- Prostate artery embolization (PAE) is an interventional radiology treatment for BPH that offers a lower risk of urinary incontinence and sexual side effects compared to surgery. PAE works by reducing the blood supply to the prostate, causing it to shrink.
Patients who undergo PAE have reported high satisfaction, no urinary incontinence, or sexual side effects. Minor side effects include frequent and/or difficult urination, pelvic pain, blood in the urine, blood in the stool and diarrhea. Most of these side effects resolve on their own in a few days.
Compare that to TURP, which usually requires 3 and 6 weeks to recover, and a catheter is left in place for 2 to 3 days. Complications and risks include excessive bleeding, retrograde ejaculation, and impotence.

Had PAE done jan 17 23 and within 2 months it worked Mar 14 23 i had urinary retention and a catheter for 3 months I had to pay out pocket 12000 since My prominence medicare insurance decided not to cover it even though it's medicare approved i am appealing. PAE it works great
I HAVE ENDURED most of the BPH issues. I Had 3 biopsies, 2 T3 MRIs with no benefit. It got worse and worse with a catheter use, both manual or attached with constant infections. Finally I gave in for a TURP or the surgery where they go into your prostate down your urethra and literally cut the extra size away and open up the bladder to drain. The procedure is not THAT bad and is less painful than other commenters say the PAE has. I have had 2 heart catheterizations with out any of the pain described about PAE. The bottom line is the TURP did open the flow, but did NOTHING to the other symptoms, like frequency, urgency, or night time issues. My prostate is rather large so the PAE likely needed doing first, so you need to push for a PAE if you are also larger. I can empty about half of my bladder but am constantly going, going and going. The medications they will want to shrink your prostate are a very big gamble. One triples the chance of you getting dementia, the other means you will have a 70 percent greater chance of getting a worse kind of cancer. PAE IS THE BETTER STARTING POINT OR AT LEAST A Rezume If you are under 60 or even younger. The TURP did uncover a much worser kind of cancer just as promised if you take the medicines they will want you to take to shrink the prostate.
I had my PAE on 14 October 2021. You will need to be driven home! It took two days for the sedation medicine to completely leave my body. I already see results within several days especially during the day. I am getting up less and less each night. I looked for years for something not hard in the body (laser, rezum are heat (swelling), aquablation does not hit entire prostate, urolift, from my research fail). I am scared to death of needles but happy had it done. Within 45 minutes of pae completion I was wheelchaired down to friend waiting for me.,,
You made the right choice. The Urolift did nothing for me. Getting a PAE in a couple of weeks, hoping for better results.
@@katazack I think you will be very happy. Worst part it took a couple of days to get sedation out if body. But I noticed relief in two days - able to empty bladder and stream was was better. Been two weeks and my sleeping improved from waking up every ~45 minutes to every 2 hours or longer. Good luck! Post how you did, please…

@@katazack ..I guess one can have a PAE after a Urolift? I also have a Urolift and was now considering a PAE , but couldn’t find information on whether it could be done after a Urolift.
You drove and drove yourself home?!?! I’m having it on June second. My urologist really pushed Rezume. Read too many horror stories about it. Glad I did the research.

@@luigi5890 I am having unanticipated adverse affects - I now have to self-catheterize every night and my retention volume went up to ~ 250ml. I was told that it would get worse before it got better, but this was never something that was explained as a potential side-effect. How long after your procedure did blood in your urine stop?

I tried a prostate artery embolization "PAE" nine weeks ago with zero results,. Prostate was at 120. Will be doing the Aquablation June 13th.

@@DeathSentryCoH Never got an answer to that. Dr. said there was a 10-15% fail rate. Dopey me never asked why. But i need to find the answer. Nevertheless, i went ahead and had the aquablation procedure don June 13th. All the plumbing is now working.

Days 8, 9 and 10 post procedure were misery for me.
So what becomes of the dead tissue now dwelling at the entire circumference of the eurethra.
Since this procedure stops blood flow to the prostate, what happens with all that dead tissue, necrosis kind of thing?
Why not say where you got the procedure done? Not many places in the US do it, and I understand that it is a procedure very dependent on doctor skill.

Now, in 2024 there are other procedures like holep, greenlight lazer, and resumz.
Do your research and look into these other options. I'm getting holep in a other two weeks. HoLEP is a one and done procedure. With a skilled doctor there are little to no side effects.
Is PAE is also suggested for Patient with Ca Prostate with Gleason 8? Can anybody answer
My doctor will not perform a PAE with a Gleason over 6.
Amazing, the advances of medical science and PAE is one of them. A procedure much easier for the patient than other options. I had mine done on Oct 20th. BUT, as Paul Harvey used to say “and here’s the rest of the story”. I’ll start with PAIN - for a day and one half after the procedure you will feel pain. Similar to a metal shaft being shoved from your rectum to your bladder. It hurts but after that time period it starts to diminish. Actually mine took 4 days to go away. FREQUENCY - if you think you had to go pee a lot before the procedure, wait until after. Easily twice or three times as often. BURNING - oh yes, yes, yes. FEELING - along with the frequency goes the constant feeling that you have to deficate but you don’t. Day 7 - pain is less but still there, had to call doctor for additional meds. But given the horrendous recovery of other procedures, DEFINITELY worth doing. THE TRUTH AND NOTHING BUT THE TRUTH. GOD Bless.
I had mine procedures 3 weeks ago and went through similar experience, Day to Day 4 was painful and Day 5 I got better. After 2 weeks I fully recovered and the stream of my pee was better than before, I am still under medication Tamsulosin as the full effect of the procedure won’t kick in not until 4 weeks to 6 weeks. Will start to reduce the intake Tamsulosin this week. Let’s see.
@@tkam6100 I had zero pain after PAE. However, after five weeks I still was not able to pee. Went ahead and had the aquablaion procedure done. Again, no pain, but had to stay two days in the hospital. had some blood clotting in urine. Results were as hoped. All the plumbing is now working.
This sounds wonderful, but I watched a walk through by a guy from Pennysylvania and he said and the images showed the variability of arteries in that area between people and even between both sides of the prostate. He said amusing things like no not that one that goes to the penis or to the anus. Sadly on a another site it gave stats and the high percentage of ischemic anuses as side effects. It is my impression that it is really hard for them to tell what they are doing. Plus, my doctor said that it is a awfully large amount of radiation to the prostate to do the real time scanning. I have become skeptical. Oh, and how is it that only surgeons are greedy and radiologists not? I don't get that.
Ya know, I've listened to md's talk about TURP and how it would benefit me, I've read the broschures. The side effects that were mentioned were made to sound insignificant. I have to wonder what this fellow is leaving out.

@@MF-kb2nv : Hopefully you didn't do TURP, it's old school now. I'm 78, last PSA was 32, up from 12 a yr ago, have had BPH since my 40's, so had a Prostate MRI w/contrast, 'n no cancer was found, but prostate found to be 143g in size, too large to consider PAE. Did the research, as my 1st Urologist wanted me to have a TURP and I declined. The new "gold standard" is a laser HoLEP procedure, but of course he didn't tell me that, as he doesn't do that type surgery. Whichever procedure you decide on, make sure your Urologist has done at least 200 of 'em, you don't want a "Trainee" working on your manhood. If your prostate is larger then >80g do a HoLEP, or HoLAP or Green Light if less than
He did not mention UROLIFT or REZUM. I would like to see a true comparison of these 3 options. PAE vs REZUM vs UROLIFT.

@@gtibruce For prostates? I think it is a little over a decade. For other procedures several decades, at least. I remember seeing ads for varicose vein procedures when I was young, in the '70s.
i have done this procedure and i am very much ok before for peeing i take too much time now its very easy sexuel life also very good no side effect nothing only i want to ask one question are taking any medicine after this procedure why i am asking i am taking Veltam F tablet every night is it need to go for this tablet pls help me in this.
I asked my urologist about PAE and he said ..."its not a good procedure...". At the time, I did not understand that PAE is not a procedure done by Urologists, but rather by an Interventional Radiologist. This makes it difficult for patients to understand the treatment options for BPH, and whom to trust on the recommended procedure. Why is it this way?
I would have asked him who it's not good for, him, or me.
Let me guess, your urologist was a surgeon (95%) and surgeons like to cut.
Of course he did. They are surgeons and the more prostate removals that they do the better for their curriculum vitae. I had it done--by an interventional radiologist--the only doctors who can do this. This is the best procedure I've done in my whole life! I would recommend it to anyone whose body is healthy. The embolization went thru an artery in my right leg. Recovery time took a week--I had to lie still until the puncture in my leg healed. I can do anything now. No problem. I did notice I had lost some of my erections--but two years later, they have come back. I urinate normally and not like I use too--getting up late at night and nothing coming out or having to go into emergency and have a catheter put in for a week or month. Your urologist will not tell you about this procedure. I had to research this information and find out for myself.

My urologist never mentioned pae as an option and now I know why. Because it's not money in his pocket.
I'm 51 with a 40cc prostate and very minor symptoms (dribbling a few drops after each urination). Is it too early to get a PAE or perhaps an alternative like iTIDS or Rezum ? I GP recommended that I should wait. Will an early PAE prevent any further intervention?

I took 400mg of Saw palmetto pills and 3 tabelspoons of pumpkin seed oil, changed my diet to plant-based low carb, and 6 month later, US scan showed that my prostate has shrenk down to 34cc. I'll continue with this practice for another 6 months and see of the trend continues.
I am thinking of having a pae procedure. Can i talk to you about it?

I am actually a believer in natural treatment. My father suffered from BPH and had several surgeries unfortunately, no cure. It was all expensive procedures which exposed his health to risk at 68 years of age. Read reply to know how he found help….
I'm always looking for natural treatments fro my BPH. On Tamsulosin for 2 months now. Works ok; not great. Any help would be greatly appreciated.

Been on Tamsulodin have had none of possible side effects, I was getting up 4-6 times per night now it's only once so very pleased but I guess the drugs will only last for so long then a more permanent solution will be needed

I'm just curious, was this patient a smoker? It's obvious he was overweight which causes most problems for humans. If a younger man is reading this, do yourself a favor and don't become fat, and leave the tobacco alone.

You would have to check with the interventionist radiologist office and I'm sure they can tell you.

This guy is not accurate there's a 1 in 100 chance that you could have rectal bleeding or Pinot bleeding
