You are speaking my language, my dude! I have been using the Pomodoro Method which is pretty much what you described: 25 minutes focused on the task, 5-minute break, repeat 3 more times, then you get a 30-minute break! Making lists is good, but another thing you can add to that is a time log. Basically, I plotted out how much time I spent doing any task/meeting so that it would help me see realistically what I was able to accomplish in a day! You can do this before you start your day, and then modify it as the day goes on, it's been so helpful!
A technique I like to use is having my self talk be the voice of authority. That internal dialogue of "it's boring" or "i'll do it in a moment" is actually quite destructive. So I use that voice to effectively be the stern parental figure and tell myself things like, "you need to do the dishes now because if you don't, you'll have to wash them while you're cooking and that'll just make things so much harder", or "you need to read those boring articles otherwise you're not going to learn that thing you've been wanting to learn for years". I find it helps best when I say it out loud!

Good idea! I suppose in essence that’s looking at the consequences of NOT doing what you need to do - I like it!

How much easier is doing these tasks with meds as opposed to not being on meds ?
It makes a big difference for me..
