Problem with sacrificing your toe for your thumb is now you have to consider the absence of not being able to use your toe to balance yourself; learning to walk again because the arches on the foot will be more painful going forward and not having the toe to pro-pulse to walk forward comfortably will lose the structure as well . Not judging the decision. It's all based on the patient's livelihood. If he needs a functional hand and decides to go this route, then fine. The next alternative will be for him to have custom insoles for his feet containing a carbon fiber spot for where the missing toe once was so he can properly balance, and the arches on his foot won't be painful when he walks. He'll have an artificial big toe in the orthotics. Me personally, I'd rather go with a prosthetic thumb. To me, structure is more important first. Foundation is key -- like the architecture of a building or a skyscraper. With a prosthetic thumb, I can at learn how to use it as a tool to aid what is left of my hand.

It can be. But DNA and such have to be an exact match. And that's like finding a needle in a haystack.
