    This one always gets to me. A family member of mine is an ATC and there’s no way that one operator should have been on duty. The second operator should have been called back from his break when the maintenance crew came in and started disconnecting stuff. Very sad. Should never have happened.

      It's a wake up call. Peter, already couldn't handle the other airline and he tried calling but the land line was down , he should call back the other one on break immediately. Peter was not matured.

      Anything to blame to absolve the criminal negligence of the SkyDrive crew. Well done crafty one ! Peter was the direct, immediate cause of the crash. He would be perfectly aware of the TCAS function and yet he chose to issue instructions to a single aircraft. At the absolute minimum he should've given descend instructions to one aircraft and climb to the other. He did not. Instead he fixated on the Russian aircraft. He also gave erroneous information to the Russian crew - telling them the DHL flight was on their right whereas it was actually on the left. This would have the crew looking in the wrong direction for traffic. When they did spot the DHL plane, on their left, it was hopelessly too late. While Peter Nielsen was only one link in a chain of criminal indifference and incompetence, he was nonetheless the person who finally doomed both aircraft and he is not without blame. Skyguide were forced to pay a pittance to the survivors of those murdered and were not held criminally responsible - the most any of them received was a small fine. Indeed, they had the audacity to sue the Russian Airline for damages.

    It was all a chain of coincidences, from the children missing their original flight, the unscheduled repairs at the control tower, confusion about TCAS etc. It was tragic and sad

      That's always how it is. No single thing is supposed to be able to down a liner so it takes many factors coming together. There are always multiple checks for everything so something has to missed several times by several people for a mistake to happen

      There was no confusion on the TCAS though, the pilots were merely following the air traffic controller guidance which incompetency led to this horrible disaster.

      I don't agree. It has nothing to do with coincidence. Thats like saying it was the children's destiny to die in by flying, and the controller Peter was destined to be murdered by one of the fathers. It was a shame that psychics weren't there to intervene. The repairs should have been scheduled for a time when there were more people working not less. But would that have really mattered.

      I wonder how the killer's wife & daughters would feel knowing he went on a revenge hunt and stabbed Peter Nielsen to death in front of his wife & children? Sicko!

      At first I was happy that he found his daughter and could feel he was the first to get to her after the tragedy. But, now I think this was more traumatizing for him to witness the scene and is why they were discouraged of allowing families of the victims to participate. Maybe it is better that others find deceased and deliver the news in the fashion a police officer would in a consoling manner. Plus I think after this he needed counselling and should avoid the news and not get too involved in what the media had to say as they were looking to blame people without the investigation being completed.

      @@stacey4u2luv Yes I agree he shouldn’t have been allowed to be part of the search. Just finding the pearls alone would make me insane knowing they were once around my daughters neck. His mind heart and soul left this man that day.
      I just hope anyone reading my comment above doesn’t think I condone what he did to the tower controller.
      I just found it amazing in all that tragic loss of life and airplane parts he was able to find the pearls and his daughter pretty much intact.
      Just a horrible situation for all involved. One I never wish on anyone.

    I heard that the other traffic controller was taking a nap on the clock. He should have been in there helping Peter. Especially since they were doing work on the phones etc. Many things worked together to cause this heartbreaking tragedy 😢

      You heard??? Every second of every movement of both men was deeply and repeatedly investigated, and taking a nap during a shift is impossible both morally and physically for a Swiss air traffic controller at the Zürich airport. Get a grip, check your sources.

    Peter was not to blame. Unfortunately I know that deep down he will always blame himself, no matter what anyone says. But the fact is, this was not peters fault. He was extremely let down by a large number of other people who were supposed to be working in conjunction with him to keep everyone safe. Not one single person is to blame for this. This was an absolute tragedy that was not caused by one single person. All of the passengers, the pilots, their families, those in air traffic control, will always be in my prayers 💔 x

    This is the first one I have watched about Peter Neilson that lets me know they have a vase with a single rose in it as a reminder of Peter. This is good as it is a constant reminder to them not to put too much pressure and work on any ATC.

      ​@@wildncrazyguy21It was the fault of Skyguide for not having the proper precautions in place. Such as disallowing maintenance while only one controller is in the room. Understand that the human being makes mistakes. Mistakes should not end in disaster. Peter was overwhelmed, had communication cut off, and was left without critical systems functioning. Any human in the same position would have been very likely to end up the same. The last procedure to enact would be that a controller MUST instruct both planes. Such as telling one to descend and the other to stay level. Peter did not purposefully cause the accident nor did he know what could have happened. Placing the blame on him is merely a scapegoat for the ineptitude of the company

      @@budderkupp1282 Yes, absolutely true. Management is never held responsible. I worked in mining and mine safety. We were required to recommission a mine that had been shut down for being non-profitable. The equipment was already end-of-life and several years of neglect meant the system was inoperable. The entire mine control systems, and all the wiring, was intrinsically unsafe. But the choice was to patch the system back to life or walk away. Walking away meant I would never work in the industry again (the mining company saw to that). Inevitably, a train carrying ore ran away and smashed into the active mine all, killing many people and triggering a partial collapse. Several engineers went to jail - and exactly no-one who was responsible. By the time this happened I had already left the country in search of work.

    Poor Peter and All The Poor People including the Children who were involved in Those Airplanes. Peter didn't mean to let That Happened, he just was trying to Avoid The Collision. It wasn't Peter's Fault😢 If only Hes Co-workers didn't left him that Day, Than They would have Helped him Avoiding The Collision...

    Faulty nonworking equipment was a major factor as well as the tragic choices made in this collision. The ATC did not deserve to be murdered. Tragedy here all around.🙏😥

    That man who killed the guy on the computer's that watches the plane's should have went to the company! Because they were sent in to turn off the phone's to the plane's he couldn't get in touch with them to tell them that they were heading right into each other he tried the best he could they should have grounded the plane's until they had those lines fixed and test them to make sure they were working properly!     Some news reporters are worse than vultures. The issue could be laid at the feet of the contrller taking an extended break. The issue could also be laid on management, scheduling two controllers and maitenance is stupid. Communications must all have redundant backup systems. My personal opinion management is responsible. There should have been policy that all workstations should be manned at all times. No exceptions.

      Peter was the direct, immediate cause of the crash. He would be perfectly aware of the TCAS function and yet he chose to issue instructions to a single aircraft. At the absolute minimum he should've given descend instructions to one aircraft and climb to the other. He did not. Instead he fixated on the Russian aircraft. He also gave erroneous information to the Russian crew - telling them the DHL flight was on their right whereas it was actually on the left. This would have the crew looking in the wrong direction for traffic. When they did spot the DHL plane, on their left, it was hopelessly too late. While Peter Nielsen was only one link in a chain of criminal indifference and incompetence, he was nonetheless the person who finally doomed both aircraft and he is not without blame. Skyguide were forced to pay a pittance to the survivors of those murdered and were not held criminally responsible - the most any of them received was a small fine. Indeed, they had the audacity to sue the Russian Airline for damages!

    I find it very interesting how killing as revenge is a thing among us humans. Which benefits does it got? I mean, does it feel better? Does it give your wife and kids back?
    Is it better to spend life in prison mourning alone than in your own house with all the happy memories before the accident?
    Not being able to grief with other family and/or friends? Not being able to even visit their gravestone. And this guy Peter, wasn't even guilty!! The investigation wasn't even finished yet!!
    He had the odds against him and his mistake was one of many problems that night.
    With this logic Peters family should kill this father for being a suspect as revenge.
    But what do i know? I've never been in a situation like this so who am I to judge?
    But my logic brain would rather prefer that a person I really hate and blame for killing my family stays alive. As punishment he has to live with what he's done.
    And I would make sure to remind him on a regular basis. Just rub it in how much damage he has done to other human beings. If I take his life it's like I'm setting him free.
    If he's to weak he can take his own life. I would't care less.
    But this is only if the person was found guilty.
    Peter wasn't. I can't imagine how he felt that night and afterwords. His punishment was the knowledge of being a part of a tragic accident.
    His murderer set him free from guilt.
    May Peter and all the other victims rest in peace!

    There are two airplanes that are going to collide, and you only check with one and give them instructions while completely disregarding the other plane. Does that even make sense? He was trying to save time, so that he could go back dealing with the other screen. But not only that, you don't even wait to see if the situation is under control. I understand what led to this situation in the first place, but I also believe he handled it very poorly. He did not deserve to die tho. The blame was mainly on these corrupted organizations that prioritize money over safety.

      That's how it's done. You tell one plane to climb or descend that's safer than both planes changing altitude like here. Generally someone will be where they aren't supposed to be and someone is on track so the one off course is told to deviate. If both are told to react that increases the number of potential conflicts. Had only one descended it would have been fine but both did. TCAS isn't really supposed to ever be needed. If it activates several things have already gone wrong.

    This whole situation is a tragic chain of events. I can't imagine how the father who lost his kids felt, but also I can only imagine how Nielsen felt after the disaster in general. I wouldn't be able to live with myself in either his or the father's shoes. This show sometimes really makes me appreciate my life, kids and everything that's important to me. My ultimate nightmare is dying in a plane crash or losing loved ones ,( especially my parents right now because they've been going to Fl every other month now,). And yet I watch these to fall asleep lol. It's the narrator's voice .something about British accents 🥱

    Every time I get to the Avianca Crash I'm always reminded of the Attorneys remarks and that of the Spanish Passengers that lived talking about the word Priority rather than Emergency. There are words like Pan that means a cooking utensil in your kitchen but to a controller Pan Pan means something completely different. If they had said that they were declaring a Fuel Emergency the aircraft would still be intact and they would still be alive but may have some professional troubles after the incident. Basic English is the primary language of Aviation world wide, but there are code words that you must say in order to have "priority status" there a bunch of other aircraft in the hold too that want to get down, If you decide not to divert to Boston when you had the fuel and almost run out of fuel holding in New York , YOU must say the word EMERGENCY. That's kind of it, westerners flying into China or Japan use the word Pan Pan or Emergency and those words are immediately recognized.

    How anybody could blame Peter for the collision is beyond me…he did everything humanly possible. It was more the fault of the maintenance workers who disabled the phones and radar. Should have someone to replace a co worker who needs a washroom break instead of overloading another controller.

He just neglected both planes that were about to go on a collision course, never bother to check the altitude of both planes which were clearly descending at the same time, and just went on to check on the Airbus instead.     Even for the Avianca flight, no sense of urgency whatsoever. Especially, the pilot in communication with the air controllers. Too much attitude and arguing in the cockpit. Baffling. I was shocked when the word emergency finally left his mouth when it was too late. I thought he would go down while assuring that all was good.

    TCAS needs to make its algorithm simple and known so humans can use those same rules. If the controller had known that TCAS was going to make the Russians climb he wouldn't have ordered it to descend. Something like whoever's going more East or North with North overtaking East goes up and the other goes down. Might be hard in complex environments, so instead have the radar displays *show* that TCAS has intervened and how (like a red arrow pointing up or down, and the involved planes flashing or something) so controllers are aware deviations are ongoing. Hard to believe they didn't incorporate this already, as TCAS uses transponders, which is what secondary radars are displaying anyway.

    This tragedy didn't have to happen. If Peter Nielsen would've done 1 thing it all could've been avoided. When the engineers came in and told him what they were going to do, he should've grabbed his manhood and told them they must wait until the 2nd controller comes back from break and it's not negotiable. It doesn't matter if it 1 aircraft and its the Wright Brothers first machine, the engineers must wait.
    Every controller should be made to watch this episode, in training, and once a year after. This will drive home the point that no controller should ever find themselves in this position..EVER!
    Never allow outside forces to tell you to deviate from what you know is a responsibility that could lead to a situation where lives are in your hands. So while I feel so bad for Nielsen. It was his fault, because he believed he could handle the situation he was put in, and he didn't look at the whole picture after he was told what they were going to do.
    Sad because Nielsen paid for his mistake with his life.

    This was a horrible mistake there’s plenty of blame to go around. But what company allows a air traffic controller to go on such a long break and expects another air controller to watch two screens. That is impossible to give your full attention to a screen. Common practice no that’s asking for a a disaster to happen. Who allows phones to be totally disconnected and controllers don’t have the resources to call out or screens to give lag time it is critical for any company screen to work properly just because you have a slower than normal air traffic in the sky it puts all those planes at risk. The company is responsible for poor performance lagged safety procedures overworked employees. Peter I’m sure never went a minute without recalling what happened that night but it wasn’t his fault sure when he contacted one plane to descend and didn’t the other haunted him. But the truth he has to many things going on doing two positions at once watching to screens which common sense tells you that impossible to give your full attention and safety to any aircraft and faulty equipment no phones it is tragic after so many lives lost changing so many people parents forever children lives lost than he was killed and added another death. I won’t say anything about the father I believe he went totally insane after the death of his entire family. He heard so many news reports blaming the Peter and in his mind not stable took his revenge on Peter. Right absolutely not. But grief and loss and unable to cope can do change things to mind. during loss as the mother of one child killed we didn’t want anything to happen to Peter! Enough deaths. The ones that should have took responsible is the company the string of events from Peter down to the pilots the company set the stage for all the events that followed. but as we know it’s the bottom line make everyone else to blame. The mighty dollar must be protected instead of lives and knew there practices were dangerous. My all rest in peace.

    it is arguably the best MAYDAY/Air Crash Investigation episodes, i mean normally human suffering after a deadly crash is just sad and a buzz kill no matter how detailed they are about the sufferings fats because of the children and the horrible end... even i despise peter n. and think he's done a crime and ofcourse the chain of command of SKYGUIDE, But The one who really GOT OFF THE HOOK IS THE ATC TAKING A BREAK leaving PETER THERE< he isn't maybe as observant as other ATC's that's not his fault and SKYGUIDE TRAINING IS LAUGHABLE BACK THEN EVERY OTHER ATC CENTER APPROACHERS AND TOWER ALWAYS SAID UNDER THESE CIRCUMSTANCES
    "BUT IF TCAS ADVISORY EXPEDITE TCAS ADVISORY was already saving hunreds of lives for 5 years, THUS......

    I guess, on the first one here, he should have started screaming loudly he needed help immediately in case someone hears him and helps, even if we know no one will hear. It was truly a freak accident because they were both descending to avoid each other and they descended to the exact same spot. Very sad.

    Expecting someone to do their job after snatching away all their tools is not correct. Peter was very very unlucky that day.
    This incident could have been prevented. Very sad.

    I do feel bad for this, Peter like I mean, yeah the guy was experienced and everything like that, but he had a workload put on him that he never should have realistically had to try to deal with that was very unfair to him

    Why would you need to track a waypoint when given a straight in approach. You fly runway heading, dump altitude, report altitude and land. Screwing around with computer inputs at that part of the approach is just plain stupid.

    At least something positive came out of AA 965. Now, when TOGA power is applied, speedbrakes are automatically retracted. This at least takes one task away from the pilots when executing a terrain escape maneuver.

    i don’t think this was anyone’s fault. I mean y’all realize the phones broken, the airplanes alerts, and the poor dude going back and fourth. It’s hard because the guy told them to descend and didn’t know that the planes alert told the other plane to descend, so i guess the lack of communication there. In this case i guess the pilot took Peter’s word over the technology (which yes wasn’t right in this case), but it’s understandable you don’t know what you’d do in the situation and they decided to trust the human. Not to mention Peter didn’t get the alert for the collision another technology thing. So i just wish the mechanic told Peter the whole thing, which the video said they didn’t. Ofc the mechanic didn’t mean for it to happen, just in future should let the controlling people know that certain alerts wouldn’t work. Imagine if it was working that day, Peter probably would’ve caught it in time. But then again, i think it showed that maybe some of the controllers might rely to heavily on the technology yk? Honestly seems like a huge misunderstanding and communication. Mechanic not telling Peter everything, DHL not telling Peter the alert told them to descend, Peter telling the other plane to descend, Peter not being able to move a different plane to a different center, and the poor dude who caught it not being able to reach Peter, or talk to pilots due to the rules.

    Very sad story! It gets worse when you find out that Vitaly Kaloyev later found Peter and murdered him for what happened to his family. He went to prison was sentenced to 8 year.

    TCAS instructions should have been standardized worldwide from day 1. This accident was totally preventable
