You're right, the co-op campaign takes place after the singleplayer campaign. After that, she releases Chell and starts testing with the robots. So P-Body’s hat and glasses is part of the “Robot Enrichment” thing. I can’t remember where, but there is a menu where you can customize your robots (give them accessories and skins) and I think get custom gestures. Some items are given free - like a beanie for finishing singleplayer (I think) - but most require payment.
You actually did sequence break the co-op campaign…sort of. The bit with the bird was a free DLC called Peer Review that takes place after the main co-op campaign. I believe GlaDOS references the humans you found in it. In any case, glad you played (and enjoyed) it!
There's even a message before going into the Art Therapy courses that says "WARNING! DLC courses take place after the main co-op campaign!" or something like that.
Wheatley found the Co-op bots during the solo campaign. At the end when Glados was back in charge and Chell sent to the surface, she figured that she didn't have access to the whole facility, so she deployed the co-op bots to learn that there are a bunch of humans in stasis out there. Then you find the humans. Then she wastes them all in about a week, where all the "future" shenanigans happen that culminate in finding the birds. The End.
When Glados says "her" she is talking about Caroline. When Wheatly says "her" he is talking about Glados, and Chell doesn't speak because she is a mute lunatic

The camera lock on the vault was purposely put there by any survivors of GLaDOS' first neurotoxin wave. It is designed only to be unlocked by a human gesture. Not sure how you and your friend played, but back when me and friends played, we always did a gesture in front of GLaDOS at the end of each chamber; and over time she gets progressively annoyed until the end, where you have to do that to solve the puzzle.
