    I just had the the balloon shoulder surgery done for a non-repairable tendon 12 days ago. I also had a repairable full thickness done as well as a bicep tendon repaired as well. Total of a 3 1/2 hr surgery. Recovery has been going well. Haven’t started PT yet but only had uncomfortable pain for about 2-3 days but manageable and as long as in sling I feel pretty good. Definitely know I’ll have work ahead of me but so far so good.

      Sounds like you have the same tendons tore off the bone as I do. I have three tendons that are full sickness tears and severely retracted. My doctor says he's not very confident that this surgery is going to heal. I'm glad to hear you're doing good, my doctor never mentioned a balloon of any sorts

    Like others, I am enjoying your videos which are very informative and educational. Like others, I stumbled upon your channel while researching the surgery I have coming up on 16 March. My tear is a repair of a16 year old previous repair which was full thickness and retracted. The tear now is full thickness of the supraspinatus and retracted 36 mm. Also the infraspinatus is torn. The surgeon's plans are to perform repair with release of the supraspinatus and infraspinatus, using an Allograft possibly. The original tear used a porcine graft which lasted a long time. He plans on doing a suprascapular nerve decompression, subacromial decompression, and distal clavicle excision. He said the subacromial spacer balloon is an option also. The first surgery in 2004 started out to be via arthroscope, but ended up being an open procedure. The doctor now is concerned about adhesions which he will get rid of. He is the Director of the Tulane University Department of Orthopedics, Professor of Clinical Orthopedics, Chief of the Division of Sports Medicine, and Director of the Tulane Institute of Sports Medicine. He spent lots of time explaining the pros and cons of different procedures, and stated I am too young, at 63, for a reverse shoulder replacement but he left muscle transfers on the table if they are needed. He hopes to preform this arthroscopically, but it could end up as an open procedure again. Wish me luck and my prayers go out to all of the patients out there with the same problems.

    I had the balloon repair in December, 2022. This procedure has been used overseas for 15 years and just the past year has been approved here in the states. It was only approved at certain facilities. The balloon did dissolve within the 3-6 month time frame and only created a minimal space. I tore the bicep off the bone. Unfortunately, in my situation it made minimal difference. I had PT for 7 months and I only have range of motion but no strength. I am now scheduled for the reverse shoulder repair. I, honestly, wish I had done the reverse surgery instead of wasting 7 months doing PT that really didn’t help. Everyone is different so it may help others. This was just my experience with the ballon surgery.

    Much thanks Dr. Orcutt for all the knowledge you share. I found your channel in 2020 when I had a terrible fall. Resulting in…
    ( easier to cut n paste) I don’t know all the damage by heart!. 😂
    ~Full thickness tear in the supraspinatus and full thickness tear infraspinatus rotor cuff tendons.
    ~Atrophy of the corresponding muscle belly.
    ~Suspect chronic acromioclavicular joint separation with a tear of the trapezoid portion of the coracoclavicular ligament and acromioclavicular ligament.
    ~Suspect adhesive ligament
    ~Suspect anterior labral tear
    -Small glenohumeral joint effusion.
    While I was not looking forward to a RC surgery I was surprisingly even more disappointed to I would not be a candidate for the surgery because of the severity of my tears.
    I see my orthopedic about once every four months to get a Cortizone shot. Each visit we discuss this procedure. He said I would be an excellent candidate. As you stated getting it covered by insurance has been extremely challenging.
    PLEASE keep us followers updated.
    Much Thanks for all you do! 😊

    I had a massive rupture of the supra with 2.3 CM retraction. Surgeon was able to pull the tendon back and anchor but she also inserted a Stryker balloon. I am 7 weeks post op and PT tells me I'm at least 2 weeks ahead of rehab protocol. Dr. will MRI me at 3 months and again at 6 months to determine the status of the repair. Long term results have been about a 30% failure rate for the reattachment surgery with a massive tear such as mine. She is going to perform similar procedure on subsequent patients. Based on the results thus far, It's definitively been a positive. We'll see at 6 months when the balloon has dissipated.

      Hi Dr. Orcutt!
      My question is, in the video you said the balloon reabsorbs in 3-6 months, but then on a comment you say it dissipates in 2 years. Wondering what the length of time really is? My doctor just suggested this balloon implant procedure for me since he says I'm too young for total reverse shoulder replacement surgery.
      Why am I considered too young? I'm 55. What are the possible complications with younger age

    If the benefits are short-term, how long does it take to go back to your normal activities? I have a total rotator cuff tear and a proximal bicep tear. I am a nurse practitioner and struggling between to do or not to do especially I can still workout and have very limited pain.

    • @rotatorcuffexpert
      @rotatorcuffexpert  2 года назад +1

      Adnan, It does not appear, at least in the literature that I have read, that the results are short lived. I think your best bet is to talk to a reputable surgeon in your area that does a lot of rotator cuff repairs ( It shouldn't be that hard to find one since you are an NP). They will need to see your MRI and do a good physical exam to be able to give you your options. Good Luck and let us know how it goes.

  • @1RedEarthProductions

    The reason it works is that it essentially replaces the CA ligament that “old school” surgeons take down during decompression. That allows the cuff to rehab without the humeral head migrating, thus strengthing the remaining cuff muscles as well as the deltoids so that when the balloon degrades, those muscles are strong enough to keep the head centered in the joint.

    • @rotatorcuffexpert
      @rotatorcuffexpert  2 месяца назад

      Almost, It recreates the depressive forces that the superior rotator cuff create. With a massive tear there is no depressive forces and that leads to superior migration of the head and glenohumeral dysfunction.

    • @1RedEarthProductions
      @1RedEarthProductions 2 месяца назад

      @@rotatorcuffexpert Especially if your surgeon stupidly takes down the CA ligament which was perfectly healthy. Leave that intact and you still have that ligament acting as the girdle across the top to prevent upward and forward motion of the glenohumeral head.

    • @1RedEarthProductions
      @1RedEarthProductions 2 месяца назад

      My second surgeon specifically said that is why he does not take down a healthy CA ligament in a subacromial decompression due to the liklihood of migration of the head in the joint. Had no idea going in that my first guy was doing harm since he never told me healthy tissue would be cut away to repair a partially torn supra.

    • @rotatorcuffexpert
      @rotatorcuffexpert  2 месяца назад

      @1redearthproductions. Thanks for your comments. The CA ligament does perform a function in the shoulder which is up for debate among orthopedic surgeons. That being said, the main depressor of the humeral head is the superior rotator cuff and that is what the balloon is recreating, the superior rotator cuff, not the CA ligament .

    • @1RedEarthProductions
      @1RedEarthProductions 2 месяца назад

      @@rotatorcuffexpert But do you agree that the CAL The CAL acts as a static restraint to superior humeral head migration and load distributor across the shoulder girdle which essentially helps prevent superior and anterior migration? Not sure what the debate is…I will always wonder that if my surgeon had left it intact as my second surgeon says he does during a rotator cuff repair, if I would even be needing an InSpace balloon. What I suspect is the true debate is actually one of ability, not functionality. It just seems wrong to take down perfectly healthy ligamants that have roles to perform, especially when studies have shown that it takes 25-30% more force by the rotator cuff muscles to keep the huneral head centered in absence of the CA ligament, so why in the world would it be taken down if the goal is to regain function after a tear? Regardless, the balloon is my last ditch effort to become pain-free short of a reverse total shoulder which would likely sideline my vocation as a sports photographer. Supporting 10 lbs of gear for hours at a time on a compromosed shoulder or one with a restricted range of motion after a reverse would simply not work for me. I have great range and ok strength after a failed repair and now my supra is unrepairable and when the Regeneten patch disentigrated, it took the infraspinatus with it. And the “dissolvable” staples that should have been absorbed im 3 months? 8 months later they were found floating around in my shoulder still attached to fragments of the collagen patch.
      Not sure what the CA debate is, but everything that is done should always be “first, do no harm” when it comes to any surgery. I will always believe that debates like this come down to the skill level and convenience/time needed of a given surgeon. Just an observation from one who wishes his stabilizing ligaments would have been left intact which may have prevent the need for a second, now third, and possibly a fourth surgery just to eliminate my pain.
      That being said, you are doing a good thing here. Knowledge is power and had my surgeon told me that he would be cutting away perfectly healthy tissue anytime before I was put to sleep, the first surgery would never have happened because I have had more pain since the initial repair than I had after the accident. And yes, that makes me bitter.
