How to Take An Aqualizer Bite Updated - Priya Mistry, DDS (the TMJ doc)

  • Hello Friends and in particular, Fellow Dentists! In this video, I go over the "Aqualizer Bite" technique. It's a way of measuring the patients bite and building an orthotic that will treat muscles & joints as well as protect the teeth from the forces of clenching and grinding.
    This technique, when done properly, can decrease headaches, ear pain, stuffy ears, jaw pain, neck pain and even minor clicking/popping noises in the jaw. It may not help unlock a jaw that is locked closed as this typically requires myofascial release and very technique-sensitive osteopathic manual jaw manipulation, in addition to an orthotic.

    Thank you for the video Dr. Priya Mistry. Which lab do you use? I am using Glidewell but seems like they are not getting the bite index part correctly.

      I currently use a small local lab who doesn’t want me to disclose their name. Sorry! You could try Williams lab in California. They’re great!

    Hi Doc, thanks for all your videos! Non-dentist here, but curious about this orthotic. What are the bite indexes? Are those indentations in the guard for the upper teeth fit into or are they those weird metal things that stick out of a guard? Would love to see the final product but can't find any images via typical internet searches. I haven't watched *all* of your videos (yet); apologies if you've shown this somewhere. Thanks again!

      There are no metal parts to my orthotics. There are actual indentations in the plastic. It’s not helpful to show a picture of it because it looks similar to a typical nightguard but it’s all about the prescription built into it. Think of eyeglasses - they all look similar but the prescription built into them makes all the difference for the wearer.

      Thanks for the explanation! And the orthotic would have more ...mass? (Is that the right way to think of it?)... on the lingual side than the cheek side?

    If the indexes are 2mm deep, how thick is the orthotic? I think you mentioned that the aqualizer you use is 3mm, although you no longer routinely use that technique

      That can vary from patient to patient.

      Yes, I want to buy 2-5 pack AQUALIZER MEDIUM SLIM. I have a 3D CT bone scan files. Mouthguard is 2mm and i have a small mouth.

    Hi doc- TMJ sufferer here. About 2 years. It has been reduced a lot with night guard and I wear orthotics during the day. That brought it down 60%. Then chiropractor brought down the pain some more. I can’t seem to get over the hump though. Everyone talks about grinding and clenching especially unconsciously. I might be doing this, but don’t really catch myself doing this it all. I only have my TMJ on the right side. Do I have stress? Yeah. Can I do much about it? I think I’ve done what I can. I’m ready to accept that this will just be a part of me the rest of my life. But I’d like to make one more push somewhere to see what I can do. Live in Ohio so can’t exactly see you. Do you have any suggestions? Thanks!! Can general inflammation from diet be the true culprit of TMJ? Or do you think it’s more physiology. I could go on a extremely low inflammatory diet as a last ditch effort.

    I have extensive Tori on the bottom of my mouth. Will this be a problem for a lower orthodic?

    Do you take insurance for TMJ? Dentist in my area only take self pay. Just curious. Thanks

      We bill Medical Insurance if you have TMJ/TMD coverage. Dental insurance doesn’t cover it because TMJD is a medical problem.

    Mam does tmjd cause itchy ears while moving the jaw. If yes can you please explain how.? Thanks a lot in advance

      Itchy ears is not a symptom I hear of frequently. Sorry but I don’t have a good answer for you.

    If I showed a dentist this could this help them make a splint for me help all my symptoms brain fog Dizziness vertigo pain neck jaw vision hearing and so on....I feel I have to try help myself as no one OZ knows how help me and I'm in very bad way...wish you lived in Oz.
    I cant function and very dark days and my kids suffering too watching me go down hill fast.

      You could ask your dentist to watch this but typically practitioners don’t like to be guided on how to do things. Sorry you’re going through this and best of luck to you.

    How can I find an orthodontist who does this in Luxemburg? They don’t even know what that is and I’ve been in orthodontic treatment since I’m 4 years old

      ​@@theTMJdoc I got the Aqualizer ultra with medium volume. I'm about 6ft tall and would say I have an avg. sized jaw. I believe it fits. I was planning to try and get an orthotic from my dentist, but since the Aqualizer is not offering relieve I don't know if that would be even a good thing to try.

      @@fips001 have you tried wearing it during the day instead? It may be that it was not sitting correctly at night and it’s hard to know when you’re sleeping.

      @@theTMJdoc yes. It feels kind of weird to me because I have to bite down to even feel the water pockets. Meaning that either I’m relaxed but not on the pockets or I’m biting (muscle tense).
      I would think my jaw should find an optimal position in a relaxed state, but I don’t see the aqualizer doing anything in that state.
      Edit:
      To clarify, my mouth can be still closed and my upper lip touching the roof of the palate in that relaxed state.
