  • In this video, Dr. Wood walks through a case study of a man diagnosed with BPH.
Prostate artery embolization (PAE) is an interventional radiology treatment for BPH that offers a lower risk of urinary incontinence and sexual side effects compared to surgery. PAE works by reducing the blood supply to the prostate, causing it to shrink.
Patients who undergo PAE have reported high satisfaction, no urinary incontinence, or sexual side effects. Minor side effects include frequent and/or difficult urination, pelvic pain, blood in the urine, blood in the stool and diarrhea. Most of these side effects resolve on their own in a few days.
Compare that to TURP, which usually requires 3 and 6 weeks to recover, and a catheter is left in place for 2 to 3 days. Complications and risks include excessive bleeding, retrograde ejaculation, and impotence.

Can PAE successfully target large median lobes? History: UroLift 2 years ago (intact and working) . No improvement with UroLift. New Doc. discovered high bladder neck and large median lobe. prostate size 55g. Lobe is where all my issues are stemming from. Want to preserve current Sexual level. On daily coumadin ( heart valve). I've not had much luck finding out if a median lobe can be treated successfully with PAE. Closest IR who does PAE is Jacksonville FL, about 5 hours one way.

A prostate that large doesn't leave you with many options, the PAE is best suited for large prostates such as yours.

Who is not a candidate for PAE?
In general patients with small prostate glands (< 40 g) are likely not ideal candidates for PAE, although data in this area is variable. Unlike many surgical interventions large prostates appear to respond well to PAE (Kurbatov et al.Jan 18, 2019 found in internet

I'm all for pae except except no long-term studies on the health effects of and how much there's been any standardization in reducing the amount of radiation that a patient is exposed to and of course never a mention of it which is very duplicitous.
It's like you guys don't want to talk about it... I also went on a big old rabbit hole on the amount of possibilities that the obstruction beads could be placed in a wrong area and that area would then be starved of blood and die like your penis!!.. or some surrounding structure like that.. I didn't even know that could happen scary

Rezum is only beneficial in treating glands < 120 mL, and there are limited long-term outcome data. PAE is technically easier with larger, more vascular prostates.Apr 2, 2019 found in internet
