Did you use booty mods by chance ahah?
theres a bug w those mods that messes w your save file and does that

It's kinda crazy we are still seeing this visual problems. It's actually been a problem for a few years now too. Just see if you can remove all your mods and launch a fresh game to double check. Is that solves your issues, double check over dependencies for your mods. Sometimes custom bodies etc will require a framework that may or may not be out of date.

I personally haven't experienced this issue on the PC version, but going through the patch notes it's a bit vague on what "fixes" they have done. They seem to be specific quest fixes. Best bet would be to just report this issue on the forums or CDPR's official Twitter posts about patch updates.

How do people get cyberware like what's in the thumnnail?

They are just Cyberware mods and body replacers on the Nexus Mods if you're on PC. In this particular case it only affects V as it's a unique framework for the player character.
