      Hi - I watch you all of the time. My depression/anxiety has been so bad I am now facing eviction and that, in turn, is triggering worse depression/anxiety. Mt psych disappeared after covid ( I lost a sister to covid and before that, a brother to suicide0. but before that he told me Buproprion would help with my 'motivation". I just became worse. I was on Effexor before and only having mild results after slightly raising the dose). so mt new psych - who reired last year- just kept rasing the Buproprion. I was not getting better. (Yes, I was in therapy, not much help- no modalites- just talk, used, also grief therapy- when depression doesn't lift, it just doesn't help much). So now I have a nurse who tries, but seems to fill what I suggest (?). So- that's why I ask so many questions. I'm really afraid I won't ave the time to go through the side effects again. And with so many commercials making depression seem to be cured with one pill while still coping byhiding behind a smiley face (how are these people getting out of bed?)!adding to the stigma and people close to me noot understanding/believing me and what I'm going through (major MDD with an anxiety disorder) I have bottles laying around- still good? I would love to call for a free consult but I don't have much money after that. Please- I'll take any suggestions (on Obamacare/Medicaid so hard to find a doc/other treatments) right now.

      If you take any other med with Trintellix then the nausea can be intense. So I just take it by itself with 3/4 cups of water of so between breakfast and lunch midway. One thing the interaction did help me with was I tool Lamotrigine/Lamictal for bipolar and it really shortened my cycles. Once on Trintellix I immediately discovered I could not take them together. So I would take Trintellix first and midday take the Lamotrigine. Well after a couple months I noticed I forgetting to take the Lamotrigine. I'd correct myself, but after 6 months I was beginning to realize I didn't really need the Lamotrigine. That's been 8-9 months ago now. I told my doctor, but continue to get the Lamotrigine. On my next visit I'm going to discuss discontinuing Lamotrigine.
      Okay I spelled all this out for the reader because I realize self diagnosis and even self withdrawal of a med can be detrimental to whole objective. As you can tell I'm going about it wrong. Here's another thing. Cost. Lamotrigine is a $4 med, whereas Trintellix is about $500-$600 med. So here I am, thinking it's okay to self regulate a $4 med. WRONG! lol Yeah it's time to make that appointment. I am methadone patient for 20 years, and it's the only other thing I take. This video is helpful.

    I have been using this medication for 3 months now. I was becoming suicidal after being on Wellbutrin for many years. I like it so far, it has helped my anxiety tremendously. the only side effect was severe sedation for the first month., but i think it was due to the other medication i take, Prazosin and Lamotrigine. I haven’t noticed any improved focus though. I’m grateful to have Medicaid and dont have to pay $500. Highly recommend.

    I was actually good on escitalopram in terms of mood BUT the cognitive delay was soooo bad that I cannot even speak without pausing to remember what I was saying. I've been on vortioxetine for more than a month and I can really feel the results 🥰

      @@PilarBorrellaSalas i was switched by my psychiatrist and have been on it for 6 months. So far so good. Glad my insurance covers for it but if not it will be too costly 🙈

    My psychiatrist changed my antidepressants many times (one of them being Trintellix) over 4 years period, trying to find "the one that is working".
    Now, I started thinking if the chemical imbalance is real, after so many major studies in the last few years showing almost no difference between antidepressants and placebo.

      Maybe useful if your suffering from Dementia of some form as for it's value in treating MDD from my experience it's an awful drug causing emotional paralysis and suicidal tape loop ideation. I guess all you can do is try it and cross your fingers just like any other current anti depressant..not a game changer.

      I’m in the exact same boat. I was also taking Citalophram. I had to stop it because it was making me very irritable and angry. I start my first dose of Trentellix tomorrow morning. How has your experience with it been so far?

    I switched from this to Viibryd for 3 months. That stuff made my brain feel like mush. I'm finally a month back on Trintellix and I feel like a fog has been lifted from my brain. I wish it was more affordable. It's really putting me up against the wall, even with the savings card. It's seriously killing me, but the alternative is being a zombie or being a suicidal basket case. I finally feel like making content again instead of it just being performative, I feel so much better. I can't even put a price on it. I'm grateful my guy will give me samples if I need them, but I just wish they'd drop the price. I can't go back to effexor or anything like that, I won't do the brain zaps again. I'll just have the find it in the budget.

    Trintellix is the only anti-depressant that has ever really worked for me in what seems like a lifetime of trying different meds. *The problem is the cost.* There is no generic available in the US so a one month supply is over $400. I’ve literally been living off of samples from my doctor but this leads to missed dosages when I can’t rely on prescriptions. The patents to Trintellix don’t expire until *June of 2027.* Meaning there won’t be a generic version of the drug available until then (over 4 years from now). 😢

    Most informative video I ever seen about Vortioxetine! Just an important point you didn't mention which is Vortioxetine and emotional blunting. How it is less likely to cause it in comparison with SSRIs/SNRIs

      @@blessieyang8070 it doesn't cause it, but any medication that increases serotonin may cause this, vortioxetine is less likely to cause this due to its multimodal effect. If serotonin causes emotional bunting it is a clue that low serotonin is not your problem.

      @@blessieyang8070 It wouldn't hurt to try. Bupropion can also help with that as well. Typically emotional blunting is a result of low dopamine and norepinephrine.

    If you're looking for tough questions then I have one for you: taking two SSRIs concurrently is generally considered to be an absolute contraindication. But some docs seem to be happy combining Trintellix with SSRIs. Why is that considered safe but the other not?

      I wouldn't consider it completely safe. You would still need to watch for serotonin syndrome. Risk is less than with SSRIs since it has many antagonist actions on different serotonin receptors, which account for increases in the different neurotransmitters, and not just serotonin.

      I’m shocked that you say you used alrazolam for 10 years as I’m a recovering Xanax addict who went on withdrawals and lost the plot due to alprzolam Xanax yes it’s a great medication to take for feeling amazingly good but i got addicted and went crazy and ruined my life and lost the people I love around me it messes up your head the next day when u wake up your just so moody also depends on dose I was taking 7 1mg a day

      @@anthonybenash3457 I just started on trintellix would you believe I’m on trintellix and quetipine because of addiction to Xanax and weed for years I was abusing the Xanax tho I take 7 maximum pills everyday like I was a super high all the time so whatever. I did to my brain now I’m ruined and have to take anti depressants and Phycotits due to having anxiety and phycosis and literally losing the plot because of Xanax and yes 2 .5 mg twice a day sound like it wouldn’t do much I’ve taken that much before 1 mg in day and I noticed it made me have nice feeling too any amount gives you kinda good calm euphoric buzz

I have dealt with general anxiety disorder for years and I’ve been doing way better with the Xanax personally. I know the risks of addiction so I have never asked to have my dosage increased.

    I have Bipolar 2. I take Trintellix on top of a mood stabilizer called Lamictal. Can you do a video on how Lamictal works for Bipolar and treatment resistant depression? Thank you so much for all that you do! The information you've shared has been so helpful.

      Thanks for your comments. I am french colombian living in france with bipolar 2 trouble. Thanksfully medicine are taken in charge by the medical insurance. I would say i feel better during the day with the trintellix but I have nausea 1/3 and strange dreams.

      @@OneDanae yes I had much stress with living with an emotionally unstable person. After the drug I could calm myself down. Also it doesn't have particular negative effect on intellectual performance during the day. Only problem with the drug is having slightly less libido.

If it doesn’t improve maybe I’ll try Trintellix. I’m always in a daze it seems and having memory issues and I’m only 44. You recommend Trintellix? How long it take to work for you?

      @@jmoss2543 first ~10 days i started with 5 mg, then have taken 10 mg constantly for the last months. In the drug description it says it takes 3-4 weeks to have the effect , but I could feel the difference after a few days. I didnt have any experience with anti-depressants before, but I really liked this one and I'm happy that it works for me. Anti depressents' effects might be different for each person, but I had some luck on this one.

    I was given samples of this medication and was so happy with the results. I have been on many different anti-depression meds for yrs. After a period of time they seemed to have lost their effect. Unfortunately the cost, even with my Medicare part D insurance is not in my budget!

    Excellent video. I am a PMHNP student graduating in December. I still struggle with choosing the best antipsychotics for my patients. Could you go over a topic over first generation and second generation antipsychotics ? Thanks you!

      That's a great video idea. When choosing between them I look at the side effect profile and risk/benefit for my patient. There are many choices so I can understand the confusion.     I was put on this following a near death experience. I never thought I felt very depressed but do have anxiety and PSTD. So far, I notice it has improved the rapidity of my thoughts, given me vivid dreams, and seems to have increased the clarity of my thoughts. I've yet to see an improvement in anxiety, but every incremental dosage had a difficult period where I was ferociously angry. Currently, I'm at 10mg and feel less aggressive. My doctor plans to increase me to 20mg. I wonder if it decreases empathy?

    2:22 If something improves memory, attention and processing speed I can quarentee you it will raise the overall IQ since all those three are in major role when measuring IQ. So it would basically be impossible that it wouldnt improve IQ.

      You would think so, but we don't have any studies to confirm this, so can't claim it. As well as don't want people taking psychoactive drugs just to try and raise IQ. These drugs cam cause more harm than good for some people.

    2 months in and the nausea and cold sweats keep coming after taking the medication, even when paired with anti nausea meds🙃 it hasn't helped any symptoms and we cant increase the dose because of the side effects. Still lots of mental blankness, actually increased frustration and interrupted sleep. But hey i guess keep trying if it works for other people 🙄

    I have been taking vortioxetine 1 month now. I have reached the dose of 20 mg starting with 5 mg. Although i have less anxiety i have noticed less connection to my emotions and all senses overall, is this a side effect? It feels like emptiness and numbness....does it get better as the time goes by?

      This is a flattening affect that cam occur due to too much serotonin, which lowers dopamine. Have your doctor lower the dose, or add Bupropion if tolerated and no contraindications exist.

      @@DivinelyTimed 2 and 1/2 weeks in and my appetite is back! I still get anxiety attacks at night and sudden lows throughout my day, but not as much and I have a more positive outlook in life. I am also getting more focus on my studies. Slowly getting there, not perfect yet. Hoping to feel the full effects of the pill on my 4th week!

I just want to wake up and not dread the day. Dare I say I hope to smile genuinely and have meaningful interactions with people! Also improve my self care and energy level. I have high hopes every time I start a new med but as of late that initial excitement isn’t there anymore because nothing has worked thus far.

    Can you tell me if you think virtioxetine causes weight gain? And can it help PMDD? I take Prozac for PMDD and don’t love it. I was wanting to try trintellix for PMDD and anxiety

      This would not be my first choice for those symptoms. As far as weight gain, it is considered weight neutral. Have you tried sertraline or escitalopram?

      @@LevelheadedMind I have tried Zoloft and citalopram … and Prozac.. I don’t think I handle the other SSRIs very well. But I’ve been researching the Trintellix and all SNRIs .. it’s such a trial and error. Thank you so much

    Can you please expand on how Vortioxetine increases norepinephrine, dopamine, glutamate, acetylcholine and histamine? Everything I have read says it absolutely does not have any affect on anything other than serotonin. Is it an indirect action? I am considering giving this med a try and am wanting to understand it as best I can. Thanks.

      No direct actions but by antagonistic and agonism on various serotonin receptors and pathways, it leads to actions on those multiple neurotransmitters. Stahls Psychopharmacology text book explains this very well if you want to learn more about down stream effects.

      You can add medications such as clonidine, propranolol, or hydroxyzine. You would need to be careful with any serotonin medications as these can increase risk of serotonin syndrome. Buspar can also be added but with caution, since it is a partial serotonin agonist.

    5 mg atleast from the beginning felt alright after couple of weeks. 10 mg almost paralyzed me, I was so tired, horrible brain fog and my anxiety just got worse by the day. I had to stop it after 2,5 weeks I had to work and continue on living which wqs impossible. I wonder if this stuff is too strong on me, I dont suffer from depression, I take it for pretty bad general anxiety disorder.

      Yes you can stop these medications if done correctly under the direction of a provider. As for organ damage it is fairly rare but liver damage can occur with some of these medications, typically in higher dosages risks of side effects are increased.

      Hi, I didn't see any interaction with Ashwaghanda according to UPtoDate interaction checker. As for the tinnitus, this can cause issues with balance and dizziness. So if it doesn't resolve, I would first make sure it is a side effect and see an ENT then consider switching treatments if it is indeed from vortioxetine.

I'm seeing you from Portugal, I started with vortioxetina a week ago, in my case I fell a little social anxiety, to sing in public, to eat with stranger, be a centre of attention is not a good thing for me, even with my friends. So I start to avoid parties, sing in public, and even be with my group of friends. Because I started to feel, sensations of panic.
    Is this drug or substance the most suitable for this clinical condition?

    I take one low dose aspirin a day and I just started taking 10mg vortioxetine. What bleeding are these docs referring to exactly?

      Serotonin can affect platelets ability to clot. Therefore there is in an increased risk of bleeding. Watch out for signs such as bruising easily, nose bleeds, blood in stool, etc. It would be prudent to have your blood levels checked at least every 6 months to monitor as well.

    Are there any supplements or activities you can do to increase the therapeutic effect without increasing the dose of the medication?
    I currently use a TDCS headset from flow and try to exercise because I read they can help improve the results. I also take a fish oil tablet every day.
    I am on 10mg but I don’t want to go higher due to risk of increased side effect.

      No, this is not considered safe. Bupropion can significantly increase the blood concentration of Trintellix making you more susceptible to the side effects. Please talk with your doctor about this concern.

      It is typically only once daily. Ask your provider why it is beign prescribed in this frequency, perhaps there is a metabolism issue?

      Once a day is preferred , especially since it has a long half-life. It is sedating for most, so I recommend the total dosage to be taken before sleep. If others are aroused or stimulated, then you should take in the morning. Make sure you always communicate with your doctor before you leave his office, so that you dont have uncertanties which can delay your treatment and therapeutic benefits 👍💯

    Would you say its better for anxiety than Lexapro? I take 15 mg per day and I am doing well on it but I also at times feel a bit numb/or like I can’t focus.

      In my opinion Lexapro is better at treating generalized anxiety and even panic disorder. If you have depression as a dominating illness and some underlying anxiety, Trintellix would help.       Yes, it can increase your risk for serotonin syndrome so you and your provider will need to monitor for this risk and symptoms of serotonin syndrome.

      Bupropion will reduce metabolism of Trintellix making you have adverse toxic effects. If this combo is being given it must done under expert guidance, many risks as well as serotonin syndrome risks.

    Mam I am having functional disorder dr.prescribed me vortioxetine ten mg....l took for one month...my symptoms improved ....then halfed the dose to five mg .....I took that for one month more ...now I started having buzzing sound in my ear ...mam why this ringing in my ear it was no there before

      Why did you reduce your dosage upon symptom improvement? This is an indication that the medication is starting to work ! Never, ever do this! unless it's causing a serious adverse reaction. The tinnitus is most likely from a slight withdrawal from 10 to 5 mgs . Please see your doctor and Never self-medicate or play doctor . You keep messing around like this and you run the risk of Treatment resistant Depression/GAD etc..

    Metformin, is this really good for you, because I have to now take other supplements to fix what Metformin depleted, like Vitamin B12 and Folic acid is there nothing else to replace this with?

      As a Psychiatric Nurse Practitioner prescribing a medication like metformin is out of my scope of practice. I really can't comment on whether it is good or bad. However, for type 2 diabetes with proper diet and exercise some patients can come off of this medication. So discuss your concerns with your provider and develop an action plan to start working towards a goal of coming off this medication.

    My dr is taking me off Wellbutrin and on trintellix. I’m on 300mg bupropion and he has me going down to 150mg but at the same time start taking 5mg trintellix for one week. At week 2 I’m going off bupropion completely and up to 10 mg trintellix. Should I be worried about drug interactions? Or just be aware of changes and re asses as needed. Thanks!

    Vortioxetine does not work for me....6 months on 20mg...energy zero, motivation zero...a waste of time, probablly less side effects but if depression dont goes away, you are not able to do nothing. I hope work for someone...

      I'm sorry to hear it hasn't helped you. Not every medication is going to be the right fit for everyone. Also make sure to look at other causes, such as poor diet, inflammation, methylation issues, autoimmune disease and gut dysbiosis are some of the few causes of depressive symptoms to explore. I hope you find the right treatment for you and get to the root cause of your symptoms.

      There are studies that suggest CBD is an inhibitor of CYP2D6 which Trintellix is metabolized by, so it may increase the percentage of available Trintellix in your system leading to increased side effects. THC is not on the CYP2D6, but induces CYP1A2, which has no effect on Trintellix.

    The nurse's role is to follow the doctor's orders. There is, of course, a great deal of room for one's own opinions and, not least, to remind doctors of new or increasing symptoms and side effects, and to generally look out for the patient's best interests in a role similar to that of a medium between patient and doctor.
    But precisely when it comes to pharmacology, nurses have limited knowledge of pharmacology. Superficially, you can understand certain basic mechanisms of action, but at the end of the day, a nurse has extremely limited knowledge of pharmacology/psychopharmacology. That is why we have different professional roles. A doctor or clinical pharmacologist like myself has the most knowledge. It is not about devaluing nurses, but just as a pure fact. Doctors are doctors and nurses are nurses

      Overproduction of glutamate can, yes. But you need glutamate for attention, focus, and energy, which is what this medication provides to some patients and the benefits of stimulating this system.

      @@LevelheadedMind Would be hard finding a Doctor that acknowledges this condition because very few Doctors are even aware of it most are of the believe once the medication is stopped everything goes normal if it doesn't ''it's in your head'' as in you're thinking about it ( placebo). The condition got recognized in 2019 by the European medical agency. Anyhow from what I am aware theirs no treatment currently for it some medicines like Wellbutrin (Bupropion) which is a NDRI provide relief for some, Viagra (Sildenafil), and Cialis (Tadalafil) for some.

    Hi dr! Do you find it weird if a neurologist prescribed me 10mg for 2 weeks then 10mg every 12 hrs (for a total of 20mg a day) for JUST 3 months? Suppose to regulate 4 hormones (serotonine, norepinephrine, melatonine and other) bc I have physiological stress and then I can continue with a treatment from a psychiatrist? (Which I believe would switch to lexapro cause I was on it some years ago)

      I've never heard of Trintellix used in this way. Did you ask for a rationale to this type of treatment plan? Perhaps they have some research paper or study about this use that I am unfamiliar with. Always speak up if you don't understand your treatment plan, it's your right.

I too was kinda skeptical that's why I asked. Aside from if it's true or not from the neurologic pov could someone switch from vortioxetine to Escitalopram and get something good from it or is it a waste of time and money psychiatrically speaking since vorti it's gonna be only 3-4 months?
