Because it's bugged and works through scars atm. TBH I wouldn't normally abuse it in a proper run like this but I kinda took this run as a joke because of how much easier they made max fear with the patch.
I understand the frustration of Vow of Suffering being removed, but i dont get the ire towards Athena's keepsake. It is a very powerful option but not literally mandatory, the same challenging experience can be achieved with 55 fear + no Gorgon, the same way that bugs which reduce challenge by bypassing Scars can be avoided to self impose a purer challenge. Suffering's removal seems way worse to me since its GONE gone and no amount of player choice can replicate it
At the very start of the video they show the previous run to prove the 55 fear run isn't seeded, and during that gameplay you can see they have divine dash. Their only other boon is hera attack, and on the 55 fear run the attack slot is already filled by that very same boon, so the only valid boon from the previous night available to pull from is divine dash
