The general layout, areas of deserts, forests, mountains was "hand drawn" in lets say, low resolution. And the algorithm creates the details. In a modern engine, the amount of detail and variation could be much bigger.

Only the interior of the cities and the dungeons are not procedurally generated. Outside of cities, I think that you can't even find a place you were at a given moment if you get far away enough.

If you equate that to 1.8 meters each cell is 57.6 wide. Making the map roughly 3.1km by 3.1km. Bethesda maps aren't actually "big" when compared to the size of a person. What makes them feel big is perspective. Jogging speed is about double the length of the character per second, about 370u/s. Meaning without sprinting, and no obstructions, it takes roughly 16 seconds to cross one cell. The game is designed to be nothing but rough terrain with interruptions. In the real world 16 seconds feels like little, but playing a game, even 10 seconds can feel like a drag if you are just walking. Skyrim is 119 cells at widest, but a lot of that is ridiculous terrain that impedes movement a lot, making everything it feel like much more.

Nur craften. Viel bauen. Zusammen viel Spass haben. Es ist sehr vielfältig.
