@debbraholcomb Plastic surgery lifts tissues, but doesn't treat the skin or give you glow. Many people with plastic surgery still look old. You really need a multi-prong approach to anti-aging.

I will probably have surgery some day (when I'm in my 50s) to reposition my facial ligaments and fat pads but to date I have not. I'll do a video soon on things that I have done but this is always such a controversial topic and comments like yours keep me from posting one. I leave my older videos up when I am in my 30s and look terrible so that you can see what a difference it makes, now that I'm in my 40s and looking better, when you do less filler and more lasers and tightening devices. Since my older videos I have had an upper eyelid blepharoplasty and have done many lasers and tightening devices which I will post in a future video. In my older videos I was also underweight and incredibly stressed out with a baby and a toddler and in an unhealthy marriage. You can see it in my face. I am tempted to take these videos down but I leave them up to show people that you really can age backwards.

As I watched this, I thought, boy, this woman must be fresh out of college. Then I read some comments & when I saw where Dr. Stefani said she's in her late 40's, you could have knocked me over w/a feather !! All of the dermatologists I watch on YT have gorgeous skin. 10:18 I’m 56 yo and I’ve been using tretinoin for decades. Despite the length of time I’ve been using it I can still only tolerate 0.025% about 3-4 times a week. After applying it I apply CeraVe moisturizing cream. And under my eyes, around my nostrils and around the sides of my mouth I also apply CeraVe healing ointment. So far this has been working.

Hi Dr. Kappel, There is a bit of back and forth when it comes to putting tretinoin under the eye area. I use Altreno which is in a vehicle that is less irritating... but in general would you ever recommend putting retinoic acid under the eyes for help with fine lines? There are plastic surgeons and you tube derms where some say yes and some say no. I would love to know what you think? Thank you!❤

The BEST video on retinoids hands down. I wasn’t aware that the conversion of retinaldehyde to retinoic is reversible. Other YT videos refer to retinaldehyde as “one step away” from retinoic which led me to believe it was a 1:1 conversion. And your advice to use varying concentrations or different derivatives throughout the week makes sense but something I never thought about. I also didn’t know that a gel carrier is more “potent” than a cream. Thank you!

I learned so much from this video. I was told less is more while using Trent, so I stopped exfoliation. Now I know I can do it or try it. On Peter Thomas Roth 1.5% forever. Then I started Trent 0.05 in July and had no peeling and two spots that were a little sensitive. So far, no problems, except I need to get a refill. I use it on the middle and lower part of my face and neck and decolletege, so go thru a tube faster. Thanks again for the indepth explanation.
I can’t even imagine using tazarotene. I’ve been using Murads otc retinol for a long time 3xs a week. A few months ago, I decided to use tretnoin .025 % for 1 of those days. It is soooo strong but skin definitely looks better. Thanks for clarifying the gel vs the cream because my Dr. gave me the gel maybe that’s why I feel it’s so strong.

When I went to use tretinoin, I went onto the strongest prescription 0.1% and my face peeled off for about 6 weeks. I use it every night now and have for a little over a year and I have no sensitization issues. I also have a great moisturizer that I apply 30 minutes after applying my tretinoin. I should have probably started at the lowest percent, but hey, I am one of those crazy patients lol.

Yes although it is a cream and a mid-level percent it can be irritating. Try using it once a week with the sandwich or 1:1 with a hydrator method and see how you do.
Im on tazorotene 0. 1% and got on that for acne treatment. It never helped my acne but i stayed on it since ny derm said it was good for anti-aging. Not sure if it is actually too strong. At this stage, years in of use, no flaking or anything but i wonder if going that high could reduce anti-aging effect? Or if my skin isnt red/flakey/raw does that mean my skin is tolerating it?

Is there some scientific objective way of measuring cellular response that determines this, or is it a matter of manufacture or marketer opinion? For example, I've seen glycerin, aloe vera and vitamin e labelled in BOLD as "actives". Are they? Is there a definitive list? Thank you.
Well the "active ingredients" in a product ingredient list are going to be the ingredients that are essentially working hardest in the formulation, whereas "active skin ingredients" generally speaking are going to be describing more of how acidic, stimulating or aggressive that particular ingredient itself is. For example, I would consider glycolic an active ingredient, but if it was just tucked mildly into the bottom of an ingredient list on a product, I wouldnt consider it one of that specific products "active ingredients". So basically there's sort of two contexts to dubbing something as active. Active on its own vs being a main active within a formulation can depend entirely on the strength, stabilization and specific formulation of the product that it's within. Hopefully that makes some sense.

If it is a retinol then I’d say theses no need but if you meant a prescription tretinoin then I’d say yes give it a try by starting once a week. On the day you use the tretinoin then don’t use the retinol that day. Does that make sense?
I am 64 and have been experiencing fragile skin on my arms for around 20 years now. All I have to do is lightly bump into a cardboard box and the blood comes right to the surface. At the same time if I rub against that same box, the skin peels back and I bleed. I have seen a Dermatologist and what I got from it was that the damage was already done but yet he prescribed Tretinoin. I used it for around 6 months with no change. Why prescribe this if there's nothing to be done? Is there something that can help in reversing this condition?
Can you come up with a visual chart about active ingredients? Something that’s like Do’s and Don’t?! So we can see can what definitely not to do! Sometimes it’s hard to remember all the different details about the ingredients and some of them have “acid” in the name which can come across as dangerous if combined with other things!

Loved this video! I'm been using tretinoin about 2 times a week and your MDAIRE RETINOL and the other nights. Once in a while I'll use the retinol during the day too. I noticed you put your retinol on last in your layering routine. What if I put the vit. c, retinol and then the NMF Hydrator last? Would it still be just as effective? Or is your way to buffer the retinol? Thank you for this informative video. Would love a video on peptides vs growth factors. (I know you talked about it a little bit in a previous video.) You could do the hydrator last but i like to use it as a buffer before the retinol. Either way works fine tho! How is your skin tolerating it that way so far?
I’m curious if tretinoids melt fat. Every single time I use it all over my face or the back of my hands, the next day I notice all the areas that should have fat, look hollow. So I only use it on certain areas and not all over

Hello Dr Stefani, I am taking pharmacy classes, but I am simply a beginner. However, I have an interest in vitamin A derivatives. What concerns me is that I heard from my professor that Tretinoin does affect the baby's development even if you are not yet pregnant. She advised against tretinoin. I want to hear more from you since I couldn't find info online. Moreover, our classes for vitamins are still the basics, but honestly, my curiosity will kill me. Great advice here! I was prescribed tretinoin .1% off the bat as a teen for acne and it messed up my skin so badly with no benefits that I gave up before the first month and avoided retinol for years. I just got back to tretinoin now in my 40s. Started with .001 and it’s making a difference. Only regret is that I didn’t start on it in my 30s.
But my questions is, if you have inherited gene irregularities (which we all do) this stimulating mechanism (retinol, ect.) is going to stimulate those irregularities also, the ones that cause skin cancers, etc. I say this because this actually happened to me. I started using a product (not yours) that had this stimulating mechanism of action and after 6 months of using it I got skin cancer. It actually was a skin cancer my mother or father never had (my parents actually never had any skin cancers, they spent their lives in the sun with no sunscreen whatsoever) My father passed at 84 and my mother is 80 now.
I am 69 years old and have enjoyed a lifetime of good skin. Not at all sensitive (like my husband who sadly has VERY sensitive skin). At 65 I started using retinol responding to all the hype about it. YIKES. I had quite a journey. Just like you say, higher percentages are not better. Just recently I put a mild retinol on my chest. It was not a success. Angry, red patches that are incredibly sensitive appeared - itchy, painful :( I'm using B5 thick cream by La Roche Posay that works better than anything to ease the pain of retinol sensitization. If I have even a hint that a retinol I'm using on my face is causing that slight burn, I stop immediately and switch it to using it on the back of my hands and feet that have some signs of sun damage. Luckily my face, neck and décolleté don't, due to regular use of spf, sun avoidance and upf clothing. I've always worn gloves while cycling (I'm a cycling enthusiast) but I've been guilty of wearing sandals all summer and forgetting to put spf on the tops of my feet. I do now! Now I do use retinol every night but I'm careful to keep it in the lower percentages. I'd love it if you would do a video on red light devices at home. What do you think of them? I'm interested in your products for they are extremely expensive for me (I'm in Canada).

When I have a car again at 49 I'm going to dermatologist to get a tretinion prescription to mix with hydrators for neck face hands .I look younger to majority of people most say under 40. I started using Tretinoin two months ago (with no history of any other Vitamin A products) and I used a pea sized amount once a week for 4 weeks and then increased it to twice a week…so far I haven‘t experienced any irritation and have already seen changes in my T zone (my biggest problem area) I think people get too overzealous with Tretinoin and think they‘re invincible and that their skin is different and that it’s gonna be able to handle amounts that are WAAAY too much😅
I get my Tretinoin from a company called AGENCY! Iv been using this brand almost two years now. It’s an easy process and they take pics of your skin and everything

I would avoid the lips and go very slowwww with the neck. For the neck I'd only use 0.025% tret and I would definitely use the sandwich method for application with a hydrator ;)

My face . My face I depend on for employment at various types of businesses they always put me in front to talk to customers and represent so I ne ed this to be ,stay competitive. I n Ed to look 30 something at oldest because of the way society is .I'm not shallow and superficial !! My face is . My job security ( and that's the good choicey nice jobs also). That's how the work works is for me and I deal with unprofessional female jealousy sabotaging jobs within past 3 years because of they're problem with women who can look good how they envision using hair color brow mahe up ,make up hair styling . So I need to keep myself up at my age . As for personal life I don't have one till I'm more financial stable ,it's been difficult! ( Soon with car again easier more consistent to achieve that )
@@Roaming_Realms2B I could see that my skin barrier was in better health. More resistant to travel/weather. I noticed increased skin healing time with the use of the vitamin C formula. Also the RXR formula made transitioning from never using retinol to easy and poreless skin. So much that I could go without makeup and people thought I was wearing it.. the neck formula is really like Botox in a bottle for neck lines.. have not used the wash yet but I’m sure it’s next level as all these products are! Hope this helps! You won’t be disappointed.

At this stage, years in of use, no flaking or anything but i wonder if going that high could reduce anti-aging effect? Or if my skin isnt red/flakey/raw does that mean my skin is tolerating it?
I've just discovered your channel and I'm learning so much. Thank you! I have a question: How is a skin care ingredient deemed an "Active" or not? Is there some scientific objective way of measuring cellular response that determines this, or is it a matter of manufacture or marketer opinion? For example, I've seen glycerin, aloe vera and vitamin e labelled in BOLD as "actives". Are they? Is there a definitive list? Thank you.
Well the "active ingredients" in a product ingredient list are going to be the ingredients that are essentially working hardest in the formulation, whereas "active skin ingredients" generally speaking are going to be describing more of how acidic, stimulating or aggressive that particular ingredient itself is. For example, I would consider glycolic an active ingredient, but if it was just tucked mildly into the bottom of an ingredient list on a product, I wouldnt consider it one of that specific products "active ingredients". So basically there's sort of two contexts to dubbing something as active. Active on its own vs being a main active within a formulation can depend entirely on the strength, stabilization and specific formulation of the product that it's within. Hopefully that makes some sense.

I use all your products they are excellent! I have a prescription retinol that I quit using when I switched. Should I be incorporating it into my skincare? Starting once a week?
RX is a .05 cream.
If it is a retinol then I’d say theses no need but if you meant a prescription tretinoin then I’d say yes give it a try by starting once a week. On the day you use the tretinoin then don’t use the retinol that day. Does that make sense?

I am 64 and have been experiencing fragile skin on my arms for around 20 years now. All I have to do is lightly bump into a cardboard box and the blood comes right to the surface. At the same time if I rub against that same box, the skin peels back and I bleed. I have seen a Dermatologist and what I got from it was that the damage was already done but yet he prescribed Tretinoin. I used it for around 6 months with no change. Why prescribe this if there's nothing to be done? Is there something that can help in reversing this condition?
Can you come up with a visual chart about active ingredients? Something that’s like Do’s and Don’t?! So we can see can what definitely not to do! Sometimes it’s hard to remember all the different details about the ingredients and some of them have “acid” in the name which can come across as dangerous if combined with other things!

Loved this video! I'm been using tretinoin about 2 times a week and your MDAIRE RETINOL and the other nights. Once in a while I'll use the retinol during the day too. I noticed you put your retinol on last in your layering routine. What if I put the vit. c, retinol and then the NMF Hydrator last? Would it still be just as effective? Or is your way to buffer the retinol? Thank you for this informative video. Would love a video on peptides vs growth factors. (I know you talked about it a little bit in a previous video.) You could do the hydrator last but i like to use it as a buffer before the retinol. Either way works fine tho! How is your skin tolerating it that way so far?
It's tolerating it pretty well so far. If I get itchy or flakey I back off the tret for a few more days. @@DrStefaniKappel
I’m curious if tretinoids melt fat. Every single time I use it all over my face or the back of my hands, the next day I notice all the areas that should have fat, look hollow. So I only use it on certain areas and not all over

I am taking pharmacy classes, but I am simply a beginner. However, I have an interest in vitamin A derivatives. What concerns me is that I heard from my professor that Tretinoin does affect the baby's development even if you are not yet pregnant. She advised against tretinoin. I want to hear more from you since I couldn't find info online. Moreover, our classes for vitamins are still the basics, but honestly, my curiosity will kill me. If you can't respond, I kindly ask for recommended literature I can read.

I was prescribed tretinoin .1% off the bat as a teen for acne and it messed up my skin so badly with no benefits that I gave up before the first month and avoided retinol for years.
I just got back to tretinoin now in my 40s. Started with .001 and it’s making a difference. Only regret is that I didn’t start on it in my 30s.
I hear this story all too often. Hopefully after watching this video that will save someone from that same experence.

It can irritate the skin even more it’s it’s over a year old. I’d toss it 😕
But my questions is, if you have inherited gene irregularities (which we all do) this stimulating mechanism (retinol, ect.) is going to stimulate those irregularities also, the ones that cause skin cancers, etc. I say this because this actually happened to me. I started using a product (not yours) that had this stimulating mechanism of action and after 6 months of using it I got skin cancer. It actually was a skin cancer my mother or father never had (my parents actually never had any skin cancers, they spent their lives in the sun with no sunscreen whatsoever) My father passed at 84 and my mother is 80 now.

I am 69 years old and have enjoyed a lifetime of good skin. Not at all sensitive (like my husband who sadly has VERY sensitive skin). At 65 I started using retinol responding to all the hype about it. YIKES. I had quite a journey. Just like you say, higher percentages are not better. Just recently I put a mild retinol on my chest. It was not a success. Angry, red patches that are incredibly sensitive appeared - itchy, painful :( I'm using B5 thick cream by La Roche Posay that works better than anything to ease the pain of retinol sensitization. If I have even a hint that a retinol I'm using on my face is causing that slight burn, I stop immediately and switch it to using it on the back of my hands and feet that have some signs of sun damage. Luckily my face, neck and décolleté don't, due to regular use of spf, sun avoidance and upf clothing. I've always worn gloves while cycling (I'm a cycling enthusiast) but I've been guilty of wearing sandals all summer and forgetting to put spf on the tops of my feet. I do now! Now I do use retinol every night but I'm careful to keep it in the lower percentages. I'd love it if you would do a video on red light devices at home. What do you think of them? I'm interested in your products for they are extremely expensive for me (I'm in Canada).

When I have a car again at 49 I'm going to dermatologist to get a tretinion prescription to mix with hydrators for neck face hands .I look younger to majority of people most say under 40. I started using Tretinoin two months ago (with no history of any other Vitamin A products) and I used a pea sized amount once a week for 4 weeks and then increased it to twice a week…so far I haven‘t experienced any irritation and have already seen changes in my T zone (my biggest problem area)
I think people get too overzealous with Tretinoin and think they‘re invincible and that their skin is different and that it’s gonna be able to handle amounts that are WAAAY too much😅

Retinol disrupts my sleep. I wish I could use it, but I can't, and I've tried numerous brands. Eventually, I began researching online how it affects me only to discover, I'm not alone.

I would avoid the lips and go very slowwww with the neck. For the neck I'd only use 0.025% tret and I would definitely use the sandwich method for application with a hydrator ;)

@@Roaming_Realms2B I could see that my skin barrier was in better health. More resistant to travel/weather. I noticed increased skin healing time with the use of the vitamin C formula. Also the RXR formula made transitioning from never using retinol to easy and poreless skin. So much that I could go without makeup and people thought I was wearing it.. the neck formula is really like Botox in a bottle for neck lines.. have not used the wash yet but I’m sure it’s next level as all these products are! Hope this helps! You won’t be disappointed.
