I purchased a cold therapy machine and slept in it. It made all the difference…I used it for many weeks. Was so worth the money as it conforms to your shoulder and you set it for how long to be cold and how long to be off the cold. Loved it. My pain was not manageable without medication and cold therapy. I’m 3 weeks out.

Good video and information. Had a massive rotator cuff tear repair and bicep tenodesis 7 weeks ago. Did not want to take any opioids. The 1st night was rough after the nerve block wore off, but the pain was very tolerable with anti-inflammatory, Tylenol, and ice. Had myself all worked up about the pain being horrific without opioids but was pleasantly surprised when it wasn’t. I think ice is the key.

I’m happy that worked for you. I had traumatic massive tears, open and arthroscopic repair and 9 days out I still need a half dose of my prescribed post op opiate just at bedtime or I can’t seem to get to sleep. I’m ok with all the others rest of the time. My pain has been pretty dang severe. My surgeon says he’s not at all surprised-I think each person is different and it depends on the type of injury and repair. So for others who aren’t ok without a little more help-just check in with your surgeon and as long as you are doing what they recommend, you’re ok.
My surgery is 12/27/22.
What did you use to deliver the ice?
@@johnkru1295 Coreflex cryro pneumatic shoulder wrap the ortho gave me, along with the dreaded sling. Didn’t use the pneumatic function, just kept rotating the cold pack inserts as needed.
Great recipe for constipation. Has worked 100% of the time for me.
2 tablespoons apple juice
2 Tablespoons prune juice
2 tablespoons milk of magnesia
Stir together
Heat for approx 20 sec in the microwave.
Within 2 hours the magic happens.
I had Achilles surgery, knee replacement and shoulder surgery all within 14 months and this formula helped me through all 3.

Thank you for all the information-very helpful and more than my doctor shared with me. I watched this before my surgery and again afterwards. I would like to add that having a heating pad to sit on my lap was helpful as the ice machine on my shoulder made me shiver and the heating pad was so comforting.

Excelente reviver. Exactly how my surgery was. I had the block and same medicines. All the info you give in this one video. I had to search and put together by looking at several different videos. My surgery went fine but my surgeon only told me 10 percent of what you are saying. I wished I had found your video right at the beginning. Your info is very concise and fast. If you see it at first before and others you will have to play it slow to take notes. I will only add exercises for the first 2 woks after surgery. I set up three nests throughout the house and move from each when I get bored. Four now. I hate being lazy but over active lifestyle is how I tore my joint up lol😊

Wow this is full of brilliant information. Thank you so much for making these videos. I am on day 9 after having my right shoulder arthroscopic acromioplasty, rotator cuff repair
and biceps tenotomy and this is so helpful. On day 7 post op, I ceased Cepalexin and Buprenorphine. I tried to go without any sort of pain meds but that surely did not last long. I am managing well on panadol osteo twice a day along with ice packs after passive exercises 3 times a day as designed by my surgeon. Thank you as from a patients perspective, these are the best!!

Really helpful info. I’m just 3 wks in my surgery. I had a bicep tenodesis repair to. It’s really difficult getting things done living alone because as luck would have it, it’s my dominant arm! I’m not used to doing much with my left hand.

Excellent video, I should have viewed it before my shoulder surgery. Surgery was a breeze but the stiffness experienced for the following 8 -10 weeks was unbearable.

Same here. My surgeon never told me to keep moving it - and the pain was extreme (I ended up in ER on morphine at day 5!) and it is too painful to move it. I’m a week out and really struggling.
Dr. Orcutt- your videos are invaluable as I prepare for rotator cuff surgery! I had surgery on my elbow about six years ago to clean up damage from my auto-immune condition. They did a nerve block. The surgeon said he could do it laparoscopically but thought he might do better with an open incision. That was fine by me (about a 4 mm incision). The nerve block lasted almost 36 hours (I was beginning to think they'd done damage) and when the block cleared, I didn't even need Tylenol! For the last few years due to psoriatic arthritis and degenerative disc disease I've needed about 1-3 50mg. tramadol q d. During flares I've used up to 7.5 mg. hydrocodone q d. I don't need to kill the pain, just make it bearable. I hope this doesn't bite me during the post-op pain control. I've been biting the bullet lately and avoiding analgesia in hope of more effective post-op analgesia. The NIH says hypnotherapy is harmless and helps some patients (placebo?). Am I over-thinking this? Cheers

Great video - very informative. You addressed a lot of things my surgeon never touched on. In fact, he's said very little to me, and I'm going into surgery in 3 days. But dude, seriously, slow it down. I had to reduce the play back speed to understand.

I had my surgery 8/20 and I’m still in a lot of pain.
Slushy ice packs are the best.
1 part rubbing alcohol
2 parts water
In a qt freezer ziplock bag.

Thanks, doctor. Great video series. I am at day 6 post-op. Doing well. I chose not to take opioids because the incidence of vomiting is outweighed by a minimal amount of tolerable pain managed with OTC analgesics.

You can take an anti nausea medication, like Zofran.
I have okay range of motion. And started with bands and lite weight to get strength back.

Almost all opioids will cause constipation with prolonged use. You can either do a home brew or take 1 capsule of Colace daily or 1 senokot daily. The only Opioid that I know of that does not cause constipation is Tramadol 50-100 mg BID/ twice a day.

Was thinking 3 weeks using ST disability so as not to burn vacation time and allow for proper rest and lack of stressing the arm/shoulder. Not physical labor but all computer related and not possible to sit at desk and keep arms up.

Thank you. I had an I think extensive rotator cuff repair surgery 9 days ago. on my dear dominant arm.I am still in quite a bit of pain. Is this normal? Would ice help at this point.? . My surgeon wants me in an immobilizer sling for another 21/2 weeks. I am taking extra strength Tylenol and maybe 1 or 2 Oxycodone a day. I live alone .
@@jocatran I am 7 days after my right rotator tear surgery. I feel no pain just some stiffness and slight soreness. I ice and I do my pendulum exercises several times a day! I take Advils everyday. How are you doing?
I no right. These videos give much more details about surgery, why we’re having so much pain at night, the importance of keeping the sling on for 6 weeks. I actually saw a video of the exact same surgery. It really clarified everything!

Hi doctor... I really like these videos. They're very informative. I had a straight Total Shoulder Arthroplasty with just the cap, my doctor said a stem wasn't needed. The rotator cuff was in good shape so he didn't have to do the reverse. My question to you, which is off topic. After I rehab.. can I slowly go back to conventional resistance exercises utilizing bands, dumbbell and kettlebells.? He also had to do a biceps tendonesis. What are are your thoughts?? I'm 65

Hey David, thanks for the comments. I'm glad you are recovering well enough to consider exercise. Every one is different so it is important to ask your surgeon these questions. That being said, typically in a patient that I perform a TSA once they have healed (3-6 mo ) after surgery I will allow them to do light weights and bands. The biceps tenodesis has little to with restrictions in my opinion once it heals. The big concern in glenoid loosening. Putting a lot of stress through the joint with heavy lifting can cause premature loosening of this component and result in needing to perform revision. Be careful and talk to your doc.

Thank you very informative Two weeks ago I had shoulder surgery. I had a combo of Frozen shoulder and partial rotator tear. My ortho doc didn't prescribed me any nerve pain med. It was first percocet. Did follow up and I mentioned pain and he prescribed hydracodone. I guess I should have been specific and also said nerve pain,because that is what I am truly suffering from now more than anything.

Exactly how robust is the tendon reattachment during the first 24 hours. A solo male, I had to readjust the sling pillow and lift it out while still in the sling. I also discovered why seniors in their 60's and marry one another after trying to put on a pair of socks. How much of a reach etc. would it take to re-injure the attachment after 36 hrs. I had to straighten my injured arm out and maybe just reached a little bit. Thank you, James.
James, thanks for subscribing! If I have someone with a big tear and is hard to fix, then it would be easier to retear than a small easy to repair tendon. So unfortunately it depends. I’m a nurse too,had my surgery this morning. Doing well,but wow the soreness is intense. Once the block wore off. I was a little smug about it, I have good tolerance. Doc said after 2 Nd there should big improvement in that. Wishing you a good recovery!

rotator cuff and biceps tenodesis repair. Left shoulder was not bad but the right shoulder was more severe with a full thickness tear (not sure if that is the biceps) and 2 tears to the rotator. It's been a month today and I still hurt and can't sleep. The bregg polar cube helped immensely with circulating ice water (used a t shirt cut in half lengthwise to protect skin). I don't take the painkillers but I wish I could find a way to stop it aching and be able to sleep
Leora, So sorry you had to go through surgery on both shoulders. I think ice is a great thing after shoulder surgery. Talk with your doc about other meds you may be able to take to allow you some sleep. There are a lot of different ways to decrease that ache. Pls let us know how you are doing. 🎉Had a massive rotator cuff and bicep surgery 7 days ago. Nerve block wore off. I did started taking Gabapine, hydrocodone, and meloxicam before the nerve block wore off. And also took laxative. My mistake is that I didn’t ice the first my 48 hours. Then I started using ice machine now. My shoulder seems to not healing. It’s still painful. My fingers, lower arm and elbow are still numb since after surgery. I can only move my fingers up and down and side but I can’t bend, make a fist or move my fingers together. No sensation at all. My upper arm is swollen / edema and no progress at all. I tried to not take my med to see if I can to tolerate the pain but it’s painful. My surgeon said to give it a time to heal, at least 2 weeks. Be patient, I am trying to be patient but I’m very worried and anxious. Why is my lower arms still numb? My surgeon thought he pulled a nerve while reattaching my bicep tendon, he had it analyzed and pathologist said it’s not a tendon, it’s blood vessel. I’m in a lot of pain if I don’t take meds, why is that?
Julie, So sorry you are having pain. Just because it is still painful does not mean that it is not healing. It is necessary to take your pain meds for weeks after surgery. Wish you had been my surgeon. Getting ready to get it done again.
My mom is a nurse. She made me some great ice packs. She used 50% rubbing alcohol and 50% water. Stacked them in freezer. Quart sizes and gallon baggies. Double bag ziplock. Then she made me some really neat covers for ice packs using thin white cotton kitchen rags. It’s super hard brushing teeth for me. Yes, constipation can kill you after surgery. My aunt died from an impaction from back surgery. I use ClearLAX with a little OJ diluted with water. Watch steps. Especially women with breast and an arm sling. You can fall down the stairs, fall over shoes, toys, and even get your leg hung up in a long cord like a phone charger. Cook ahead. Have somebody come every couple days to wash your dishes, bring food and help you bath. It will save money if you don’t need them there 24/7. My family helps me but it’s not free. Ha!

Summer, girl thank you for your story and all the recipes. Now I know the mixture for ice packs. Plus the laxative w oj perfect!. My hair dresser washes my hair. I take very slow and thoughtful showers since I only have one hand to use. Mostly the important body parts. I also hv my moms chair to sit on so that helps. Living alone today I figured out how to put my shoulder ice pack on by watching RUclips. I use the wall and or the corner of a wall to assist me when I hv to hold a strap in place as my other hand us busy with the other end of the strap, so far so good. I agree with the nerve block. I had it for ankle surgery and it was great for pain control. It did feel very weird not being able to feel or move my foot for 24 hours but i just watched TV and completely forgot about it. I wasn't getting out of bed for anything so it didn't matter anyway lol

I had a fracture and dislocation of my right ankle. Do you think the glucosamine chondroitin and UC-2 can prevent the post traumatic arthritis or would you say i’m doomed to getting arthritis at some point in the future?
Never doomed! Some things you can't change (like your injury) but you can do supplements, PT (exercise) stay strong, stay at an ideal body weight, etc. Good luck Coolwater83
I just had rotator cuff surgery on my superstanious, labrum repair, and biceps tendon repair and all the videos I watched and the nurses and doctors told me I would not need to take my prescribed pain meds until the next day. Well at 9pm on the dot the same night as surgery I was in excruciating pain I had a nerve block pump they sent me home with but I don't think they put it in right or something because I was in the worst pain I've ever had in my life 8 hours after surgery the pain was so bad I took 8 percocet from 9pm to 8 am and the pain never eased up
I'm sorry for your pain. Yes, the block sometimes doesn't work. I suggest to my patients to take meds when they begin to feel pain, and if they are not having any pain still take meds prior to going to bed the first night. So they have something in their body before the pain hits the next day. I hope the pain is getting better. Good Luck!

Larry, thanks for the question. Typically we don't recommend a brace early on after shoulder surgery. I think it can play a role after you have healed and are getting back into more normal activities. What surgery did you have?
@@rotatorcuffexpert doc has to see scan. “Will” have an anatomic total shoulder with an augmented glenoid or a reverse total shoulder with an augmented baseplate per his initial review.
I like Phillips MOM caplets. With full glass of water because I dont like otc fiber or colace. Colace gives me cramps. Plus you get some extra magnesium.

Hey David, I had to look up what a Macebell exercise routine entails. First off it is best to talk with your surgeon. If I have a patient that I performed a TSR, once they recover from surgery I allow them to go back to their favorite exercises with restrictions. Lifting heavy weights with strange angles may predispose the replacement to come loose. Tread carefully.

How much movement am I allowed within my upper body as long as I keep the arm in the sling and in front of/tucked in by my side /preserve the integrity ? Am I allowed to move my fingers, hand , wrist of the arm affected or NOT AT ALL?

I have experienced 2 full thickness tears as well possibly a partial tear of a third. However this will be determined by my surgeon after I have my MRI in 6 days. I am not certain at this point if I will have an open surgical or laparoscopic procedure.
Once you have a MRI, you and your surgeon can come up with a plan of care. Also check out my videos on how to choose your shoulder surgeon. Even if you already have a surgeon this is good information.

Two complete tears (1 of them retracted) and one partial tear. This will be in Philadelphia. Where are you that they’re not letting you have surgery now?
@@jakesmith8583 Yes no surgery due to an over abundance of needed surgeries as well due to Covid all the OR's at the moment are only performing emergency surgical procedures. And I have the inside scoop, I am a retired nurse and know all the surgeons, they had a 4 to 6 month waiting list just to have a 1st visit and I got in in 2 weeks. I will keep you guys posted to my progress.
Hi Dr. Orcutt. I am 2.5 weeks post surgery non dominant arm. I have had rotator cuff surgery on my dominant arm in addition to labral repair separate surgery. I am having a lot more pain with this one. Seems to be carrying on longer and different. Not supposed to see the doc for 4 more weeks. How do I know not to worry as my pt seems to be befuddled as well. I am not a pill taker and don’t want to be an alarmist.

Do you have any suggestions for pain at 8-9 weeks post op? I can handle the pain during the day with Motrin and Tylenol (yes, I take 3 of both at one time just to even get some relief), but during the night I always wake up around 4-5am in sever pain and can’t sleep any longer. My surgeon doesn’t want me on narcotics anymore (after my 6 week appointment) so that’s not an option. He says pain of 6-7 is normal. I just want to sleep! I am elevated with a wedge pillow as well as many other pillows when I sleep along with the cold/ice machine. Any suggestions would be great! Thanks.
Jennifer, that is a tough one. I typically expect my patients to need some pain meds for the the first 3 mo especially at night. Maybe talk again with your doc. There may be something they are comfortable with just for sleep. Good luck and let us know how you are doing.
Hello. Is combination of metamizol, diclofenac and dexason intramuscular applications (injection) ok after rotator cuff surgery? I have diverticulitis and I wish to avoid the pills.

Milk of magnesia comes in caplets now. Highly recommend gentle stool softener and very mild stimilant laxative. Just enough with pain meds. Take eith full glass water.

Dr. Good info. I'm having rotator cuff surgery Nov 12th. He's going to give me oxycodone. If that is taken as prescribed, should that keep me comfortable?
I took oxy for 5 days. Pain wasn't bad after 5th day. I had 4 anchors inserted, and had really bad tears. The worse part was being in the sling 6 weeks. Good luck

If your having a rotor cuff block, and you have a small cold can you still have surgery?
A lot depends on your surgeon and the anesthesiologist. Usually a small cold is ok, but you never know until you check. Good Luck!
Can a nerve block be used on my left shoulder since I have an ICD implanted on that same side in my chest? My rotator cuff arthroscopic surgery is scheduled in the next few days.
Dennis, In general a block can be performed in that shoulder. Obviously it is up to the anesthesiologist. I have not seen that to be a problem. Good luck and let us know how your are doing in your recovery.

I have a torn biceps and rotator cuff. For 2 months, steroid injection wore off after 2 weeks, PT seems to make pain worse. My surgeon can’t get me in for 2
months yet. I’m miserable, can’t sleep, ice helps temporarily. Would it be ok to seek another surgeon opinion if I can be helped sooner than 2 months.

I did have a opioid med that was weak. Didn't kill the pain that well

I've read horror stories about the nerve block not working and person in horrific pain. What will the doctor do if that happens? I''m so worried about my reverse shoulder surgery in three more weeks if this happens?
Superski, That happens sometimes. Usually the anesthesiologist will try to do an additional block or sometimes the surgeon will need to provide more pain meds. Good luck and please let us know how you are doing in you recovery.
My question is what are the findings with ice vs heat in light of the newer research that seems to show that recovery/healing is improved with heat and gentle range of motion and the creator of the ICER plan has recommended it be abandoned?
I am not sure of what the ICER plan is but in the early days after RCR, there is a lot of inflammation and blood flow to the shoulder. Most of the time this is a huge response. Using ice in the first few days helps decrease the inflammation and helps decrease pain. Gentile ROM also does this. Heat can be a tool, but not in the early days of rotator cuff healing.

Tree, It depends on your medical health and history etc. If we are talking in generic terms, in a healthy, otherwise uncompromised patient they can take gabapentin and a narcotic on the same day and even at the same time. Hope this helps
Curious on your thoughts about CBD, THC or marijuana to mitigate the pain in lieu or to supplement etc the pain. I go for a SLAP repair, debridement and re anchoring of possibly two of the bicep tendons. Not looking forward to it and about a week and a half to go until the procedure. And I do mean from a legal dispensary not from the street lol. Sorry if already asked and answered in the comments. Much appreciated and great videos
Gerit4357, Good question. I practice in Georgia, where there is limited legal ability to obtain CBD, THC so i am not an expert for sure. My concern is how does it interact with other meds that you would be taking during your recovery. With all my surgeries and still needing more extensive work done because my shoulders keep retearing, my doctor sent me to a pain specialist. I was shocked when the pain doctor wanted me to eat edibles or smoke the marijuana vape pens... I told them absolutely not!!! I don't want to be high to make the pain go away. I think they're all crazy for suggesting it. It was the most bizarre doctors appointment I've ever been too. I told him I felt like I was in the twilight zone... What has the world come too??? People abuse pain meds and people that truly need it for a longer period of time are now faced with this insanity of doctors wanting us to use marijuana. Now with that said... I have heard that it does wonders for pain but I DONT USE MARIJUANA AND IF I DID WOULD PROBABLY HAVE A FULL BLOWN PANIC ATTACK. I tried it once in high school and thought I was dying then. I couldn't feel my tongue and absolutely hated the feeling.

I am about to have rotator cuff repair, but my doctor suspects frozen shoulder and thinks I might need 2 surgeries, first to handle frozen shoulder and then repair of the rotator cuff, I already am on a pain management program for joint issues for complications dur to my ehlers Danfoss syndrome and have been on pain medication for 12 years, I am worried that my regular pain medication won't help me, will they give me something for a few days and what do you think about even going through with the surgery because it seems the likelihood of recovery is pretty low because my lack of mobility in that shoulder any advice would be greatly appreciated!!
Shana, That is a difficult problem. Often it is possible to address both the frozen shoulder and the RCR at the same time. Being on chronic pain meds is a big deal controlling pain after surgery. Skutch, any time you are doing a procedure there are risks involved. The nerve can be injured, that is why most use Ultrasound to make sure they are not damaging the nerve. Hope this helps.

Had my surgery 8/20, 6 1/2 weeks now and I’m still in alot of pain. Actually more than pre surgery.
I had a full tear in suspraspinatus and a partial in ifraspinatus

Certainly seeing your surgeon is the next most important step. To evaluate your motion. Your strength should be pretty bad right now, only 6 wks from RCR. Hopefully you will be able to start strengthening soon. That usually helps the pain. Good Luck!

I’m 6 weeks after having rotator cuff and bicep tenodesis surgery. What worries me at this point is that I wake up in the morning with pain and swollen shoulder then it gets better throughout the day.. Don’t know if this is somewhat normal at this point. Any thoughts
@@rira65 yes I would say that’s normal due to your body lying flat while you sleep. Standing up with gravity pulls the swelling down. Hope this helps.

Iveth, A lot depends on where you live and what your income is. In my area if the patients income is low then they may qualify for indigent care and the surgery may be free.

Hi Dr. I'm 3 weeks post minor RC repair 1cm tear supraspinatus.
First weeks pain was mild last 10 days has been excruciating muscl pain, like cramp, spasm in the trap area.
I fesr I may have damaged the repair by pushing things a bit.

In general it is normal to have pain in the traps and around the shoulder blade as these muscles are working harder than normal to support the repaired rotator cuff. Nerve block did not last me 10 hours so I feel like you need to have the pump where you can control the pain for the first two or three days because all I did with narcotics was throw up

I had my rotator cuff surgery and a lot of filing etc 6 weeks ago. I have followed my surgeon's advice to the letter but my shoulder is aching. It is actually getting worse. I use Turmeric massage oil which eases the pain temporarily. What can I do to alleviate the constant pain?😢
Hi Shirley and thanks for the comment. Pain is often one of the most difficult things after RCR because it lingers so long. It sometimes seems crazy how long your shoulder can hurt after surgery. There are many things that can help pain so please talk to your surgeon again. They know your medical history and what is safe for you to take.

Have you gotten exercises to do? I’d go back to the surgeon to see what can be done. I use ice every couple of hours. After surgery, I would wake myself up every couple of hours to ice and to take my meds so it wouldn’t be so painful later. I’ve used a TENS unit for pain relief also.

@@vincentpiparo8899 Not bad. Get stitches out tomorrow and start physical therapy Wednesday. The nerve block lasted about 24 hours, so you really don't feel much. But you'll want to start your pain meds sometime before the nerve block wears off. I was on hydrocodone 5mg/acetaminophen 325mg once every 6 hours. Then three hours after that, I took Ibuprofen as well every 6 hours. I quit all meds by Thursday evening/Friday morning. So say at noon I took hydrocodone, at 3pm I'd take Ibuprofen, at 6pm I'd take hydrocodone and so on until Friday morning. Wasn't too bad. Had a mild sharp pain every now and then but kept it iced up. Hot shower 48 hours after. That felt good for the muscles. Then iced it down about an hour after that. Recliner didn't work for me. It still put too much pressure on the back side of my shoulder. Had to prop up a futon matress on the spare bed so I could sleep with a pillow on my good side sitting up. Drink plenty of water, took a few stool softeners until I was off of meds. Rate of motion is severly limited for now but I guess PT and stretching will make it come back. Other than that, it's tight, lack of motion and starting to bruise now. Guess I'm a late bruiser.
