5 Things I Wish I'd Known About CPAP Masks from the Get-Go

  • I'm still getting the hang of this whole CPAP thing, but in the six months since I started therapy, I've learned SO MUCH about masks that I wish someone had told me before I started a symbiotic relationship with one.
The most important things I wish I had known:
    1) Masks come in all shapes and sizes, and what works for one person won't work for another.
    2) If you hate the first mask you're prescribed, most DMEs will let you swap it out within the first 30 days, or they'll just give you a new one.
    3) Many modern masks, like the Philips Dreamwear and ResMed "i" series, are modular, so if you have a full face mask, you can just swap two relatively inexpensive bits and have a completely different mask.
    4) Even a lifelong mouth-breather like me can learn to nose-breathe, although what works for one person won't necessarily work for everyone.
    5) I don't care who you are, there's almost certainly a CPAP mask that will work for you. Don't give up. Be stubborn! Decide that you're going to find a mask that works and you will.

Great stuff, Dennis. One comment my sleep doc made when we were discussing mask choices for my forthcoming CPAP: "All masks leak." I'm sure that's partly true, but I'm also sure there are degrees of leakiness. Some leak more than others depending on the face, facial hair, etc. Keep up the fine work, sir!

    • @AdventureswithCPAP
      @AdventureswithCPAP  3 года назад +19

That's true! It's absolutely true. But it's complicated. These things have to ventilate so you're not breathing in your own exhaled CO2. And each type of mask has a different intended leak rate. You'll typically find that anything less than 24 liters per minute is the correct amount of leakage for the average mask with the average user at the average pressure. Anything above that, the machine has to work harder to maintain your prescribed pressure.
So, yeah, I need to get into the habit of being more specific about this stuff. I use "leaks" as shorthand for "large, unintentional leaks outside the design parameters of the mask and machine." But that's not obvious! And I promise I'll do a better job of spelling that out going forward.

I have heard it from many people. You can hear them cough or literally stop breathing. Many never get to sleep at all they never reach REM. They claim they just can't tolerate on their face and they get clostrophbia so they do nothing. So many days get shaved off of your life if you don't get your head together and you learn how to tolerate some discomfort. Get ahold of yourself and control your life. The first day after I woke up after using my first CPAP. Was amazing. I hadn't slept like that since I was a kid. I was a new man. That was 31 years ago. I can not sleep without it. Without it I have bad headaches. It's miserable. I dread possible power outages. Long term ones for 3-5 days means no sleep for me for 3-5 days. I am on my 6th machine. Do yourself a favor get it and use it. Loose your stupid ego. No one is perfect. There is no shame in discovering you're not either. You wanna be a real man? Take control of your heath!

I’ve spent six years studying failing CPAP users for a full year after one month of non-compliance and have a huge catalog of things that keep people from succeeding with CPAP. I will retire in six months and plan on starting YT channel called Sleepy Tom, the CPAP Evangelist. I would do well to give a small percentage of the valuable info you have given in this presentation. I would love to add my two cents worth from a professional stand-point. Let me know. @@YES-IM-TROLLINGAre you a Truck Driver that don’t want the DOT all in your life? 😂 the aggravation of dealing with all this crap? If so I’m right there with ya, I have to go this week to set up a sleep study, because they got me dozing off a bit on camera, I do believe I may have a touch of sleep apnea but honestly my MAIN ISSUE is my jobs stupid “schedule” or lack of…… you never know when you’re gonna work, how long you’re gonna be gone….. you just wait by the phone and they call you and say “be here at __________ time” and it could be anywhere from 18 hours from the call, all the way down to can you come in RIGHT NOW!! Usually it’s about a 10 to 12 hour heads up, the problem with that is it’s NEVER the same time Daily they may give you 10 hours off in between, they may give you 48 hours off in between so YOU NEVER CAN PLAN OUT YOUR SLEEP!!! So what ALWAYS HAPPENS TO ME, I just sleep when I’m tired and wake up naturally, and IT ALMOST ALWAYS WORKS OUT, that they call me after I done had a big 10 hour good sleep, then they will call me maybe 2 hours after I wake up, and say “be here…. 12 hours from now” so now I’m in a situation where I am WELL RESTED, done Been awake 2 hours, got to be at work in 12 hours….. AND I JUST CANT GO TO SLEEP!! So now RIGHT WHEN IM GETTING SLEEPY…… it’s time to go to work….. and I usually get runs that I have to use up my whole 14 hour clock, plus I live an hour away from my job…… so now I’m at a point where towards the end of my run I’m pushing at OVER 30 hours STRAIGHT AT BEING AWAKE!!! And this has been the case for the couple times they got me dozing off, but now THEY ARE FORCING ME TO DO THE SLEEP STUDY, well I will probably show to have sleep apne, because I have had some rapid weight gain over the last few years…. And I do wake up sometimes from night terrors of someone choking me or smothering me, or like I am drowning in a pool and can’t breathe…… SO I WILL PROBABLY FAIL THE SLEEP STUDY (so to speak) but I believe THE REAL PROBLEM IS MY JOB AND THE STUPID HOURS, and they TELL YOU “ if you think you’re tired and can’t do the run just say no to the run SAFETY IS #1 😁” then hit me with the fake smile……. But just as I thought they would do the times I have told them I don’t think I’m going to be able to get enough sleep so I can’t take this run, well they get mad at me, they purposely punish me by giving me multiple days off with no work, they know that I enjoy getting hours so they punish me basically saying to me without actually saying it “ either you take what we got for you to do or you will sit your ass at home with no money“ ANY TRUCK DRIVER KNOWS THIS PAIN….. so anyway now I’m at a point where I’m going to be forced to probably get a CPAP (which COULD BE BENEFICIAL, OR COULD BE A NIGHTMARE) but more importantly, I have ALWAYS worked long hours, ran off little sleep, but I NEVER HAD A PROBLEM DOZING OFF DRIVING til I got THIS JOB with the stupid scheduling…… my problem is it really I can stay awake for a long time with very little sleep (if I have a Night where the sleep apnea doesn’t affect me that bad) so my job is treating me like I am this sleepy ass that can’t stay awake but really my problem is I am INCREDIBLE AT STAYING AWAKE 😂 and I just can’t GO BACK TO SLEEP….. from the time they call me til the time I need to clock in at work…. So I think I probably just need to find another Job but right now Trucking ain’t TOO HOT, so I figured I’d just submit to the sleep study, get the stupid machine….. and hopefully I will benefit from it….. and maybe then I can start job hunting….. BUT YOU SEE….. now I’m gonna have the DOT up my ass when I don’t really think my sleep apnea is that bad YET, however my worst nightmare is to run over a minivan filled with an innocent family, so if the CPAP/BPAP can even help A LITTLE BIT prevent that…… I’m down to suffer a bit…..but still I believe the main problem is MY JOB And I know there are many jobs in trucking like my job where they preach safety……. BUT IN REALITY THEY JUST WANT YOU TO HIT THE ROAD WHEN THEY WANT YOU TO NO MATTER IF YOU ARE GONNA BE SLEEPY OR NOT…. I think I really just need to search for a totally different career but that is not something I can do overnight so until then I guess I will do all I can do to keep from running over someone….. anyway sorry for the loooong reply, this was really meant for you, and the sleep tech guy AND ANYONE ELSE to read who may be dealing with the same thing…… I had the QUICK THOUGHT of letting someone else do my sleep study FOR ME, because I genuinely feel like….. though I MAY HAVE A TOUCH OF SLEEP APNEA, that is not my MAIN PROBLEM, if I have a schedule I have NEVER HAD A PROBLEM getting enough sleep to stay awake/aware during my shift or multiple shifts when I work two jobs, so I hate to spend all this money and have the DOT up my ass when I feel like I really don’t need that in my life, but I quickly decided if I can DO ANYTHING…… THAT MAY PREVENT ME FROM RUNNING OVER A FAMILY, then I will do that…… my ultimate goal is to find a job with a better MORE PREDICTABLE schedule, OR just completely switch careers….. so not sure if this is your issue, I also wanted to see if this sleep tech “Tom” I think he said his name was…… had any advice….. again sorry for the long reply….. I’m off work now with nothing else to do…. 🤷🏻‍♂️

As a person who worked for a DME as a technician I despise restocking of the masks particularly because they're so many types. Seems like the prescribing nurse would push the products we had in overstock rather than what the patient needed. Now that I'm a CPAP user in training I've learned it's incredibly important to get sent home with several different types of masks to practice with. Also keeping in mind of the occasional sickness which could require a totally different fullface mask then what you normally use. Sick while on CPAP is horrible especially with a phlegmy cough with clogged nasal passages. I love your videos and you speak with compassion.

When I am dead asleep my sinus will swell shut almost instantly . Thank God I automatically switch and still breath but then it is rough for me to sleep like that. They sent me seven different mask to try and now of them would seal up on my face. Their Tech told me that I was on my own. I gave them back the machine and went to a different place. The Tech had me try a full face in her office and it felt good but she also gave me a mask that just covered my nose. It did good. Since my nose shuts off on me I been using the full face. Noticed that the ones that didn't work had a cloth on the rubber sealing part to my face. Thank You.

Hi Dennis, great story. I loved your Vader improv. When I was diagnosed with sleep apnea many years ago, I received a full face mask that I hated. No matter what I did, sleep on my side, sleep on my back, constantly adjusting the straps on my full face mask, it leaked all night long. I asked my self how could they give someone such a terrible mask. It was gigantic and looked like I was going scuba diving.
It took months before my sleep doctor and DME allowed me to try a different mask. I found a nasal mask called the Nasal Aire II that I loved due to its minimalistic style and size. I was a mouth breather, so I had to train myself to breath through my nose and keep my mouth closed. I'm sure you would have hated the Nasal Aire II because of its nasal plugs. I like it because it provides more direct pressure inside my nasal passages and leaks less than nasal pillows.
Once I started to become an advocate for my CPAP therapy, my quality of sleep improved. I learned from other people's experiences on CPAP forums and used Sleepyhead to gain a better understanding of my sleep sessions from start to finish, instead of the minimal information reported at the end of the night on the CPAP machine. This allowed me to better engage issues with my sleep doctor and advocate changes that needed to be addressed.
So, I can really relate to your experiences and appreciate your efforts to share this with new and existing CPAP users. I learn something new almost every day. Thank you very much.

(Beard and ex-boxer with nasal/jaw/facial bone issues) and then a year later refit my mask once to get a better seal when I rolled a bunch displacing it, and when I cut off the beard, as promised, for the kid's graduation - before returning to the older one when the beard came back. I'm a bi-pap guy and literally wear it 100%, camping, traveling, everywhere and it changed my Life. A lot of this I'd learned, but this should be essential watching for new Cpap And Bi-pap users. Really well done and spot on dude, Thank you!

What i did do was go to a dental specialist and got a emd, a dental appliance for sleep apnea. It has been spectacular. Thanks again for your informative presentation!! I'm sure there are many types of these - but I had one and it just ended up rolling around in my mouth or sticking to my top teeth while I slept with my mouth open. Maybe if I had heard about Mewing as talked about in this video I could have tried it. But was useless when I opened my mouth. Any thoughts?

I literal just stumbled upon this tongue trick two nights ago. Just laying there trying to stop the air blasting thru my mouth. I finally figured out if I keep my tongue pressed against the roof of my mouth and the air stopped. Amazing. Well some of you may say duh. Well it took me awhile to think of it and SHAZAM I find this video. Thank you for helping people.

About 25 years ago I was sure I needed a CPAP, but they didn't want give me one.
So I bought one myself without prescription.
I read a lot on forums, asked a lot of people on the internet, and set the pressure myself.
I was my own CPAP specialist.
Right from the start I didn't have any problems using my CPAP.
And when I did there was always a forum with people to help me with tips.
Didn't have a sleep doc for over 15 years.
Only in the last decade I have officially a CPAP, but many times I know more about CPAP
    than the specialists.

Interesting. The reimbursement cabal does not recognize snoring, in an of itself, as a source of poor sleep. However, I have seen patients in the lab who do not meet the criteria for AHI diagnosis, yet they display a high propensity for awakening to their loud snores. CPAP does treat this problem at relatively low pressures. Sounds like you took the bull by the horns. As you age the likelihood is high that you would eventually become diagnosable and, therefore, treatable by the reimbursements sources. I hope you intervened before other consequences of daytime sleepiness could work their way into your life, e.g., hypertension.

I'm sure somewhere in the comments this is probably mentioned but you only need a prescription for the CPAP machine itself. I just purchased a new style mask off of Amazon without a prescription less than 10 minutes ago. The prescription (lie) is created by DME's to keep you coming back to them rather than looking online for better prices.

Oh good grief!! You should be a paid consultant!! I gave up on my CPAP years ago...think it was a BiPap. Regardless, I could not get the help I needed to make it work for me. Rather than helping me with the available options, they were piecemealed out to me and I'd be met with an eye roll when I, again, each time, explained my problem. I gave up. That was over a decade ago. I'm on Medicare now and Bipap-less 😂 My PCP advised me that I should be using a CPAP of some sort 🤣😂🤣 I don't know if I've got the patience for any of them at this point in my life!! Having said that, there are studies that show that apnea can cause a host of physical issues. Blood pressure and heart issues primarily so it's worth taking a look at once you've seen a video or two... especially one like this!! Dang...why does being healthy have to be such a PIA!?! 😁

I started with the ResMed and the person doing the fitting set me up with the small pillows. I had a terrible whistling sound with every breath and problems chafing. Switched to the medium size and it fixed everything! So yes, patients really need the ability to just try every option until they find one that works for them. Doctors and DMEs need to stop just pushing the one thing they know or have experienced themselves (or are being given incentives to push).

Great video brother.. It took me 5-6years to finally give in and commit to use the CPAP for at least a full month. I'm a light sleeper, so it was a challenge. Finally made the habit stick 3years ago and never looked back. I live in Co Springs now (Higher Altitude) so I rigged my O2 Concentrator into the mask. Benefits 1.) filtered air no more allergies. 2.) the Snoring would scar up my throat / sinuses 3.) When I replace my filters , it amazes me, how much crud it saved me from..

Good info and I got a kick out of the cap, I wear one as well (my wife is not a fan LOL). I have been using my CPAP religiously for years and a few things that work for me. I have a ResMed full face and I am a mouth breather. 1- I use a lip balm on my lips because regardless of using the humidifier they still suffer without it. 2 - I cut a piece of paper towel the shape of my mask and put it between my face and the mask. If you have the humidifier going your face will eventually get moist (at least mine does) and then the mask will start to leak. The paper towel will stop this. Finally I use nasal strips to keep my nose open. Doing this along with regular cleaning and filter replacement has worked great for me for years.
Pleasant sleep and happy dreams... :)

I almost died from not breathing. My electrical system was zapping me and luckily I was able to get the CPAP. Come to find out I had central sleep apnea caused from severe stress. I practically had to diagnose myself because no doctor for 3 months could figure out what was wrong with me. 7 emergency room trips and finally one doctor in the ER said he thought sleep apnea. Did test and they came back as mild sleep apnea so the gave me CPAP but wasn't working and I researched and found out about central sleep apnea. They tested me again and found it to be that and put me on bipap instead. Once stress went away I'm off it, but don't put off getting it because you can die in your sleep.

I've been using a cpap for 14 years now, and tried many different kinds of masks.
I recently switched to the Resmed airfit N30 nasal mask, and I couldn't be happier! It barely feels like I'm wearing a mask, and is super soft, which I love! And, I can wear my glasses to read until I fall asleep too!! As nasal masks go, I tried one that the air was fed into the mask at the top of the head. I hated it. The weight of the hose would move the mask and I'd get leaks. The nasal mask I use now has the air hose connected to the part that goes over your nose, and the head strap is so thin and lightweight that I barely feel it when I'm wearing it. I can't use nasal pillows, they really hurt my nose... the way a phone does when you have it up against your ear too long. It is super important to find the right kind of mask that feels really comfortable for you. Also, when you get your first cpap, it can take a couple of weeks to get used to, so don't give up. You need quality sleep. Once you get used to it, you'll find you can't sleep without it.

As a "soon to be" CPAP user, this was a FANTASTIC video. Everything you stated was easy to understand and made perfect sense. I say this because this is the exact type of advice I expected at the sleep clinic when I did my overnight sleep test to determine the appropriate settings. The tech who helped me get setup for my test asked me "do you breathe through your nose"? I said usually, but sometimes I will breathe through my mouth at night. Her response was "OK, you need a full face mask". In the morning, I asked if I will have the opportunity to try other masks when I get my CPAP machine so I can determine which would work best. The response I got was "no, they discourage that. You need to stick with the mask they give you." I just did my test last week and haven't received my prescription yet, so now I feel I am armed with the info I need when ordering the equipment and possibly switching to a different mask if needed.
The other thing that discouraged me was after asking the tech if I would be able to pick the type of CPAP machine I wanted, she said "no, you get whatever they send you. They are all the same anyway". I haven't spoken to the DME supplier yet, but I certainly hope that I have options. I am the one paying for it after all (above what insurance will cover).

    • @AdventureswithCPAP
      @AdventureswithCPAP  8 месяцев назад

You have options! Anyone who tells you that you don't is doing you a disservice. If you get to the DME and they try to give you a Philips, tell them "No thanks," demand a copy of your prescription, and order a Resmed from lofta.com or cpap.com or wherever you want to online!

I was given a nasal pillow headset when I got my first CPAP in 2000. (I think the model was a "Breeze")This thing had a hard plastic and metal spine over the top of it. I learned to bring a roll of electrical tape for field repairs when I traveled. It was fragile. I got a replacement of a silicon Resmed Swift FX in 2010. Still nasal pillows however. It's not being manufactured any longer, so I'm now trying the Resmed model you showed at 19:00, but I think I prefer the silicon nasal pillows as I am accustomed to that design and they seem to provide a more secure fit. I didn't know for a very long time that I could choose alternative masks. Also, I travel a lot for work and some of the best money I ever spent was a mini CPAP for travel. I see an awful lot of people lugging around those familiar CPAP travel bags in airports.

    Recently been prescribed a CPAP.
I have not even received it yet.
Thank you for sharing your experience!!!
Funny thing is for years instead of sniffing I would press my tongue against the roof of my mouth like your Mu? technique and swallow hard.
The motion creates a vacuum clearing off my soft palate. It keeps my sinuses open and keeps me from sniffing crap in my lungs creating Bronchitis then coughing it up in my sinuses creating a sinus infection.
I tried the Mu? technique while watching, just a bit of vacuum to hold the tongue in place. Wow, this to my wife’s pleasure kept my mouth shut.
I don’t need to be sleeping to snore but this kept my tongue from sliding backwards.
Again thank you for sharing your experience!

Hey so I just went from struggling with the F30i to nose pillows after watching this and trying a few other things. Remarkable difference. And it also can keep my mouth shut the same way as you. I'm 300lbs and a mouth breather at night typically. Good info in the video! Thank you!

I like the one that your wife like, been wearing it 5 years, tried several others, plus some resmeds, I'm pissed at Phillips, I took the foam out and will never get another Phillips but I will keep thier masks, I wasn't even thinking about the DS2 but after you're video I KNOW they can't pay me to take one. I did the same as you, my wife started a year after me. She was fortunate to try all mine to decide. Thanks for the video.

LOVED this video - it reminded me of my own long and excruciating journey searching for the right mask setup that worked for ME :-) - been using CPAP for ~10 years. Took a while, but I eventually settled for the ResMed P10 nasal mask, along with a tape to literally tape my mouth shut every night. (3M Medipore H 2" x 10 Yard Hypoallergenic Soft Cloth Surgical Tape). This has worked for ME for the past 7 or 8 years. Then again, I am clean shaven :-) , might work for others who mouth-leak a lot. If you are new to CPAP - be patient, and good luck with your own journey :-)

Thanks for this little talk, I didn't know about the tongue exercises but do now and I use the resmed pillow mask you are learning to like, 8 am getting good numbers but I can't side sleep with 5hwt pillow mask and am relocated to sleeping on my back... Great advice to part out a mask to save money, THAT is MONEY!!

The gel nasal pillow beat up my nose sooo much. I will definitely give the silicone version a go.

I have a resmed airsense 11. My first mask looked like a fighter pilots mask. Completely covered nose and mouth. Hated it. It rubbed next to my eyes. I use the one that sits under my nostrils and covers my mouth. F30i I believe. My equipment is supplied by apria. The lack of support for a new user is mind-blowing. I had to figure things out on my own. I needed the mask i use now because of the reason i stated and also because the other, the hose hooked to the front of the mask and it was uncomfortable because i am a side sleeper. The one that hooks to the frame on top of my head is better. It has been a journey. One main problem i have still is i am swallowing air in my sleep and i wake up bloated and in pain and well, you can imagine the embarrassing outcome of that! 😂 i was allergic to the foam lined version of my first mask.

Just got my first CPAP machine, a ResMed 11 and trying to figure out how to breath thru my nose with the issues you described. I got both a full face mask (which I hate) and silicon nose mask that I like if I can only learn to nose breath when sleeping, something I’ve never done. I’ll try the learning technique you described. Thanks for your video which could not have come at a better time for me.

I am so glad I found your channel. I have just been diagnosed with sleep apnea. I am currently waiting for my CPAP machine. I did not know there were so many options for masks. Thank you so much for this video

Great look into your cpap journey Dennis and really great you found the right combination for you. Also a quiet mask and device combination is really good for non OSA partners trying to sleep next to you. :)

Thank you ao much Dennis for sharing your wisdom and experience. You helped me realize how important it is to address sinus issues. For too long i was ignorant about my sinuses. All the different doctors, nnps didnt ever ask about my sinuses or the ents never fuly addressed it. A functional medicine doc was the first to address it in more detail but i tend to forget the impotnce of daily sinus irrigation. Your video has been so helpful. Wish I had found it sooner. Thank you so much!😊😊😊

You didnt mention mouth tape. As long as a person can breathe well through the nose, mouth tape can work. It keeps my lips from getting chapped too. I apply a lip balm first then cover upper and lower, in a relaxed position, and i just use painters tape from the dollar store. If i dont use it, then my mouth will fall open and get so dry, its a big leak.

Thank you so much for taking time to make this video. I just got a CPAP today. Let me share a poem with you.
"Grandma has a habit, of chewing in her sleep. She chews on Grandpa's whiskers and thinks its shredded wheat" Thankfully I dont have a beard (yet) I am an older female. Hope you are doing well. God bless.
