Many of my coworkers struggle to get by taking care of their kids, and some work multiple jobs in order to do so. I can't imagine how hard it is on them, given I struggle as is getting enough sleep with how long our shifts are.

I always find that so strange because either way there's still the same amount of doctors so it can be sorted by need or money. For example, I'm Canadian and I needed surgery. It was a fairly long wait, but I understood it was because I was low priority (benign growths that would eventually cause issues).
