Tests: hscrp : (if below .3, don't need colchicine)global inflammation marker , check white blood cell count should be below 5. Esp. Check Eosinophils 1% & monocytes 6%.       @@MK-sc8md glad you said all is well 🙏🏼. Had CIMT Friday and plaque all good but they found a nodule on thyroid 😩. I turned 57 yesterday, great birthday present huh? Searching for information on how to naturally shrink it but no one seems to have an answer 🤷🏼‍♀️.

    Inflammation is the enemy and caused by our life choices. Nicotine, chronically elevated gluclose or blood sugar is the main culprit. Plaque is distributed differently depending on the cause of Inflammation. Sugar does not effect periferal blood vessels. Fine nerves and heart. Nicotine effects entire arteral blood stream. Rare other causes: genetic vascular disease and lupus. Other immune diseases increase vascular inflammation. Chronically elevated uric acid causes inflammation.
    1) don't smoke het rid of nicotine 2) ketogenic diet..up to two years or more, if older, use meds. Alternatives to statins: CAC score to find risk >80 at risk. Nuclear medicine stress test ti follow up. 81mg aspirin per day if high CAC test. Clotting cascade disrupted. Reduces platlet activation so clot dissolves. 2nd drug: decreases white cells- colchicine .3 to .5g daily. Test inflammatory markers. HSCRP marker

      NAC helps activate Glutathione , powerful anti-oxidant, and is thought reduce inflammation and oxidative damage. So indirectly you're getting a healthy benefit. Same for Vit C as it helps wth absorption of other vitamins and minerals.

      Same here. I haven't smoked since 1987 and my CAC at age 66 (end of 2022) was 0. I've also remitted my diabetes, got off BP drug, weigh what I did in mid 1980s before having children. And amazingly my very grey hair is now darker; my boss has confirmed this. I started low carb at the end of 2021. It's wonderful what changing what you eat can do for your health. I've been eating a lot more meat since the start of this year.

    Hey doc, my CAC was a 7 in one artery. I’m going for a CIMT tomorrow. Been on ketovore since December 23. Lowered HR-CRP to 0.8, lowered A1C to 5.6, trigs went up a bit from 116-125 and HDL went from 50-66. BP always perfect and I feel amazing. LDL high and total cholesterol over 300 🤷🏼‍♀️. Arthritic pain is gone and no brain fog. I’m a former smoker and was around secondhand smoke my entire life from my nana. Going for a GTT Saturday with IR. ApoB is high also.

      Losing weight causes your LDL to go up. Ignore the noise and turn down ANY medications to control cholesterol. Keep eating very low carb. Carnivore high fat is best or as close as possible. Stay away from spinach and other oxylate heavy veggies. All plants have defense chemicals. At least make them a minor part of your diet.
      Cardiologist will try to scare you away from keto/carnivore. Ignore their advice and listen to Cywes. All cardiologists seem to be indoctrinated and wrong about everything. I had one tell me I’d be better eating Oreos. No lie. What a hack.
      Cimt is a waste of time. CAC is all you need. Especially with a CAC of 8. That’s very low. Carnivore will stop progression and maybe even reverse it with some regular fasting.

      Triglycerides cud be easily lowered by omega3 6 - 10 grams , add extra EPA 3 grams, and B3 niacin slow release - 1000 mg.

    Do NOT get a CAC Test unless you have a universal life insurance policy in place! I was denied extra term life insurance because of a positive score. Luckily I already had Universal.

    I am a 65 year old male, and most of my life I ate a standard diet. One year ago I went on a keto diet, and my weight went from 230 lbs. to 171 lbs. My waist went from 42 inches to 34 inches within 3 months. I feel so much better, and I went for a CAC test and it is 326. Heart disease does not run in my family. I now eat more a carnivore/keto diet. My glucose stays between 85 and 105 all the time. my cholesterol is 340 triglyceride's are 84 HDL 55 LDL 265. I don't know if I should do anything or not. I feel great and I HATE going to doctors, so I will probably just hang out and wait to die!!

      I"d keep doing what you are doing. Your trigs/HDL ratio is sub 1.5x which is great. Maybe take a baby aspirin daily.

      Look at a life extension product called Super K or any product that has a combination of k2 mk7 k2 mk4 and k1. There's also Cyclodextrins that appear to be able to reverse calcification but it's not cheap and has to be taken in enema form for 3 months. So I'd really do my homework on those but the k2 is cheap no known side effects and studies have shown it can get results in some cases

      Hopefully, you are doing intermittent fasting, which is very easy to do on a keto or carnivore diet. I eat between 12:00 p.m. and 5:00 p.m. daily. This intermittent fasting for me increased my HDL from 46 mg/dL to 79 mg/dL in 6 months. If you can also periodically do some extended fasting, that will further improve your health. I do 72-hour water only fasts (occasionally adding electrolyte supplement only) every couple months. Start out with a 24-hour water only fast and try to build up to longer fasts.

      I am in the similar situation as yours my doctor keep telling me that I have diabetes and wants me on meds i told him lets look for the root cause That i want to see an endocronologist and a gastro doc. but I have absolutely no symptoms whatsoever of diabetes. He also said that i have high cholesterol as well. I have no gallbladder and a complete hysterectomy no treatment ever for that.I am on the low carb diet eating less than 50 grams of carbs a day. No sugar one meal a day fasting between 16 and 18hrs per day. But not losing much weight. I am 145 lbs though. I am tired and frustrated with doctors. I am now looking to see if I could find a functional doctor in my area. and looking for a new primary care doc as well. I wish that health care system was better in the U S allow people to go see alternative docs. Etc

    Keto 4 years, off statin 3 years. Weight down 45+ pounds and BP now excellent off ARB/diuretic. Hga1c 5.1 post high of 6.5. 67 y/o thriving retired primary care male physician. Psychiatrist wife has similar results!

    76yo male here. 5 years ago started keto. 3 years ago CAC was 23. 1 year ago CAC 32. 2 months ago a VERY painful heart attack, stent installed. So much for low CAC "protection".

      My hubby super fit 74 , incredibly focused on low carb diet, no sugar, just had a stroke, his cholesterol was a bit high and blood pressure also but we believed him so fit this stroke has been like a bolt out of the blue

      @@antheablackmore5838 IMHO the high cholesterol & cardiovascular disease can be symptomatic of lifelong "dirty" food intake (sugar, processed, seed oils, carbs). I'm heading for a CVD event that can only be mitigated by late-in-life changes to clean nutrition. Not a horrid alternative to agonizing cancer & chemo I guess!

    Dr. Rob, thank you for yet another wonderful video. I feel and live your passion! For your readers: I am 65 yrs old. I was a smoker in my late teens to middle twenties. During that time, I was also addicted to sugar. It was not until my late fifties, after looking at my annual bloodwork for the last thirty-years did I find out that for all that time, I've been pre-diabetic. Family PCP never alerted me of that fact, and I doubt he even knew what it was. Strong family history of heart disease, and been on a statin and aspirin for more than twenty-years (thought I learned lessons from my family). I was always physically active, and even boxed while I was in college. Two-years ago I had an ER visit for what I thought was a stroke but thankfully turned out to be benign positional bigeminy. In the ER, although I noticed it-doctor/nurses were not alarmed, I noticed my BP was sky high. Upon arriving home, I thought this was unusual as two months previous (when at my annual PCP visit) my pressure was normal. Long story short, I dug into the cause for the high BP: insulin resistant, obese, stressed (was soon to retire-workload transition). It was then I learned of this CAC Scoring test. Feeling pretty confident, I scheduled the test. My score came back as 1050!!!!! Never imagining what it may feel like, seeing that score must have come close to what Mike Tyson gut punch feel like! How can that be? Never ANY symptoms during my physical exertion (cardiologist suggested that over time my heart may have grown collateral arteries). Lastly, for that time, I bought into the low-fat high carb standard American diet…believe I was safe. Through tons of research, I learned that that Weekly, now, I practice e calcification is part of the healing process needed from decades of endothelium damage. However, it is the angry plaque (work conducted between the calcification and media layer of the artery) that ruptures. Of course, I scheduled the standard stress test, passed that with flying colors (not much of a confidence builder as a stress test will only show an anomaly if you’ve previous had an MI, or have a 70%+ occlusion). I next scheduled a consultation with a locally renowned cardiac surgeon. Based on my stats(stress test results and physical activity level), he was quite confident that most if not all of my plaque was in the media of my arteries, and nearly all calcified (which the latter can reduce your chance of an MI to close to those who had a CAC score of near zero). He went on to explain it’s those people that have a CAC score between 100 - 500 are the ones he sees more on his table. Actually, pissed I’ve been lied to for decades, I took immediate action. Today (just over 1.5 yrs later), I’ve lost 85 lbs. (5’ 10”)(235 - 150), waist 32, I ruck sack (60 lbs.) 2x p/week(5.5 miles), with cardio and strength training most other days. My Vo2 Max has increased from 20 to 39, I entertain a low-carb meal plan, eliminated all added sugar (and HFCS) from my diet (wife not only makes our own sprouted grain breads, but also ketchup (no HFCS)). Eliminated all seed oils, processed and UP foods from diet, and more. Weekly, now(maintenance mode), I practice time -restricted feeding (usually 18/6), with two weekly 24h fasts. Three times per year I do a three-day fast. As a retired engineer I track all of my bloodwork (some of the BW I ordered, my cardiologist had to be trained on). Besides what was mentioned, my bloodwork (to include, A1C 5.1, fasting insulin (3) & CRP, and many more) have never been better. I am too old to remember what I felt like when I was eighteen, but this has to be pretty close. Next Feb (2y mark), I am going to pay for another CAC score to see if perhaps I have limited future increases, or better-yet, stopped it’s progression. So, why did I write all this? To try to persuade you that YOU are control of your wellbeing, and NOONE will care for you as well as yourself. Lifespan is NOTHING w/o Health span! Lastly, if we would apply one-half as much effort we apply to our investments to our health, I believe, we may be able to eradicate heart disease in the future! Is there any guarantee I will not die before my next CAC score, no. But, if that is a case, I’ve had the most enjoyable two-years of health I could ever imagine!

      Ralph...thats motivation just reading your notes. Im 77 and need to do everything you have done but i dont have it. A year ago i gave up sugar and high processed foods and ate hi protein and fat..low carbs. Nicotine as teenager only. But i have constant sciatic nerve pain. Im motivated to exercise more now but its my wgt that gets me down. Ive just decided to eat much less food but when i do it will be the hi protein and fat..low carb. Its hard but i have nothing else to do anyway. I just thought id say how great you did..are doing and youve helped motivate me. TY.

      ​@@fmpabxx7834 Yep, probably almost as alarmed when I read the results! Back in Feb 2023, as I wrote, post my local cardiology visit I demanded a consultation with the lead surgeon (20y OR exp) of the cardiac surgery team in one of our area’s leading hospitals.
      My wife and I came armed with CAC results, past physical (exercise/workload) history, and current physical capabilities. The more I explained all this to him (now, this is a guy that holds hearts in his hands), the less I was able to get a rise from him! At the end of the consultation he said two words, “don’t worry”!!!! What? As a soon to retired engineer, I built a career on worrying and risk mitigation! He went on to mention that he’d bet a year’s salary that most all of my plaque is located within the “media”, or muscle of the arteries, and if I had any soft plaque, considering the exertion/exercise I place myself under, it would have ruptured already. Lastly, he told me “although I know you wont listen when I say to put all the tracking spreadsheets away, do so and come an see me in two-years”.
      Through later research I learned that “calcified plaque” is not dangerous, furthermore if one can reduce their insulin resistance, practice time-restricted feeding, with occasional intermittent fasting (autophagy), the body can actually “consume” any signs of arterial inflammation “think pus”, thereby totally calcify one’s plaque. Secondly, plaque is “associative”, not “causative”. In other words, plaque is a sign of former endothelial damage and the body’s attempt to repair the damage. Low insulin resistance, can thereby reduce future arterial damage, and hopefully allow the body to complete the repair process and thwart future damage. Hence, totally calcified plaque returns the MI risk to almost match that of someone with zero “angry” plaque!
      Anomalies I learned and found interesting was about marathon runners. There are many of these physically conditioned athletes that have CAC scores upwards of two-thousand or more! The medical community labels these athletes to “Have a Heart of Stone”.
      Another interesting point I learned is that a low CAC score can be even more dangerous that one over a thousand! Why? It all depends on the percentage of that plaque which is “angry”, containing foam cells and not completely calcified, that has even great risk of rupture. Lastly, a “zero” CAC score sues not necessarily mean you are not in danger! Why? You can have endothelial damage, nearly all foam cells, and no calcification as the former is NOT detected during the CAC test. For people with a zero CAC score, to be sure, I’d highly recommend them getting a CIMT (Carotid Intima Media Thickness) test. Different than a “flow” test, and done by a skilled technician, this test measures the “thickness” (aka if there is plaque) within one’s carotid artery.
      In short, it’s never too late to become metabolically healthy via eating as natural as possible, eliminating all added sugars(inc. fructose, HFCS, etc.), processed and UPF foods, and eliminating all seed oils from one’s diet. Think Paleolithic meal plans.
      My goal, as I’ve learned from many of the cardiologists I follow is that “one wants to die WITH their plaque, not from it”.

    Dr Cywes, as one of your students I am gonna go on the limb and surmise why Nicotine develops plaque on the extremities and everywhere as chronic elevated blood sugar inflammation concentrates near the heart arteries and coronary arteries is as follows. Nicotine is absorbed in the arteries and increase blood pressure also restricting blood flow by hardening the arteries. Even throughout the body. Nicotine users are generally skinny people, they eat less and have lower blood sugars. Where as chronic blood glucose, the glucose is attached to red blood cells making them stiff and less flexible. The glucose molecules have sharp edges. The heart is pumping the blood with these sharp molecules attached, the vortex around the heart arteries is greater causing the blood to bounce off the artery walls causing little cuts from the attached glucose. Thus the inflammatory response and plaque accumulation. Now back to the less flexible cells from the glucose attached and with sharp edges, when it passes capillaries in a single cell row at the extremities, no flexibility and sharp edges cause diabetic neuropathy at the extremities.

      Yes.
      Glycated RBCs tend to strip the glycocalyx which causes endothelial inflammation like sickled RBCs.
      When the glycocalyx is damaged this will also obliterate the negative charge which in health is protecting the endothelial cells.
      When the heart contracts it greatly reduces blood flow through the Coronary arteries as they get compressed.This pulsatile stress in the presence of inflammation is unique to the coronary arteries.

    CAC score 0, however, they saw calcifications in the thoracic aorta. Dr is sending me to cardiologist. Dr wanted me to go on Statins. I refused. 67 y/o female non-smoker.

      I had a CAC score of 312, at 60, now I'm 65. Cardiologist refuses a 2nd CAC, because I didn't follow the statin recommendation. I changed to low carb, intermittent fasting, now towards carnivore.
      I started D3, K2, Magnesium, Vit C, Boron. A year ago started Nattokinase 14000 units daily plus Chromium and Berberine.
      Do I also need to take baby aspirin or is nattokinase sufficient?

    I have Hashimoto's disease, sjogrens, I.C., fibromyalgia, (A .C. in remission). I don't smoke, I don't drink alcohol at all. I stopped eating meat and pork 14 years ago. Recently, I had a scan done, and they found fatty liver disease, a small cyst in one kidney, gallstones, and one kidney stone. I am guilty. I do love sweets. I'm not diabetic how could I have so many issues when all I take is vitamins and herbs and stay away from canned , frozen, and boxed foods. How is it possible my body continues to turn on itself? Doctors don't bother to try to find out why? I have been in pain management 30 years for chronic pain syndrome my central nervous system is damaged

      please send your blood to Igenex labs in California for testing or order their new home kit. You may have Lyme disease (undiagnosed) which is causing you constant pain. Most doctors do not know about this disease.

    I am not a perfect carnivore but after being on this diet about a year and half my CAC score done before I had started my score was 28 and just last week the number dropped to20.4

      My score was 305 in January 2023 and I'm planning of getting retested in January 2025. Did you do anything besides the diet? I take nattokinase, K2 MK7 and Dr. Schulze's Heart formula. I eat mostly carnivore but occasionally will have a low carb veggie.

    Here is my problem, and its a legitimate concern. I have a colon tumor to deal with. And every “flapping gum” on the internet is screaming that red meat is a major risk factor for colon cancer. So one can see how a keto diet might be a big concern for me. Do I agree that a keto diet can be very beneficial for overall health? Yes. My nephew is on a carnivore diet and he lost almost 100 pounds and feels great. But with my situation, I have to watch my colon health very closely. Another concern is my taking an aspirin (81 mg) each day. I read that aspirin can increase uric acid levels, and I have a history of gout and kidney stones. I haven’t determined my uric acids levels yet, but I will do so shortly and I assume this level will be elevated.

    I'd like to see the data/papers etc, on nicotine causality of plaque. I'm a M65 and on carnivore low carb for 18 months. Just had a CTA and my CAC score was a 25. I quit smoking 10 yrs ago. smoked for 40 yrs. I have been vaping low dose nicotine for 10 years. I can't find any data for years that nicotine causes plaque. Hi dose nicotine will inflame arteries yes. In fact, all studies show only that nicotine has very little physical effect on the human body accept that it is an extremely addictive chemical. Thanks for your professional postings. Love it. Respectfully...

    I have one or fewer gout attacks annually, I'll be 74 on November 14, 6" 3" 170 Lbs, low carb ketogenic diet, My Uric acid runs in the high 7"s with the most recent (July 2024) being 7.9.     WHAT did you say as far as cochicine dose? Sounds like ".3 to .5 egrams daily." Did you mean to say milligrams or micrograms? The difference is crucial! Typical colchicine tablet is 0.6 mg. It does have a crease for cutting it in half (0.3 mg). So, if you meant to say, "point 3 to point 5 milligrams daily," please clarify.

      CAC is only a gauge for hard plaque. Soft plaque is definitely more dangerous because it's not as stable and can break off/cause clots.

    But that "healing process" is the formation of plaque. The LDL is needed/used for that. If you experience a prolonged episode of inflammation (and this can happen out of our hands), it would seem to be beneficial to have less LDL to lay down that plaque. Correct? Won't the high protein from the carnivore/ keto diets elevate uric acid levels?

    I refuse to ever take any statins. I dont smoke but did from 17 til 22 yoa..quit cold turkey. I quit sugar and processed foods one year ago and ate carnivore and keto diets. Then i saw my dr..got blood study and am pre diabetic. Now im doing exercise daily and trying to lose wgt. I dont want to take sugar meds. My echo of heart shows inherited heart hypertrophy. Ill try 5 mg ASA. My Dad had MI at 91..hx of 40 yrs smoking. So im sad that at 77 i am diabetic and hypertensive. Ill eat protein and fat and lowest carbs and see what happens with increased activity daily. I dont need much food to exist.

      Several youtube MDs say that happens because the red blood cell live longer on carnivore, and therefore have more time to glycate. If that is true, the higher score may not reflect higher blood sugar.

    For anyone considering going on a statin, think again. My friends doctor convinced him to go on statins due to high LDL cholesterol despite a 0 CAC score. This was 2 years ago. He just got another CAC scan due to chest pain and it came back as 83! He has now found out that statins will often increase a CAC score and do so very rapidly. His doctor has tried to convince him this is due to a plaque stabilizing effect of the statins and that we need to advance the disease in order to stabilize it. To me that doesn't make much sense at all, especially since the chance of having a major cardiac event is less than 1% in the next 10 years if you have a 0 CAC score. My friend is very upset and angry at the doctor for not letting him know that statins will often increase a CAC score, or give you a non-zero score if it was 0 before hand. Sure, he probably had some soft plaque but there is good evidence this can be reversed by diet and lifestyle changes, taking vitamin K supplements and eating oily fish. None of which the doctor mentioned.

I was dealing with high blood pressure and high cholesterol. He said that I am a walking time bomb and I could have a stroke like my mother. She passed from pancreatic cancer. I don't eat meat, and gluten free. I cut out carbs, lost 20 lb, and I still have high cholesterol and blood pressure issues. At times very high or extremely low. My mother suffered these things no matter what I do it's genetics. At least that's what my doctor says

    I had my first afib attack in May 2003. I spent a day and night in cardiac icu, during which time I had a heart echo, which the docs said I had a very young and great looking heart, despite me being on the upper side of 66. I also had a nuclear stress test I passed with flying colors.
    Exactly one month later I had a massive heart attack, triple bypass, and 25 % EF as a result.
    I never smoked, though my mom and people I was around in confined areas did. I did eat the wrong foods.
    Since, the drs insist on statins. I now refuse to take statins, instead prefer to eat a good diet. My triglycerides are down, still high, but much better. LDL is around 180 and still dropping, but still way down from before.
    I tried a low dose for over a month and ended up in the ER from side effects.
    My eating habits, lifestyle, and family genes, are to blame for my heart attack. I am doing my best to correct the things I can control. The others, such as genetics, are out of my hands.

He jogged 7 miles a day and did 200 sit ups. He started having chest pain when he jogged. He had a stress test which he passed. 10 days later he was dead. He has familial high cholesterol-not caused by diet but genetics. You may have the same issue.😅

      Part of my problem is genetics. Those in my family that don't die of heart issues by their mid-70s, die of dementia in their 80's.

      It might just. L-citrulline and citrulline malate are sports supplements you can get. I take the latter for gym; supposed to improve blood flow and lower blood pressure slightly.
      Taking it everyday indefinitely would be expensive tho

    I've been pure carnivore (BBB&E) since January, 2024. I've lost 25lbs and my formerly high BP is now quite low. The problem is I HAD A HEART ATTACK three days ago and had to have 4 stents placed. I had no less than three cardiologists lecture me on the need for a statin and the need to eat more fruits, whole grains, and vegetables. They have scared the hell out of me. I'm back on a statin (and now plavix) and I'm quitting carnivore. BTW, my LDL is 283 mg/dl (195 prior to carnivore), Total Cholesterol is 351, HDL is 52 and Triglycerides 80. Does anyone think I'm doing the wrong thing?

      Carnivore here with high LDL. From everything I have learned your figures are perfect. TG/HDL ratio 1.53 is good. High LDL is irrelevant if you are metabolically healthy.

    Does nicotine from smokeless (snuff) also cause the same inflammation?. I did it in my teens and early 20s. Stupid kid.
    More worried about my low fat diet upbringing honestly. Terrible diet for everything. High fat Carnivore is my best I’ve ever been.

    My husband smoked from teens to 26. Stopped. At 86 when prostrate exam being done they found small cancers in his bladder and said were from smoking. (60yrs later).. his sister died at 56 A 90 a day smoker 😖🥹😳

    Is it just nicotine or is it smoking/vaping it?
    I chew nicotine gum…I’ve never smoked. I started chewing a piece or two of 2mg nicotine gum during my 20-25 mile per day bike rides. Not every ride, but most. It’s great for focus.

    I am 83. I have never smoked and I have always tried to eat a healthy diet. 3 years ago I developed diabetes. Two years ago I began having AVNRT episodes and angina pain. I am presently in Ukraine so it is difficult to get good medical advice. An EKG recently showed à potential block in the right artery. I take insulin and I try to stay on a low carb diet. A doctor here wants me to take stations. What do you suggest .

    I need advice... 60 year old male, fit body, lifting weights... prior life was smoking, drinking beer everyday, eating crap. Now, carnivore, dropped all habits 3 years ago.
    Had both Popliteal arteries bypassed... Had a CAC scan last night, today's score is 1467! 500-something in both left/right arteries.
    I have an appt. for a cardiologist in Oct.
    PCP says a Statin in comments... Does this really help when eating carnivore?

      It's my belief that there are a number of things than can be used instead of aspirin. I take nattokinase, K2 MK7 and Dr. Schulze's Heart formula as well as magnesium.

      As I know (from the web), Vitamin K2 (menaquinone) is essential for normal blood clotting.. Where aspirin is for thinning. One study found that eating one clove of garlic (about 3 grams) daily for 16 weeks reduced thromboxane by 80%. Thromboxane is a substance produced by platelets that causes blood clotting. I hope this helps. :)
