I learnt the hard way with don't number one, I titrated from 30mg to 50mg far too quickly because I was chasing the euphoric feeling. The side effects and aftermath of taking 50mg set me back one to two months in the titration process. I experienced headaches and nausea on a daily basis whilst taking the 50mg and for a couple days after. The Doctor took me off vyvanse temporarily. He put me on 5mg fast release for 4 weeks whilst I recovered and now I am realising that 20mg is my dose (I requested the start very slowly). Incredibly I am getting 12 hours of use case from 20mg when I was only getting 8 from 50mg plus nasty side effects. The bottom line - start really slowly and do not chase the fuzzy feeling, chase alleviation of your symptoms. I find it very useful to write down the symptoms that I want to alleviate i.e. focus, prevent excessive thoughts etc. If the medication stops working, these targets will not be hit.

Thank you for making this! I just started ADHD meds for the first time a couple days ago, and I've definitely been feeling ridiculously good and productive. My doctor didn't tell me about a honeymoon phase associated with my meds at all, so I feel I definitely would have fallen for that trap of increasing the dosage to chase how I'm feeling right now.
But, only being a couple days into taking an ADHD medication, I think it's quite possible that a lot of the more euphoric feelings might just be me being excited about how the meds are working as well as they are. Even after getting formally diagnosed about a month ago, I was still a bit afraid that they got it wrong and all my symptoms were just my own personal shortcomings, and just things I would have to deal with forever. But the meds so far have made significant improvements to my symptoms, so that finally felt like the proof I was waiting for.

For dosage, I had a strange and interesting experience (I'm on concerta), lower dosage actually gave me worse side effects+ a crash in the evening, while increasing the dosage made me feel more stable. To be honest there has been a mild effect getting some stuff done.... trust me a mild effect to stop racing thoughts is MASSIVE... productivity improved marginally. I might be going with a full tablet (10mg) for one morning and then assess and maybe 5mg the same afternoon. reason to increase is to help focus and stay on task better.

Hope you're getting on well, helpful videos to put my own journey into perspective appreciate the content you have out out so far. 👍
I went to the doctor about 5 years ago, got diagnosed as lockdown hit. Started in concerta then titrated up but found that it sent my sensory overload into overdrive, made me super anxious and increased social anxiety, ended up shutting myself away actively avoiding social contact of any form (always been slightly that way but increased with concerta).
LancUK who oversee my prescriptions switched me to Elvanse 30mg which suited me a lot better, titrated up to 50mg and find I'm like a zombie, I can focus on absolutely nothing (as opposed to being unable to focus, I can't drag myself away from sitting stupified) for hours and hours just sitting staring at the tv not taking in what's going on, also find the insomnia messes me up massively, I'll take a dose at 6/7am then not be able to sleep until 5am the next morning which results in missing doses for the next few days as I try to get my sleep pattern back to normal.
Every now and again however I'll have a day where I take it and it's like the stars have alligned, I'm well rested enough and the medication works and I feel motivated to get stuff done and stay on task.
Got a review coming up soon so I'll see what they say but I suspect I'll be reducing back down or switching to something else.
Sidenote my doctor tried me on sertraline for depression and anxiety which didn't agree with me, went for 6 months to a year without trying anything else before going back to the doctor.
They put me on Mirtazapine 15mg then 30mg which has definitely helped, also had the added affect of helping me off to sleep as I take it before bedtime.
    Hope you're keeping well. 👍

  • @roro-mm7cc
    @roro-mm7cc Год назад +3

Definitely agree with the never ending dose increase thing + exercise being super important. If you have been on a higher dose for awhile it can sometimes be better to decrease the dosage even though you may think you need more. For me I was unhappy on 50mg so the doctor increased to 2X30mg per day ie 60mg total. I actually found 60mg to be even worse than 50. I decided to try just taking 1 x 30mg which ended up being way better in the long run after a few days of settling into it.
My theory is that this is due to the long half-life of lisdexamfetamine. On the lower dosages you will basically completely eliminate the drug from your system by the time evening comes allowing your brain to replenish dopamine etc overnight. High dosages may initially feel better due to a sharp spike in lisdexamfetamine but this is a very short term thing. Due to the long half life, if you take a high dose there will still be a significant amount of the drug in your system all the way through the night giving your body less of a chance to replenish dopamine stores and return to baseline before the next morning. Basically high doses constantly expose your nervous system to dexamfetamine and prevent you from being able to naturally replenish/restore your brain in the evening. Elvanse also had a diuretic effect meaning you become dehydrated over time due to loss of electrolytes which could also be a cause of decreased efficacy - this side effect will be exasperated by higher dosages. Also not eating enough (another side effect more intense with high doses) prevents you getting the nutrients you need to replenish dopamine.
All in all I think high dosages of elvanse are a bad idea and it is most effective (long-term) in low doses. If you genuinely need a higher dosage I think it would be better to not go with elvanse but instant release dexamfetamine because this is eliminated from the body much faster. You can’t just constantly expose yourself to dexamfetamine and expect to feel good forever your body needs a chance to recover at least overnight.

    Hello Oliie,
Thanks for uploading your experiences on the channel, they are really helpful. I would like to ask you, this week I started with elvanse 30mg after having a bad experience with concerta, and the first 2 days were amazing, but from the 3rd to the 5th I started to sleep a little less than usual (6h instead of 7,5h or so and of worse quality) and in the afternoon, when the medicine loses its effects, the headaches prevented me from doing anything else but going to bed. It's a very bad feeling, so much so that I'm afraid to take a pill again. Do you think it has to do with the fact that I should try a higher dose that covers more hours per day or that the body has to get used to it?
I need help, because I know that it is up to me, besides the psychiatrist, to find what works for me. The truth is that the sensation experienced in the first two days of use after having very low self-esteem, especially at work, has been incomparable. Now I understand the differences between those who have adhd and those who don't, and they have a big advantage.
    Thank you in advance.

      Hi Christian,
Thanks for the message, i appreciate you watching! Happy new year to you!
I think you'd be best speaking with your prescriber really - they should make these decisions along with yourself. In my opinion, which is purely based on anecdotal experience, my body didn't have to 'get used to' the medicine as such. I didnt suffer majorly with headaches or insomnia, but that isnt to say the medication isnt right for you if you are having these symptoms! Perhaps go back to your Dr and discuss the symptoms youre experiencing and see if they can offer any insight on ?dosage changes or ?type of treatment.
Sorry i cant be of more help, but please do keep me posted with your progress!
      Ollie.

Hi Dan. You're right, the waiting list was one of the main reasons behind my choice to go private. My treatment is with Anchor Psychiatry - I believed they are only based in Norfolk though! In terms of advice, Reddit forums are great! Take everything with a pinch of salt, but for the most part its full of people going through a similar process. Good luck, Dan!

Hope this caddra chart can help you ask for long acting med like Concerta/Elvanse and or nonstimulant, so less slump end of day. Www.caddra.ca meds chart, and handbook free there. I found I had to learn or got far too low doses given the family member and poor results in NZ and not 2 meds. Also this www.additudemag.com/autism-medication-treatment-help-parenting/
Covers the 3 types considered to get life better depending on symptoms. True meds are not panacea but right meds are amazing for quality of life n success. Same two pills a day suited for 3 years. Like insulin 4 diabetes.
