Layering Skin Care Like A Dermatologist

  • Layering of skincare products is important because factors such as molecular weight, PH and solubility can affect the way these products interact with your skin and with one another. Te three key steps in any skincare routine include: Cleansing, exfoliating and toner. The products that are layered after that should be applied in a certain order to maximize the efficacy and prevent skin ailments like irritation and break outs when not used correctly. In summay, a good skincare regimen should include antioxidants/sun protection during the day and collagen repair (retinoids and peptides) at night. After watching this video, you should understand the basics of layering these products and why it is important to go in a certain order.

    I earned my Medical Degree from UC Irvine and completed my residency in Dermatology at UCLA. Upon completing my residency, I joined the full-time faculty at UCLA as a Clinical Instructor and Professor in Dermatology and Dermatologic Surgery. I then went on to complete advanced fellowship training in Dermatologic Surgery at UC Davis and Cosmetic Dermatology at the Laser and Skin Surgery Center of Northern California with world-renowned expert, Dr. Suzanne Kilmer. After fellowship, I was appointed the Director of Dermatologic Surgery at the Laser & Skin Surgery Center of Northern California where I remained active in clinical trials and have been on the forefront of the latest advances in aesthetics and dermatology. As an Orange County native, I love the SoCal community and currently enjoy being a Dermatologist in Newport Beach, CA.
    My areas of expertise include cosmetic dermatology with a focus on injectable neuromodulators (Botox, Dysport, Xeomin), as well as dermal fillers (Juvederm, Voluma, Volbella, Vollure, Sculptra, Restylane, Belotero, and Radiesse). In addition, I travel nationally and internationally where I teach other physicians injection techniques and about the latest advances in dermatology. I am highly-trained in the use of lasers and energy-based devices to treat all aspects of photoaging, including fine lines, wrinkles, skin laxity, brown spots and facial vessels. I offer a comprehensive approach to body contouring with the use of minimally-invasive procedures. In addition, I specialize in Dermatologic Surgery including Mohs micrographic surgery, blepharoplasty, skin cancer excisions, sclerotherapy, surgical earlobe repair and cosmetic tattoo excisions.

      Do you have any specific brand examples of actives, humectants, and hydroquinones? These are the three I am confused about but would really like to add them into my skincare routine.

      Stacie Buesing on IG, she uses Alastin nectar, Alastin restorative skin complex, and Skinmedica HA5 as humectants. She used skinbetter Alto as a vitamin C and it’s literally my favourite thing in the world! She also uses zo skin exfoliating peel pads for her acids. Skinbetter makes peel pads too (I kind of prefer them but they’re stronger)

    This is so interesting. Most of the dermatologists whom I've heard speak say that physical scrubs are the devil, and that you shouldn't use acids every day. Fascinating to hear a different perspective.

    Medical school flashback as I sat taking notes from your video. Loved it! The only item that may trip people up is Hyaluronic acid as it is primarily a hydrator despite it's name. Like other subscribers, I'd also love a comprehensive list of medical grade skin care products you'd recommend.

    I understood that SkinMedica Essential system with GF was a large molecule and always should go on first even before Vitamin C in order to penetrate.

      @@evelynm6961 she does not wear foundations, concealers or powders. She may wear lip gloss or tiny bit of eye makeup. She has lash extensions, and her eyebrows are micro bladed. Her skin is all natural, no makeup. She is BEAUTIFUL !!!!

    I just purchased SkinMedica Growth factor and the uplifting eye. I’m super excited. I also use ZoSkin GSR I have the sheet from Zo so I’m still confused when I use the Biopelle stem cell eye cream last? And I’m using the Biopelle Tensage growth moisturizer bc I just love it. I use the RetinA/Zo hydroquinone at night. I do have a sheet from my Esthtician... but since watching you I’ve added some products. My dr doesn’t carry SkinMedica ? But I am very excited to receive this tomorrow. I am talking a break from ZoSkins ( though I love it) I’m certain I’ll love this just as much. Thank you again. I received a sample of the NIA24 eye cream .. ( I saw you use that ) so I’m saving that until my Biopelle is gone or I’ll switch on and off. Xo

      I use the osmosis line but love the vitamin C balm by truth treatments. When should I put the c balm on because it’s more of a Vaseline feel but my skin is absolutely radiant the next morning

      +Marsha Woods I don’t ever like to post anything negative but this product sounds scientific but in my expert opinion is lacking the important AHA and antioxidants that the skin needs. Very little data to support the active ingredients in this product. It’s also expensive and similar in price to medical grade but lacks technology. None of the key pathways are targeted so I wouldn’t recommend it 😒

    Dry mature skin does not need 2 cleanses a day. Or an exfoliant. Or a toner. Or an acid. Especially if any rosacea is present.
    Hydroquinone should be out on dry skin first.

    *skin prep 1.) Gentle Liquid/gel cleanse twice a day (no hard soaps/bars and always after micellar water or balm/oil cleanse if applicable).
    *skin prep 2.) Exfoliating cleanse once a day-once a week (what ever works best for you).
    *skin prep 3.) Toner/essence to get any extra dirt/oil off face.
    *active layering 4.) Apply VIT-C *or* AHA/BHA/PHA FIRST
    OR
    VIT-C, then AHA/BHA/PHA. (Let vit-c sit for a minute before applying next step)
    Then, apply Growth factors/PEPTIDES over Vit-C/AHA/BHA/PHAs.
    Then, HUMECTANTS/HYDRATORS like HA, Glycerine, Ceramides, water based moisturizers without oils.
    Then apply your retinol (for during day) or retinoid (for during nighttime).
    Lastly, apply sunscreen if applicable.

    Another great video!! Question for you- I use the ZO vitamin c and exfoliation accelerator and hydroquinone. According to their rubric you would use the hydroquinone, then the exfoliant and then to C. It’s sort of creamy. When I used skinceuticals, i always used that first since it was more of a liquid.

    I look forward to these videos as they are so informative! You mentioned using hydroquinone then Retin A after serums at night so my question is if I put serums on 1st does that decrease the effectiveness of my hydroquinone? My Derm suggested I use it alone in the AM and can mix equal parts of Retin A at night. Would love your expertise on the matter as my serums such as Vit C, peptides, growth factors, HA5 etc are sitting Idle in the closet as I thought couldn't use in conjunction with hydroquinone. I greatly look forward to your response!

      +Katie A ig you’re using a hydroquinone, I’d use that first but it’s ok to mix with serums and retunA. Many compounds (like Triluma) combine retinoids with HQ. As far as products sitting on the shelves, I’d say toss it after a year bc the active ingredients may not be as active over time. Hope this helps!!!

    Hi doctor! Thank you for such an informative video. I’m curious on your thoughts on Dove sensitive skin beauty bar (fragrance free) for the face and body? Doctors seem to love it, but I’ve seen various videos about bar soaps not being good for skin.

      +Crystal R. Noooo I don’t recommend it babe. Dermatologists only like dove soap for body (not face) for people with eczema or xerosis. And that was like 10 years ago. I’d stick with s body wash and facial cleanser.😉

      +lisa munisteri Vitamin C has to be activated at a ph of 3.5 and is a highly unstable molecule. I like revision skin cares Vit C and skinceuticals CE ferulic. I am at a dermatology conference in Chicago and learned this morning that ferulic acid greatly enhances the absorption of vitamin C. ✨

      lisa munisteri What a great idea. I do use L-ascorbic Vitamin C powder. I use a small, dark glass bottle and combine the powder with water, shake, shake, shake and it absorbs. Now I will also add some ferulic acid to the bottle. I make just enough to last about 10-15 days. After that amount of time I can see when it starts to go bad because the liquid goes from clear water-looking to a yellowish color. I clean out my bottle and make another batch. This seems to have been working well for me.

    Great video. So helpful to not only know what products to use but the order to use them to achieve maximum results. How long should you wait to apply make up after applying sunscreen?
    Today I saw advertised a new all in one Collagen Peptide capsule that is a multi collagen capsule packed with 5 types of collagen, hyaluronic acid and vitamin c by Sports Research available on Amazon. Have you heard of this new product? Would you share your thoughts with us? I have been taking the Vital Proteins collagen peptide powder 20g a day that you recommended.       That would actually be a good idea to do so. Retinoids can dry and irritate the skin, so HA would be very helpful, as it attracts 1,000 times it's weight in water.

      Yes...in fact, there's a new FDA-approved tretinoin that my dermatologist prescribed me called "Altreno" -- it's a tretinoin cream that incorporates hyaluronic acid within its formula so that it's more moisturizing and less irritating compared to other brands.

      As far as I am aware, HQ is applied to the whole face, as it lightens the spot and the skin under it. So to have an even skin colour it is applied to the whole face.

      Salicylic acid is a an exfoliator so that would make sense, when there is less dead skin, a product will absorb better

    There is lot of fear mongering regarding hayflick limit of cells
    it says too much exfoliation can exhaust the number of cells especially due to the usage of retinoids and can lead to premature ageing

      +Alice dsouza Nooo my beauty. This is not true. That’s like saying working out is bad because you’re muscles are too stimulated to get stronger. Exfoliation increases cellular regeneration and renewal. You wanna keep those cells stimulated and active and behaving younger. Otherwise they become sleepy, inactive, old and senescent and stop working in a healthy way. Does this make sense?

      My mother has been using Retin A and exfoliating for over 40 years. She’s in her 70’s now and has no wrinkles. Beautiful, glowing skin. She’s also fair and this has been an amazing treatment for her. I would say that she is a testament for tretinoin, personally. It’s never thinned or damaged her skin. Quite the opposite. I started using it in my early 20’s as she was my role model. Hope this helps . 🤗

    Thank you, that was a lot of information. Have you ever given us a graphic on products we can use in different categories. When I look up Peptides, Humectins, growth serums, there are so many different price points to choose from. Sorry if I have missed this if you have already gone through it. I did get to see your CVS trip. That was a hoot. BTW, I have post menopausal chin hairs, some are grey or white. I get waxing done every other week. My esthetician tells me that I have some ingrown hairs and maybe I should lay off the glycolic pads a week before I go in to see her. She seems to think that wiping the top skin layer off may make my skin heal over the new hair follicle so it doesn't break through, does that sound right to you?

    Please explain the following: AHAs to be used at this stage 8:54, or at this stage 12:03 ?
    So, I understand that these acids are humectants, but you have included the same thing in two different stages. So..........?!?!?! How do I do this? Supposing I have a couple of different AHAs, do I use them at the first stage, after prepping my skin, or do I use them after the peptides & growth factors?

    How much facial sunscreen do we need to put on?
    Can you give us some good hydrators? I do use HA5, do I need something else as well?

      I know you didn't ask me any of this, but I hope I can help.
      *Pores*
      Pores are for the most part fixed, due partly to genetics. But most of the things that you can do will be to minimize the appearance of pores. Pores are basically the opening where our hair follicles are, even though it's usually shorter and finer on the face, yes there's hair all over our faces!
      1) Niacinamide can reduce the appearance by reducing oil production (oil glands also located within the pore).
      2) Chemical exfoliation (AHAs and BHA) can also reduce the appearance. They work by chemically sloughing off the dead skin cells and extra sebum from the surface of your skin.
      3) Retinoids, but especially tretinoin (retin A), can help speed up cell turnover (essentially, the time it takes for the fresh skin underneath to surface, and the old skin on top to leave). Tretinoin is also one of the best things you can use to reduce wrinkles, dark spots, reduce texture on the skin, and encourage thicker and more supple skin.
      4) Physical exfoliants can work to, but I personally wouldn't do so more than 1 or 2x a wk. They can be abrasive, and if used incorrectly, do more damage than good. Esp if you have certain skin conditions, such as rosacea, acne, eczema, etc. I would actually advise against using them at all in that case. Make sure it's a finely milled scrub, nothing worse than huge sugar granules or walnut shells scraping against your skin everyday (those are common scrub ingredients). But they aren't necessary in skincare.
      The methods that I know of that will give more permanent results are microneedling, and laser treatments, all done by a dermatologist though. They can also perform higher grade and more intense (and effective) chemical peels than what's offered otc.
      Keeping your skin clean, moisturized, drinking enough water and wearing sunscreen are the best things you can do on a day to day basis to maintain the appearance of smaller pores.
      *Sunscreen*
      1) You need about 1/4-1/2 tsp for the face and neck. About the size of a nickel sized dollop. Though I don't like to wear less than 1/2 a tsp. Because we often aren't that great at applying sunscreen correctly, I want to at least make sure I'm always wearing enough. Usually applying it in 2 layers is more effective, as it ensures the sunscreen filters will form an even film over the skin.
      2) You want to be applying every 2 hrs. Unless you experience enough sweating, or come into contact with water- then you should reapply asap.
      3) Also, after you've applied it, wait 15-20 min before going outside. It needs to form that film on your skin to be as effective as the bottle states.
      4) And if you will be applying makeup after the sunscreen, wait at least 8-15 min after the sunscreen (and dab it on, don't wipe and smear it on if possible). Putting makeup over sunscreen will inevitably disturb it, so waiting decreases the chance of ruining that sunscreen film entirely.
      5) I should also note that the best sun protection is keeping your skin away from the sun, so a wide brimmed hat, sunglasses, and covering other exposed parts will give you the best protection. That isn't always feasible though, but do what you can. I wear long sleeves and have uv protection sleeves (pulls up the arm and covers the hands) outside everyday, Because it's too annoying for me to reapply over and over on my hands and arms, since it gets wiped or washed off so easily.....just my preference.
      *Hydrators*
      Hydrators come in the form of humectants. Humectants work by drawing moisture to the skin. Hyaluronic acid (HA) applied topically works as a humectant. But there are many humectants: glycerin, honey, urea, sorbitol, some glycols (like butylene glycol), peptides, and lactic acid (Which is also a mild chemical exfoliant). Pretty much all skincare has some form or 'forms' of humectants. So adding the HA as a standalone product should be enough, since there are usually enough humectants in your moisturiser already.
      Anyway, back to HA. It attracts and holds up to 1,000 times it's weight in water. But that extra moisture mostly sits on top of the skin, and is transient. For more lasting results, you'd need to see a derm. They inject the ingredient into the skin. Most research shows that topically applied HA can't penetrate the skin barrier because the molecules are too large. But other research has shown that if the HA comes in a smaller molecular form, there is a possibility of it penetrating. Both lines like The Ordinary and Hada Labo have products that contain multiple forms of HA, which increases the odds of one of them penetrating.
      I know this is a lot. It's a lot simpler than it looks. The info behind everything is the big part. An actual skincare routine doesn't have to be complicated though. Cleanser, moisturiser [and retinA if you can handle it], sunscreen - all that's necessary for healthy & slower aging skin. Hydrating serums and actives are all extra steps that you can take if you want, or if you have a skin issue. Good luck!

    You’ve talked several times about staying away from moisturizers and I understand the reasoning however I’m wondering if you would discourage the use of moisturizers from reputable lines such as Alastin, skin medica ect as well.     Hello I have a question. I just purchased Avene from my doctor, they told me it was a prescription retin A. After research I realized it’s not as strong. Is there a big difference between the Avene and prescription retin-A? Thank you.

      Yes there is a difference. Avene Ystheal is a strong retinol but a retinol, not a tretinoid as retin-A. Avene should be converse into the skin in retinoic acid to be effective , but it will not be as effective as retin-A that doesn't need conversion.
