    When my Airsense 11 was set to full face I was having trouble getting enough when ramp was on at pressure level 4. When it was set to Pillows it was providing more airflow and was a lot easier to breath at pressure level 4.

    I also read that it's the hose. There are other masks that have the hose... the Evora comes to mind. I believe it's to make the response punchier during breathing transitions, something you'd have to read *at the mask*, not at the machine. Like a different algorithm or rise rate when switching from exhale in inhale. Just a WAG.

  • @stuartcrisp2027
    @stuartcrisp2027 6 месяцев назад +6

    An extra foot on a 6 foot hose is roughly 15% extra length, which equates to a reasonable extra flow resistance, especially at higher pressures. Pressure is measured only in the machine and drops along the hose so the mask pressure shown in OSCAR is a calculation based on known factors, e.g. length and diameter of hose, flow rate, temperature and humidity; but also on some estimates and assumptions such as intended leak, mask resistance (with pillows having higher resistance than full face) and maybe mask volume. So it does seem likely that the pillows setting is to overcome the extra resistance of the tube.
    I use an Evora Full and found I needed a higher (BiLevel) pressure support than with a Vitera. I had originally attributed it to mask volume, but the tube resistance now makes more sense to me. I have not tried pillows setting with lower PS yet, but it is something I will do after my next review with my specialist.

    • @stuartcrisp2027
      @stuartcrisp2027 6 месяцев назад +1

      @@flowsy5294 Because I use a BiLevel, my pressure support (and therefore IPAP) is set 1 cm higher than I think I need with a Vitera. EPAP is unchanged. I have yet to experiment with reducing PS by 1 and using pillows mask setting instead. I have a different test running at present.

    I use a Resmed F3 mask and I find the change from Full Face to Pillow lengthens the ramp up time to fill the mask and improve the seal. That is shown in your Oscar readings.

    I've been wondering this for a long time! It's always (slightly) bothered me if I should be using the pillow setting on the Brevida. I tried similar testing and ended up thinking the same thing. It probably does have something to do with pressure change response rate or leakage calculation. Or maybe it's simply a way to lock us in to ReMed masks 😀.

    Remember how old the machines are and what masks were available when they came out. That is what any algorithms should be taking into account at the time. As this is a new mask I would say this one doesn’t match the profile of ‘full face’ at the time the machines were released. So they have said the best match to profile they have available to them

    After switching from F30 to the F40 the resmed 10 stopped auto shutting off after I removed the mask in the morning, had to manually shut off the machine, switched to the pillow setting and now it once again shuts off automatically.

    Jason, I commented on your other video about using the pillows for this mask and it seems I was getting more air,than usual. I’ve switched back to full face,so unless they tell me it’s harmful,I’ll be staying with full face

    • @Freecpapadvice
      @Freecpapadvice  6 месяцев назад

      The only way it’s going to be harmful is if the pressure delivery is changed somehow. (Too little too much).
      This brings up the question of what someone would do it they are using the FPbSleepStule that doesn’t have this adjustment.

    When I saw Nick's video explaining that the F40 needed to be set to pillows due to the short tube, I switched from Nasal to Pillows for the Bleep Eclipse, since it also has a short tube and the same logic should still apply. After I switched, the leak rates were still the same (literally 0.0 leak throughout the night no matter if it was set to nasal or pillows). However, there was a slight reduction in what the machine said my AHI was after I switched to pillows. I know that the machine's calculation of AHI is probably inaccurate, but I am just reporting what MyAir tells me. I am on a fixed pressure of 10, so the machine's auto algorithm isn't a factor here.

      I can see this being affected. I’m so jaded against all flagged events that this really isn’t something that I considered. But I think you may be correct. I’ll have to look at my data with it…..but it’s so inconsistent anyway! 😂

    • @RebelRobot01
      @RebelRobot01 6 месяцев назад +1

      @@Freecpapadviceit would be interesting to see what you find. Since Resmed apparently also suggests setting the N20 to nasal, you might consider doing a similar comparison of the N20 at different mask settings as you did for the F40.

  • @en2oh
    @en2oh 6 месяцев назад

    Given that you have reduced variability by doing this under optimum conditions, you might expect the resists to ne essentially the same. The difference in settings may be the range over which the system will believe things are ‘normal’ and not throw an error code. I suspect that the pillows setting has the widest tolerance and full face the least with nasal somewhere in between. Just a thought… nice video. Tnx for sharing it

    My experience is that the F40 sounds like it's leaking, but it isn't leaking, checked with the one finger test while mostly asleep. Verified per Oscar in the morning. Thinking there's a leak while asleep makes me slap my face; strangely, this wakes me up.
    The F40 also doesn't have plastic parts like a regular FFM; it's mostly silicon. Maybe this has something to do with the "pillows" setting.

    Jason, I'm curious to know what size mask you're using with the F40. I have a facial profile similar to yours and find the fitting guide wants me in a S-W size mask though I'm 6'5" and 220lbs. So far I've become a fan of the F40 although the leak rate is higher than the F30i that I'll use some nights. When I get tired of the noise with the F30i, I'll switch out to the F40 for a few nights.

    I’m wondering if pillow setting will fix my auto start/stop problem. I use an evora full and my ResMed air sense 10 will auto start but not auto stop. The auto start/stop works fine when I use my f20 mask, but the evora full won’t work with stopping (it starts fine).
    This theory developed after I saw the f40 video you did the other day. I still need to test it.

    I love the fit of the F40, but feel too much back pressure when I exhale causing me to inhale more deeply and breathe more rapidly. I can breathe more freely with the F30i but do have leak problems. There is clearly more venting with the F30i. I can’t feel any air exiting the mask with the F40. Anyone else have this problem.?

    Evora FF doesn't even mention what setting to use at all. The F40, F30, F30i, all don't have XS masks and the head frame in small is way too big. You told me to switch to full face for leaks (and didn't mention my head looking abnormally small for a woman 😁) so I'm pretty sure it's not me. Resmed just forgot women need different sizes.
