extremely painful gout attacks since my mid 30s. I'm 59 now. I'm otherwise

    • @NoNo-dq4mc
      @NoNo-dq4mc Год назад +9

This is a pretty basic overview and he left out what I consider a very important factor. Gout is a genetic disease. If you have hyperuricemia, you cannot lower uric acid and become normal again.
You can only lower uric acid possibly several points even if you loose weight, avoid purine, exercise, don't drink alcohol, hydrate, etc.
It is may help avoid flare ups, but without meds you can't meaningfully lower uric acid.
I'd consider that the most important and defining factor of the explanation of the cause of gout.
Also, several types of alcohol do not raise uirc acid themselves. (Basically not beer) they however limit it from being urinated out. Some studies show wine had no affect on uric acid. I doubt it though seeing wine is processed throu the liver just like all other forms of alcohol. There are stats of risk of flare ups based off studies of different types of alcohol.
Besides the genetics that cause gout, I would have also mentioned xanthine oxidase and it's role in production of uirc acid. Also, the process of uic acid becoming urate crystals. Like ph levels and what it binds with to form. Basically gout is urate crystals in joints. I don't remember him saying that either.

    • @crayrayc
      @crayrayc Год назад

@@NoNo-dq4mc no, he hit the nail on the head! everything else is just rhetoric from doctors and pharmaceuticals. your body creates uric acid naturally as a way of self cleaning/regeneration.. the crystals however are formed via inflammation due to the body's reaction to the sugars that are overloading everything.

Isn't it amazing how this information is not only not widely known, but neither is it promoted, advertised, or otherwise being disseminated to the public, but there is all kinds of advertisement and stuff for which medications are made to "alleviate" other things? Here, the prescription for alleviation is to just stop eating and drinking some things. There's no money to be made in that.....and that's why we don't know about this information. Thank you for putting it on RUclips. I learned these things about 10 or 12 years ago and have pretty much eliminated gout flares because of it. I learned it on a website some guy made who experimented on himself with his own gout issues to see what would cause a flare and what would keep them away.

2 years ago, i had back to back gout attacks within 6 months. It started in August, had it for a week then after 2 weeks, it'd start again all the way to February/March of 2022. I didn't understand why i kept getting it since i was just eating chicken, veggies, fruits and even started eating beyond meat products plus no alcohol or meat (pork/beef/turkey). I came across a video that says meat does NOT cause gout. It gave me a pause and made me think about what else i was consuming besides the obvious to avoid gout attacks and it made me realize that yes, i avoided everything my dr told me to avoid but i was still eating pastries/desserts and drinking bubble teas/sodas etc. My dr's have never told me to avoid sugar but it was the main cause for my gout attacks. I haven't had bubble teas/sodas/pastries/desserts for a year now, been eating more meat/eggs and haven't had gout attack since then. Also, my bad cholesterol went down, blood sugar is normal as well as my blood pressure. Have also been cooking my food with butter and extra virgin olive oil instead of vegetable/canola/peanut oil etc. Nowadays, we really have to do our own research to help ourselves because most doctors would just prescribe medicines instead of healing it with dietary and lifestyle changes first.

Great video, doc. I just ran into this while drinking a cup of real grape juice drink, about 20 ozs of a real wine (8% alcohol!) a day and an occasional five beers a night. My legs and feet swelled up for the first time in my life. It really terrified me. The oddity here was a grossly sweet frozen cheesecake made everything come to a head. I never knew a single incident of food poisoning could cause your legs and feet to swell so bad, but it can. I stopped the sweets, dropped the wine and beer, and went on a seven day fast with no solid foods - just lots of spring water, many types of teas only sparingly sweetened with real honey and just a half cup of fruit juice if I got lightheaded. I made it the full seven days fasting with no solid food. My legs and feet went totally back to normal. My eyes got better and my brain fog, that I didn't even realize was that bad, went away. I look forward to my next seven day fast soon. And I am definitely reconsidering any more alcohol consumption at all. I never want to live with swollen legs and feet. I wished I knew this fasting and complete refusal to ingest anything sugar producing was a cure when my wife was alive. She had swollen legs the last couple of years of her life and the Lasix didn't help it at all. You are 100% correct about the alcohol and anything else full of sugar. It is really bad for you.

There's more going on with gout than just sugar and alcohol. Six years ago, I had my first gout episode. Four years ago, I gave up sugar and fruit. I've never drank. I ended up in keto, even though that wasn't a goal, I also lost 20 pounds and was not overweight. One year ago, I had the worst gout episode yet-- both feet, and I couldn't walk without pain, which was difficult for me because I walk between four and six miles per day. I'm on Allopurinol now, and it's working so far. Sugar and alcohol are not the full reason for gout. YMMV.

I’ve been getting gout for twenty years. I finally realised my triggers were aspirin at any dosage and anchovies. I can drink litres of OJ (no added sugar) and a nice glass of red occasionally without a problem. Now instead of my big toe, the crystals are forming in my heels. I go to the gym, sweat it out for two or three hours with lots of water and after a couple of days, it’s gone.

Give up the OJ or sooner, rather than later, that will be your new trigger.
It takes approximately 16 medium orange to make one litre of juice (no sugar added). The fact that you said you drink litres implies that you drink at least 2 litres a day. That is 32 medium oranges WITHOUT the added benefits the orange fibres provide, basically 2 litres of mostly fructose. I'm sure you wouldn't consider eating 32 medium sized oranges. Replace the litres of OJ with litres of water and maybe also eat an orange or two.

I used to drink close to 6 liters of soda a day - or a 12 pack when I was drinking cans (I naively thought that I would cut back by switching to drinking from a glass). I would get gout at least once every couple of years and it would usually depend on a lot of factors. Now that you mention the kidney connection that makes sense. I used to take a LOT of ibuprofen (thank you United States Military) and it was usually after a cold or some other sinus affliction. I would take pseudoephedrine for my sinuses (when it was available) and I would take a lot of ibuprofen because when the rebound would happen (and with sinus medicine it's not a if, it's a when) I would get severe headaches. It was usually after these events that I would have several things go wrong. Gout is the warning usually, then I would get an upper GI bleed from too much IB, which I would then fight for three days until I got dehydrated and went to urgent care. All of that was related to IB. I have since stopped taking IB, and only take Tylenol for pain. I have since stopped drinking soda as well. I haven't had any issues (knock on wood) in almost 3 years since I changed my habits. Oh, I forgot to mention, I also would up with diverticulitis. My mother died from complications of diverticulosis, so that was a wake up call for me.

When I discovered this years back, I have no more gout attacks, diet moderation is also the key, stop gorging yourself on foods, reduce portions and use more water instead of juices, beer and sodas.

Many people have high uric acid but never get gout. Why? Gout = uric acid + inflammation. I can eat anything I want now because I eliminate the inflammation part of the equation with turmeric, MSM, blueberries, and other ingredients. If you're a beer lover and don't want to give up your beer....controlling the inflammation and not getting dehyrated is your key to having a reasonable amount of beer. Add in some liposomal C and celery extract with lots of hydration to remove uric acid even quicker.

I don't drink alcohol, don't eat any fructose/sugar, I'm low carb with time restricted eating, eat quite a lot of fat but do not eat large amounts of protein, never eat shellfish and only occasionally eat fish. Blood pressure is typically around 110/70. Last CRP a couple of months ago is below 1 (still higher than I prefer) and recent triglycerides around 0.8 mmol/L.
Interestingly, my uric acid level is often at the top end of the normal scale (0.46 mmol/L or 7.7 mg/dL) and never below 0.4 mmol/L. It has been at that level ever since I first measured it a couple of years ago. I've never had the slightest negative effect from it, even when I was high carb.
I've heard uric acid referred to as being a paradox. It is the strongest antioxidant in the human body, yet high levels can cause inflammation and a variety of problems, but some people with high uric acid never appear to have any issues from it.
I'm still keen to understand if it is actually a problem for me or not. I got my own uric acid meter so I can check it more often and see if anything can affect it, but no luck so far.

It would appear I have a genetic tendency for high uric acid production. There is research that indicates 'Most patients with asymptomatic hyperuricemia never develop gout or stones. Pharmacologic treatment for asymptomatic hyperuricemia carries some risk, is not considered beneficial or cost-effective, and generally is not recommended.'
Recently I've found an almost 100% carnivore diet has kept my level under 0.40 mmo/L (as low as 0.36) and has not gone up towards 0.46 so I will continue that for a while. Yeah, I have not been taken any alcohol, and hardly any sugar, and I still got a very bad gout flare up. I remember that I suspected that broke my ankle, even though there was no trauma. The flare up lasted about a month, and I was in so much pain that I could not walk for 5 days. Well, now I suspect that it may be my attempt at a carnivore diet that caused that. I was on it for a few months, and I took almost no fiber. Well, I am not doing this anymore. So, about 2 weeks after I recovered from my first attack, I get a second one last week. Thankfully, it was mild and I only had problem walking for part of a day. I suppose I can look at this second attack in two ways. 1. I failed to prevent a second attack so I am still doing something wrong. Or, 2. the second flare up is kind of an aftershock. So, my lifestyle changes are working because it was so mild. Well, I don't know which is more accurate. Only the 3rd flare up will help decide that. If I continue to get flare ups regularly, then 1. is more accurate. If I get few flare ups and they reduce in intensity, then 2. is more correct. Wish me luck.

The advice is to do all one can to prevent gout from recurring a second time and thus becoming chronic because it leads to other bigger problems.

i had a lot of injuries as a hockey player. broken bones, torn ligaments and muscles, but compared to gout in the knee, the pain was nothing. after a few attacks, i cut alcohol and sugar from my diet and can eat normally. avoid skin from fish and chicken but can eat anything else. drink 2-4 litres of water a day and am complaint free.

I had bad spells of gout in my 20s - this was put down to almost constant dehydration hard physical work in a mine, in temperatures in excess of 100F. I would drink a 10 pint can of water in a shift [more if I could get it] and sweat it all out without having need to pee. After moving to a shallower and much cooler mine attacks ceased.
My current less serious gout problem arose from the need to use large quantities of digestive enzymes made of pigs pancreases that are chock full of purines. A low dose of Allopurinol has this under control.

I got gout and high uric acid from drinking concentrated chicken broth I made myself without eating any carbohydrates at all.
It was 100% carnivore and my uric acid was around 14 mg/dL.
The pain in my ankle was terrible.
So purines do matter even on a carb-free diet.
I also got gout from eating a plant-based ketogenic diet from avocados and macadamia nuts twice even though I only ate the carbohydrates found in those two foods.
It turns out that high fat diets, even when low in carbohydrates, can impair the excretion of endogenous uric acid.

Some meat have high level of purines, so it's not a great pick if you have gout yeah. It's extremely hard to avoid all fructose AND purines, so managing your intake and staying hydrated is probably your safest bet.

I had uric acid kidney stones and my nephrologist put me on alopurinol AND drinking baking soda daily (1 level tablespoon in 8 oz. of water, twice daily) and drinking at least 80 oz of water daily.

I was on the protocol that I mentioned in my original post for about 3 weeks under direction of my urologist, have continued with drinking the correct amount of daily water (with lime juice), and altered my diet accordingly. No issues since. Good luck.

Same happened to me, had to call an ambulance. And when I was at a residential program where they served meat at every meal, I had a gout flare up after two days, different people have different chemistry's and different biological dysfunctions. This one thing across the board is what is wrong with modern medicine (particularly American). Adding "for most people" goes a long way in your credibility in my eyes.

Red meat can be problematic too. And sugar is everywhere, especially in the US, just look at what’s in normal bread. Carbs, especially white wheat, white rice etc. is metabolised into sugar as well. And: we’re all susceptible to different things.

AND the there is familial gout from which my husband has occasional flare ups (6 or so yearly) especially after eating seafood or asparagus. He has a sweet tooth and moderately drinks wine/beer. His mother had it (died at 98 from lung cancer due to heavy smoking), his mother’s sister had it (died of heart problems at 95), and his maternal grandfather had it (died tragically young at 86 in a car crash). One “size” does not fit all…

I've found my biggest issue is vitamin d, hydration, and sleep. I tend to be deficient in all of those. I'm otherwise a decently active dude. My recent flare-up really affected my mental health. I use a lamp for vitamin D and stopped using it during the summer, thinking things would be cool. Things were not cool. I'm going to use that lamp every day and any extra sunlight I get will be extra. I finally broke it out after about 6 weeks of not using it and I felt relief and reduction in swelling and pain and an improvement of mobility in just a few hours.
