    Summary:
    Another common fear that any patient with hernia has is, “Doctor, you are going to do my hernia operation, will it come back again? I have a relative who is an aunty and who has undergone a hernia operation five years ago. Today, she says that her hernia is back. I have also heard similar stories from my friends. Hence, everyone is discouraging me from going for a hernia operation saying, “What is the point? If you get it done now, it will come back after five or ten years. Will I get back the hernia?”
    The short answer is, “It depends”. The long answer is, “It depends on what?” Hernia repair is an operation where we try to re-create the weak abdominal wall using reconstruct from the muscles and also by using a mesh. However, the final success of the operation depends on multiple factors. This is because, when the healing is good, the scar is healthy. If the scar is healthy, the hernia does not come back. What are the factors that will decide how successful the hernia operation is going to be? There are certain factors that depends on the patient himself or herself and there are certain factors that depend on the treatment.
    Patient factors include patients with diabetes. They have heavy blood sugar. Their tissues quality is poor and thus, their healing is poorer. If they do not have good sugar maintenance, their scar tissue that is going to be formed will be weak. Patients with diabetes can have a higher rate of a failure of hernia operation in the long run, if they do not maintain a good sugar control.
    Obese patients - In the patients who are extremely obese, their muscle mass is thinner and the patient gains a lot of weight following a hernia repair. Due to excess stretching, the scar tissue is stretched or pulled and thereby, the hernia repair can give way.
    Smokers - The quality of the cement...the body scar can be referred to as cement. It is the cement that the body produces to heal any wound. The quality of this cement or collagen is directly affected by smoking. If the patient continues to smoke after hernia repair, the quality of the scar is very poor and the hernia can come back again.
    Patients who have received various radiation therapies - If they have received radiation therapy for any sort of cancer, the tissues are generally damaged due to radiation and the hernia can recur.
    There are other factors too, such as patients who have chronic severe coughing, or patients who do very strenuous activities very early after the hernia operation - these patients can have a weakening of the scar.
    The second group depends on the treatment.
    What sort of treatment was done - What is the experience of the surgeon, the technique used for the hernia repair and was the technique appropriate for that type of hernia. As I told you earlier, certain hernias are best treated laparoscopically and certain hernias are best treated by open surgery. If the surgeon chooses the right operation for the right patient, the chance of success is very high.
    Then, we see if a mesh was put - is a mesh put for the repair of hernia. If it is used, what sort of mesh has been used? These are all the factors that will contribute to the final result whether the hernia is going to be cured forever or whether it will come back after a few years.
    There is no simple answer to this question, “If I do a hernia repair now, will it come back again?” It depends on so many of these factors and that will be individualized. Generally, we discuss the result of the operation prior to the surgery, with the patient, where we access all these factors and give them a rough estimate of the chances of the success of the hernia operation.

    Good video. I met with my surgeon two weeks before my surgery and I'll share what I learned from him. He told me the biggest difference between laparoscopic and open repair would be in time to return to physical activity (two weeks for laparoscopic versus minimum 6 weeks for open). Both involve some level of post surgery pain and and both share similar recurrence rates of 1 to 3% according to him. Laparoscopic is less invasive and thus has a shorter painful period following surgery and recovery time. The biggest factor for recurrence (all other things aside like patient health and compliance with healing restrictions, lifestyle, surgeon experience etc) is mesh versus no-mesh. That is the one decision a patient can make to lessen likelihood of recurrence regarding options for inguinal surgery besides resting for 6 weeks -- opting for mesh. The downside of using mesh is there is an apparent higher likelihood of some chronic pain that ranges from minor to debilitating although the debilitating chronic pain is becoming less likely given advances in techniques and mesh material. So I am now ten days into my recovery having had open inguinal mesh surgery (and I had a small umbilical hernia repaired since I was under anesthesia that did not require mesh due to small size). I am just walking around the house but not lifting anything to allow to heal. I am using stool softener to lessen straining with bowel movements. I am an active person who works out every day and remind myself that the most people who come to RUclips and watch hernia videos and make comments are those who are nervous or who had problems (the 1 to 3% plus the nervous people like me). I will continue to heal slowly like Dr P suggests, taking care of myself and comfort myself that my decision to use mesh was wise as it significantly reduces the likelihood of recurrence. In 6 weeks I will go back to the gym to continue being healthy and fit and my surgeon says from that point on to cut my weight training in half then slowly build up over time and I should return to normal with minimal likelihood of recurrence (100% - 1 to 3% = 97%). Optimism.

    I am 8 weeks post surgery for upper gastric meshhernia repair. I think the importance is to if possible find out what the initial cause was? For me it was severe ibs and my stomach swelled so bad that the inner pressure tore open an area in my upper stomach and out came a lipid hernia. From now on I watch my diet, eat small and regular and avoid stress. I also think the after care is most important on whether or not the hernia repair heals properly. I have not forced my self to lift things that were heavier than around 7lbs, the first 6 weeks I would even lift my cat or a 4pint of milk. Fortunately I have home help. Also getting up out of bed the correct way and lying flat when sleeping to ensure your abdomen relaxes and this helps heal the incision area. Just being gentle with yourself. 😊

    Two years and mine is back......but not exactly in the same spot
    No heavy lifting,no coughing,no diabetes here: i am quite devastated!
    I do not want a second op, i am 56 years old and have been taking it slowly since i knew there was a bigger chance for me to get a recurrence.
    No good at all....but that's just the way it goes!

    Five months post laprascopic non mesh surgery and it’s back. Maybe at 80 years of age I was just too old. I thought my surgeon was excellent so not blaming her. I had virtually no pain after surgery but the diet before and after was horrible. So now I’m back to morning jumps etc. I won’t have surgery again and I don’t think anyone would want to do it. My hernia was 12 cm so very large. Evidently half my stomach was in my chest which caused really bad breathlessness. I couldn’t walk but virtually sat and did nothing for months. I’m coping now but disappointed .

    Three months before I had umbilical hernia surgery. Again my belly button protruded and did ultrasound and detected hernia. No idea how it came back . Maybe stress , constipation etc. Anyway surgeon advice for LP. Hope I will get well after this.

    Hello doctor... in 2019 I did umbilical hernia surgery... it was open typed surgery, for first 2 years I didn't had any problem. In 2021 I delivered my baby girl, soon after that I was able to notice a small bump like a pimple, in my stomach where the surgery was done. Initially I was able to see only 1, now in 2023 I can visibly see 6 bump in my stomach 1 after another in a straight line.. I can feel sharp pain whenever someone touches my stomach or I got hit..

    After surgery, the intestines are always pressing against the groin. The intestines will find eventually find a weak spot push there, in time resulting in a breakthrough.
    The person can feel the intestines pushing, and should take preemptive action.
    Coughing frequently could result in a hernia protruding.

    I had hernia repair l refused the mesh l was offered bio mesh but it wasn't needed .lm in alot of pain a day later but l will be fine.

      I had inguinal hernia surgery 4 months ago and I feel soreness right below my below button where the groin area is. I keep reading that it takes 3 to 6 months for the new tissue to form with the mesh and that can be the cause of it for me. I am going for a CT Scan next Friday. Nervous as heck.

    I need 2nd surgery today.1st surgery was great.had mesh inserted.less than 1 year my hernia ruptured after I lifted 17 lbs.it popped sound i knew it came back . right now ,not wanting to repeat a surgery.2020 was bad,covid,gallstone surgery,colonoscopy,and 2 deaths, all within 6 months,now my hernia has expanded and now has pain.pain is a 7-8 right now what to do,any advice?

      Likely that is some nerve damage or stressed muscles based on how you are walking and sleeping post-op. It will likely subside in a few more weeks. If it worsens I would get in contact with your physician.

    Really good explanation but I’m afraid with the NHS there was no following consultation infact released within 2 hours with no pain relief. 1 / 5 / 22 apart from knowing the mesh is there one year later still holding back the inguinial hernia. The mesh is uncomfortable but not as bad as carrying a hernia

      Usually small incisional hernias will not be a problem during pregnancy . It is best you consult a surgeon and get it checked before planning the pregnancy .

      Dr told me mesh was fixed and I am feeling still pain and uncomfortable after surgery still my belly button is outie and occasionally bulging and painful about 5 cms radius of belly button and belly muscle is paining majorly right side of bellybutton muscle is paining regularly and I am unable to eat mouthful of food and if I eat a little bit also feeling heavy and if I eat heavy my stomach was paining
      and majorly I don't like my outie tummy
      so please guide me what to do sir

    Lifting weights increases your chance of recurrence by 85%. Surgeons won't tell you that. Best thing to do is yoga, walking, hiking, swimming, pilates, etc. Do not go back to lifting weights. Crossfit is probably the worst thing you can do.

    I have been operated upon the right side ok inguinal hernia laproscopically with a mesh...
    After 5 days of the operation there appeared a swelling on the hernia site. Although there is not much pain in that I suspected it to be the failure of the operation. My surgeon told me that it's a seroma formation that will take about 2 months to get disolved on its own. But I m still not convinced... Kindly guide if there is anything to worry?

      And today its the 26th day after the surgery, the seroma or whatever it is, still persists as it ws earlier.

      Brother do you feel numb the hernia Sergery site or not ? My operation is done 6 weeks ago but now it starts paining as i started walking...

    Sir mujhe tha hernia 2016 me surjury hui and now mjhe fir dubara surjury krwani pd rhi h and right now i m 23 yr old. But i can't understand me na to koi heavy weight nhi lift kr rha tha i just quit gym after that and koi bhi diabetes nahi h.

      @@mrmaddy6889 same thing is with me i had surgery 2 weeks ago left side is hanging fine but right side is fixed near the upper part of scrotum. Please give suggestions anyone.
