I saw one video, that for mazda couldn't add one key, at least two keys should be programmed, tried for mazda 5, 2010 and failed one key programming, with IM608 of course.

I have had same issue too and it turned out to be defective chips now I get my keys from a private store for locksmiths only. Amazon and ebay are good but some are really bad quality
