Why you feel a sense of panic when you drift off to sleep (or when you wake during the night)

- If you are living with chronic insomnia, you likely have a high level of arousal. In other words, you are highly alert and when you do relax, it is very easy for you to return to a state of alertness.
This high level of arousal (known as hyperarousal) can make it more difficult to fall asleep and stay asleep.
When we combine hyperarousal with the natural changes that occur in our body as we are about to fall asleep and when we are sleeping, we can easily become very alert and worried during the night - and even experience symptoms similar to a panic attack as this worry produces an intense fight or flight response.
Understanding that many of these triggering symptoms are normal (for example, a sensation that you are breathing more slowly, your heart is slowing, you are feeling cold) can help reduce the arousal response and improve your sleep.

Was having a seriously tough time with sleep deprivation and crazy anxiety and panic because of it. Was feeling like I was having a heart attack, among other things while trying to sleep. Only half remember what was said because your voice was so calming it lulled me straight to sleep and i just accepted the anxiety heart attack feeling and other weird stuff and they passed on through.

I don’t know what to call this, but I randomly wake up in the middle of the night and then as soon as I fall back asleep, I get woken up by this weird feeling I don’t know how to describe feels weird in my legs and brain and it happens for hours I try to fall asleep like 30 times and every time I do I get woken right back up it only happens as soon as I start sleeping I fall asleep get woken up over and and over.
This happens to me too. What I do is lean into it. I used to try to control it and that only made it worse. What’s happening to you is exactly what I experienced. You’re not alone.
Something similar happens to me I get to sleep and then after being asleep for some time I'll wake up with my ears ringing heart racing and this strange feeling like something vibrating up and down my body

This has started happening to me occasionally recently at the age of 43. It's so scary! It's usually when i'm SUPER tired, which makes it evenworse. I do work late and my mind is usually running fast, and I audiobook a lot at night before bed (used to help me sleep but now I think maybe it's a problem). I also started taking a blood pressure medication to help with migraines about a year ago, I'm going to discuss that with my doctor to see if that's a potential issue.

I find watching the tv with bluelight glasses can help me take my mind off hyperarousal and focuses it on something more relaxing which can help me sleep.

This is me😔 I am driffting off to sleep and suddenly I get this fear and racing heart. And can't go back to sleep until 5 or 6 a.m. I fear that the following night the same thing will happen. I know that it is me, I don't have any health issues. I just fear I won't be able to fall asleep and then indeed I can't. I feel very frustrated. What can I do?
Hopefully, you picked up some tips on what you might be able to do in the video, Dominika! Remember that you will ALWAYS sleep, sooner or later! If being in bed starts to feel unpleasant, it can be particularly helpful to get out of bed and do something relaxing and enjoyable until you feel conditions are right for sleep. This can help reduce arousal while also building sleep drive.

I’ve also noticed the dreams I’m having are terrible and horrifying when I’m in this state, for example I’m terrified of rats and after falling back asleep this morning I had a nightmare of like a thousand of them swarming someone
That definitely sounds unpleasant! If nightmares are an ongoing problem, you might want to look into Imagery Rehearsal Therapy.
You may have sleep apnea. Terrible dreams are one of the symptoms of sleep apnea.
25 years over the road , i retired 12 years ago , still to this day i do that , its a form of ptsd , if i fell asleep behind the wheel people including me , would die ...that kind of affects ya after 25 winters and seeing the things i saw ..but now i believe i got lots of health problems , dont know where to start ..
Yes this is me. Sadly this started two months ago when I started feeling very weak with anemia. It felt like I was dying. But I had anemia and my first panic attack. I was experiencing anemia weakness with anxiety symptoms but it felt like I was dying. So every night I’m fighting sleep thinking I’ll die in my sleep.

@@notnadayet7020 it does get better i have been having these off and on it helps to know that it’s anxiety and your not dying but it can definitely be hard to know that when your panicking. it does get better but for be a get it every month or sometimes more than 2 days in a month it depends. but yes it gets better:)
Thanks for sharing. Difficult thoughts and feelings come from your brain trying its best to look out for you - and they can feel really unpleasant and generate their own set of uncomfortable thoughts and feelings, too. Since this stuff is unpleasant, we naturally want to avoid it - so we might try to fight or avoid all these difficult thoughts and feelings. That's when things can become more difficult since we cannot permanently control what goes on in our minds. Sometimes, a more workable response might involve practicing moving away from trying to fight or avoid all this difficult stuff. To acknowledge it and make space for it instead. To observe it rather than go to war with it. To be kind to ourselves in return. This way, we aren't making things even more difficult by adding struggle on top of it all.

Hey please tell me if something like this happens with you or not. Like when you fall asleep you get to experienced sleep paralysis but you are still sleeping as you are trying to wake up and then you fall into deep sleep where you experience your nightmares.
@@messy_.1221 its different with me…all thats happening is air hunger all the time like i try to take deep breaths or either yawn..& nights are worst i wake up in like 30-40 mins of sleeping…i panic a lot while sleeping..i feel pain in chest & back..reports are normal..i just entire day watch my own breaths..at times its so crazy & frustrating to get a deep breath..its been 3 weeks i have been facing this..also like while sleeping if i get a dream i get up gasping for air & then its a trouble to get to sleep..struggling alot!

Mine is awful. I'll hear things that aren't real. Any small (real) noise puts me into fight or flight. It's just terrible.

Hello i have a concern too about my sleep problem, when i fall a sleep and wake up i feel like i cant breath..and the next is im having a panic attack feels like im loosing my breath..
Here’s how mine goes. I go to bed, starting to drift off, slightly feeling the panic setting in, my breathing randomly get disrupted as if I forgot how to breathe, then I snap/jolt gasping for air. What is this?

@sidharth Choudhary i cant say for sure but mine went away after i took vitamin d pills because i did a blood check and i was severely low on vitamin d. Plus I’m stressed a lot. And I don’t go out in the sun very often
it could be sleep disordered breathing. this started happening to me more often last summer. I started having panic attacks because of it. I took an at home sleep study and an in lab sleep study but it feels like I am going to choke when my body relaxes, my heart starts racing and then I go into full blown panic. and then I'm scared I'm gonna choke in my sleep.
Yes I get this during bouts of sleep anxiety! Hate them! I have to train myself to ignore them and drift off again.
Glad to hear you're able to drift off in the end! Every time you do that, you train/remind yourself that you can sleep even if you feel that sense of strong anxiety when you first begin to drift off to sleep.

I had this for years no matter where I try to do. I have had palpitations for years then I had several Holter monitors tests done.Now the last two tests came back with NSVT. Life’s a crap creek!
i'm sorry that happened to you :( how many tests have you done if i may ask? and how long did you have the holters at a time? I've only done a holter once for 48 hours and the doctors noticed tachycardia but said i "dont need to worry about it" but I'm still worried 😭

Very concerned that I can't breathe, this is me. The issue is combined with heavy congestion from allergies. Not able to breathe through the nose at all. I'm currently on 6 hours sleep for the last 2 nights combined. Tonight will be Intesting.

That sounds really difficult, Jordan - and you are not alone! If it feels as though you stop breathing and wake with a gasp, it might be worth talking to your doctor just to rule out a sleep issue such as sleep apnea.
I get a tremor when drifting off and waking. It’s so awful
I have this issue. I was actually relieved because I suspected heart failure.
So I got checked and they said my heart is healthy. And tbh I don’t get much sleep, but a few times when I go to sleep, I’ll hear ringing in the ear it’s like loud, my chest area feels weird like around the heart as if there’s as squeeze and I can’t move but it lasts for a few seconds, but sometimes I’m too sleepy to wake up and try but when I do it stops. How do you lower hyperarousal? I cannot sleep at my partner's house because my body won't let me fall asleep. My mind is relaxed but my body won't let me let go. its getting in the way of my relationship! (and all my past ones!)
I think one of the best ways to tackle hyperarousal is to abandon the anxiety struggle - since anxiety tends to be most powerful when we try to avoid it or fight it!

I experience this kind of situation 2 months ago for one week that when I'm trying to fall asleep or when I'm about to fall asleep suddenly I experience breath pause then it wakes me up its like your body stop breathing automaticaly I know that our body is capable of breathing on it's own but in my experience I feel breath pause when about to sleep or when I'm about to fall asleep most of the time when I’m seconds away from falling asleep I lose my breath and stop breathing also I experience brain zaps suddenly it wakes me up It's like hypnic jerk in my brain I experience this 2 kinds of problems when I'm trying to go to sleep 2 months ago for 1 week and now I experience it again I don't have any problem when I'm already sleeping my problem is when I'm about to fall asleep or when I'm trying to fall asleep I experience this kind of problem. One day I did try to drink beers and it make me drunked so bad that I fall asleep without experiencing it.

This is me at the moment and its literally awful. I feel a rush of adrenaline to the chest though and i get it literally every time I try to drift off. Maybe thats thr fight or flight response serting in. I am exhausted. I am getting about 4 hours sleep a night, if that. I have tried everything. Mindfullness techniques, I do think I have probably associated my bed with feeling like I am going to die pretty much.
I totally empathise, I used to get this (still do occasionally). These adrenaline rushes are just one symptom of hyperarousal. The thing with hyperarousal is the more you focus on it, the more your brain will think it is something it needs to do. If you can start tolerating the symptoms and eventually accept them they will fade away. I talk to mine! If I get an adrenaline rush or my heart starts beating I will just say something like 'OK, we're going to do this again are we? Well brain, do what you need to do. I'm just going to lie here and rest until you're finished'. It happened last night to me. I woke in the early hours, wide awake, having a hot flush and started to feel a little anxious. I was awake for an hour or two just riding it out. Eventually, I fell back to sleep. I came on here just to remind myself that it is nothing to worry about. Stay strong, it will get better I promise!

Hi. I completed almost all possible heart exams, blood tests..., seen by 4 cardiologists in the last year...and nobody found abnormal things.; Just some extrasystole of little significance while on 24h Holter. All they say I have some panic attacks or adrenaline rush. For me is happens just before I go alseep. Especially if I lay down on my side and if I am a bit bloated. Sudden heart pounding that I feel in my chest and neck with normal and regular rhythm. MY heart rate is between 70 and 100 bpm when this happens. I normally stop it with a Valsalva maneuver. The attack lasts maybe 2 minutes if I let it do its thing. I even managed to do ECG with my fitness bracelet while these happened. The cardiologists told me that the beating is OK and is not even palpitation or arrhythmia. I feel anxious when going to bed thinking that it might start again. I really hate this. I do not get scared when it happens. Just surprised.
I get have the same problem. It happens during the transition between being awake and drifting to into sleep. The worst part is it's like I forget how to breath, so I jolt myself awake because it feels like I am dying. It can happen over and over again for hours.. it's the worst.

That sounds really difficult and I am not surprised that you feel anxious when you go to bed - that's your brain doing its job and trying to look out for you. How do you respond to the appearance of that anxiety, and how helpful do you find that approach?

Now I cannot fall asleep at night and I avoid the bed because the anxiety start to grow against my will and soon I feel strange, then something starts vibrating inside me and the palpitations / pounding heartbeats in my neck and trunk start out of nowhere...I manage to stop them with a Valsalva maneuver. I managed to do ECG with my smartwatch 2 times when in full attack and the heart rate was just about 85 bpm. My doctor says to me that the ECG are OK, so is just in my head. My anxiety was somatized. I try to sleep with Melatonin and is not working. So, now I work at night and sleep without problems in the morning after 06 AM until 1-2 PM. All these attacks are coming on me 90% on the situation at night while I want to sleep. What is strange is that I do not feel scared when is happening. Except for the first time when I considered is a heart attack and I went to Emergency Department. Then I was only like calm...and curious amd I was trying to measure my pulse, oxygen, blood pressure...like a scientist. But even though I know it is anxiety, something that is deep into my brain is keeping me out of bed in night.
I always fall asleep and wake up to my back and chest burning!

I experience something akin to this a lot, it usually occurs when I'm trying to fall asleep. I doze off, then BAM I'm awake with a rapid heartbeat and a 'tense' feeling in the chest like the adrenaline rush you get on a rollercoaster. I have to breath deeply to calm down. Lately it's been happening a LOT, it can happen every time I'm trying to sleep and it can take hours of trying to sleep before I actually finally doze off. I also wake up suddenly in the middle of the night with a similar sensation, but sometimes I am shouting out in my sleep and, this is gonna sound weird, but I am literally *poking* my mouth *rapidly* with my fingers. I don't know why I do this - possibly I am panicing in my sleep and I'm trying to force myself awake. I believe this to be a separate condition, and the two problems have 'merged' into one.
I have thought it might be sleep apnea contributing to this, but I don't have any other symptoms usually associated with the condition, and I don't lay on my back at all in the night. I do have year-round cold like symptoms though - lots of mucus in the back of the throat and this obviously builds up over night, forming a kind of 'bubble' in the throat which may be causing my brain to believe I am choking or something is stuck in my throat, even though I am probably still breathing perfectly fine. The point is, there may be something physical triggering these panic attacks (except for the ones that occur when falling asleep, those are a more recent development), but whatever the problem it has become so acute that I am beginning to dread going to bed at all.
It's little surprise that you dread going to bed when you experience these symptoms! Although you don't feel that sleep apnea is a problem, it may be worth talking to your doctor about your concerns. At the very least, an overnight sleep study can help rule out any physical cause of the physiological responses you are describing.
@@InsomniaCoachI will be seeing a Doctor in June, but for now I am keeping a sleep diary making note of how things like caffeine, physical activity, sexual activity and even outdoor air quality (PM2.5) impact my sleeping quality. So far I have about a months worth of data, and will continue for a full year. Some nights I can sleep quite well without any panic attacks, though I do wake up a few times.
@@HuntSmacker It's unlikely that any of those things are having much effect on your sleep. Temporary sleep disruption can be triggered by an infinite number of issues but chronic insomnia is only caused by three factors:
1. Homeostatic disruption (in other words, you are not sufficiently sleepy for sleep)
2. Circadian disruption (usually caused by an inappropriate or irregular sleep schedule)
3. High levels of arousal (this includes cognitive arousal, physiological arousal, and conditioned arousal)

@@InsomniaCoach I beg to differ, I have noticed instant changes from certain activities, however I have yet to collect enough data to prove this. And naturally every person is going to react differently. Insomnia runs in the family on my mothers side, though my father generally sleeps through the night. I have also recorded myself (audio only) sleeping to try and hear if anything unusual occurs while I'm sleeping (snoring, sudden gasps for air, that sort of thing) but so far I don't have enough to work on.
I've always been a somewhat light sleeper - some people can sleep through all kinds of noises, but for me even the slightest thing can wake me up, usually with a boost of adrenaline in a fight-or-fight state to go with it. Perhaps that is something deep rooted in human genetic code which was designed to protect our ancestors? Either way, it's really annoying and does worry me if such adrenaline bursts have any long term effects on health like a person overdosing on stimulants.

I wake up in a panic and hot and then I take off running. Just anywhere! When I was a teenager, I almost jumped out the window! It’s so strange. Happens a lot like 4 times a week, if not more.
Been having this for 2+ years now. Lately it's getting worse, as it is accompanied by some type of electric pain in my head and numbness in my legs / arms. I wonder if this is not a symptom of an underlying decease, such as MS. What do you think?
If that's a concern, I would suggest discussing that with your doctor. If there's no medical explanation for your symptoms, they could be physical symptoms of heightened arousal.

For it's like when i close my eyes i feel scared but i try to ignore it as much as possible then it gets more intense my body starts feeling numb,cold,itchy my heart beats fast and it keeps me up after trying to put myself to sleep about 4-5 times then i eventually sleep it started 3 months ago and for the past 6 days i've been trying to sleep on my side and the same thing happens again everytime i try to sleep on that side the same thing also happens when i try to sleep on other parts of the bed aswell
Many people find that trying to ignore thoughts and feelings makes them more intense and powerful. It's a bit like building a dam to hold back a river. Have you tried acknowledging your thoughts and feelings and practiced allowing them to exist and to come and go as they choose instead? Without that dam, perhaps those thoughts and feelings will be better able to flow and won't feel quite so powerful?

As I’m falling asleep I feel a sense of panic or there’s someone or something in that room but there is nothing there and my heart rate changes but doesn’t increase by much but my breathing changes a lot not in a sense I need to catch my breath but a feeling of there’s no air in me

Have you done any videos on the effects of going to sleep early or maybe on sleeping too much? Why I ask: I can go to sleep early and fall asleep quickly and sleep throughout the night. 9+ hours. But then I feel like hell the next day. Anxious and just a general malaise. Where as if I get a shorter properly timed, not too late, not too early, I feel much better. Any idea on why this might be?
It's hard to say for sure what's going on (and remember I am not a doctor and I do not give medical advice!).
Generally speaking, long sleep durations are often associated with reduced sleep quality since sleep is often less consolidated when obtained over long periods of time.
If you tend to feel unrefreshed and sleepy during the day even after long periods of sleep you might want to talk to your doctor about the potential presence of sleep apnea. Otherwise, if you feel better when you don't allow yourself to remain in bed for 9+ hours, it might be helpful to reduce this a little!

So you should not undersleep or oversleep. Oversleeping can cause your brain to be sluggish and cranky overtime, you can feel malaise, strange headache, heavy eyelid. You should set an alarm that wakes you up after 7.5 hours

Hello Monty! Sometimes when we try to control what can't really be controlled (such as our heart rate, what we think, sleep itself) we might struggle more with those things. You might find the book "DARE" by Barry McDonagh helpful if you are struggling with panic attacks.

I usually fall asleep fine. But once in a while (usually when im extra tired) right as im about to fall asleep, i forget to breathe. Its always after exhaling, i just dont inhale again, like its too much effort or something, and after a few seconds, i realize it and get woken up with racing heart. This will happen 100 times before i can actually fall asleep. From what i can read online, apnea is when youre already asleep and you stop breathing. This is clearly before i fall asleep. Im right about to fall asleep and im aware that ive stopped. Its awful

I don’t know what this is, but this really scares me. I’m an 11 year old girl that gets pajic attacks, severe anxiety, sleep anxiety and thinks something is wrong with me. I wake up every time I drift to sleep and i unintentionally burst awake like my body doesn’t want me to fall asleep. When I have a scary dream, wake up with fire sensations around my body, and continuously keep myself awake. I feel numb and scared wondering why I feel like this and it bothers me so much. Is somnophobia real ? I sometimes feel I have it , because as much as I love pleasant dreams, it is scary that how our body is in state of paralysis while we are sleeping.

Somniphobia is outside my area of expertise. If you feel that sleep itself is scary, it's little wonder that sleep is proving to be difficult! I'd suggest speaking with a healthcare professional who specializes in behavioral sleep medicine if you feel you need help with this.

Good question - I'd ask whether trying to fight or avoid that fight or feeling is more pleasant and makes things easier. Many people find that struggling with their thoughts and feelings can make things more difficult and can end up consuming more of their energy and attention.
I feel like my heart is racing, not slowing. I'm always cold from hypothyroidism :( ..then when I'm randomly awake, my brain is like full on looping about everyone dying.

That sounds difficult. You might want to acknowledge whatever you are thinking or feeling, be kind to yourself in return, and allow your thoughts and feelings to come and go as they choose. Many people find that the more they try to fight or avoid certain thoughts and feelings, the more difficult they can become.

What is the difference between sleep apnea and nocturnal panic attack?
Sleep apnea happens when the airway is blocked. We don't really know for sure what causes panic attacks - but they're often related to the relationship we have with the thoughts and feelings that show up inside us. The more we try to fight or avoid them, the more powerful they can become.

I dont have stress at night. I take my valium and endone before going to bed. Yet as soon I fall asleep,I start to talk, and a frightened feeling wakes me up.

@Insomnia Coach yes ive spoken to my dr and psychologist about it. I do breathing in 1234, out of 1234 normal breathing pattern, as counting and holding unless I'm medatitating, heightens the fight or flight feeling (adrenalin).
We have tried different treatments, medications, and techniques.
I have many health issues and have been classed as complicated case. As I'm on so many medications, my tolerance is pretty high to most drugs.
I've set myself a regular bedtime, read, or do a meditation session before falling asleep. Sometimes it works, and im grateful for those hours I'm able to sleep undisturbed.

Just drifted off 3 times. Before falling asleep i noticed my heart rate was low. Mid 50's i have anxiety so its usually 80-90 even resting. Oh i could also hear it feel my heart beating on my pillow . Anyways 3 times i woke up with that nasty gasp for air, scared, and on the 3rd one almost had a panic attack. Now i am here without one ounce of sleepy in me

Get further away from the camera and mix up the eye contact/body language a bit. It will make your videos much more comfortable to watch amd make peopel more receptive to your point
“Hyper arousal” I wrote about migraine people who are known to have a sensitive brain are more likely to have this.

Just as i start to doze off i can breath in normally but then my throat seems to collapse and I can’t breath out . Then i choke , my heart races , I panic and jerk fully awake and race out of bed. I am over weight . I am utterly exhausted.This is every night for years. My Doctor is no help.

I was just diagnosed with sleep apnea. 40 events per hour and oxygen levels drop to 80. So I wear a CPAP now. But I think there is something else going on. Since my sleep is garbage I tend to fall asleep sitting on my sofa while watching TV or on my laptop. I dose off I don't go into deep sleep. And at that point when you are still conscious but are going to sleep I stop breathing. Is like my mind goes to sleep and turns off the breathing. So I don't think this would be sleep apnea as if is happening right when I dose off. And if I am sitting, the usual causes of sleep apnea aren't there I assume. As they say that sleep apnea is caused by tongue/muscles relaxing and dropping into your airwaves. So does anyone know if there are other causes to make you stop breathing? Or anyone experience similar?

HELLO EVERYONE, PLEASE READ. I have a possible answer to the problem. I started feeling this way about 3 months ago and couldn't figure out what it was. I was scared to fall asleep thinking that I could possibly die in my sleep. For some reason or another, I decided to take some adrenal supplements, and that feeling completely went away. If anyone decides to do this, please come back and say whether it helped or not.

@Taquitosdeasada the adreanal glands produce a hormone called cortisol. When you are stressed, your cortisol levels rise, which is what it is supposed to do, that's normal. However, when you are super stressed,it can cause the adreanal glands to become fatigued. I have anxiety, and I am super aware of everything that happens with my body, and it causes me to be super focused on everything, even subconsciously. When the hyperaousal situation started happening, which I didn't know what it was at the time, I started doing my research and found out that anxiety could be a cause for it, which led me to taking the adreanal supplements. It really has helped my sleeping situation. It may help your situation too. There is no harm in trying them. They sell it on Amazon. I hope I was of some help. God bless.
