  • Exercising the rotator cuff when it hurts is one of the worst things that could be done. This tends to add stress and tension to the rotator cuff tendons and keep them from healing properly.

    This is all very compelling information I have not heard from my PT and sports med ortho doc related to my 2 year long shoulder pain. I was dx’ed with impingement syndrome, scapular dyskinesia, and learned that i have significant hypermobility in both shoulders. X-rays are pristine so no arthritis. I have been extremely disciplined in doing the PT work (9 exercises in all) and still I have pain. I’m a 63 yo female with years of strength/resistance training under my belt. Every day I want to cry out of frustration but i just keep at it. My doc says my rotator muscles are very strong via exam so he doesn’t think there is any type of tears in muscles. I’m just so depressed about the lack of progress pain-wise and how much strength I’ve lost in upper body. It just seems like my muscles are writing checks the rest of my anatomy can’t cash anymore bc I can’t, apparently, get the biomechanics right. I’m going to look at more of your content and just keep going. Ugh.😩

    Very helpful information. I’ve been on a very long road with my shoulder issues. And even though it has been almost pain free this year, I still have setbacks and have to really pamper it and move forward much more slowly than I want to. It is a constant learning process. The things you mentioned here really resonate with me. This was perfect timing!
    I’ve learned a lot over the years from great videos like yours but there are so many things to know with the shoulder, and this kind of information fills in a lot of the gaps in the typical things people hear. Thank you for sharing your knowledge with us. It’s so helpful!

    Thank you. I am trying to recover with lots of light shoulder exercise and stretching but it’s a very slow process. I think I caused the problem through heavy weight training.

    Hi Doctor David, I love your videos! You had a video on knee pain not to long ago where you showed us by turning your foot inwards and outwards while keeping your leg still to lessen knee pain. I have osteoarthritis in both knees really bad and I have had excruciating pain in my knees for several months where it was so painful to even walk. Even though I still have knee pain, doing these exercises has relieved some of my pain where I can walk in lesser pain now and I wanted to thank you so much!!! Can you do some videos on shoulder and neck pain with osteoarthritis in them. I have osteoarthritis in both shoulders but I only have it in my neck on the right side as of now, and it doesn't help by being a side sleeper either and it's really pain full at times that I twist and turn all night and wake up really exhausted on some days I also can't sleep on my back because I get headaches when I do, due to a incident that I had when I was in junior high school. I did by a Sutera Dream Pillow which has helped with the migraines I was having with the shoulder and neck pain but no matter what my doctor has suggested for me to do I can't find no relief in them please help!!!

    Okay found some. Hope you get to see this... I have had an accident flying from the horse 20 years ago. I know I hit my shoulder on some Iron beam. The anckle & underleg was crushed smooshed, so that had the main care. I live alone, so needed to walk on cruches. So the shoulder could not heal properly. I could do pressure or pull if I helped my arm to go up. I could not lift it to shake a hand. I could do the pressure walk with cruches. It turns out I had a difficult weird small tear in the bone up front. That was the pre-history. Now a few months ago I slipped on the Ice and landed straight on elbow/underarm. But ofcourse UP into also my shoulder. Now 😢 I can hardly tuck my shirt in my pants. Still. I can not reach till fave level. It pulls. Or moving arm in 90 degree position to the rights also yanks. And the sudden cracks are not nice. I also have to say, add, that in the last year before this last Ice crack sometimes it felt like a tender? Needed to pop on the inside upperarm / front. Now the triceps or back arm hurts like I have been beaten and it's bruised like black. I have zero believe in the therapists here, cause when I got the accident 20 years ago amd told them about the shoulder brake, they only are focused on getting that huge needle treatment. And I know that is not what I mean with my pain. Inflamation I guess they use it for? That's not it. And for me with the Fybromiyalgia & lack hormone D muscles power easily breaks down.     I needed this video today. My right shoulder hurts a lot which I think was from lifiting weights. Some days it feels like it is healing where I have no pain. But then I can be sitting in a chair, reading a paper back book, when suddenly it begins to hurt again. I've tried arnica cream on the area and even a heating pad which makes it feel better. I don't know what else to do other than wait for it to heal. But since I am right-handed it is impossible not to use my arm for everyday activities. I'm going to watch your other videos on this to see if there is anything else i can do to help the healing process.

      @@epmanualphysicaltherapy Thank you. I will watch the videos. I also remembered that I had pain soon after playing way too many rounds of solitare online. I think it was the mouse-clicking that did it. I was working out with weights, as well. I stopped and the pain eased up but as soon as I started again it returned. Then it got so bad that anything made it hurt. But I will check out those videos now. Thanks so much for getting back to me. Oh one more thing. I used to see a chiropractor regularly but its been years. I think I might have to visit him again.

    I have question: how do differentiate between a rotator cuff tear and bursitis?

      That is a great question, Sherry! Unfortunately, it is very difficult to describe without being able to show you. There are also various levels of severity when it comes to shoulder bursitis. One of the best things that can be done to determine whether there is a rotator cuff tear is to think about the mechanism of injury. If there was a very specific incident that caused the injury and a tear or pop could be felt, then it is possible that a rotator cuff tear occurred. Hope this helps!

      @@epmanualphysicaltherapy thank you. There was no particular injury just pain. I know I have a full thickness tear (diagnosed 10 years ago) but I’ve been able to ‘PT it’ to not hurt. Some of your exercises to determine if it is a tear hurt but others do not. I’ll just keep doing the ones that don’t!! ☀️

    I had both of mine fixed. I am so mindful of not tearing them again. You cannot have them fixed twice.

      Hey Mark! Definitely a good idea to be cautious after the surgery. It is very frequent that people re-rupture their surgically repaired tendons.

    Hi Dr David, thank you for your videos. I have been in agony for 2 months and finally got an ultrasound scan that shows a tendon tear in my shoulder. Can this heal and do you recommend any exercises or rest please?

      Sorry to hear about your tendon tear in your shoulder Steve :-( It is possible for this to heal. There are many methods to help this and the "correct" way to help heal it will differ for each person based on their condition.     Hi Dr. When should I start exercising rotator cuff injury that is from a fall? It’s been 3 days l. I’m getting conflicting information. Move it or don’t move it? I can barely lift my arm at this point. X-ray confirmed it’s not broken or dislocated. Please help

      Hey Alicia! Great question! There is no precise answer to this question, however, it currently sounds like the best thing for you to do is keep the arm supported in a sling.     I have an impingement i think, clciks a lot with some soreness if i rotate it. Ive been doing the exercises youve said not to do! If theybare bad exercises, what are good exercises? Shouldni be adding some weight when the soreness stops?

    So if you have pain, when do you strengthen? In another video “2 shoulder exercises that are safe for a rotator cuff tear” you’re laying down w/arm on pillow moving your arm front to back slowly, aren’t you saying here basically not to do them? Just chking. I did them and the pain I had in my shoulder/neck actually lessened. You mentioned these exercises are for ppl w/more severe pain, but here your saying not to work in pain. I’m just confused. Thx. I have a tear and impingement. Should I not being doing them?

    I watched one of your videos earlier about 3 of the worst and best exercises for a rotator cuff tear confirmed by an mri. One of the exercises you said was best was one where a woman was lying on her back with a pillow under the shoulder for support. You then had her rotate her arm back and down slowly for at least a minute, to be repeated several times a day. But in this video, you seem to condone that now. I have a partial tear, confirmed by mri, in the supraspinous portion of my shoulder. Doing the pillow exercise, along with the shrugging exercises you showed in that previous video, has been extremely helpful but now I'm concerned if I'm doing the right thing. Could you maybe clarify? Thanks.

      Great question Debbie! The shoulder rotation exercise shown in the other video can definitely be a great exercise for a torn rotator cuff. The purpose of this video is to warn against doing any exercises that cause or increase pain. Any exercises at all, including the ones we recommend, should not be performed if it causes or increases pain. It's not that these are necessarily bad exercises, but it is very possible that someone may be performing it incorrectly or it may not be the right time to perform that motion. Having said that, if these exercises are helping you, then definitely continue with them! Hope this helps clarify a bit!

      Thank you. Would you consider doing a video, or at maybe briefly discuss shoulder arthritis and rotator cuff tears. I had a partial tear in my right rotator cuff and, after what seemed like endless months of PT, I had an MRI. It showed severe arthritis and a partial tear so, I had both total shoulder replacement and rotator cuff repair all at once. Now, six years later, an MRI has shown pretty much the same thing in my other shoulder - a partial tear with advanced arthritis. However, the level of pain now vs my other shoulder is like night and day. I was in tears before my surgery but this is is so much more tolerable, even though my orthopedic surgeon said my MRI looked almost the same as it did before. I am very active and have been for decades and have no plans on giving up. However, since it's not just a rotator cuff tear I'm dealing with, I wonder what would be the best treatment option.
