Dorothy: Anterior and Posterior Hip Replacements

  • Dorothy, a dental assistant, started having trouble with her hips about ten years ago. The pain on one side progressed over the years and she went for an X-ray.
It was clear she needed a hip replacement. Many friends recommended Dartmouth-Hitchcock for her care.
"I was very comfortable with the hospital," Dorothy said. "They were very thorough. That gave me a lot of confidence. The doctor was just amazing - he described the surgery and I could really sense, 'This is what this man does every day! There's nothing to it.'"
The surgery was successful and she had limited mobility for six weeks, as well as post-surgery pain medication.
Knowing her other hip would need to be replaced soon, she didn't wait until the pain became unbearable. She went back to Dr. Stephen Kantor, who had performed her previous surgery. He told her that the new anterior approach, in which the incision is on the front of the body, not the back, would be preferable in this case.
"The difference between the posterior and anterior were amazing," Dorothy said. "I had absolutely no pain after surgery. ... I never took a single pain medication, not even a Tylenol. It was amazing. I had no restrictions. I was walking hours after the surgery."

Just had posterior done five days ago. No pain meds at all. Got out of bed myself. It ALL depends on your physical shape going in. If you are overweight and out of shape your recovery will take longer.

2 weeks ago yesterday I had an anterior hip replacement. 2 days after surgery I was walking outside around the block with a walker. Zero restrictions on movement. Yesterday I received the doctor's OK to return to work. I am back nearly to normal before my hip went bad. That is the worth of the direct anterior method over the posterior method. I have been walking about 2 miles per day for the past week.

I am so glad your surgery went so well but I am having a very hard time believing that you did not require any pain medication at all not even a Tylenol. I'm about to go through hip surgery myself and I'm trying to make good decisions but some of these videos just don't seem real to me. I hope they are. I hope they are.

22 weeks ago had posterior repl which was 5 yrs overdue. Left hospital 27 hrs after surgery. 18 hrs later took the only painkiller since. Generally stuck to anti-inflammatory foods for 12 weeks. Believe my procedure was successful.

Yeah but you're not a sportsman. This is the issue we need resolved. We know it works for normal stuff (albeit the positioning of the cup is apparently far more difficult with the anterior approach - we're just looking into that now).

I've had the posterior approach, I'm a rock climber and mountain biker and was able to get back into the sports about 2 months after the op. I have been offered the anterior approach for my other hip and I am a bit undecided.

Thankyou, you've answered my question. Now I can concentrate on loosing excess weight in order to have the anterior approach.

I am in my late 30's . Had both anterior and posterior. Main thing is to find a good doctor who successfully did lot of surgeries without any major complication. I had two different doctors also. I have less range of motion with anterior. But problem with posterior is extremely painful with even lot of pain meds. I still have burning pain at the surgery location after four years with posterior hip.

@@rs-zl7re I am obese... my surgery is in two weeks. Anterior. It is my understanding obesity is not an obstacle to anterior surgery - it is not helpful for rehab in either case.
