb in December 2017. Since going low carb, no more T2D, Sleep Apnea, brain fog, GERD, and more. Also since going low carb: significantly bettered my Triglycerides, Glucose, A1C, etc, but also have elevated total cholesterol and LDL.
    I've also had a CAC test and a CTA with contrast. I believe my data can help others, but I need a professional that wants to dive in.

      Only thing that is worth looking at are triglycerides and HDL. If you are anywhere near a 1:1 ratio then that is excellent. I do look at inflammatory markers for all people, including those following a carnivore/keto diet because of the oxidation from cooking meat, especially meat that is loaded with antibiotics and hormones. Preparation and cooking method also plays a huge role. Contrary to what this physician recommended, I do recommend checking OxLDL and MPO, as well as Lp-PLA2, sedimentation rate, HS-CRP, and all the interleukin markers. I also run a GI-MAP test because so many people have gut issues or underlying gut issues which then leads to a host of problems and other autoimmune diseases. There is more in preventative care than to focus on just heart disease. What good is it to have a healthy heart when you contract cancer. You have to make sure all your detox pathways are open and functioning properly. Gut function as I mentioned, proper exercise, and I also recommend infrared sauna as well as rebounding or the use of a vibration plate to help with the lymphatic system. Even far and near infrared light therapy and EWOT is something I have been using the last 8 years or so with patients. The human body is very complex and supporting one bodily function or organ can or may have a negative affect on another organ and its function. BTW, I've been in healthcare some 30 years with a specialty in integrative functional medicine and endocrinology. Take care and best of luck.

    OK, so I have a dilemma, metabolically I am fine, triglycerides, 62, HDL 92, LDL 163 and APO - B 130. Recent CIMT scan all clear. Do I need to worry about my high APO-B? And what if it is not oxidized? And how do you know if it is oxidized? Can you have APO - B which is not oxidized, and therefore not an issue? Not much information on this online at all

    • @JasonActualization
      @JasonActualization 6 месяцев назад +3

      ApoB doesn't become oxidized, it is a carrier protein and a proxy for the total amount of lipoproteins, excluding HDL. The higher the ApoB, the more opportunities for lipoproteins to become oxidized and thus atherogenic, but it is not causative in and of itself. People who are metabolically healthy can 100% still develop heart disease if they have an imbalance of oxidation and antioxidant activity. As long as you are avoiding pro oxidative agents such as seed oils, and are emphasizing saturated and monounsagrated fat while minimizing PUFA, and consuming potent antioxidants such as pomegranate juice, you are golden.

    Why are we still debating the best ways to estimate risk? We don’t have to guess anymore, we can look. A CAC will tell you if you’ve been growing plaque, and a CCTA will show you if you’re still growing plaque. Discussions about risk are obsolete-just look. If you’re metabolically healthy and eating your favorite diet and you’re not growing plaque, great! But if you’re still growing plaque on CCTA, as many do, you’d be wise to open your mind to treatments that have been shown by randomized human trials to improve outcomes.

      @@texasjherrera Coronary CT angiogram. It's a CT scan with contrast that closely evaluates the presence of plaque in your coronary arteries. It detects both soft (immature/vulnerable) plaque and calcified (mature/stable) plaque. It's not cheap, and insurance only covers if you've got documented ASCVD.
      But a CAC (coronary artery calcium scan) is almost as useful and only costs about $80 cash. It's also a CT but without contrast that quantifies the amount of calcified plaque in your coronaries. It can't see soft plaque, but as long as your CAC score is zero, you've got essentially a 5 year warranty, meaning your risk of heart attack in the next 5 years is about zero.

      @@jennifermarlow. That’s actually a very reasonable reply-thank you. Except I don’t think doctors are prescribing statins to people with normal lipid levels. And doctors do understand-they understand that if we give everyone with high LDL-C a statin, we reduce heart attacks and strokes. If I’m your doctor, and you come to me with high LDL-C and my job is to prevent your first heart attack, what should I do? I can tell you to exercise and lose weight, which I’ve presumably done a hundred times before, but if your LDL-C remains high, your risk remains elevated. I can ignore it, or I can treat it.
      At least that’s the old paradigm, which is what these two are still arguing in this clip. The beauty of imaging studies like CAC is that we now have a third option: instead of guessing about risk of disease, we can actually look for the disease itself. Anyone whose doctor suggests a statin might simply request a CAC to see if it’s really necessary.

      @@mfkleven thanks for explaining that. I had a CAC and it was zero. I have a family history of CVD and my LDL cholesterol is “high” according to my physician, perhaps I’ll ask for another in 5 years from the last in order to get him to stop recommending I take a statin for no reason.
      I’m curious about the other scan though. I’ll see what the out of pocket cost is, and maybe get one just to establish a baseline at my age.

    • @mfkleven
      @mfkleven 6 месяцев назад

      @@texasjherrera That sounds like a very reasonable plan that I think most doctors would support. The question is, “If I’m not growing plaque, why do I need to lower my cholesterol?” The answer, at least concerning heart disease, is, “You don’t.”
      However, brain health is another issue to keep in mind. Hyperlipidemia in mid-life is associated with a 2.3x risk of dementia in later life. It’s not clear if the absence of coronary plaque would place you safely outside that risk or not. It may, but I don’t think we know yet. My CAC is zero and my CCTA shows no soft plaque, but I still opt to take 10 mg of ezetimibe to keep my LDL-C about 70 mg/dL because it’s very safe and I don’t want dementia.

    I'd be curious what your research finds for those of us that are genetically prone to high Lp(a). I know they have some medications in development / trials to lower it. In my case, with Repatha, I brought my stubbornly slightly high LDL from 135 to 67. Fit, active, everything is else is optimal. But, that darn Lp(a)

    I do not understand why many people still talk about HDL cholesterol and LDL cholesterol or good and bad cholesterol. It is confusing. There is only one cholesterol. Cholesterol is cholesterol. No good or bad cholesterol. LDL and HDL are NOT cholesterol. They are lipoproteins, little submarines, who transport cholesterol, triglycerides, vitamins.

    • @JasonActualization
      @JasonActualization 6 месяцев назад +5

      Correct, with a major caveat. The problem with high LDL is that there are more opportunities for it to become oxidized, but the real problem is that our dietary guidelines have optimized for the mitigation of LDL, not what confers these particles with resistance to oxidation. It turns out that these strategies are at odds with one another, and that attempting to lower LDL via reducing saturated fat actually renders these particles more susceptible to oxidation, hence why the goal post is ever being shifted/lowered with respect to target LDL in statin therapy.
