How to Properly Exfoliate

  • Exfoliating you skin should be part of your skincare regimen. Exfoliation allows the dead skin cell layer to come off and allows fresher younger skin cells to replace the old ones. Exfoliation can either be mechanical (with the use of a scrub) or chemical ( with a natural acid like glycolic acid). This also signals to the skin cells deeper in the skin to increase celullar renewal. Its all about being "out with the old nd in with he new" as the dead skin cells that may have irregular distribution of pigment get debrided and replaced by skin cells with a more even skin tone. In addition, exfoliating removes dirt, make up and debris that sometimes cleansers alone cannot remove. Weather you exfoliate once a week or once a day, try and Incorporate this into your routine my beauties.

    I earned my Medical Degree from UC Irvine and completed my residency in Dermatology at UCLA. Upon completing my residency, I joined the full-time faculty at UCLA as a Clinical Instructor and Professor in Dermatology and Dermatologic Surgery. I then went on to complete advanced fellowship training in Dermatologic Surgery at UC Davis and Cosmetic Dermatology at the Laser and Skin Surgery Center of Northern California with world-renowned expert, Dr. Suzanne Kilmer. After fellowship, I was appointed the Director of Dermatologic Surgery at the Laser & Skin Surgery Center of Northern California where I remained active in clinical trials and have been on the forefront of the latest advances in aesthetics and dermatology. As an Orange County native, I love the SoCal community and currently enjoy being a Dermatologist in Newport Beach, CA.
    My areas of expertise include cosmetic dermatology with a focus on injectable neuromodulators (Botox, Dysport, Xeomin), as well as dermal fillers (Juvederm, Voluma, Volbella, Vollure, Sculptra, Restylane, Belotero, and Radiesse). In addition, I travel nationally and internationally where I teach other physicians injection techniques and about the latest advances in dermatology.     Thank you!!!! I totally understand that pigment explanation, yes it takes a little thinking and concentrating but I get it!!!! If you exfoliate regularly then the sleepy cells don’t have time to collect more pigment. I really love how you go into the science, you offer a different perspective than the other you tube derm and it is great 👍 you are all contributing to my wellness and my skincare learning journey

    Such a fantastic explanation thank you. I use differin almost daily or retinol. I also use facial wipes with extra micellar water to remove sunscreen and makeup before I use a hydrating cleanser. I find my skin sensitive and smooth, sometimes irritated. Since I have oily skin, I use a scrub with salicylic acid in the shower so I'm getting chemical and physical exfoliant 3 times a week. That seems to be really working well for reducing congestion, no more blackheads or acne, glowing skin, and it's convenient and cost-effective. I hope that that helps someone :-)

    Great video - very informative. Have been using Biologique Recherche P50 as a chemical (acid) exfoliation daily and Clinicians Complex Microdermabrasion Cream as a manual exfoliant twice a week - works really well and helps with pigmentation. Love you channel and all the science behind your recommendation 👍😘

    I think you explained this process very well! I am definitely going to up my exfoliation game. I just purchased The Ordinary AHA 30% + BHA peeling solution. I have oily skin, with large pores and sebaceous hyperplasia. I am hoping this will help. Thanks for informative video!

    Wow, thank you for sharing all that information. Truly, you are wonderful for teaching us. I am 65 and I use a glycolic acid wash and Clarasonic brush every day in the shower. I alternate two products in the shower. I use a Peter Thomas Roth glycolic cleanser and an Elizabeth Grady glycolic cleanser. I follow the Peter Thomas Roth cleanser with a Health Food store rose water tonic then a Peter Thomas Roth glycolic exfoliating pad. After the Elizabeth Grady glycolic cleanser I use their polishing scrub in the shower, then I follow that with the rose water tonic. Depending on what I am doing that day, I either get dressed and use a sunscreen with make up or if I am staying in, I mask once a week or put on my night time serums and creams. (I usually shower in the afternoon, after I have gone to the gym and done a few errands... in case you were wondering. Thanks again.

    Thank you for posting very informative video. I am currently dealing with acne. I have seen a few dermatologists from my country and their principles are quite different from what I read from Western practitioners.       Jan Truszkowski yes you can use both. It’s too complicated to explain the science behind growth factors in the skin so to keep it simple it allows the cells to communicate effectively with one another and optimizes cellular renewal.

    Would love to know your thoughts on melasma. And your thoughts on xenoestrogens and melasma and is it true melasma gets better AFTER menopause? I am estrogen dominant so I have been researching this for a few years and taking a DIM supplement (Estroblock Pro by Dr Nick Delgado that completely got rid of my horrible cystic hormonal acne) but I notice when I take it regularly, my melasma lightens. 😁 I love to exfoliate but I usually avoid scrubs, since it seems to darken my melasma...then again blow drying my hair darkens it too. Grrr. I do love lactic acid, Vit C, retin-a and hydroquinone...and of course SPF. Now using Lytera, taking a lil break from hydroquinone. But if you have time sometime to give your thoughts on melasma, that would be awesome! I've had many different derms tell me completely different things. But you have the BEST info. Thanks so much!!!! 💜

    This makes sense!! Great explanation, I can picture it. I'm happy to learn it's ok to exfoliate once a day! I thought once every couple of weeks was the rule. I use a product called good skin and my clarisonic mia. I use tretinoin 0.1% my dermatologist gave me! Love your videos!! I look forward to them. 🙂❤

    I love to exfoliate and do it every day. I'm glad to hear you talk about mechanical exfoliating as I do that regularly. I hear a lot of people saying how bad it is for your skin, causing micro tears. Do you think that's an issue?

    Is it best to apply your body lotion immediately after the shower onto wet skin to lock in the moisture?
    I’m so glad you gave the thumbs up to daily exfoliating. I love doing it and my skin looks great but I’ve recently heard that it can ruin your moisture barrier and cause inflammation. I was afraid that I might be doing more harm than good but you made me feel better! ♥️

      Ju Du perfect example of why you should site your sources of information. Any board certified dermatologist will say that is simply untrue and wrong! These beauty bloggers who do not have the appropriate background and education make statements like this and mess everyone up. Don’t listen to them my beauties. Would you let someone who’s not an Obgyn deliver your baby? My point exactly.

    💋💋💋 Thank you so much for yet another fabulous skincare vid!!!! I have exfoliated multiple times a week for many years now. My skin looks kinda gray & dull if I don't exfoliate at least every day or two. So glad to know I'm not the only one and that it's actually a good thing to do it that often. What are your thoughts on microdermabrasion as a form of exfoliation weekly or monthly? Keep doing what your doing!!!! 🤗😘🤗😘 So in love with your channel & posts! 💋💋💋
    P.S. Content idea....... Adult acne, cystic acne and how to treat it & heal it as quickly as possible. My skin has been better since really focusing on it but I still have what seem like certain problem pores! 😏🙄 Is that even a thing? Because it seems like I have certain pores that flare up over & over!!!! 😤😡

      asullivan114 perfect! Microdermabrasion isn’t my favorite as it can make the pores look bigger. Think of them as triangles so when you microderm the tip of the triangle off the opening looks bigger. Hydra facial is a better option than the old skool microderm technique

    Hi Stef I have been very interested watching you treat loose neck skin with Thermage. Unfortunately where I live in Christchurch, New Zealand I haven't been able to find any practices that offer this and most of the clinics that advertise 'radio frequency' tightening are performed by nurses or clinicians not doctors. Would I get the same results from a radio frequency course of treatments. Thanks

    Oh noes I had heard that too frequent of exfoliation was overkill and detrimental to your skin and now I've been missing out on giving myself the best possible self care, shoot. So even if you use retin-A, it would be an extra benefit to build up to a daily scrub or glycolic treatment? I had thought once dead skin cells were exfoliated they wouldn't grow back by the next day, but I see now that it's also about cleaning debris off of the surface and out of pores

      joni4taz I live exfoliation accelerator by @zoskinhealth bc it is a 10% glycolic acid and helps remove brown spots too. I’ll get back to the rest of the comments after work today I promise! I’m in surgery right now 😘

    You had me at melasma & uneven/lackluster skin! It was like you were speaking to me. Question: does Azaleic acid count as an exfoliant? Also, is it okay to physically exfoliate if you have some small broken capillaries?     Great video! I enjoy your chanel a lot! In your opinion why is there a difference between sleeping with makeup on ( that would be around 8h) and wearing it on your face for 16h? Why do dermatologists emphasise on taking makup off at nighttime but it is ok to wear it during the day? What do you mean by "letting your skin breath"? Maybe you could do a future video of pros and cons of physical vs chemical/enzyme exfoliation.

      Madalina Nastase great question! It’s important to have about an 8 hour period for your skin to be open to the outside external environment. Do if this happens pens day or night doesn’t really matter. Allowing it to breathe means that the natural process of exerting sweat, sebum and dead skin cells is not blocked by a blanket of makeup over it. Make sense?

    Can you please tell me what the two products you named for exfoliating your body? I have extremely dry body skin. I’ve tried everything and haven’t found anything that works. And do you use them daily just like your facial exfoliation?

      Cinde Davis The exfoliating body lotion was Amlactin which has ammonium lactate. It’s also fragrance free 😊 You can buy at CVS.

      Amanda Sharif Yes but I wouldn’t spend a lot on scrubs for the body. You can even use a DIY like sugar and honey scrub and for chemical exfoliation for the body I’d recommend AMLactin from the drugstore that doesn’t have to cost $$$

    Which exfoliating polish do you recommend? I’m using Epionce products (including Lytic lotion). I’m thinking I should scale back the Lytic to once per day since with twice per day I’m experiencing some light peeling.

    Dr. Kappel, I'm a new sub n so happy to have found ur channel. It's so helpful for someone like me, 53 struggling w/menopause n finding ways to help combat aging. My ? I have is it ok to do 10% GA daily n 20% weekly or how often? Also, will exfoliating help with sagging skin? It feels like my skin has fallen 2in. since meno😳, thanks again for ur time n knowledge

    I started using the Neutrogena Grapefruit 2% Salicylic acid face wash for the past few days and for my oily forehead it is looking great. I feel like its really helped reduce the oil on my forehead, but I am getting dry flaky spots on the corners of my nose, on my chin, and around my mouth. Is this because the wash is too harsh for me or is my skin just not used to it yet? I do moisturize with the Neutrogena hydro boost moisturizer after the wash

    I’m thinking about purchasing the Alastin regenerating skin nectar that I see in a lot of your videos. Do you also use the Alastin regenerating retinol?     I use kojic acid soap three times a week face and body it's just adding the foam to your skin and wait one min, no scrubbing just rinse off its helped get rid of some sun damage too and for me rubbing the skin to exfoliate with product is to aggressive 😍

      Renee Ezell She said to use ammonium lactate which is an ingredient in the brand Amlactin. It’s also fragrance free. You can buy it at CVS.

    Love your videos I have a 12-year-old son that is starting to break out and I was wondering is it OK to use a Chemical exfoliate on him And if so what would be a good option because I see him breaking out but he also has sensitive skin

      I would say yes, but I would use a cleanser with salicylic acid in it you can leave it on the skin for a minute or so then rinse it off. That way it won't aggravate sensitive skin because you're washing it off. It will be on long enough for it to be effective 👍

    Funny. My mom has never ever exfoliated her skin and she has skin like a baby at the age of 70. In fact, all she ever did and still does is "wash" her face just with some drugstore toner (?) and use a moisturizer. I on the other hand have to exfoliate or I have a dinosaur face :(

      Youngr by Stefani Kappel MD but my skin is sensitive and becomes so irritated and blotchy from strong chemical exfoliates. My skin will literally be an oil slick, right, red and comes up in tiny whiteheads. What can I do???

      @@karlachappo5397 I also have the same issue (altho I have combo skin). But I only face this problem when I use glycolic or salycilic. Try using Mandelic acid only; its life saver. I like the one that is made by Vivant :))

    Weird question and not exactly sure how to explain it. But, I have always hated how the area above my lips look "too puffy" I guess this would be my nasal nasolabial folds. Except the area sticks on my face instead more sunk in? like on your face or a normal face. Do you know what fixes this?

    Just recently discovered you. Love your channel! I use 10% lactic acid 2 times a day and I use .1% Retin A daily at night. I have no problems with peeling, sensitivity etc. However, when I use a 25% glycolic, such as Drunk Elephant Baby Facial one time for 10 minutes I get super dry and flaky. Why would that be and is that the goal?

      InTheMiddle I’d stay away from drunk elephant but the regimen you described is good. You may stop peeling and that’s ok. It’s like going to the gym and not getting sore as you become more fit

    Hello Dr I am 17 year old Indian girl .. I don't have any kind of problem on my face and no allergies nothing but I have some tanning on my face ... and few days ago I have started my skin care routine... and i dont apply sunscreen ... but dove facewash toner and moisturiser... I don't know am I doing it right to keep my skin healthy and glowing ... can you please give me any suggestions regarding my skin care

    Hi! So I’m on accutane right now, 30mg and am wondering what products you reccomend. My skin is really dry so I need a good moisturizer but don’t want to clog my pores. I also have a lot of dead akin that needs to be exfoliated but I can’t be too harsh with it. Thank you doc!! 💕😘
